Pustulosis exantemática generalizada aguda desencadenada por la vacunación con Pfizer-BioNTech frente a la COVID-19
Acute generalized exanthematous pustulosis (AGEP) is a type of uncommon skin lesion characterized by the rapid appearance of disseminated, sterile pustules over an erythematous skin background. This reaction is usually triggered by drugs, mainly antibiotics, although other causes are infections and vaccines.
We report the case of a patient who developed AGEP after the administration of Pfizer-BioNTech vaccine against SARS-COV-2. To our knowledge, only six cases of AGEP due to SARS-COV-2 vaccine have been reported, and none with Pfizer-BioNTech vaccine.La pustulosis exantemática generalizada aguda (PEGA) es un tipo de lesión cutánea poco común caracterizada por la aparición rápida e insidiosa de numerosas pústulas estériles sobre un fondo eritematoso. Esta reacción normalmente es desencadenada por fármacos, principalmente antibióticos y más raramente por otros tipos de desencadenantes como infecciones o vacunas.
Se presenta el caso de una paciente que desarrolló PEGA tras la administración de la vacuna de Pfizer-BioNTech frente al SARS-CoV-2. Según nuestro conocimiento solo hay seis casos descritos por la vacuna frente al SARS-CoV-2 y ninguno de ellos por la vacuna de Pfizer-BioNTech
Space and time in the tectonic evolution of the northwestern Iberian Massif: Implications for the Variscan belt.
Recent advances in geochemical studies of igneous rocks, isotopic age data for magmatism and metamorphism, quantitative pressure-temperature (P-T) estimates of metamorphic evolution, and structural geology in the northwestern Iberian Massif are integrated into a synthesis of the tectonic evolution that places the autochthonous and allochthonous terranes in the framework of Paleozoic plate tectonics. Because northwestern Iberia is free from strike-slip faults of continental scale, it is retrodeformable and preserves valuable information about the orthogonal component of convergence of Gondwana with Laurentia and/or Baltica, and the opening and closure of the Rheic Ocean. The evolution deduced for northwest Iberia is extended to the rest of the Variscan belt in an attempt to develop a three-dimensional interpretation that assigns great importance to the transcurrent components of convergence. Dominant Carboniferous dextral transpression following large Devonian and Early Carboniferous thrusting and recumbent folding is invoked to explain the complexity of the belt without requiring a large number of peri-Gondwanan terranes, and its ophiolites and highpressure allochthonous units are related to a single oceanic closure. Palinspastic reconstruction of the Variscan massifs and zones cannot be achieved without restoration of terrane transport along the colliding plate margins. A schematic reconstruction is proposed that involves postcollisional strike-slip displacement of ~3000 km between Laurussia and Gondwana during the Carboniferous
RICORS2040 : The need for collaborative research in chronic kidney disease
Chronic kidney disease (CKD) is a silent and poorly known killer. The current concept of CKD is relatively young and uptake by the public, physicians and health authorities is not widespread. Physicians still confuse CKD with chronic kidney insufficiency or failure. For the wider public and health authorities, CKD evokes kidney replacement therapy (KRT). In Spain, the prevalence of KRT is 0.13%. Thus health authorities may consider CKD a non-issue: very few persons eventually need KRT and, for those in whom kidneys fail, the problem is 'solved' by dialysis or kidney transplantation. However, KRT is the tip of the iceberg in the burden of CKD. The main burden of CKD is accelerated ageing and premature death. The cut-off points for kidney function and kidney damage indexes that define CKD also mark an increased risk for all-cause premature death. CKD is the most prevalent risk factor for lethal coronavirus disease 2019 (COVID-19) and the factor that most increases the risk of death in COVID-19, after old age. Men and women undergoing KRT still have an annual mortality that is 10- to 100-fold higher than similar-age peers, and life expectancy is shortened by ~40 years for young persons on dialysis and by 15 years for young persons with a functioning kidney graft. CKD is expected to become the fifth greatest global cause of death by 2040 and the second greatest cause of death in Spain before the end of the century, a time when one in four Spaniards will have CKD. However, by 2022, CKD will become the only top-15 global predicted cause of death that is not supported by a dedicated well-funded Centres for Biomedical Research (CIBER) network structure in Spain. Realizing the underestimation of the CKD burden of disease by health authorities, the Decade of the Kidney initiative for 2020-2030 was launched by the American Association of Kidney Patients and the European Kidney Health Alliance. Leading Spanish kidney researchers grouped in the kidney collaborative research network Red de Investigación Renal have now applied for the Redes de Investigación Cooperativa Orientadas a Resultados en Salud (RICORS) call for collaborative research in Spain with the support of the Spanish Society of Nephrology, Federación Nacional de Asociaciones para la Lucha Contra las Enfermedades del Riñón and ONT: RICORS2040 aims to prevent the dire predictions for the global 2040 burden of CKD from becoming true
Thrust geometry and the location of gneiss domes: implications of thermal models of the Central Iberian crustal domain during the Variscan
England and Thompson (1984) and Peacock (1989) developed a method to produce thermal models of the pressuretemperature-time (P-T-t) paths of regional metamorphism
in thickened continental crust. Their models place the physical conditions undergone by rocks initially at different depths in a temporal frame and permit a better understanding
of the orogenic processes involved. One common response to thickening that is observed in orogenic belts is the formation of gneiss domes (Whitney et al., 2004). Gneiss domes are an important feature of the Iberian crust affected by the Variscan orogeny and are abundant in the core of the Central Iberian arc (Martínez Catalán, 2011). A rootless suture and the loss of the roots of a mountain chain: The Variscan belt of NW Iberia
Ophiolites of different Paleozoic ages occur in North-West (NW) Iberia in a rootless suture representing the remnants of the
Rheic Ocean. Associated allochthonous terranes in the hanging- and foot-walls of the suture derive from the former margins,
whereas the relative autochthon corresponds to the Paleozoic passive margin of northern Gondwana. The Paleozoic tectonic
evolution of this part of the circum-Atlantic region is deduced from the stratigraphical, petrological, structural and metamorphic
evolution of the different units and their ages. The tectonic reconstruction covers from Cambro-Ordovician continental rifting and
the opening of the Rheic Ocean to its Middle to Upper Devonian closure. Then, the Variscan Laurussia–Gondwana convergence and
collision is briefly described, from its onset to the late stages of collapse associated with the demise of the orogenic roots.

Une suture sans racines et la perte des racines d’une chaîne montagneuse : la chaîne varisque du Nord-Ouest de
l’Ibérie. Des ophiolites d’âges différents affleurent dans le Nord-Ouest de l’Ibérie dans une suture sans racines, témoin de l’océan Rhéïque. Les terrains allochtones sur et sous la suture dérivent de ses deux marges, tandis que l’autochtone relatif appartient à la
marge passive du Nord de Gondwana. On peut déduire l’évolution des plaques dans cette partie de la région circum-Atlantique à
partir des données stratigraphiques, pétrologiques, structurales, métamorphiques et géochronologiques. Cette évolution inclut le
développement d’un rift continental et l’ouverture de l’océan Rhéïque pendant le Cambro-Ordovicien ainsi que sa fermeture
au Dévonien moyen à supérieur. On décrit aussi l’évolution de la convergence et collision varisque entre Laurussia et
Gondwana, du début jusqu’aux derniers stades d’un effondrement associé à la perte des racines orogéniques
Owning a Pet Is Associated with Changes in the Composition of Gut Microbiota and Could Influence the Risk of Metabolic Disorders in Humans
Pet ownership positively influences clinical outcomes in cardiovascular prevention. Additionally, cardiovascular disease (CVD) has been previously linked to microbiota dysbiosis. We evaluated the influence of owning a pet and its relationship with the intestinal microbiota. We analyzed the gut microbiota from 162 coronary patients from the CORDIOPREV study (NCT00924937) according to whether they owned pets (n = 83) or not (n = 79). The pet-owner group was further divided according to whether they owned dogs only (n = 28) or not (n = 55). A 7-item pet-owners test score was used. Patients who owned pets had less risk of metabolic syndrome (MetS) (OR = 0.462) and obesity (OR = 0.519) and were younger (p < 0.001) than patients who did not own pets. Additionally, patients who owned dogs had less risk of MetS (OR = 0.378) and obesity (OR = 0.418) and were younger (p < 0.001) than patients who did not own pets. A preponderance of the genera Serratia and Coprococcus was found in the group of owners, while the genera Ruminococcus, an unknown genus of Enterobacteriaceae and Anaerotruncus were preponderant in the group of non-owners. In patients who owned dogs, Methanobrevibacter and two more genera, Coprococcus and Oscillospira, were more common. Our study suggests that the prevalence of MetS and obesity in CVD patients is lower in pet owners, and that pet ownership could be a protective factor against MetS through the shaping of the gut microbiota. Thus, owning a pet could be considered as a protective factor against cardiometabolic diseases

Variscan Metamorphism
Various segments of Variscan crust are currently exposed in Iberia in response to successive tectonic events during the Variscan orogeny itself and subsequent extensional and compressive events during the Alpine cycle, all accompanied by surface erosion, and collectively contributing to their exhumation. We review the main characteristics and geodynamic contexts of the metamorphic complexes developed in Iberia during the Variscan cycle, which include: (i) LP-HT complexes associated to the Cambrian-Early Ordovician rift stage; (ii) HP-LT complexes associated to subduction; and (iii) syn-to-post-collisional, MP and LP/HT complexes from the hinterland to the foreland fold-and thrust belts. All the above contexts are illustrated with case studies. Finally, a review of Variscan metamorphism in the Pyrenees and Catalan Coastal Ranges, located far away from the Rheic suture is also presented
