    Investigation génétique de NAFLD dans le diabète de type 2 via construction d’un modèle de prédiction de la maladie et par criblage du locus PNPLA3-SAMM50

    La stéatose hépatique non-alcoolique (NAFLD) est une altération hépatique fréquente dans le diabète de type 2 (DT2) et est associée à diverses complications telles que la mortalité. L’établissement d’outils de prédiction non-invasifs de NAFLD est primordial. Mon projet de maîtrise avait pour objectif d’établir des marqueurs génétiques de NAFLD dans le DT2 via deux stratégies : 1) une sélection non-ciblée des marqueurs génétiques (SNPs) via la méthode LASSO et 2) une sélection ciblée de SNPs rapportés comme liés à la maladie ou à des altérations associées. Une population de 4098 patients avec DT2 d’origine caucasienne (ADVANCE) a été utilisée. Des données statistiques sommaires d’études pangénomiques ont été exploitées pour sélectionner, via LASSO, les marqueurs génétiques (SNPs) à inclure dans le score de risque polygénique (PRS). J’ai également développé un modèle de 3210 SNPs ajusté par des covariables capable de prédire les taux élevés de ALT (AUC=0,69) et la mortalité non-cardiovasculaire (AUC=0,66). Le criblage du locus candidat PNPLA3-SAMM50 a mis en avant une diversité des associations génétiques aux différentes altérations métaboliques comme les taux de ALT (substitut du diagnostic de NAFLD) (rs2294915, P = 1,83x10-7), à la mortalité non-cardiovasculaire (rs2294917, P = 3,9x10-4) et à l’efficacité de la thérapie intensive antidiabétique chez certains patients de la population (porteurs GG de rs16991236, P=0,007). Mes travaux ont permis de mieux comprendre le fond génétique de NAFLD dans le DT2 et laissent envisager l’établissement d’outils de diagnostic et de suivi de la maladie plus adéquats.Non-alcoholic fatty liver disease (NAFLD) is a liver disorder more frequent in type 2 diabetes (T2D) and is associated with complications such as mortality. For this reason, establishing non-invasive tools for predicting NAFLD is crucial. My master’s project aimed to establish genetic markers for NAFLD in T2D using two strategies: 1) a non-targeted selection of genetic markers (SNPs) by the LASSO method and 2) a targeted selection of SNPs reported as associated with the disease or its related abnormalities. A population involving 4098 patients with T2D and Caucasian ancestry was used. Summary statistics data of pangenomic studies were exploited for the selection of SNPs to be involved in the polygenic risk score (PRS). I also designed a model of 3210 SNPs adjusted by covariates and able to predict the high rates of ALT (AUC=0.69) and non-cardiovascular death (AUC=0.66). Mapping of the candidate locus PNPLA3-SAMM50 allowed the observation of diversity in terms of genetic association with the metabolic abnormalities such as ALT (surrogate of NAFLD) (rs2294915, P = 1.83x10-7), non-cardiovascular death (rs2294917, P = 3.9x10-4) and the efficiency of the intensive antidiabetic therapy within a subgroup in the population (individuals with GG of rs16991236, P = 0.007). My studies allowed for a better understanding of the genetic background of NAFLD in T2D and open perspectives for establishing more adequate tools for diagnosis and follow-up of the disease

    Introduction: Insulin gene VNTR was associated with polycystic ovary syndrome (PCOS) in some studies but not in others. This couldb be due to the heterogeneity of the definition of PCOS and/or the use of inappropriate gene mapping strategies.Material and methods: In this investigation, the association of VNTR with PCOS was explored in a population of women from Central Europe (377 cases and 105 controls) in whom PCOS was diagnosed according to Rotterdam criteria. Seven SNPs: rs3842756 (G/A), rs3842755 (G/T), rs3842754 (C/T), rs3842753 (A/C), rs3842752 (C/T), rs3842748 (G/C), and rs689 (T/A) were genotyped in a portion of the population (160 cases and 95 controls) by sequencing or by SSO-PCR. Analysis of linkage disequilibrium (LD) pattern allowed selecting three tagSNPs (rs3842754, rs3842748, and rs689), which were genotyped in the rest of the population by KASPar.Results: Six haplotypes were reconstructed, among which three (h1, h2 and h6) were more frequent. Statistical analysis allowed observation of the association of the SNP rs3842748, through its GC genotype, with obesity in PCOS (P = 0.049; OR CI95% 1,59 [1.00–2.51]) and in classical PCOS (YPCOS) (P = 0.010), as well as the correlation of the SNP rs689 and the pair of haplotypes h1/h1 with higher levels of testosteronaemia in the PCOS group, although this was at the limit of significance (P = 0.054)Conclusion: These results are in accordance with some studies in literature and highlight the role of insulin gene VNTR in complex metabolic disorders. Mogło tak być z powodu heterogeniczności definicji PCOS i/lub stosowania nieprawidłowych strategii mapowania genów.Materiał i metody: W niniejszym badaniu, związek VNTR z PCOS zbadano w populacji kobiet pochodzących z Europy Środkowej (377 przypadków chorobowych oraz 105 osób kontrolnych), u których zdiagnozowano PCOS według kryteriów rotterdamskich. Siedem polimorfizmów pojedynczego nukleotydu (SNP): rs3842756 (G/A), rs3842755 (G/T), rs3842754 (C/T), rs3842753 (A/C), rs3842752 (C/T), rs3842748 (G/C), oraz rs689 (T/A) wytypowano w części populacji (160 przypadków chorobowych i 95 osób kontrolnych) poprzez sekwencjonowanie lub SSO-PCR. Analiza wzoru niezrównoważenia sprzężeń (LD) pozwoliła na selekcję trzech SNP znacznikowych (tagSNP) (rs3842754, rs3842748 i rs689), które wyselekcjonowano w pozostałej części populacji metodą KASPar.Wyniki: Sześć haplotypów odtworzono, z których 3 (h1, h2 i h6) występowały częściej. Analiza statystyczna pozwoliła na obserwację związku SNP rs3842748, poprzez genotyp GC, z otyłością w PCOS (P = 0,049; OR CI 95% 1,59 [1,00–2,51]) i klasycznym PCOS (YPCOS) (P = 0,010), jak również korelacji SNP rs689 i pary haplotypów h1/h1 z wyższym stężeniem testosteronemii w grupie PCOS, chociaż wynik ten znajdował się na granicy istotności (P = 0,054).Wnioski: Powyższe wyniki są zgodne z niektórymi badaniami w piśmiennictwie i podkreślają role VNTR genu insuliny w złożonych zaburzeniach metabolicznych.     Impact of FTO genotypes on BMI and weight in polycystic ovary syndrome : a systematic review and meta-analysis

    Aims/hypothesis FTO gene single nucleotide polymorphisms (SNPs) have been shown to be associated with obesity-related traits and type 2 diabetes. Several small studies have suggested a greater than expected effect of the FTO rs9939609 SNP on weight in polycystic ovary syndrome (PCOS). We therefore aimed to examine the impact of FTO genotype on BMI and weight in PCOS. Methods A systematic search of medical databases (PubMed, EMBASE and Cochrane CENTRAL) was conducted up to the end of April 2011. Seven studies describing eight distinct PCOS cohorts were retrieved; seven were genotyped for SNP rs9939609 and one for SNP rs1421085. The per allele effect on BMI and body weight increase was calculated and subjected to meta-analysis. Results A total of 2,548 women with PCOS were included in the study; 762 were TT homozygotes, 1,253 had an AT/CT genotype, and 533 were AA/CC homozygotes. Each additional copy of the effect allele (A/C) increased the BMI by a mean of 0.19 z score units (95% CI 0.13, 0.24; p = 2.26 × 10−11) and body weight by a mean of 0.20 z score units (95% CI 0.14, 0.26; p = 1.02 × 10−10). This translated into an approximately 3.3 kg/m2 increase in BMI and an approximately 9.6 kg gain in body weight between TT and AA/CC homozygotes. The association between FTO genotypes and BMI was stronger in the cohorts with PCOS than in the general female populations from large genome-wide association studies. Deviation from an additive genetic model was observed in heavier populations. Conclusions/interpretation The effect of FTO SNPs on obesity-related traits in PCOS seems to be more than two times greater than the effect found in large population-based studies. This suggests an interaction between FTO and the metabolic context or polygenic background of PCOS

    Diversity of Y-chromosomal and mtDNA Markers Included in Mediscope Chip within Two Albanian Subpopulations from Croatia and Kosovo: Preliminary Data

    The aim of this preliminary study is to analyze genetic specificity of Kosovo Albanians comparing with neighboring populations using new genetic tool - MEDISCOPE gene chip, to investigate the feasibility of this approach. We collected 37 DNA samples (9 Croats, 17 Albanians from Croatia and 11 Albanians from Kosovo) from unrelated males born in Croatia and Kosovo. Additionally, samples were expanded with female individuals and mtDNA analysis included a total of 61 samples (15 Croats, 23 Albanians from Croatia and 23 Albanians from Kosovo). This pilot study suggests that the usage of the MEDISCOPE chip could be recognized as an efficient tool within recognition of the population genetic specificity even within extremely small sample size

    Large effects on body mass index and insulin resistance of fat mass and obesity associated gene (FTO) variants in patients with polycystic ovary syndrome (PCOS)

    BACKGROUND: The polycystic ovary syndrome (PCOS), a common endocrine disorder in women of child-bearing age, mainly characterised by chronic anovulation and hyperandrogenism, is often associated with insulin resistance (IR) and obesity. Its etiology and the role of IR and obesity in PCOS are not fully understood. We examined the influence of validated genetic variants conferring susceptibility to obesity and/or type 2 diabetes mellitus (T2DM) on metabolic and PCOS-specific traits in patients with PCOS. METHODS: We conducted an association study in 386 patients with PCOS (defined by the Rotterdam-criteria) using single nucleotide polymorphisms (SNPs) in or in proximity to the fat mass and obesity associated gene (FTO), insulin-induced gene-2 (INSIG2), transcription factor 7-like 2 gene (TCF7L2) and melanocortin 4 receptor gene (MC4R). To compare the effect of FTO obesity risk alleles on BMI in patients with PCOS to unselected females of the same age range we genotyped 1,971 females from the population-based KORA-S4 study (Kooperative Gesundheitsforschung im Raum Augsburg, Survey 4). RESULTS: The FTO risk allele was associated with IR traits and measures of increased body weight. In addition, the TCF7L2 SNP was associated with body weight traits. For the SNPs in the vicinity of INSIG2 and MC4R and for the other examined phenotypes there was no evidence for an association. In PCOS the observed per risk allele effect of FTO intron 1 SNP rs9939609 on BMI was +1.56 kg/m2, whereas it was +0.46 kg/m2 in females of the same age range from the general population as shown previously. CONCLUSION: The stronger effect on body weight of the FTO SNP in PCOS might well have implications for the etiology of the disease

    FTO gene polymorphisms and obesity risk: a meta-analysis

    <p>Abstract</p> <p>Background</p> <p>The pathogenesis of obesity is reportedly related to variations in the fat mass and an obesity-associated gene (<it>FTO</it>); however, as the number of reports increases, particularly with respect to varying ethnicities, there is a need to determine more precisely the effect sizes in each ethnic group. In addition, some reports have claimed ethnic-specific associations with alternative SNPs, and to that end there has been a degree of confusion.</p> <p>Methods</p> <p>We searched PubMed, MEDLINE, Web of Science, EMBASE, and BIOSIS Preview to identify studies investigating the associations between the five polymorphisms and obesity risk. Individual study odds ratios (OR) and their 95% confidence intervals (CI) were estimated using per-allele comparison. Summary ORs were estimated using a random effects model.</p> <p>Results</p> <p>We identified 59 eligible case-control studies in 27 articles, investigating 41,734 obesity cases and 69,837 healthy controls. Significant associations were detected between obesity risk and the five polymorphisms: rs9939609 (OR: 1.31, 95% CI: 1.26 to 1.36), rs1421085 (OR: 1.43, 95% CI: 1.33 to 1.53), rs8050136 (OR: 1.25, 95% CI: 1.13 to 1.38), rs17817449 (OR: 1.54, 95% CI: 1.41 to 1.68), and rs1121980 (OR: 1.34, 95% CI: 1.10 to 1.62). Begg's and Egger's tests provided no evidence of publication bias for the polymorphisms except rs1121980. There is evidence of higher heterogeneity, with <it>I</it><sup>2 </sup>test values ranging from 38.1% to 84.5%.</p> <p>Conclusions</p> <p>This meta-analysis suggests that <it>FTO </it>may represent a low-penetrance susceptible gene for obesity risk. Individual studies with large sample size are needed to further evaluate the associations between the polymorphisms and obesity risk in various ethnic populations.</p

    BACKGROUNDThe Fifth Evian Annual Reproduction (EVAR) Workshop Meeting discussed knowledge regarding contemporary genetics in female reproduction.METHODSSpecialist reproductive medicine clinicians and geneticists delivered presentations based on published literature and current research. The content of this report is based on the expert presentations and subsequent group discussions that took place during this Workshop.RESULTSNumerous ovarian genes with a role in infertility have been identified. Future challenges for genetic screening of patients, such as those with polycystic ovary syndrome, primary ovarian insufficiency or endometriosis, include the identification of high-throughput strategies and how to apply these findings to infertile patients. The identification of high-quality embryos in IVF using objective technologies remains a high priority in order to facilitate single-embryo transfer. Gene expression profiling of cumulus cells surrounding the oocyte, and proteomic and metabolomic approaches in embryo culture media may significantly improve non-invasive embryo quality assessment.CONCLUSIONSThe way forward in advancing the knowledge of genes involved in reproduction was considered to be through genome-wide association studies involving large numbers of patients. Establishing international collaboration is required to enable the application of such technologies in sufficient numbers of patients

    Study of the molecular mechanisms of malignant melanoma differential resistance to vinca alkaloids

    Le mélanome malin (MM) est un cancer très réfractaire aux thérapies anticancéreuses, dont les vincalcaloïdes (VAs). Afin d'étudier le rôle de la GSTM1 (glutathion S-transférase 1) et la MRP1 (multidrug resistance protein 1) dans la résistance acquise du MM aux VAs, nous avons établi 4 modèles cellulaires de résistance à la vincristine (CAL1R-VCR), à la vindésine (CAL1R-VDS), à la vinorelbine (CAL1R-VRB) et à la vinflunine (CAL1R-VFL), par exposition continue de cellules du MM (CAL1-wt), pendant un an, à ces anticancéreux. L'expression d'ne GSTM1 fonctionnelle est spécifiquement observée (RT-PCR, western blot, activité GST totale) dans les cellules résistantes. Le curcumin (inhibiteur de GSTM1), la BSO (inhibiteur de synthèse de glutathion) et le MK571 (inhibiteur de MRP1), réduisent considérablement le résistance acquise à la VCR et à la VDS mais pas à la VRB ou à la VFL. Toutefois, tous ces VAs réduisent spécifiquement l'activité GSTM1. Ces données montrent l'implication différentielle de GSTM1 et MRP1 dans la résistance aux VAs. Pour déterminer les mécanismes moléculaires de cette chimiorésistance, nous avons réalisé une étude pangénomique (biopuces Affymetrix HG-U133 Plus 2.00) sur les lignées CAL1 (wt et R). Le regroupement hiérarchique (par Cluster et TreeView) des données des puces a révélé une similarité entre les profils d'expression génique de CAL1R-VRB et CAL1-wt mais aussi entre ceux de CAL1R-VCR et CAL1R-VDS. L'analyse bioinformatique (par IPA) des transcrits les plus différemment exprimés entre les lignées cellulaires, a mis en évidence 6 réseaux géniques connus pour leur rôle dans la chimiorésistance tumorale. Le programme FatiGO a révélé 3 termes biologiques sur-représentés (> 60%) dans CAL1R (ribosome, filaments intermédiaires du cytosquelette, récepteurs olfactifs) tandis que l'étude fonctionnelle (invalidation génique par siRNA, test de viabilité) de GPR143, KIT et SLC45A2 (gènes interagissant avec NF-κB et CCND1 (facteurs de la chimiorésistance tumorale), très exprimés dans CAL1-wt et muets dans CAL1R) a montré la faible tendance des deux premiers à être impliqués dans la résistance aux VAs.Malignant melanoma (MM) is a very refractory tumor to anticancer therapies, including vinca alkaloïds (VAs). To investigate the role of GSTM1 (glutathione S-transferase μ1) and MRP1 (multidrug resistance protein 1) in MM acquired resistance to VAs, we established 4 cellular models of resistance to vincristine (CAL1R-VCR), to vindesine (CAL1R-VDS), to vinorelbine (CAL1R-VRB) and to vinflunine (CAL1R-VFL), by continuous exposure of MM cells (CAL1-wt), for one year, to these anticancer agents. The expression of a functional GSTM1 is specifically observed (RT-PCR, western blot, total GST activity) in resistant cells. Curcumin (GSTM1 inhibitor), BSO (glutathione synthesis inhibitor) and MK571 (MRP1 inhibitor), considerably reduce the acquired resistance to VCR and VDS but not that to VRB or VFL. However, all these VAs specifically reduce GSTM1 activity. These data show the differential involvement of GSTM1 and MRP1 in resistance to VAs. To determine the molecular mechanisms of this chemoresistance, we performed a pangenomic study (Affymetrix HG-U133 Plus 2.00 microarrays) on the CAL1 lines (wt and R). The hierarchical clustering (by Cluster and TreeView) of array data revealed a similarity between the gene expression profiles of CAL1R-VRB and CAL1-wt, but also between those of CAL1R-VCR and CAL1R-VDS. The bioinformatic analysis (by IPA) of the most differentially expressed transcripts between cell lines, highlighted 6 gene networks known for their role in tumor chemoresistance. FatiGO program revealed 3 biological terms overrepresented (>60%) in CAL1R (ribosome, intermediate filaments of cytoskeleton, olfactory receptors), while functional study (gene invalidation by siRNA, viability test) of GPR143, KIT and SLC45A2 (genes interacting with NF-kB and CCND1 (tumor chemoresistance factors), highly expressed in CAL1-wt and mute in CAL1R) showed the weak trend of the two formers to be involved in resistance to VAs

    Dans un contexte de densité urbaine fortement créateur d’ombre, nous nous intéressons au lumiduc : dispositif d’éclairage consistant à transporter la lumière du jour à l’intérieur des espaces profonds. Ce dispositif dont l’histoire remonte au 19e siècle, a vécu des bouleversements historiques le plus souvent très divergents qui ont mené à l’invention du lumiduc contemporain au milieu du 20e siècle. Ainsi, dans ce travail de recherche, nous avons examiné d’une part les différents paramètres techniques du dispositif à travers une lecture dans la littérature spécialisée, et d’autre part l’imaginaire urbain rattaché à son usage à travers certains cas de figures utopiques. Enfin, nous avons réalisé une étude prospective posant la question du devenir de la forme architecturale et urbaine se basant essentiellement sur une stratégie d’éclairage par des lumiducs
