    V605 Aquilae: a born again star, a nova or both?

    V605 Aquilae is today widely assumed to have been the result of a final helium shell flash occurring on a single post-asymptotic giant branch star. The fact that the outbursting star is in the middle of an old planetary nebula and that the ejecta associated with the outburst is hydrogen deficient supports this diagnosis. However, the material ejected during that outburst is also extremely neon rich, suggesting that it derives from an oxygen-neon-magnesium star, as is the case in the so-called neon novae. We have therefore attempted to construct a scenario that explains all the observations of the nebula and its central star, including the ejecta abundances. We find two scenarios that have the potential to explain the observations, although neither is a perfect match. The first scenario invokes the merger of a main sequence star and a massive oxygen-neon-magnesium white dwarf. The second invokes an oxygen-neon-magnesium classical nova that takes place shortly after a final helium shell flash. The main drawback of the first scenario is the inability to determine whether the ejecta would have the observed composition and whether a merger could result in the observed hydrogen-deficient stellar abundances observed in the star today. The second scenario is based on better understood physics, but, through a population synthesis technique, we determine that its frequency of occurrence should be very low and possibly lower than what is implied by the number of observed systems. While we could not envisage a scenario that naturally explains this object, this is the second final flash star which, upon closer scrutiny, is found to have hydrogen-deficient ejecta with abnormally high neon abundances.     Evolved stars dominate galactic spectra, enrich the galactic medium, expand to change their planetary systems, eject winds of a complex nature, produce spectacular nebulae and illuminate them, and transfer material between binary companions. While doing this, they fill the HR diagram with diagnostic loops that write the story of late stellar evolution. Evolved stars sometimes release unfathomable amounts of energy in neutrinos, light, kinetic flow, and gravitational waves. During these late-life times, stars evolve complexly, with expansion, convection, mixing, pulsation, mass loss. Some processes have virtually no spatial symmetries, and are poorly addressed with low-resolution measurements and analysis. Even a "simple" question as how to model mass loss resists solution. However, new methods offer increasingly diagnostic tools. Astrometry reveals populations and groupings. Pulsations/oscillations support study of stellar interiors. Optical/radio interferometry enable 2-3d imagery of atmospheres and shells. Bright stars with rich molecular spectra and velocity fields are a ripe opportunity for imaging with high spatial and spectral resolution, giving insight into the physics and modeling of later stellar evolution

    Cross-sectional and longitudinal construct validity of two rotator cuff disease-specific outcome measures

    BACKGROUND: Disease-specific Quality Of Life (QOL) measures are devised to assess the impact of a specific disease across a spectrum of important domains of life. The purpose of this study was to examine the cross-sectional and longitudinal construct validity (sensitivity to change) of two rotator cuff disease-specific measures, the Rotator Cuff-Quality Of Life (RC-QOL) and the Western Ontario Rotator Cuff (WORC) index, in relation to one another and to other joint and limb specific measures in the same population of the patients suffering from rotator cuff pathology. METHODS: Participants enrolled were consecutive patients who received physical therapy for management of impingement syndrome or received treatment following rotator cuff repair, acromioplasty or decompression surgeries. All subjects received physical therapy treatment and completed four outcome measures at 3 single points (initial, interim, and final). Cross-sectional convergent validity was assessed at each of the 3 time-points by correlating the WORC and RC-QOL's scores to each other and to two alternative scales; a joint-specific scale, the American Shoulder and Elbow Surgeons (ASES) standardized shoulder assessment form and a limb-specific measure, the Upper Extremity Functional Index (UEFI). Non-parametric statistics (Spearman's rho and Wilcoxon-Mann-Whitney tests) examined the construct validity. The standardized response mean (SRM) was used to examine sensitivity to change. RESULTS: Forty-one participants entered the study and their scores were compared at 3 cross sectional single points. The correlation coefficients among the 4 measures varied from 0.60 to 0.91. Correlation between corresponding domains of the WORC and RC-QOL varied from 0.45 to 0.85. The known group validity was not significantly different among individual sub-scores and total scores. The final SRMs were (1.42), (1.43), (1.44), and (1.54) for the ASES, RCQOL, WORC, and UEFI respectively. CONCLUSION: The WORC and RC-QOL exhibit similar cross-sectional convergent validity in patients suffering from rotator cuff pathology. The sensitivity to change was very close among all scores, with the UEFI having the highest sensitivity. Further research is needed to examine the extent to which each physical or emotional domain contributes to prognostic or therapeutic decision-making

    Association Between Advanced Maternal Age and Maternal and Neonatal Morbidity: A Cross-Sectional Study on a Spanish Population

    Background and objective: Over recent decades, a progressive increase in the maternal age at childbirth has been observed in developed countries, posing a health risk for both women and infants. The aim of this study was to analyze the association between advanced maternal age (AMA) and maternal and neonatal morbidity. Material and methods: A cross-sectional study of 3,315 births was conducted in the north of Spain in 2014. We compared childbirth between women aged 35 years or older, with a reference group of women aged between 24 and 27 years. AMA was categorized based on ordinal ranking into 35-38 years, 39-42 years, and >42 years to estimate a dose-response pattern (the older the age, the greater the risk). As an association measure, crude and adjusted Odds Ratios (OR) were estimated by non-conditional logistic regression and 95% Confidence Intervals (95%CI) were calculated. Results: Repeated abortions were more common among women of AMA in comparison to pregnant women aged 24-27 years (reference group): adjusted OR = 2.68; 95%CI (1.52-4.73). A higher prevalence of gestational diabetes was also observed among women of AMA, reaching statistical significance when restricted to first time mothers: adjusted OR = 8.55; 95%CI (1.12-65.43). In addition, the possibility of an instrumental delivery was multiplied by 1.6 and the possibility of a cesarean by 1.5 among women of AMA, with these results reaching statistical significance, and observing a dose-response pattern. Lastly, there were associations between preeclampsia, preterm birth (<37 weeks) and low birthweight, however without reaching statistical significance. Conclusion: Our results support the association between AMA and suffering repeated abortions. Likewise, being of AMA was associated with a greater risk of suffering from gestational diabetes, especially among primiparous women, as well as being associated with both instrumental deliveries and cesareans among both primiparous and multiparous women

    The CMS Phase-1 pixel detector upgrade

    The CMS detector at the CERN LHC features a silicon pixel detector as its innermost subdetector. The original CMS pixel detector has been replaced with an upgraded pixel system (CMS Phase-1 pixel detector) in the extended year-end technical stop of the LHC in 2016/2017. The upgraded CMS pixel detector is designed to cope with the higher instantaneous luminosities that have been achieved by the LHC after the upgrades to the accelerator during the first long shutdown in 2013–2014. Compared to the original pixel detector, the upgraded detector has a better tracking performance and lower mass with four barrel layers and three endcap disks on each side to provide hit coverage up to an absolute value of pseudorapidity of 2.5.     G Protein Coupling and Second Messenger Generation Are Indispensable for Metalloprotease-dependent, Heparin-binding Epidermal Growth Factor Shedding Through Angiotensin II Type-1 Receptor

    A G protein-coupled receptor agonist, angiotensin II (AngII), induces epidermal growth factor (EGF) receptor (EGFR) transactivation possibly through metalloprotease- dependent, heparin-binding EGF (HB-EGF) shedding. Here, we have investigated signal transduction of this process by using COS7 cells expressing an AngII receptor, AT1. In these cells AngII-induced EGFR transactivation was completely inhibited by pretreatment with a selective HB-EGF inhibitor, or with a metalloprotease inhibitor. We also developed a COS7 cell line permanently expressing a HB-EGF construct tagged with alkaline phosphatase, which enabled us to measure HB-EGF shedding quantitatively. In the COS7 cell line AngII stimulated release of HB-EGF. This effect was mimicked by treatment either with a phospholipase C activator, a Ca2 ionophore, a metalloprotease activator, or H2O2. Conversely, pretreatment with an intracellular Ca2 antagonist or an antioxidant blocked AngII-induced HB-EGF shedding. Moreover, infection of an adenovirus encoding an inhibitor of Gq markedly reduced EGFR transactivation and HB-EGF shedding through AT1. In this regard, AngII-stimulated HB-EGF shedding was abolished in an AT1 mutant that lacks Gq protein coupling. However, in cells expressing AT1 mutants that retain Gq protein coupling, AngII is still able to induce HB-EGF shedding. Finally, the AngII-induced EGFR transactivation was attenuated in COS7 cells overexpressing a catalytically inactive mutant of ADAM17. From these data we conclude that AngII stimulates a metalloprotease ADAM17-dependent HB-EGF shedding through AT1/Gq/phospholipase C-mediated elevation of intracellular Ca2 and reactive oxygen species production, representing a key mechanism indispensable for EGFR transactivation
