    Use of Botulinum Toxin in Orofacial Clinical Practice

    Botulinum neurotoxin (BoNT) is a potent biological toxin and powerful therapeutic tool for a growing number of clinical orofacial applications. BoNT relaxes striated muscle by inhibiting acetylcholine’s release from presynaptic nerve terminals, blocking the neuromuscular junction. It also has an antinociceptive effect on sensory nerve endings, where BoNT and acetylcholine are transported axonally to the central nervous system. In dentistry, controlled clinical trials have demonstrated BoNT’s efficiency in pathologies such as bruxism, facial paralysis, temporomandibular joint (TMJ) disorders, neuropathic pain, sialorrhea, dystonia and more. Aim: This study’s aim was to conduct a systematic literature review to assess the most recent high-level clinical evidence for BoNT’s efficacy and for various protocols (the toxin used, dilution, dosage and infiltration sites) used in several orofacial pathologies. Materials and methods: We systematically searched the MedLine database for research papers published from 2014 to 2019 with randomly allocated studies on humans. The search included the following pathologies: bruxism, dislocation of the TMJ, orofacial dystonia, myofascial pain, salivary gland disease, orofacial spasm, facial paralysis, sialorrhea, Frey syndrome and trigeminal neuralgia. Results: We found 228 articles, of which only 20 met the inclusion criteria: bruxism (four articles), orofacial dystonia (two articles), myofascial pain (one article), salivary gland disease (one article), orofacial spasm (two articles), facial paralysis (three articles), sialorrhea (four articles) or trigeminal neuralgia (three articles). Discussion: The clinical trials assessed showed variations in the dosage, application sites and musculature treated. Thus, applying BoNT can reduce symptoms related to motor muscular activity in the studied pathologies efficiently enough to satisfy patients. We did not identify the onset of any important side effects in the literature reviewed. We conclude that treatment with BoNT seems a safe and effective treatment for the reviewed pathologie

    Attitudes and burden in relatives of patients with schizophrenia in a middle income country

    BACKGROUND: Most studies of family attitudes and burden have been conducted in developed countries. Thus it is important to test the generalizability of this research in other contexts where social conditions and extended family involvement may be different. The aim of this study was to assess the relationship between the attitudes of caregivers and the burden they experience in such a context, namely Arica, a town located in the northernmost region of Chile, close to the border with Peru and Bolivia. METHODS: We assessed attitudes towards schizophrenia (including affective, cognitive and behavioural components) and burden (including subjective distress, rejection and competence) in 41 main caregivers of patients with schizophrenia, all of whom were users of Public Mental Health Services in Arica. RESULTS: Attitude measures differed significantly according to socio-demographic variables, with parents (mainly mothers) exhibiting a more negative attitude towards the environment than the rest of the family (t = 4.04; p = 0.000).This was also the case for caregivers with a low educational level (t = 3.27; p < 0.003), for the oldest caregivers (r = 0.546; p = 0.000) and for those who had spent more time with the patient (r = 0.377; p = 0.015). Although attitudes had significant association with burden, their explanatory power was modest (R2 = .104, F = 4,55; p = .039). CONCLUSIONS: Similar to finding developed countries, the current study revealed a positive and significant relationship between the attitudes of caregivers and their burden. These findings emphasize the need to support the families of patients with schizophrenia in this social context

    Tesis (Magíster en Gestión Pedagógica y Curricular)A través de una investigación cualitativa, se establecieron consideraciones críticas al Diseño Universal para el aprendizaje, como estrategia efectiva en el modelo educativo chileno. En donde se concluye que existen dificultades a nivel de la comprensión de los procesos de la enseñanza y el aprendizaje, y de la implementación de esta estrategia en las escuelas chilenas. Por otra parte, se establece que para ser efectiva esta estrategia debe ser parte de un modelo educativo desarrollador y no de un modelo tradicional, en donde se transforma en otro intento fallido de alcanzar la calidad educativa.Through qualitative research, universal design for learning was criticized, as an effective strategy in the Chilean educational model. It is concluded that there are difficulties at the level of understanding the processes of teaching and learning, and the implementation of this strategy in Chilean schools. On the other hand, it is established that for this strategy to be effective must be part of an educational model developer and not a traditional model, where it becomes another failed attempt to achieve educational quality

    The meaning of hysterectomy for a group of chilean men partners of women who have undergone hysterectomy

    Se objetivó conocer el significado de la histerectomía para un grupo de hombres chilenos, parejas de mujeres histerectomizadas (HPMH). Estudio cualitativo, con entrevistas en profundidad realizadas a 15 hombres, parejas de mujeres histerectomizadas, entre Mayo y Septiembre del 2010, previa aprobación de dos Comités de Ética. Para el análisis de los datos se utilizó la perspectiva fenomenológica descrita por Giorgi, y se realizó el análisis de contenido según Krippendorff. Los criterios de Creswell fueron utilizados para evaluar la credibilidad del análisis y asegurar la validez descriptiva. Emergieron cinco dimensiones que representan aspectos únicos del significado de la histerectomía para los hombres parejas de mujeres histerectomizadas: síntomas, comentarios, atributos del útero, preocupaciones, y cambios en la sexualidad. Educar los hombres parejas de mujeres histerectomizadas es una acción fundamental para apoyar a las mujeres que serán sometidas a una histerectomía, siendo necesaria su incorporación en el plan de cuidados de ellas.The objective of this study was to understand the meaning of hysterectomy according to a group of Chilean men, partners of women who have undergone the procedure (MPWH). This qualitative study was performed with in-depth interviews. A total of 15 men, partners of women who have undergone hysterectomy, were interviewed between May and September of 2010, under the approval of the Ethics Committees. Data analysis was performed using the phenomenological perspective proposed by Giorgi, and content analysis was performed according to Krippendorff. The Crestwell criteria were used to evaluate the trustworthiness of the analysis and guarantee descriptive validity. Five dimensions emerged, which represented unique aspects of hysterectomy according to the men: symptoms, comments, the attributions of the uterus, concerns and changes in sexuality. It is essential to educate MPWH in terms of the support required by women undergoing hysterectomy. Therefore they should be included in the care plan designed for women undergoing hysterectomy.O objetivo deste estudo foi conhecer o significado da histerectomia para um grupo de homens chilenos, parceiros de mulheres histerectomizadas (HPMH). A pesquisa qualitativa foi realizada com entrevistas em profundidade, realizada com um total de quinze homens parceiros de mulheres histerectomizadas, entre maio e setembro de 2010, com aprovação prévia dos Comitês de Ética. Para a análise dos dados foi utilizada a perspectiva fenomenológica descrita por Giorgi e realizada a análise de conteúdo de acordo com Krippendorff. Os critérios de Crestwell foram utilizados para avaliar a credibilidade da análise e garantir a validez descritiva. Surgiram cinco dimensões que representam aspectos únicos do significado da extração do útero para os homens parceiros de mulheres histerectomizadas: sintomas, comentários, atribuições do útero, preocupações e mudanças na sexualidade. Educar os homens parceiros de mulheres histerectomizadas é fundamental para apoiar as mulheres que serão submetidas a uma histerectomia, sendo necessário incorporá-los no plano de cuidados das mulheres histerectomizadas
