    Parathyroid Hormone-Related Protein Promotes Inflammation in the kidney with an Obstructed ureter

    Parathyroid hormone-related protein (PTHrP) promotes fibrogenesis in the acutely damaged kidney. Considering the relation between fibrosis and inflammation, we studied transgenic mice that overexpress PTHrP in the proximal tubule. When unilateral ureteric obstruction was induced in these transgenic mice, we found that they had more renal tubulointerstitial damage, leukocyte influx, and expression of proinflammatory factors than their control littermates. Reversal of PTHrP constitutive overexpression in these transgenic mice or treatment of control mice with the PTHrP antagonist (7–34) decreased this inflammatory response. Losartan, which abolished obstruction-induced endogenous PTHrP upregulation, also decreased the latter response but less effectively in transgenic mice. The PTHrP fragment (1–36) induced nuclear factor-jB (NF-jB) activation and proinflammatory cytokine overexpression in mouse cortical tubule cells in culture as well as migration of the macrophage cell line Raw 264.7. All these effects were decreased by PTHrP (7–34) and NF-jB or extracellular signal-regulated kinase (ERK) activation inhibitors. Our findings suggest a critical role of PTHrP in the renal inflammatory process that results from ureteral obstruction and indicate that ERK-mediated NF-jB activation seems to be an important mechanism whereby PTHrP triggers renal inflammatio

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    We propose you a reflection about the motivational effect of the teacher’s figure in a class and we try to extract keys to increase the probability of generating a motivational environment of learning. Firstly, we will analyze the teacher’s performance. For this, we will take as starting point the four types of the teacher’s performance prototypes proposed by McLean (2002) and according to the four moments of teaching (see Alonso Tapia, 1997; 2005; Huertas, 1997), we will show prototypical examples of how each type of environment class demonstrate itself.     Resumen basado en el de la publicaciónEl objetivo es diseñar y validar un instrumento de evaluación comunicativa de las competencias en LSE que permita conocer el nivel de los alumnos sordos en diferentes aspectos lingüísticos, de cara a detectar posibles dificultades en la adquisición de esta lengua y dar cuenta del proceso de aprendizaje de la lengua de signos española (LSE) del alumno. El inventario diseñado permite evaluar de manera discriminativa competencias lingüísticas básicas de la LSE. Sería, por lo tanto, útil tanto para las primeras edades de la etapa escolar como para los inicios de la adquisición de este lenguaje. Los resultados muestran que el inventario es sensible a las variables que tradicionalmente correlacionan con la mejora de los aspectos lingüísticos.     Resumen tomado de la publicaciónEste trabajo está dirigido al estudio de las estereotipias motoras en varios grupos de niños con y sin dificultades en el desarrollo. El trabajo se concreta en dos estudios: en el primero analizamos si cambios en la situación estimular producen efectos en la presencia de estereotipias en los diferentes grupos y, en el segundo, estudiamos cómo se insertan las estereotipias en la situación de interacción, si interfieren o si, por el contrario, se ajustan a la actividad que están llevando acabo los niños.
