    Diseño de un sistema de baliza automática para puertos deportivos

    Este proyecto se centra en el diseño conceptual de una baliza automática para uso en puertos deportivos. A partir del estado del arte se ha constatado que el mercado actual es inexistente, así pues, partiendo de cero, se han creado dos posibles conceptos. Estos conceptos incluyen un diseño del sistema mecánico, electrónico y de telecomunicaciones. Dicha embarcación de servicios tendrá la función de baliza, la cual estará dentro de puerto y se encargará de ir a buscar embarcaciones y guiarlas hasta su amarre. Todo esto de forma automática. Con tal de realizar un diseño conceptual, primero se estudiará las opciones que presenta el mercado, para posteriormente presentar dos alternativas que puedan cumplir las funciones deseadas. Con la ayuda del programa de diseño asistido Solidworks 2018 se presentan las soluciones conceptuales de ambas propuestas. Para el análisis de la embarcación, a partir del planteamiento de las ecuaciones matemáticas y físicas se realiza una simulación de su funcionamiento. Esta simulación va acompañada de una hoja de cálculo que simula el controlador PID incorporado. Todo esto con el fin de tomar la decisión de cuál de las dos propuestas es la más adecuada para este proyecto y su fin

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Start-ups have been on the mouth of everyone in the financial industry since some disruptive business entered into our life with fresh ideas and a way of doing things differently. We are of course talking about Facebook, Instagram, Glovo, Uber, etc. This paper will focus on how to value a start-up. But, first of all, what is a start-up? Start-ups are not only unique because of a magnificent (or not?) idea coming from their founders, but because of their financial performance. Start-ups are fast growing companies with a high risk of default. This fact, makes them very difficult to value with traditional methods which are thought for mature companies having more or less predictable cash flows. In addition, by their nature of exponential growth, it is very important to know at which stage a start-up is (early-stage or pre-IPO?) in order to assess the risk associated with the investment. Once the nature of start-ups has been completely understood, we must define which valuation methods exist and when to use them. Traditional valuation methods (such as DCF or relative valuations) can be very useful for mature companies. However, some newly adopted methods by the Venture Capital industry have been defined in Section 2. Finally, at the end of this paper, an analysis and valuation of Typeform has been conducted – taking as a reference the Series C raised in March 2022. Founded in Barcelona, and co-based in Barcelona and California, Typeform has disrupted the online forms and questionnaires. In addition, the company is on the Top 3 candidates to IPO as an unicorn in the following 1-2 years. 4 Finally and to conclude, some final valuation conclusions have been presented and a brief description of what to expect from 2023. Given the last 3 years, where the market had an excess of liquidity, it is important to understand that the market is now more cautious and waiting for the macroeconomic environment to calm down a little bit. Examples of companies that have failed in their IPO are abundant in the market. For example, Wallbox, who merged with a SPAC to IPO in April 2021, has lost 71% of its value at that moment. Further examples can include Glovo, who has lost 60% of value since December 2021

    Mycobacterium abscessus pulmonary infection during hepatitis C treatment with telaprevir, peginterferon and ribavirin

    The first generation protease inhibitors has been the mainstay of hepatitis C treatment for the last couple of years, showing marked improvement in sustained virological response, but also increased side effects. Infection has emerged as a common complication of telaprevir and boceprevir in combination with peginterferon and ribavirin, usually caused by common pathogens. We present the case of a 65 years old man who developed a Mycobacterium abscessus pulmonary infection during treatment with telaprevir, peginterferon and ribavirin. The patient was successfully treated with amikacin, imipenem and chlarithro-mycin. The present case is relevant for increasing awareness for recognition of opportunistic infections and particularly nontuberculous mycobacterial infections in patients receiving triple therapy for chronic hepatitis C, especially in cirrhotic subjects who develop significant lymphopenia

    Global attitudes in the management of acute appendicitis during COVID-19 pandemic: ACIE Appy Study

    Background: Surgical strategies are being adapted to face the COVID-19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods: The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results: Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X-ray plus molecular testing (PCR) being the commonest (19\ub78 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6\ub76 and 2\ub74 per cent respectively before, but 23\ub77 and 5\ub73 per cent, during the pandemic (both P < 0\ub7001). One-third changed their approach from laparoscopic to open surgery owing to the popular (but evidence-lacking) advice from expert groups during the initial phase of the pandemic. No agreement on how to filter surgical smoke plume during laparoscopy was identified. There was an overall reduction in the number of patients admitted with appendicitis and one-third felt that patients who did present had more severe appendicitis than they usually observe. Conclusion: Conservative management of mild appendicitis has been possible during the pandemic. The fact that some surgeons switched to open appendicectomy may reflect the poor guidelines that emanated in the early phase of SARS-CoV-2
