    Realizzazione e validazione di un modulo di link management per reti wireless mesh

    In questo lavoro di tesi viene illustrata la realizzazione di un modulo di link management per reti wireless mesh consistente con le specifiche del draft 3.0 sull’ 802.11s. Il modulo è stato sviluppato utilizzando il framework fornito dal software Click Modular Router ed è stato installato su un prototipo di rete mesh 802.11, composto da 5 router equipaggiati ognuno con 2 schede di rete wireless 802.11a/b/g e 2 schede di rete Ethernet. Dal punto di vista software i nodi sono dotati di un meccanismo di instradamento basato sul label switching conforme all’ RFC 3031, con un protocollo di distribuzione delle label che consente di installare automaticamente i Label Switched Path (LSP), in accordo all’ RFC 5036, e in più fornisce un meccanismo dedicato per l’ installazione di rotte di backup (detour), attivabili grazie all’ interfaccia FrrDB. In una seconda fase il modulo di link management è stato esteso con un meccanismo di valutazione della qualità dei link wireless che, interfacciandosi all’ FrrDB, comanda l’ attivazione dei detour; sono stati quindi effettuati dei test per validare le soluzioni proposte

    Potential Infectious Etiology of Behçet's Disease

    Behçet's disease is a multisystem inflammatory disorder characterized by recurrent oral aphthous ulcers, genital ulcers, uveitis, and skin lesions. The cause of Behçet's disease remains unknown, but epidemiologic findings suggest that an autoimmune process is triggered by an environmental agent in a genetically predisposed individual. An infectious agent could operate through molecular mimicry, and subsequently the disease could be perpetuated by an abnormal immune response to an autoantigen in the absence of ongoing infection. Potentia bacterial are Saccharomyces cerevisiae, mycobacteria, Borrelia burgdorferi, Helicobacter pylori, Escherichia coli, Staphylococcus aureus, and Mycoplasma fermentans, but the most commonly investigated microorganism is Streptococcus sanguinis. The relationship between streptococcal infections and Behçet's disease is suggested by clinical observations that an unhygienic oral condition is frequently noted in the oral cavity of Behçet's disease patients.     Advanced Video-Based Processing for Low-Cost Damage Assessment of Buildings under Seismic Loading in Shaking Table Tests

    This paper explores the potential of a low-cost, advanced video-based technique for the assessment of structural damage to buildings caused by seismic loading. A low-cost, high-speed video camera was utilized for the motion magnification processing of footage of a two-story reinforcedconcrete frame building subjected to shaking table tests. The damage after seismic loading was estimated by analyzing the dynamic behavior (i.e., modal parameters) and the structural deformations of the building in magnified videos. The results using the motion magnification procedure were compared for validation of the method of the damage assessment obtained through analyses of conventional accelerometric sensors and high-precision optical markers tracked using a passive 3D motion capture system. In addition, 3D laser scanning to obtain an accurate survey of the building geometry before and after the seismic tests was carried out. In particular, accelerometric recordings were also processed and analyzed using several stationary and nonstationary signal processing techniques with the aim of analyzing the linear behavior of the undamaged structure and the nonlinear structural behavior during damaging shaking table tests. The proposed procedure based on the analysis of magnified videos provided an accurate estimate of the main modal frequency and the damage location through the analysis of the modal shapes, which were confirmed using advanced analyses of the accelerometric data. Consequently, the main novelty of the study was the highlighting of a simple procedure with high potential for the extraction and analysis of modal parameters, with a special focus on the analysis of the modal shape’s curvature, which provides accurate information on the location of the damage in a structure, while using a noncontact and low-cost method

    Simulated last deglaciation of the Barents Sea Ice Sheet primarily driven by oceanic conditions

    The Barents Sea Ice Sheet was part of an interconnected complex of ice sheets, collectively referred to as the Eurasian Ice Sheet, which covered north-westernmost Europe, Russia and the Barents Sea during the Last Glacial Maximum (around 21 ky BP). Due to common geological features, the Barents Sea component of this ice complex is seen as a paleo-analogue for the present-day West Antarctic Ice Sheet. Investigating key processes driving the last deglaciation of the Barents Sea Ice Sheet represents an important tool to interpret recent observations in Antarctica over the multi-millennial temporal scale of glaciological changes. We present results from a perturbed physics ensemble of ice sheet model simulations of the last deglaciation of the Barents Sea Ice Sheet, forced with transient atmospheric and oceanic conditions derived from AOGCM simulations. The ensemble of transient simulations is evaluated against the data-based DATED-1 reconstruction to construct minimum, maximum and average deglaciation scenarios. Despite a large model/data mismatch at the western and eastern ice sheet margins, the simulated and DATED-1 deglaciation scenarios agree well on the timing of the deglaciation of the central and northern Barents Sea. We find that the simulated deglaciation of the Barents Sea Ice Sheet is primarily driven by the oceanic forcing, with prescribed eustatic sea level rise amplifying the ice sheet sensitivity to sub-shelf melting over relatively short intervals. Our results highlight that the sub-shelf melting has a very strong control on the simulated grounding-line flux, showing that a slow, gradual ocean warming trend is capable of triggering sustained grounded ice discharge over multi-millennial timescales, even without taking into account marine ice sheet or ice cliff instability

    Acute Delta Hepatitis in Italy spanning three decades (1991–2019): Evidence for the effectiveness of the hepatitis B vaccination campaign

    Updated incidence data of acute Delta virus hepatitis (HDV) are lacking worldwide. Our aim was to evaluate incidence of and risk factors for acute HDV in Italy after the introduction of the compulsory vaccination against hepatitis B virus (HBV) in 1991. Data were obtained from the National Surveillance System of acute viral hepatitis (SEIEVA). Independent predictors of HDV were assessed by logistic-regression analysis. The incidence of acute HDV per 1-million population declined from 3.2 cases in 1987 to 0.04 in 2019, parallel to that of acute HBV per 100,000 from 10.0 to 0.39 cases during the same period. The median age of cases increased from 27 years in the decade 1991-1999 to 44 years in the decade 2010-2019 (p < .001). Over the same period, the male/female ratio decreased from 3.8 to 2.1, the proportion of coinfections increased from 55% to 75% (p = .003) and that of HBsAg positive acute hepatitis tested for by IgM anti-HDV linearly decreased from 50.1% to 34.1% (p < .001). People born abroad accounted for 24.6% of cases in 2004-2010 and 32.1% in 2011-2019. In the period 2010-2019, risky sexual behaviour (O.R. 4.2; 95%CI: 1.4-12.8) was the sole independent predictor of acute HDV; conversely intravenous drug use was no longer associated (O.R. 1.25; 95%CI: 0.15-10.22) with this. In conclusion, HBV vaccination was an effective measure to control acute HDV. Intravenous drug use is no longer an efficient mode of HDV spread. Testing for IgM-anti HDV is a grey area requiring alert. Acute HDV in foreigners should be monitored in the years to come

    Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Pediatric vs. adult NAFLD to MAFLD transition: a welcome but tangled path

    The term non-alcoholic fatty liver disease (NAFLD) appears unfitting both in adults and in children. As obesity and metabolic syndrome play a relevant pathogenic role, an international group of adults’ liver disease experts has proposed to rename this condition metabolic (dysfunction)-associated fatty liver disease (MAFLD). While this new more appropriate and useful definition has mostly been met with good reactions in adults, it may present a tangled path in pediatrics. Here we further stress the recommendations of the North American and the European Societies for Pediatric Gastroenterology, Hepatology and Nutrition that a hyperechogenic liver in a child affected by obesity or overweight with chronically elevated liver enzymes should not be assumed to have NAFLD only. Especially in those patients who are not in the classic age range or who have particularly severe laboratory anomalies, other genetic, metabolic (inborn errors of metabolism, IEM), endocrine, intestinal and hepatic pediatric-onset conditions, should in fact be excluded, particularly when response to a weight loss trial is not available. The term pediatric fatty liver disease (PeFLD) with three subtypes [1. contextual diagnosis of an IEM; 2. metabolic (dysfunction)-associated fatty liver; 3. unknown cause of fatty liver] has recently been proposed aiming to separate true MAFLD from IEM and/or the other above mentioned conditions, which may be rare when considered individually but represent a large group when considered collectively. Although the cost-effectiveness ratio of this attitude is still indeterminate, it is likely that the advantage of the early identification of a specifically treatable pediatric-onset liver disease associated to/mimicking MAFLD would be rewarding
