    EVALUATING THE EFFICACY OF CONTROLLED EXTENSIVE READING TASKS ON ESL LEARNING OUTCOMES

    This study sought to establish whether students who are exposed to Controlled Extensive Reading Tasks (CERTs) will have higher learning outcomes during English as a Second Language (ESL) lessons. A 3-month intervention pilot of Controlled Extensive Reading (CER) instruction, was implemented in two secondary schools in Kakamega County, Western Kenya, from February to May 2023. We compared the impacts of the CERTs treatment group in ESL lessons in the pilot school with those of the control group that did not use the intervention but utilized the same instructional components. The findings revealed that assignment to the CERTs pilot group had additional improvement in the target language skills while the control group had somewhat constant learning outcomes. The study is useful to curriculum stakeholders who should consider such results alongside existing implementation strategies in ESL policies and program designs

    Comparison of Microscopy and PCR for Detection of Giardia Lamblia and Entamoeba Histolytica in Human Stool Specimens in a Resource Limited Setting in Western Kenya

    BACKGROUND፡ Accurate diagnosis of Giardia lamblia and Entamoeba histolytica is important since these intestinal parasites account for a significant proportion of morbidity and mortality globally. Microscopy is the key diagnostic test used for diagnosis of the two parasites. Other tests including rapid diagnostic tests and polymerase chain reaction have been developed to improve the detection of these parasites. Most of these newer tests are not affordable in resource limited settings, hence the over reliance on microscopy. The objective of this study was to determine the reliability of microscopy in a resource limited setting in Western Kenya, a region endemic for the two intestinal parasites.METHODS: Polymerase chain reaction, the gold standard test, was performed on stool samples suspected for G. lamblia and E. histolytica. Microscopy was then performed on the same samples and the two tests compared.RESULTS: Microscopy was found to be 64.4% sensitive, 86.6% specific for the detection of G. lamblia. Additionally, this test was 64.2% sensitive and 83.6% specific for the diagnosis of E. histolytica. Cohen’s kappa values of 0.51 and 0.47 were determined for microscopy for G. lamblia and E. histolytica respectively. McNemar’s test revealed a significant difference between the two tests, P<0.001.CONCLUSION: This study found microscopy to be a reliable diagnostic test in this resource limited setting

    The role of indigenous knowledge (IK) in adaptation to drought by agropastoral smallholder farmers in Uganda

    44-52Majority of agro pastoral smallholder farmers in developing countries are characterised by low pay status, high illiteracy levels and are tied to cultural beliefs and spirits. These drawbacks have led them into using more indigenous knowledge as a way of adapting to droughts. This study explored the contribution of IK in enhancing farmer’s resilience to drought in crop and livestock production systems in semi-arid areas. The objectives were: (i) to examine the agro pastoral farmer’s perceptions of drought indicators and associated impacts and (ii) determinants for the adoption of indigenous knowledge drought adaptation responses. Socio-economic data was collected using oral interviews. The farmers (240) were selected using random sampling procedures and subjected to structured questionnaires and key informant guides. This study highlights that the droughts experienced were perceived to be more severe, recurrent and pervasive but also erratic. The IK drought signs were: flowering of wild plants, blowing of strong winds and appearance of flying and crawling insects. In crop-based systems, droughts reduced yields, caused plant stunted growth, increased incidences of diseases and invasive weed species. In livestock, droughts reduced surface water levels, lowered milk and beef yields, and increased diseases. The farmers reacted to droughts by practicing indigenous agro forestry, mulching, changing planting time, crop rotation and hoeing of trenches to control fires and pests/diseases. They also took their animals to wetlands for foraging and drew water for animals to drink. The uptake of IK drought adaptation practices was related to: household size, personal farming life experience, gender and age of the agro pastoral farmers. Therefore, employing IK drought adaptation responses is an important step towards increasing the resilience of agro pastoral smallholder farmers but also the conservation of IK for forthcoming generations

    Cost-benefit analysis of foot and mouth disease control in Ethiopia

    Foot and mouth disease (FMD) occurs endemically in Ethiopia. Quantitative insights on its national economic impact and on the costs and benefits of control options are, however, lacking to support decision making in its control. The objectives of this study were, therefore, to estimate the annual costs of FMD in cattle production systems of Ethiopia, and to conduct an ex ante cost-benefit analysis of potential control alternatives.<br/><br/>The annual costs of FMD were assessed based on production losses, export losses and control costs. The total annual costs of FMD under the current status quo of no official control program were estimated at 1354 (90% CR: 864–2042) million birr. The major cost (94%) was due to production losses. The costs and benefits of three potential control strategies: 1) ring vaccination (reactive vaccination around outbreak area supported by animal movement restrictions, 2) targeted vaccination (annual preventive vaccination in high risk areas plus ring vaccination in the rest of the country), and 3) preventive mass vaccination (annual preventive vaccination of the whole national cattle population) were compared with the baseline scenario of no official control program. Experts were elicited to estimate the influence of each of the control strategies on outbreak incidence and number of cases per outbreak. Based on these estimates, the incidence of the disease was simulated stochastically for 10 years. Preventive mass vaccination was epidemiologically the most efficient control strategy by reducing the national outbreak incidence below 5% with a median time interval of 3 years, followed by targeted vaccination strategy with a corresponding median time interval of 5 years. On average, all evaluated control strategies resulted in positive net present values. The ranges in the net present values were, however, very wide, including negative values. The targeted vaccination strategy was the most economic strategy with a median benefit cost ratio of 4.29 (90%CR: 0.29–9.63). It was also the least risky strategy with 11% chance of a benefit cost ratio of less than one.<br/><br/>The study indicates that FMD has a high economic impact in Ethiopia. Its control is predicted to be economically profitable even without a full consideration of gains from export. The targeted vaccination strategy is shown to provide the largest economic return with a relatively low risk of loss. More studies to generate data, especially on production impact of the disease and effectiveness of control measures are needed to improve the rigor of future analysis.<br/

    Using contact data to model the impact of contact tracing and physical distancing to control the SARS-CoV-2 outbreak in Kenya [version 1; peer review: 1 approved, 1 approved with reservations]

    Background: Across the African continent, other than South Africa, COVID-19 cases have remained relatively low. Nevertheless, in Kenya, despite early implementation of containment measures and restrictions, cases have consistently been increasing. Contact tracing forms one of the key strategies in Kenya, but may become infeasible as the caseload grows. Here we explore different contact tracing strategies by distinguishing between household and non-household contacts and how these may be combined with other non-pharmaceutical interventions. Methods: We extend a previously developed branching process model for contact tracing to include realistic contact data from Kenya. Using the contact data, we generate a synthetic population of individuals and their contacts categorised by age and household membership. We simulate the initial spread of SARS-CoV-2 through this population and look at the effectiveness of a number of non-pharmaceutical interventions with a particular focus on different contact tracing strategies and the potential effort involved in these. Results: General physical distancing and avoiding large group gatherings combined with contact tracing, where all contacts are isolated immediately, can be effective in slowing down the outbreak, but were, under our base assumptions, not enough to control it without implementing extreme stay at home policies. Under optimistic assumptions with a highly overdispersed R0 and a short delay from symptom onset to isolation, control was possible with less stringent physical distancing and by isolating household contacts only. Conclusions: Without strong physical distancing measures, controlling the spread of SARS-CoV-2 is difficult. With limited resources, physical distancing combined with the isolation of households of detected cases can form a moderately effective strategy, and control is possible under optimistic assumptions. More data are needed to understand transmission in Kenya, in particular by studying the settings that lead to larger transmission events, which may allow for more targeted responses, and collection of representative age-related contact data

    "The primary goal of this project is to establish a framework for a new human-centred, evidence-based and user-led approach to the design of facilities – including shelters and care centres – aimed at specifically supporting survivors of human trafficking as part of their psychological rehabilitation. The project will draw on the memory and direct testimony of survivors in Uganda, and of those currently resident in a recovery facility, to investigate and establish how the physical environment can promote their healing. Data collected through creative workshops and the use of observational, participatory and digital media methods will outline key architectural and landscaping design aspects of facilities that positively impact the survivors’ psychological health. The findings of this research will be disseminated as a design framework setting out evidence-based approaches for purpose-built, human-centred facilities aimed at supporting survivors."     Health financing in response to COVID-19: An agenda for research

    The global spread of COVID-19 has affected both the health and economic condition of countries, with major health system impacts. There has been an immediate need to invest in clinical services to treat patients and mount an effective public health response, requiring substantial increases in health spending. But the impact of the pandemic on the global economy also raises challenges for future health spending, with potential impacts on commitments to universal health coverage. In this working paper we outline a broad research agenda that would help countries deal with the health financing challenges they are facing, and emerge from the COVID-19 crisis with stronger health financing systems. While recognising that research priorities must be tailored to the needs of specific countries, we argue there is much to be gained by starting from a common agenda, which could enable a coordinated approach and maximise the potential for cross-country comparative work. Such a body of research will enable lessons to be drawn for (i) managing the current crisis; (ii) ensuring resilience of health systems to future shocks; and (iii) enhancing medium-term progress towards UHC

    A Simplified Spatial Methodology for Assessing Land Productivity Status in Africa

    The degradation of soil, vegetation and socio-economic transformations are a huge threat to Africa’s land production. This study aimed to (i) assess the soil and land productivity of standing biomass and (ii) determine the effect of rainfall on the standing biomass in Eastern Africa. Soil productivity was determined using the Soil Productivity Index (SPI) and a simplified model was developed to estimate the Net Primary Productivity (NPP). The SPI indicators used included soil-organic matter, texture, soil moisture, base-saturation, pH, cation-exchange-capacity, soil-depth and drainage. The inputs of the simplified model are: MODIS Soil Adjusted Vegetation Index (SAVI), soil erosion, soil nutrient content and input, rainfall, land-use/cover and agro-ecological zones. The findings reveal that the countries with the most productive soils are Mauritius, Rwanda and South Sudan—while, for standing biomass, the countries with the highest spatial extent are Mauritius (97%), Rwanda (96%), Uganda (95%), South Sudan (89%), Ethiopia (47%) and Kenya (36%). Standing biomass is dominant in biomes such as natural forests, woodlands, croplands, grasslands, wetlands and tree-plantations. High land productivity was attributed to soil quality and management, land policy reforms, favourable climatic conditions and sustainable land husbandry activities. Rainfall was significantly correlated with standing biomass in most of the studied countries (p < 0.05) except Djibouti and Rwanda. Therefore, monitoring soil health, use and land reforms are key to sustaining vegetative biomass

    Seroprevalence of anti-SARS-CoV-2 IgG antibodies in Kenyan blood donors.

    The spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in Africa is poorly described. The first case of SARS-CoV-2 in Kenya was reported on 12 March 2020, and an overwhelming number of cases and deaths were expected, but by 31 July 2020, there were only 20,636 cases and 341 deaths. However, the extent of SARS-CoV-2 exposure in the community remains unknown. We determined the prevalence of anti-SARS-CoV-2 immunoglobulin G among blood donors in Kenya in April-June 2020. Crude seroprevalence was 5.6% (174 of 3098). Population-weighted, test-performance-adjusted national seroprevalence was 4.3% (95% confidence interval, 2.9 to 5.8%) and was highest in urban counties Mombasa (8.0%), Nairobi (7.3%), and Kisumu (5.5%). SARS-CoV-2 exposure is more extensive than indicated by case-based surveillance, and these results will help guide the pandemic response in Kenya and across Africa
