    Never too old to learn - Parenting interventions for grandparents–A systematic review

    Grandparents may often be involved in childcare and in some settings, may be a primary caregiver. It may be assumed that as grandparents have raised their own children, their capabilities in terms of child care have been achieved through such experience. However, experience does not necessarily equate to expertise. Parenting interventions have beneficial effects for both parent and child outcomes. If such skills can be taught to parents, they may also be of benefit to grandparents. This systematic review explores the evidence base on parenting interventions for grandparents. The aim is to examine the implications of parenting interventions on outcomes for the grandparents, the parents and the child. A keyword data base search generated 191 manuscripts and eight studies met the criteria for inclusion. Overall, this review identifies relatively weak literature on interventions for grandparenting skills. Studies identified predominantly originated from the USA, and focused on the outcomes for the grandparents with sparse information on implications for the child or parent. However, despite such limitations, the studies all seem to report benefits of parenting interventions for grandparents. The interventions identified are varied and range from short to longer courses as well as support groups and home visits. As such, conclusions are limited. However, an enticing range of interventions with a growing evidence base of efficacy is seemingly emerging, laying the foundations for future studies honing the parenting provision for grandparents

    Area-level poverty and preterm birth risk: A population-based multilevel analysis

    <p>Abstract</p> <p>Background</p> <p>Preterm birth is a complex disease with etiologic influences from a variety of social, environmental, hormonal, genetic, and other factors. The purpose of this study was to utilize a large population-based birth registry to estimate the independent effect of county-level poverty on preterm birth risk. To accomplish this, we used a multilevel logistic regression approach to account for multiple co-existent individual-level variables and county-level poverty rate.</p> <p>Methods</p> <p>Population-based study utilizing Missouri's birth certificate database (1989–1997). We conducted a multilevel logistic regression analysis to estimate the effect of county-level poverty on PTB risk. Of 634,994 births nested within 115 counties in Missouri, two levels were considered. Individual-level variables included demographics factors, prenatal care, health-related behavioral risk factors, and medical risk factors. The area-level variable included the percentage of the population within each county living below the poverty line (US census data, 1990). Counties were divided into quartiles of poverty; the first quartile (lowest rate of poverty) was the reference group.</p> <p>Results</p> <p>PTB < 35 weeks occurred in 24,490 pregnancies (3.9%). The rate of PTB < 35 weeks was 2.8% in counties within the lowest quartile of poverty and increased through the 4<sup>th </sup>quartile (4.9%), p < 0.0001. High county-level poverty was significantly associated with PTB risk. PTB risk (< 35 weeks) was increased for women who resided in counties within the highest quartile of poverty, adjusted odds ratio (<sub>adj</sub>OR) 1.18 (95% CI 1.03, 1.35), with a similar effect at earlier gestational ages (< 32 weeks), <sub>adj</sub>OR 1.27 (95% CI 1.06, 1.52).</p> <p>Conclusion</p> <p>Women residing in socioeconomically deprived areas are at increased risk of preterm birth, above other underlying risk factors. Although the risk increase is modest, it affects a large number of pregnancies.</p

    Secularism, Racism and the Politics of Belonging

    This collection of papers is a reflection of an ongoing debate about the relationships between religion, the citizen and the state. It is a debate that is far from settled, and indeed one which may be unsettling, but a debate which too often generates more heat than light. It is, however, a crucial discussion since it goes to the heart of our understanding of modern citizenship, the role of the state and the struggle for equality. Runnymede was especially pleased to partner with colleagues at the Centre for Refugees, Migration and Belonging (CRMB) at the University of East London in co-hosting the two conferences from which these papers are drawn. CRMB’s serious and engaged approach to relating political theory to political action enabled academics, commentators and practitioners to share a platform where disagreements were aired and constructive debate and discussion enabled. We hope that the conferences act as a model for the deliberations which are necessary to address the thorny challenges raised by the authors. Many of these disagreements are reflected in the papers presented here. The collected papers discuss faith-based schooling, the veil, honour based violence, religious arbitration, and the delivery of public services by faith communities. All of these issues remain very much alive in contemporary public policy debates in the UK and beyond

    Chemical composition and antibacterial activity of essential oils from the medicinal plant Mentha cervina L. grown in Portugal

    Mentha cervina is a medicinal plant traditionally used in Portugal in folk medicine, in different gastric disorders and inflammations of the respiratory tract. In order to validate those traditional uses, M. cervina essential oils (EOs) were characterized by GC and GC–MS and their antimicrobial activity was tested against 23 bacterial strains (including multiresistant strains). The EOs were dominated by the monoterpenes pulegone (52–75%), isomenthone (8–24%), limonene (4–6%), and menthone (1–2%). The antibacterial activity of these EOs was compared to that of the main components standards. The most effective antibacterial activity was expressed by the EOs against the Gram-negative bacteria, Escherichia coli and Acinetobacter baumanni, with MIC values of 1 mg/ml. The EOs complex mixtures were more active than the individual aromatic components supporting the hypothesis that the EOs antibacterial activity is a function of the synergistic effect of their different aromatic components. These results show the potential role of M. cervina EOs as antibacterial agents and validate the traditional use of this plant

    Going to sleep in the supine position is a modifiable risk factor for late pregnancy stillbirth; findings from the New Zealand multicentre stillbirth case-control study

    Objective: Our objective was to test the primary hypothesis that maternal non-left, in particular supine going-to-sleep position, would be a risk factor for late stillbirth (≥28 weeks of gestation). Methods: A multicentre case-control study was conducted in seven New Zealand health regions, between February 2012 and December 2015. Cases (n=164) were women with singleton pregnancies and late stillbirth, without congenital abnormality. Controls (n=569) were women with on-going singleton pregnancies, randomly selected and frequency matched for health region and gestation. The primary outcome was adjusted odds of late stillbirth associated with self-reported going-to-sleep position, on the last night. The last night was the night before the late stillbirth was thought to have occurred or the night before interview for controls. Going to- sleep position on the last night was categorised as: supine, left-side, right-side, propped or restless. Multivariable logistic regression adjusted for known confounders. Results: Supine going-to-sleep position on the last night was associated with increased late stillbirth risk (adjusted odds ratios (aOR) 3.67, 95% confidence interval (CI) 1.74 to 7.78) with a population attributable risk of 9.4%. Other independent risk factors for late stillbirth (aOR, 95% CI) were: BMI (1.04, 1.01 to 1.08) per unit, maternal age ≥40 (2.88, 1.31 to 6.32), birthweight <10th customised centile (2.76, 1.59 to 4.80), and <6 hours sleep on the last night (1.81, 1.14 to 2.88). The risk associated with supine-going-to sleep position was greater for term (aOR 10.26, 3.00 to 35.04) than preterm stillbirths (aOR 3.12, 0.97 to 10.05). Conclusions: Supine going-to-sleep position is associated with a 3.7 fold increase in overall late stillbirth risk, independent of other common risk factors. A public health campaign encouraging women not to go-to-sleep supine in the third trimester has potential to reduce late stillbirth by approximately 9%

    Missing Data in Sea Turtle Population Monitoring: A Bayesian Statistical Framework Accounting for Incomplete Sampling

In the case of sea turtles, their populations are difficult to assess because of their complicated life histories. Ground-derived clutch counts are most often used as an index of population size for sea turtles; however, data are often incomplete with varying sampling intensity within and among sites and seasons. To address these issues, we: (1) develop a Bayesian statistical modelling framework that can be used to account for sampling uncertainties in a robust probabilistic manner within a given site and season; and (2) apply this to a previously unpublished long-term sea turtle dataset (n = 17 years) collated for the Republic of the Congo, which hosts two sympatrically nesting species of sea turtle (leatherback turtle [Dermochelys coriacea] and olive ridley turtle [Lepidochelys olivacea]). The results of this analysis suggest that leatherback turtle nesting levels dropped initially and then settled into quasi-cyclical levels of interannual variability, with an average of 573 (mean, 95% prediction interval: 554–626) clutches laid annually between 2012 and 2017. In contrast, nesting abundance for olive ridley turtles has increased more recently, with an average of 1,087 (mean, 95% prediction interval: 1,057–1,153) clutches laid annually between 2012 and 2017. These findings highlight the regional and global importance of this rookery with the Republic of the Congo, hosting the second largest documented populations of olive ridley and the third largest for leatherback turtles in Central Africa; and the fourth largest non-arribada olive ridley rookery globally. Furthermore, whilst the results show that Congo’s single marine and coastal national park provides protection for over half of sea turtle clutches laid in the country, there is scope for further protection along the coast.     In vitro antibacterial activity of some plant essential oils

    BACKGROUND: To evaluate the antibacterial activity of 21 plant essential oils against six bacterial species. METHODS: The selected essential oils were screened against four gram-negative bacteria (Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Proteus vulgaris) and two gram-positive bacteria Bacillus subtilis and Staphylococcus aureus at four different concentrations (1:1, 1:5, 1:10 and 1:20) using disc diffusion method. The MIC of the active essential oils were tested using two fold agar dilution method at concentrations ranging from 0.2 to 25.6 mg/ml. RESULTS: Out of 21 essential oils tested, 19 oils showed antibacterial activity against one or more strains. Cinnamon, clove, geranium, lemon, lime, orange and rosemary oils exhibited significant inhibitory effect. Cinnamon oil showed promising inhibitory activity even at low concentration, whereas aniseed, eucalyptus and camphor oils were least active against the tested bacteria. In general, B. subtilis was the most susceptible. On the other hand, K. pneumoniae exhibited low degree of sensitivity. CONCLUSION: Majority of the oils showed antibacterial activity against the tested strains. However Cinnamon, clove and lime oils were found to be inhibiting both gram-positive and gram-negative bacteria. Cinnamon oil can be a good source of antibacterial agents

    Maternal care and birth outcomes among ethnic minority women in Finland

    <p>Abstract</p> <p>Background</p> <p>Care during pregnancy and labour is of great importance in every culture. Studies show that people of migrant origin have barriers to obtaining accessible and good quality care compared to people in the host society. The aim of this study is to compare the access to and use of maternity services, and their outcomes among ethnic minority women having a singleton birth in Finland.</p> <p>Methods</p> <p>The study is based on data from the Finnish Medical Birth Register in 1999–2001 linked with the information of Statistics Finland on woman's country of birth, citizenship and mother tongue. Our study data included 6,532 women of foreign origin (3.9% of all singletons) giving singleton birth in Finland during 1999–2001 (compared to 158,469 Finnish origin singletons).</p> <p>Results</p> <p>Most women have migrated during the last fifteen years, mainly from Russia, Baltic countries, Somalia and East Europe. Migrant origin women participated substantially in prenatal care. Interventions performed or needed during pregnancy and childbirth varied between ethnic groups. Women of African and Somali origin had most health problems resulted in the highest perinatal mortality rates. Women from East Europe, the Middle East, North Africa and Somalia had a significant risk of low birth weight and small for gestational age newborns. Most premature newborns were found among women from the Middle East, North Africa and South Asia. Primiparous women from Africa, Somalia and Latin America and Caribbean had most caesarean sections while newborns of Latin American origin had more interventions after birth.</p> <p>Conclusion</p> <p>Despite good general coverage of maternal care among migrant origin women, there were clear variations in the type of treatment given to them or needed by them. African origin women had the most health problems during pregnancy and childbirth and the worst perinatal outcomes indicating the urgent need of targeted preventive and special care. These study results do not confirm either healthy migrant effect or epidemiological paradox according to which migrant origin women have considerable good birth outcomes.</p
