Pneumotórax espontâneo - novas perspectivas sobre avaliação diagnóstica e uso de cirurgia minimamente invasiva para manejo do paciente: Spontaneous pneumotorax - new perspectives on diagnostic evaluation and use of minimally invasive surgery for patient management
A pleura constitui uma membrana de tecido mesotelial que reveste os pulmões e a cavidade torácica, possibilitando o adequado funcionamento destes órgãos. Além disso, possui dois folhetos, o parietal e o visceral; quando na presença de ar entre estes, ocorre uma complicação denominada pneumotórax, o qual possui múltiplas etiologias. Nesse sentido, o pneumotórax espontâneo (PE) corresponde a uma forma de apresentação da afecção, porém menos frequente. Ademais, o pneumotórax representa uma emergência médica e pode ser tratado de maneira conservadora ou cirúrgica, contudo, há divergências na literatura sobre quais critérios devem ser utilizados para se optar pelo manejo expectante ou invasivo. Outrossim, vários casos de PE não apresentam etiologia esclarecida e são denominados de pneumotórax espontâneo primário (PEP); não obstante, tal terminologia é questionada atualmente, pois estudos indicam que sempre há uma doença pulmonar adjacente como fator predisponente, de maneira que nos casos considerados primários, apenas não houve a devida identificação da doença pulmonar responsável pelo pneumotórax. Ainda, os sinais e sintomas na doença são inespecíficos e mimetizam várias doenças, fazendo-se necessário a consideração de diagnósticos diferenciais importantes, os quais irão alterar a conduta e a gravidade do quadro, bem como a necessidade imediata ou não de procedimentos invasivos. Para o diagnóstico, além da suspeição clínica, são necessários exames complementares, como a radiografia e a tomografia computadorizada de tórax, os quais auxiliam também na identificação de complicações da doença
Measurement of the cosmic ray spectrum above eV using inclined events detected with the Pierre Auger Observatory
A measurement of the cosmic-ray spectrum for energies exceeding
eV is presented, which is based on the analysis of showers
with zenith angles greater than detected with the Pierre Auger
Observatory between 1 January 2004 and 31 December 2013. The measured spectrum
confirms a flux suppression at the highest energies. Above
eV, the "ankle", the flux can be described by a power law with
index followed by
a smooth suppression region. For the energy () at which the
spectral flux has fallen to one-half of its extrapolated value in the absence
of suppression, we find

Energy Estimation of Cosmic Rays with the Engineering Radio Array of the Pierre Auger Observatory
The Auger Engineering Radio Array (AERA) is part of the Pierre Auger
Observatory and is used to detect the radio emission of cosmic-ray air showers.
These observations are compared to the data of the surface detector stations of
the Observatory, which provide well-calibrated information on the cosmic-ray
energies and arrival directions. The response of the radio stations in the 30
to 80 MHz regime has been thoroughly calibrated to enable the reconstruction of
the incoming electric field. For the latter, the energy deposit per area is
determined from the radio pulses at each observer position and is interpolated
using a two-dimensional function that takes into account signal asymmetries due
to interference between the geomagnetic and charge-excess emission components.
The spatial integral over the signal distribution gives a direct measurement of
the energy transferred from the primary cosmic ray into radio emission in the
AERA frequency range. We measure 15.8 MeV of radiation energy for a 1 EeV air
shower arriving perpendicularly to the geomagnetic field. This radiation energy
-- corrected for geometrical effects -- is used as a cosmic-ray energy
estimator. Performing an absolute energy calibration against the
surface-detector information, we observe that this radio-energy estimator
scales quadratically with the cosmic-ray energy as expected for coherent
emission. We find an energy resolution of the radio reconstruction of 22% for
the data set and 17% for a high-quality subset containing only events with at

Measurement of the Radiation Energy in the Radio Signal of Extensive Air Showers as a Universal Estimator of Cosmic-Ray Energy
We measure the energy emitted by extensive air showers in the form of radio
emission in the frequency range from 30 to 80 MHz. Exploiting the accurate
energy scale of the Pierre Auger Observatory, we obtain a radiation energy of
15.8 \pm 0.7 (stat) \pm 6.7 (sys) MeV for cosmic rays with an energy of 1 EeV
arriving perpendicularly to a geomagnetic field of 0.24 G, scaling
quadratically with the cosmic-ray energy. A comparison with predictions from
state-of-the-art first-principle calculations shows agreement with our
measurement. The radiation energy provides direct access to the calorimetric
energy in the electromagnetic cascade of extensive air showers. Comparison with
our result thus allows the direct calibration of any cosmic-ray radio detector
against the well-established energy scale of the Pierre Auger Observatory.Comment: Replaced with published version. Added journal reference and DOI.

International Society of Human and Animal Mycology (ISHAM)-ITS reference DNA barcoding database - the quality controlled standard tool for routine identification of human and animal pathogenic fungi
Human and animal fungal pathogens are a growing threat worldwide leading to emerging infections and creating new risks for established ones. There is a growing need for a rapid and accurate identification of pathogens to enable early diagnosis and targeted antifungal therapy. Morphological and biochemical identification methods are time-consuming and require trained experts. Alternatively, molecular methods, such as DNA barcoding, a powerful and easy tool for rapid monophasic identification, offer a practical approach for species identification and less demanding in terms of taxonomical expertise. However, its wide-spread use is still limited by a lack of quality-controlled reference databases and the evolving recognition and definition of new fungal species/complexes. An international consortium of medical mycology laboratories was formed aiming to establish a quality controlled ITS database under the umbrella of the ISHAM working group on "DNA barcoding of human and animal pathogenic fungi." A new database, containing 2800 ITS sequences representing 421 fungal species, providing the medical community with a freely accessible tool at http://www.isham.org and http://its.mycologylab.org/ to rapidly and reliably identify most agents of mycoses, was established. The generated sequences included in the new database were used to evaluate the variation and overall utility of the ITS region for the identification of pathogenic fungi at intra-and interspecies level. The average intraspecies variation ranged from 0 to 2.25%. Neuroproteção na ressecção cirúrgica de gliomas cerebrais: revisão da evidência atual
Os gliomas cerebrais são tumores primários do sistema nervoso central que se desenvolvem a partir de células gliais e têm alta morbimortalidade. Seu tratamento padrão envolve a ressecção cirúrgica, radioterapia e quimioterapia, os quais possivelmente podem levar os pacientes a um prognóstico desfavorável. Nesse contexto, a neuroproteção entra como uma aliada para minimizar os efeitos colaterais da ressecção cirúrgica e melhorar a sobrevida e a qualidade de vida dos pacientes. Nesse sentido, o presente estudo tem como objetivo discutir sobre a evidência atual da neuroproteção na ressecção cirúrgica de gliomas cerebrais. Para isso, foram selecionados quatro artigos que que abordavam sobre a evidência atual da neuroproteção na ressecção cirúrgica de gliomas cerebrais, por meio de uma estratégia de busca com recorte temporal entre 2014 e 2023, nas bases de dados PubMed (Medline), Embase e Cochrane Library. Os resultados indicam que o grupo de pacientes que recebeu dexmedetomidina apresentou melhora significativa na cognição e redução da inflamação cerebral em comparação com o grupo-controle pós-ressecção dos gliomas cerebrais, além de menor incidência de efeitos colaterais anestésicos, como náusea e vômitos (p < 0,05). Ademais, foi observado que a modulação da via metabólica do glutamato/glutamina pode inibir o crescimento de gliomas e proteger o parênquima cerebral. Nesse sentido, as evidências atuais indicam que proteger as células nervosas é uma estratégia importante para minimizar os efeitos colaterais da ressecção cirúrgica de gliomas cerebrais, e a dexmedetomidina e a co-cultura de células de glioma e astrócitos que aumenta a concentração extracelular de glutamato e glutamina parecem ser importantes aliadas nessa profilaxia

Optimasi Portofolio Resiko Menggunakan Model Markowitz MVO Dikaitkan dengan Keterbatasan Manusia dalam Memprediksi Masa Depan dalam Perspektif Al-Qur`an
Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis
