    Isoflurane Preconditioning at Clinically Relevant Doses Induce Protective Effects of Heme Oxygenase-1 on Hepatic Ischemia Reperfusion in Rats

    <p>Abstract</p> <p>Background</p> <p>Activation of heme oxygenase-1 (HO-1) has been proved to reduce damages to the liver in ischemia reperfusion injury. The objective of present study was to determine whether clinic relevant doses of isoflurane treatment could be sufficient to activate HO-1 inducing, which confers protective effect against hepatic ischemia-reperfusion injury.</p> <p>Methods</p> <p>The hepatic artery and portal vein to the left and the median liver lobes of forty male Sprague-Dawley rats were occluded for 60 minutes. Reperfusion was allowed for 4 hours before the animal subjects were sacrificed. Six groups (n = 12) were included in the study. A negative control group received sham operation and positive control group a standard ischemia-reperfusion regimen. The third group was pretreated with isoflurane prior to the ischemia-reperfusion. The fourth group received an HO-1 inhibitor zinc protoporphyrin (Znpp) prior to the isoflurane pretreatment and the ischemia-reperfusion. The fifth group received Znpp alone before ischemia-reperfusion procedure, and the sixth group was administrated with a HO-1 inducer hemin prior to IR. HO-1 in the liver was measured using an enzymatic activity assay, a Western blot analysis, as well as immunohistochemical method. Extent of liver damage was estimated by determination of the serum transaminases, liver lipid peroxidation and hepatic histology. Infiltration of the liver by neutrophils was measured using a myeloperoxidase activity assay. TNFα mRNA in the liver was measured using RT-PCR.</p> <p>Results</p> <p>Isoflurane pretreatment significantly attenuated the hepatic injuries and inflammatory responses caused by the ischemia reperfusion. Selectively inhibiting HO-1 with ZnPP completed blocked the protective effects of isoflurane. Inducing HO-1 with hemin alone produced protective effects similar in magnitude to that of isoflurane.</p> <p>Conclusions</p> <p>Clinic relevant doses of isoflurane attenuate ischemia reperfusion injury in rats by increasing the HO-1 expression and activity.</p

    Effect of remote ischemic conditioning on atrial fibrillation and outcome after coronary artery bypass grafting (RICO-trial)

    Background: Pre- and postconditioning describe mechanisms whereby short ischemic periods protect an organ against a longer period of ischemia. Interestingly, short ischemic periods of a limb, in itself harmless, may increase the ischemia tolerance of remote organs, e.g. the heart (remote conditioning, RC). Although several studies have shown reduced biomarker release by RC, a reduction of complications and improvement of patient outcome still has to be demonstrated. Atrial fibrillation (AF) is one of the most common complications after coronary artery bypass graft surgery (CABG), affecting 27-46% of patients. It is associated with increased mortality, adverse cardiovascular events, and prolonged in-hospital stay. We hypothesize that remote ischemic pre- and/or post-conditioning reduce the incidence of AF following CABG, and improve patient outcome.Methods/design: This study is a randomized, controlled, patient and investigator blinded multicenter trial.     BACKGROUND Limited information exists about the epidemiology and outcome of surgical patients at increased risk of postoperative pulmonary complications (PPCs), and how intraoperative ventilation was managed in these patients. OBJECTIVES To determine the incidence of surgical patients at increased risk of PPCs, and to compare the intraoperative ventilation management and postoperative outcomes with patients at low risk of PPCs. DESIGN This was a prospective international 1-week observational study using the ‘Assess Respiratory Risk in Surgical Patients in Catalonia risk score’ (ARISCAT score) for PPC for risk stratification. PATIENTS AND SETTING Adult patients requiring intraoperative ventilation during general anaesthesia for surgery in 146 hospitals across 29 countries. MAIN OUTCOME MEASURES The primary outcome was the incidence of patients at increased risk of PPCs based on the ARISCAT score. Secondary outcomes included intraoperative ventilatory management and clinical outcomes. RESULTS A total of 9864 patients fulfilled the inclusion criteria. The incidence of patients at increased risk was 28.4%. The most frequently chosen tidal volume (VT) size was 500 ml, or 7 to 9 ml kg1 predicted body weight, slightly lower in patients at increased risk of PPCs. Levels of positive end-expiratory pressure (PEEP) were slightly higher in patients at increased risk of PPCs, with 14.3% receiving more than 5 cmH2O PEEP compared with 7.6% in patients at low risk of PPCs (P < 0.001). Patients with a predicted preoperative increased risk of PPCs developed PPCs more frequently: 19 versus 7%, relative risk (RR) 3.16 (95% confidence interval 2.76 to 3.61), P < 0.001) and had longer hospital stays. The only ventilatory factor associated with the occurrence of PPCs was the peak pressure. CONCLUSION The incidence of patients with a predicted increased risk of PPCs is high. A large proportion of patients receive high VT and low PEEP levels. PPCs occur frequently in patients at increased risk, with worse clinical outcome

    Persepsi Siswa Terhadap Atribut Layanan SMK Wisata Indonesia

    Kesuksesan sebuah organisasi, baik organisasi yang berorientasi laba maupun organisasi nir laba, seperti institusi sekolah, sangat tergantung kepada bagaimana organisasi tersebut melayani para pelanggannya sesuai dengan harapan mereka agar mereka tetap bertahan (loyal) pada organisasi tersebut. Ada beberapa alasan betapa pentingnya mempertahankan pelanggan. Pertama, biaya untuk mendapatkan pelanggan baru lebih besar dibandingkan dengan biaya untuk mempertahankan pelanggan lama. Kedua, seorang pelanggan yang puas akan suatu layanan yang ia terima akan menceritakan tentang kepuasannya tersebut kepada satu orang, sementara seorang pelanggan yang tidak puas akan suatu layanan yang ia terima akan menceritakan ketidakpuasannya tersebut kepada banyak orang. Ketidakpuasan pelanggan dapat mengakibatkan efek bola salju bagi sebuah organisasi karena semakin banyak orang mengetahui akan kebobrokan layanan organisasi yang bersangkutan. Ketidakpuasan pelanggan inilah yang paling Sering menyebabkan sebuah organisasi/atau perusahaan ditinggalkan oleh pelanggannya, tak jarang banyak yang menjadi bangkrut. Maka tidaklah berlebihan jika ada istilah yang mengatakan pelanggan adalah raja. SMK Wisata Indonesia sebagai sebuah organisasi yang bergerak dalam bidang jasa pendidikan kedepannya menghadap dua masalah utama, yakni: Pertama, persaingan di antara sekolah sejenis. Kedua, masalah kepuasan pelanggan. Penelitian ini dilakukan untuk menganalisis bagaimana kinelja atribut-atribut layanan yang diberikan SMK Wisata Indonesia dalam rangka memenuhi kebutuhan dan keinginan pelanggannya. Metode penelitian yang dipergunakan adalah metode penelitian deskriptif yang mengulas tentang deskripsi permasalahan seputar kinelja atribut layanan yang diberikan oleh manajemen SMK Wisata Indonesia kepada pelanggannya. Data primer untuk persepsi siswa didapatkan dengan menyebarkan kuesioner kepada 92 siswa kelas 3 sebagai sampel penerima jasa layanan. Atribut-atribut dalam kuesioner tentang persepsi siswa berdasarkan dimensi Servqual, yaitu reliability, responsiveness, assurance, empathy, dan tangibles. Kemudian faktor-faktor atribut tersebut dianalisis pengaruhnya terhadap tingkat kepuasan pelanggan berdasarkan analisis tingkat kepentingan dan kinelja. Pada kesimpulannya, dengan menggunakan Important Performance Analysis (IPA) diagram, diperoleh posisi atribut-atribut yang mempengaruhi kepuasan pelanggan sebagai berikut: 7 (tujuh) atribut menempati kuadran A sebagai prioritas utama untuk diperbaiki kinerjanya, 9 atribut menempati kuadran B untuk dipertahankan kinerjanya, 8 atribut pada kuadran C yang dianggap kurang penting oleh pelanggan dan kinerja pelayanannya biasa saja, serta 8 atribut pada kuadran D yang dianggap sudah baik sehingga memuaskan pelanggan

    BACKGROUND: The liver plays a central role in the whole organism's response to injury. Expression of hepatic acute-phase and heat-shock genes likely contributes to the restoration of homeostasis after stressful events. However, after prolonged ischemia, hepatic transcription of heat-shock genes can exclude the simultaneous transcription of acute-phase genes. The issue of whether hepatic 72-kd heat-shock protein (hsp72) gene expression is induced under perioperative conditions that do not result in prolonged liver ischemia and whether this might further affect the expression of the acute-phase reactant inter-alpha-trypsin inhibitor (alpha-Ti) was examined. METHODS: Pigs were anesthetized with sodium pentobarbital and ketamine hydrochloride, tracheally intubated, and their lungs ventilated. After celiotomy, a hepatic biopsy sample was obtained. Arterial blood pressure, cardiac output, and total hepatic blood flow were measured. Subsequent biopsies were obtained at 1, 2, 3, 4, and 6 h after the initial biopsy. Arterial norepinephrine concentrations were measured using high-pressure liquid chromatography. Nuclear runoff (run on) analysis and Northern blotting were applied to estimate changes in hsp72 and alpha-Ti gene transcription rates and RNA levels. Western blotting was used to estimate changes in hsp72 levels. RESULTS: Hemodynamic parameters did not change significantly over time. Arterial norepinephrine concentrations were increased at all time points. Hepatic hsp72 RNA levels increased up to sixfold while nuclear runoff assays did not detect significant changes in hsp72 gene transcription rates. The increases in hsp72 RNA levels correlated with accumulation of hsp72 (up to sevenfold). Increases in alpha-Ti transcription rates up to 42-fold were associated with respective increases in alpha-Ti RNA levels (up to 17-fold). CONCLUSIONS: These data demonstrate that hepatic expression of hsp72 is not confined to conditions that lead to prolonged liver ischemia but is also part of the response of the liver to surgery under general anesthesia.
