Current state and future of pediatric allergology in Europe: A road map

The history of pediatric allergology (PA) in Europe is relatively youthful, dating back to 1984, when a small group of pediatricians founded the European Working Group on Pediatric Allergy and Immunology—later giving rise to ESPACI (European Society on Pediatric Allergology and Clinical Immunology). In 1990, the first dedicated journal, Pediatric Allergy and Immunology (PAI), was founded. There are striking differences across Europe, and even within European countries, in relation to the training pathways for doctors seeing children with allergic disease(s). In 2016, the EAACIClemens von Pirquet Foundation (CvP) organized and sponsored a workshop with the European Academy of Allergy and Clinical Immunology (EAACI) Pediatric Section. This collaboration focussed on the future of PA and specifically on education, research, and networking/ advocacy. The delegates representing many countries across Europe have endorsed the concept that optimal care of children with allergic diseases is delivered by pediatricians who have received dedicated training in allergy, or allergists who have received dedicated training in pediatrics. In order to meet the needs of children and families with allergic disease(s), the pediatric allergist is highly encouraged to develop several networks. Our challenge is to reinforce a clear strategic approach to scientific excellence to across our member base and to ensure and enhance the relevance of European pediatric research in allergy. With research opportunities in basic, translational, clinical, and epidemiologic trials, more trainees and trained specialists are needed and it is an exciting time to be a pediatric allergologist

COVID-19 vaccination in patients receiving allergen immunotherapy (AIT) or biologicals:EAACI recommendations

Immune modulation is a key therapeutic approach for allergic diseases, asthma and autoimmunity. It can be achieved in an antigen-specific manner via allergen immunotherapy (AIT) or in an endotype-driven approach using biologicals that target the major pathways of the type 2 (T2) immune response: immunoglobulin (Ig)E, interleukin (IL)-5 and IL-4/IL-13 or non-type 2 response: anti-cytokine antibodies and B-cell depletion via anti-CD20. Coronavirus disease 2019 (COVID-19) vaccination provides an excellent opportunity to tackle the global pandemics and is currently being applied in an accelerated rhythm worldwide. The vaccine exerts its effects through immune modulation, induces and amplifies the response against the severe acute respiratory syndrome coronavirus (SARS-CoV-2). Thus, as there may be a discernible interference between these treatment modalities, recommendations on how they should be applied in sequence are expected. The European Academy of Allergy and Clinical Immunology (EAACI) assembled an expert panel under its Research and Outreach Committee (ROC). This expert panel evaluated the evidence and have formulated recommendations on the administration of COVID-19 vaccine in patients with allergic diseases and asthma receiving AIT or biologicals. The panel also formulated recommendations for COVID-19 vaccine in association with biologicals targeting the type 1 or type 3 immune response. In formulating recommendations, the panel evaluated the mechanisms of COVID-19 infection, of COVID-19 vaccine, of AIT and of biologicals and considered the data published for other anti-infectious vaccines administered concurrently with AIT or biologicals

Protocol for a systematic review of the diagnostic test accuracy of tests for IgE-mediated food allergy

Background: The European Academy of Allergy and Clinical Immunology (EAACI) is in the process of updating the guidelines on the diagnosis and management of food allergy. The existing guidelines are based on a systematic review of the literature until 30 September 2012. Therefore, a new systematic review must be undertaken to inform the new guidelines. This systematic review aims to assess the accuracy of index tests to support the diagnosis of IgE-mediated food allergy. Methods: The databases Cochrane CENTRAL (Trials), MEDLINE (OVID) and Embase (OVID) will be searched for diagnostic test accuracy studies from 1 October 2012 to 30 June 2021. Inclusion and exclusion criteria will be used to select appropriate studies. Data from these studies will be extracted and tabulated, and then reviewed for risk of bias and applicability using the QUADAS-2 tool. All evaluations will be done in duplicate. Studies with a high risk of bias and low applicability will be excluded. Meta-analysis will be performed if there are three or more studies of the same index test and food. Results: A protocol for the systematic review and meta-analyses is presented and was registered using Prospero prior to commencing the literature search. Discussion: Oral food challenges are the reference standard for diagnosis but involve considerable risks and resources. This protocol for systematic review aims to assess the accuracy of various tests to diagnose food allergy, which can be useful in both clinical and research settings

Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Kannattavuuslaskelmat matkailu- ja ravitsemisalalla : case Kuopio Wine Festival

Opinnäytetyön tavoitteena oli selvittää Osuuskauppa PeeÄssän järjestämän Kuopio Wine Festivalin ruokakadun kannattavuus. Toisena tutkimuskohteena oli lauantai-iltapäivinä järjestettyjen ilmaisaukioloaikojen kannattavuus. Aloite tutkimuksen tekemiseen tuli toimeksiantajalta, sillä ruokakadun osastokohtaisesta kannattavuudesta ei vielä ole saatu tarkkaa tietoa. Työn teoreettinen viitekehys koostui pääasiassa yrityksen kannattavuuden perusteorioista, kannattavuuden seurannan teoriasta sekä hinnoittelun teoriasta. Teoriaosuudessa käsiteltiin myös tapahtumaliiketoiminnan erityispiirteitä. Kannattavuuden selvittämisessä käytetty tutkimusmenetelmä perustui katetuottolaskennan teoriaan. Tuotetason analyysiä toteutettiin ABC-analyysin ja tuoteanalyysin avulla. Käytännön työkaluna käytettiin Excel-taulukkolaskelmaohjelmaa, johon luodun taulukon ja toimeksiantajalta saatujen tuotetietojen avulla tutkimus suoritettiin. Käytännön osuus koostui kannattavuuslaskelmataulukoiden valmistelusta, tuotekohtaisten myyntilukujen keräämisestä ja dokumentoinnista sekä tulosten analysoinnista. Taulukoita valmisteltiin kirjaamalla ylös kaikki myytävät tuotteet, arvonlisävero, raaka-ainekustannukset sekä myyntikate. Myyntimäärien lisäämisen jälkeen saatiin tietoon sekä yksittäisten tuotteiden että liiketoiminnan kokonaiskannattavuus. Saadut tulokset analysoitiin ja niiden perusteella todettiin johtopäätökset ja annettiin toimenpide-ehdotuksia. Tutkimuksen tulokset osoittivat, että Kuopio Wine Festivalin ruokakadun toiminta on kannattavaa. Laskelmien ja analyysien pohjalta voitiin myös todeta, että myytävien tuotteiden ja yksittäisten myyntipisteiden kannattavuuksissa oli selkeitä eroja. Myös lauantaipäivien osalta tutkimustulokset olivat selkeitä. Tulosten perusteella toimeksiantajalle pystyttiin antamaan toimenpide-ehdotuksia kannattavuuden parantamiseksi. Keinoja ovat muun muassa kustannusten tehostaminen ja tuotevalikoiman koostaminen kannattavammista tuotteista. Opinnäytetyö soveltuu esimerkiksi matkailu- ja ravitsemisalan kannattavuuslaskelmien toteutuksesta sekä siitä, kuinka yksittäisen tapahtuman erityispiirteet vaikuttavat kannattavuuden laskentaan. Osa opinnäytetyön sisällöstä on määrätty salassa pidettäväksi. Liiketoimintaa koskevia lukuja on salattu tai muutettu julkaistavaan muotoon.The main purpose of the study was to calculate and analyze the profitability of Kuopio Wine Festival’s street food department. The secondary purpose was to calculate and analyze the profitability of Kuopio Wine Festival’s picnic afternoons, which were open to public with no entry fee. The study was proposed by the client organization, because they had never collected accurate information about the profitability of the fore mentioned departments of the festival. The theoretical framework consists of basic theories about calculating and analyzing profitability and theory of pricing. Special characteristics of event business and its profitability are also included. The methodology used in calculating profitability was based on the theory of contribution margin. Product analyses were conducted with ABC-analysis and menu analysis based on the Boston Matrix. Microsoft Excel was used as the primary tool in calculating and compiling the results. The practical part of the thesis consisted of preparing the calculation spreadsheets, collecting sales numbers from the client organization and analyzing the collected data. The spreadsheets consisted of all the available products, their retail price, value added tax, net retail price, variable costs of each product and net income of each product. After adding the sales numbers to the spreadsheets, the total operational profitability and the profitability of each product were calculated and analyzed. The study showed that the street food department was profitable. However, there were clear differences in profitability between different products and sub units within the street food department. The client organization was given suggestions for improving their business based on the results. Suggestions included things such as being more cost efficient and compiling the product catalogue of products that are profitable. The thesis serves as an example of how to conduct profitability analysis within the tourism, hospitality and restaurant industry. It also shows how special characteristics of an event influence the analysis. Due to confidentiality reasons, some parts of the results cannot be published

EAACI guidelines on the diagnosis of IgE-mediated food allergy

This European Academy of Allergy and Clinical Immunology guideline provides recommendations for diagnosing IgE-mediated food allergy and was developed using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. Food allergy diagnosis starts with an allergy-focused clinical history followed by tests to determine IgE sensitization, such as serum allergen-specific IgE (sIgE) and skin prick test (SPT), and the basophil activation test (BAT), if available. Evidence for IgE sensitization should be sought for any suspected foods. The diagnosis of allergy to some foods, such as peanut and cashew nut, is well supported by SPT and serum sIgE, whereas there are less data and the performance of these tests is poorer for other foods, such as wheat and soya. The measurement of sIgE to allergen components such as Ara h 2 from peanut, Cor a 14 from hazelnut and Ana o 3 from cashew can be useful to further support the diagnosis, especially in pollen-sensitized individuals. BAT to peanut and sesame can be used additionally. The reference standard for food allergy diagnosis is the oral food challenge (OFC). OFC should be performed in equivocal cases. For practical reasons, open challenges are suitable in most cases. Reassessment of food allergic children with allergy tests and/or OFCs periodically over time will enable reintroduction of food into the diet in the case of spontaneous acquisition of oral tolerance.</p
