Grading the severity of obstruction in patients with Chronic Obstructive Pulmonary Disease and morbid obesity
Grading the severity of obstruction in patients with Chronic Obstructive Pulmonary Disease and morbid obesity. N. Barbarito, E. De Mattia. Aim. To evaluate the severity of airway obstruction in patients affected by chronic obstructive pulmonary disease (COPD) in the presence of concomitant restriction due to morbid obesity. Methods. Lung function test, six-minute walking distance (6MWD) test, body mass index measurement (BMI), and determination of dyspnoea using the Modified Medical Research Council Dyspnoea Scale (MMRC) were performed on each patient referred to our department according to their individual respiratory diagnosis or symptoms. Analysis was performed on smokers or ex-smokers patients, with both dyspnoea and chronic productive cough, showing non fully reversible airflow obstruction, with normal-weight (NW: BMI 22 to 24 kg/m2) or morbid-obesity (MO: BM
Prevalence of airflow obstruction according GOLD, ATS and ERS criteria in symptomatic ever-smokers referring to a Pulmonary Rehabilitation Department
Aim. To evaluate in a Pulmonary Rehabilitation (PR) setting the prevalence of airflow obstruction (AO) in either current or former smokers ≥45 years old both with dyspnoea and with chronic productive cough, using European Respiratory society (ERS) statement (FEV1/SVC <88 and <89% predicted in men and women, respectively), American Thoracic Society (ATS) statement (FEV1/FVC <75%), and Global Initiative for Chronic Obstructive Lung Disease (GOLD) statement (FEV1/FVC <70%). Methods. Lung function tests were performed in each patient who was referred to our PR department due to respiratory diagnosis or symptoms. For analysis, in patients showing AO we used post-bronchodilator lung function values. Results. In 184 ever-smoker patients with symptoms of chronic obstructive pulmonary disease (COPD), the prevalence rates of AO were as follows: ERS = 89.7%, ATS = 76.6%, and GOLD = 63.6%. Patients with AO according ERS criteria showing moderate to severe (M/S) obstruction (i.e., FEV1 <70% predicted) were 119. Patients with ERS M/S AO but without AO using either ATS or GOLD criteria were 8.4% and 19.3%, respectively. Conclusions. Prevalence of AO is highly dependent on which guidelines it is based. ATS and particularly GOLD statement can cause a large under-diagnosis even of moderate to severe COPD. Diagnosis of COPD may be overlooked if SVC is not performed
Zoledronic acid boosts γδ T-cell activity in children receiving αβ+ T and CD19+ cell-depleted grafts from an HLA-haplo-identical donor
We demonstrated that γδ T cells of patients given HLA-haploidentical HSCT after removal of αβ+ T cells and CD19+ B cells are endowed with the capacity of killing leukemia cells after ex vivo treatment with zoledronic acid (ZOL). Thus, we tested the hypothesis that infusion of ZOL in patients receiving this type of graft may enhance γδ T-cell cytotoxic activity against leukemia cells. ZOL was infused every 28 d in 43 patients; most were treated at least twice. γδ T cells before and after ZOL treatments were studied in 33 of these 43 patients, till at least 7 mo after HSCT by high-resolution mass spectrometry, flow-cytometry, and degranulation assay. An induction of Vδ2-cell differentiation, paralleled by increased cytotoxicity of both Vδ1 and Vδ2 cells against primary leukemia blasts was associated with ZOL treatment. Cytotoxic activity was further increased in Vδ2 cells, but not in Vδ1 lymphocytes in those patients given more than one treatment. Proteomic analysis of γδ T cells purified from patients showed upregulation of proteins involved in activation processes and immune response, paralleled by downregulation of proteins involved in proliferation. Moreover, a proteomic signature was identified for each ZOL treatment. Patients given three or more ZOL infusions had a better probability of survival in comparison to those given one or two treatments (86% vs. 54%, respectively, p = 0.008). Our data indicate that ZOL infusion in pediatric recipients of αβ T- and B-cell-depleted HLA-haploidentical HSCT promotes γδ T-cell differentiation and cytotoxicity and may influence the outcome of patients
Sleep, napping and alertness during an overwintering mission at Belgrano II Argentine Antarctic station
During Antarctic isolation personnel are exposed to extreme photoperiods. A frequent observation is a sleep onset phase delay during winter. It is not known if, as a result, daytime sleeping in the form of naps increases. We sought to assess sleep patterns - with focus on daytime sleeping - and alertness in a Latin American crew overwintering in Argentine Antarctic station Belgrano II. Measurements were collected in 13 males during March, May, July, September and November, and included actigraphy and psychomotor vigilance tasks. Sleep duration significantly decreased during winter. A total of eight participants took at least one weekly nap across all measurement points. During winter, the nap onset was delayed, its duration increased and its efficiency improved. We observed a significant effect of seasonality in the association of evening alertness with sleep onset. Our results replicate previous findings regarding sleep during overwintering in Antarctica, adding the description of the role of napping and the report of a possible modulatory effect of seasonality in the relation between sleep and alertness. Napping should be considered as an important factor in the scheduling of activities of multicultural crews that participate in Antarctica.

Measurement of the atmospheric muon depth intensity relation with the NEMO Phase-2 tower
The results of the analysis of the data collected with the NEMO Phase-2
tower, deployed at 3500 m depth about 80 km off-shore Capo Passero (Italy), are
presented. Cherenkov photons detected with the photomultipliers tubes were used
to reconstruct the tracks of atmospheric muons. Their zenith-angle distribution
was measured and the results compared with Monte Carlo simulations. An
evaluation of the systematic effects due to uncertainties on environmental and
detector parameters is also included. The associated depth intensity relation
was evaluated and compared with previous measurements and theoretical
predictions. With the present analysis, the muon depth intensity relation has
been measured up to 13 km of water equivalent.Comment: submitted to Astroparticle Physic
Measurement of the residual energy of muons in the Gran Sasso underground Laboratories
The MACRO detector was located in the Hall B of the Gran Sasso underground
Laboratories under an average rock overburden of 3700 hg/cm^2. A transition
radiation detector composed of three identical modules, covering a total
horizontal area of 36 m^2, was installed inside the empty upper part of the
detector in order to measure the residual energy of muons. This paper presents
the measurement of the residual energy of single and double muons crossing the
apparatus. Our data show that double muons are more energetic than single ones.
This measurement is performed over a standard rock depth range from 3000 to
6500 hg/cm^2.Comment: 28 pages, 9 figure
New MACRO results on atmospheric neutrino oscillations
The final results of the MACRO experiment on atmospheric neutrino
oscillations are presented and discussed. The data concern different event
topologies with average neutrino energies of ~3 and ~50 GeV. Multiple Coulomb
Scattering of the high energy muons in absorbers was used to estimate the
neutrino energy of each event. The angular distributions, the L/E_nu
distribution, the particle ratios and the absolute fluxes all favour nu_mu -->
nu_tau oscillations with maximal mixing and Delta m^2 =0.0023 eV^2. A
discussion is made on the Monte Carlos used for the atmospheric neutrino flux.
Some results on neutrino astrophysics are also briefly discussed.Comment: Invited Paper at the NANP03 Int. Conf., Dubna, 200
The ANTARES Optical Beacon System
ANTARES is a neutrino telescope being deployed in the Mediterranean Sea. It
consists of a three dimensional array of photomultiplier tubes that can detect
the Cherenkov light induced by charged particles produced in the interactions
of neutrinos with the surrounding medium. High angular resolution can be
achieved, in particular when a muon is produced, provided that the Cherenkov
photons are detected with sufficient timing precision. Considerations of the
intrinsic time uncertainties stemming from the transit time spread in the
photomultiplier tubes and the mechanism of transmission of light in sea water
lead to the conclusion that a relative time accuracy of the order of 0.5 ns is
desirable. Accordingly, different time calibration systems have been developed
for the ANTARES telescope. In this article, a system based on Optical Beacons,
a set of external and well-controlled pulsed light sources located throughout
the detector, is described. This calibration system takes into account the
optical properties of sea water, which is used as the detection volume of the
ANTARES telescope. The design, tests, construction and first results of the two
types of beacons, LED and laser-based, are presented.Comment: 21 pages, 18 figures, submitted to Nucl. Instr. and Meth. Phys. Res.
