    Problems associated with traffic, such as traffic congestion and pollution, have occurred in major urban areas in particular due to the increased use of cars. One possible way to reduce the use of cars is to replace commuter trips by car with other modes of transport, such as a combination of car and public transport called Park & Ride. The aim of this thesis was to understand more about factors influencing the choice of mode and to find measures that could attract car drivers to Park & Ride. A stated preference survey has been conducted in order to quantify some standard factors. The factors used in this stated choice experiment were: security at the lot, availability of spaces at the parking lot, costs at the parking lot and walking distances between the parking space and the station. The results indicate that security at the Park & Ride facility is important. Both sexes assign a high value to secure parking, but women are more willing to pay for lights at an unguarded parking lot. The results reveal that park-ing facilities, such as free parking and short distance between parking place and work at work, influence people’s choice of mode. A general conclusion is that there is potential for increasing the use of Park & Ride facilities, especially among women and respon-dents over 30 years

    The purpose of this study was to examine how people experience being in debt, what they consider was the reason for being in debt and how they experienced economic counselling. The empirical data was based on five semi-structured interviews with people having experiences from being in debt and experiences of economic counselling. The data was analysed by using theoretical perspectives from a model called economic-shame created by Bengt Starrin. The model shows how being in debt and the feelings of shame influence health. The conclusion shows that the reasons for being in debt often is due to several sudden and tragic events happening at the same time or due to each other. Together with the experiences and feelings of guilt and shame, the economical situation led to a deterioration of physical and psychological health. However, there were often circumstances arisen before the experience of being in debt which worsen the experience of being in debt. Economic counselling had great impact on the clients and the psychological counselling proved just as useful as the economic

    Screening of the two-component-system histidine kinases of Listeria monocytogenes EGD-e. LiaS is needed for growth under heat, acid, alkali, osmotic, ethanol and oxidative stresses

    To study the role of each two-component system (TCS) histidine kinase (HK) in stress tolerance of Listeria monocytogenes EGD-e, we monitored the growth of individual HIC deletion mutant strains under heat (42.5 degrees C), acid (pH 5.6), alkali (pH 9.4), osmotic (6% NaCl), ethanol (3.5 vol%), and oxidative (5 mM H2O2) stresses. The growth of Delta liaS (Delta lmo1021) strain was impaired under each stress, with the most notable decrease under heat and osmotic stresses. The Delta ivirS (Delta lmo1741) strain showed nearly completely restricted growth at high temperature and impaired growth in ethanol. The growth of Delta agrC (Delta lmo0050) strain was impaired under osmotic stress and slightly under oxidative stress. We successfully complemented the HIC mutations using a novel allelic exchange based approach. This approach avoided the copy-number problems associated with in trans complementation from a plasmid. The mutant phenotypes were restored to the wild-type level in the complemented strains. This study reveals novel knowledge on the HKs needed for growth of L monocytogenes EGD-e under abovementioned stress conditions, with LiaS playing multiple roles in stress tolerance of L monocytogenes EGD-e.     A striving for independence: a qualitative study of women living with vertebral fracture

    <p>Abstract</p> <p>Background</p> <p>Quantitative studies using generic and disease-specific health-related quality of life (HRQOL) questionnaires have shown that osteoporosis-related vertebral fractures have a significant negative effect on HRQOL, but there are only few studies that address what it means to live with vertebral fracture from a deeper experiential perspective. How HRQOL and daily life are affected several years after vertebral fracture and how women cope with this are more unclear. This study aimed to describe how HRQOL and daily life had been affected in women with vertebral fracture several years after diagnosis.</p> <p>Methods</p> <p>The study design was qualitative. Semi-structured interviews were conducted with ten Swedish women during 2008. Data were analysed using qualitative inductive content analysis.</p> <p>Results</p> <p>The findings of this study revealed three themes related to the influence on HRQOL and daily life: <it>A threatened independence</it>, i.e. back pain, anxiety, negative impact on self-image and consequences in daily life; <it>Strategies for maintaining independence</it>, i.e. coping, self-care and support; and <it>The importance of maintaining independence</it>, i.e. the ability to perform everyday activities, social interaction and having something meaningful to do. The women were striving for independence or maintaining their independence by trying to manage different types of symptoms and consequences in different ways.</p> <p>Conclusion</p> <p>HRQOL and daily life were strongly affected in a negative way by the impact of the vertebral fracture. Information from this study may provide new knowledge and understanding of the women's experiences of living with vertebral fracture from an insider's point of view in order to obtain a deeper understanding of the women's everyday life. However, further evaluation is still needed in larger study groups.</p

    Bakgrund: Mötet mellan specialistsjuksköterskor är en del av vårdkedjan, en kedja som ska fungera för patientens bästa. Prehospitalt råder det andra förutsättningar för ett överläm-nade/mottagande som kan involvera flertalet personer och professioner. Syfte: Att beskriva am-bulanssjuksköterskans upplevelse av mötet med andra specialistsjuksköterskor i motta-gande/överlämnande av patient. Metod: En kvalitativ intervjustudie, med sexton medverkande ambulanssjuksköterskor. Analysen av data genomfördes utifrån kvalitativ innehållsanalys med induktiv ansats. Resultatet Mynnade ut i fem kategorier; känna samförstånd, att bli bekräftad, att känna ansvar, behov av struktur i kommunikationen och att kunna påverka mötet. Konklus-ion: Ambulanssjuksköterskorna efterfrågade en respekt för den andres specialistområde, och ta lärdom av varandras kunskaper. Genom att vara medveten om sitt eget beteende och sin attityd kan upplevelsen av mötet mellan specialistsjuksköterskor påverkas positivt, vilket i sin tur san-nolikt också är av betydelse för kvaliteten på vård som patienten får

    Reconstructing Racially Polarized Voting

    Racially polarized voting makes minorities more vulnerable to discriminatory changes in election laws and therefore implicates nearly every voting rights doctrine. In Thornburg v. Gingles , the Supreme Court held that racially polarized voting is a necessary—but not a sufficient—condition for a vote dilution claim under Section 2 of the Voting Rights Act. The Court, however, has recently questioned the propriety of recognizing the existence of racially polarized voting. This colorblind approach threatens not only the Gingles factors but also Section 2’s constitutionality. The Court treats racially polarized voting as a modern phenomenon. But the relevant starting point is the 1860s, not the 1960s. Prior to the Fifteenth Amendment’s passage, Republicans received overwhelming support from newly enfranchised Black voters in the former Confederate States and expected that support to continue. The Reconstruction Framers were thus attentive to the realities of racially polarized voting and openly recognized that extending the franchise would empower Black voters to mobilize politically and protect their own interests. Racially polarized voting was a feature—not a bug—in the passage and ratification of the Fifteenth Amendment. Accordingly, this Article argues that the Court’s treatment of racially polarized voting as a constitutional taboo is historically unfounded and doctrinally incoherent. There are significant implications for acknowledging the role of racially polarized voting during Reconstruction. This historical insight moves vote dilution claims—and their predicate finding of racially polarized voting—far closer to the heart of the Reconstruction Amendments and challenges the Court’s hostility to race-based redistricting. It is powerful evidence that Congress is well within its enforcement authority to remedy and deter dilutive measures that exploit racially polarized voting. Finally, reconstructing racially polarized voting helps reorient voting rights doctrine toward a Fifteenth Amendment framework

    Genomic Epidemiology and Phenotyping Reveal on-Farm Persistence and Cold Adaptation of Raw Milk Outbreak-Associated Yersinia pseudotuberculosis

    Free PMC article: https://www.ncbi.nlm.nih.gov/pmc/articles/pmid/31156582/Packaged raw milk contaminated with Yersinia pseudotuberculosis mediated a large yersiniosis outbreak in southern Finland in 2014. The outbreak was traced back to a single dairy farm in southern Finland. Here we explore risk factors leading to the outbreak through epidemiologic investigation of the outbreak farm and through genomic and phenotypic characterization of the farm's outbreak and non-outbreak associated Y. pseudotuberculosis strains. We show that the outbreak strain persisted on the farm throughout the 7-month study, whereas the non-outbreak strains occurred sporadically. Phylogenomic analysis illustrated that the outbreak strain was related to previously published genomes of wild animal isolates from Finland, implying that wild animals were a potential source of the outbreak strain to the farm. We observed allelic differences between the farm's outbreak and non-outbreak strains in several genes associated with virulence, stress response and biofilm formation, and found that the outbreak strain formed biofilm in vitro and maintained better growth fitness during cold stress than the non-outbreak strains. Finally, we demonstrate the rapid growth of the outbreak strain in packaged raw milk during refrigerated storage. This study provides insight of the risk factors leading to the Y. pseudotuberculosis outbreak, highlights the importance of pest control to avoid the spread of pathogens from wild to domestic animals, and demonstrates that the cold chain is insufficient as the sole risk management strategy to control Y. pseudotuberculosis risk associated with raw drinking milk.This research was supported by the Finnish Ministry of Agriculture and Forestry grant Dnro: 1395/311/214, the Walter Ehrström Foundation, and the University of Helsinki Doctoral Program in Food Chain and Health.     Doxapram as an additive to propofol sedation for endoscopic retrograde cholangiopancreatography : a placebo-controlled, randomized, double-blinded study

    Background Endoscopic retrograde cholangiopancreatography (ERCP) requires moderate to deep sedation, usually with propofol. Adverse effects of propofol sedation are relatively common, such as respiratory and cardiovascular depression. This study was conducted to determine if doxapram, a respiratory stimulant, could be used to reduce the incidence of respiratory depression. Methods This is a single-center, prospective randomized double-blind study performed in the endoscopy unit of Helsinki University Central Hospital. 56 patients were randomized in a 1:1 ratio to either receive doxapram as an initial 1 mg/kg bolus and an infusion of 1 mg/kg/h (group DOX) or placebo (group P) during propofol sedation for ERCP. Main outcome measures were apneic episodes and hypoxemia (SpO(2) <90%). Mann-Whitney test for continuous variables and Fisher's exact test for discrete variables were used and mixed effects modeling to take into account repeated measurements on the same subject and comparing both changes within a group as a function of time and between the groups. Results There were no statistically significant differences in apneic episodes (p = 0.18) or hypoxemia (p = 0.53) between the groups. There was a statistically significant rise in etCO(2) levels in both groups, but the rise was smaller in group P. There was a statistically significant rise in Bispectral Index (p = 0.002) but not modified Observer's Assessment of Agitation/Sedation (p = 0.21) in group P. There were no statistically significant differences in any other measured parameters. Conclusions Doxapram was not effective in reducing respiratory depression caused by deep propofol sedation during ERCP. Further studies are warranted using different sedation protocols and dosing regimens.Peer reviewe
