    Güçlendirici Liderlik, Destekleyici Çalışma Ortamı ve Örgütsel Bağlılık Arasındaki İlişkiler: Rehberlik ve Araştırma Merkezleri (RAM) Örneği

    This study aims to investigate the relationship between the empowering leadership styles of the principals of Guidance and Research Centers, the organizational commitment of teachers, and the mediating role of the supportive work environment in this relationship. The sample consisted of 428 teachers working in the Guidance and Research Centers in Türkiye, and was determined according to the cluster sampling method. This study was conducted by using a correlational survey method. The data was collected and maintained using the School Principal Empowering Leadership Scale, Supportive Work Environment Perception Scale, and Organizational Commitment Scale in the 2021–2022 academic year. Regression analysis and path analysis relied on data to determine the direct predictors of empowering leadership and supportive work environment perception on teachers' organizational commitment, and the indirect predictors of empowering leadership through supportive work environment perception on their organizational commitment. We also calculated the correlation coefficients (r) to determine the relationships between variables. According to the results, empowering leadership is a positive and significant predictor of teachers' perceptions of a supportive work environment and organizational commitment. Likewise, teachers' perceptions of a supportive work environment significantly predicted their organizational commitment. Simultaneously, while empowering leadership alone explained 27% of the total variance in teachers' organizational commitment, the variance explained by including perceptions of a supportive work environment in the model was 37%. These results show that principals' empowering leadership behaviors that increase teachers' organizational commitment might affect their perceptions of a supportive work environment.Bu çalışmanın amacı Rehberlik ve Araştırma Merkezleri müdürlerinin güçlendirici liderlik stilleri ile öğretmenlerin örgütsel bağlılıkları arasındaki ilişkiyi ve bu ilişkide destekleyici çalışma ortamının aracı rolünü incelemektir. Araştırmanın örneklemini Rehberlik ve Araştırma Merkezlerinde görevli küme örnekleme yöntemine göre belirlenen 428 öğretmen oluşturmaktadır. Çalışma ilişkisel tarama modeli üzerinden yürütülmüştür. Verilerinin toplanmasında, Okul Müdürü Güçlendirici Liderlik Ölçeği, Destekleyici Çalışma Ortamı Algısı Ölçeği ve Örgütsel Bağlılık Ölçeği kullanılmıştır. Değişkenler arasındaki ilişkileri belirlemek için korelasyon katsayıları (r) hesaplanmış; güçlendirici liderliğin ve destekleyici çalışma ortamı algısının örgütsel bağlılık üzerindeki doğrudan yordayıcılığı ile güçlendirici liderliğin destekleyici çalışma ortamı algısı aracılığında öğretmenlerin örgütsel bağlılığı üzerindeki dolaylı yordayıcılığını belirlemek için regresyon ve yol analizlerinden yararlanılmıştır. Araştırmanın sonuçlarına göre güçlendirici liderlik, öğretmenlerin destekleyici çalışma ortamı algılarının ve örgütsel bağlılıklarının pozitif ve anlamlı bir yordayıcısıdır. Bununla birlikte öğretmenlerin destekleyici çalışma ortamı algılarının örgütsel bağlılıklarını pozitif ve anlamlı bir biçimde yordadığı sonucuna da ulaşılmıştır. Aynı zamanda araştırmada, öğretmenlerin örgütsel bağlılığında güçlendirici liderlik yalnız başına toplam varyansın %27’sini açıklarken destekleyici çalışma ortamı algısının modele dâhil olması ile açıklanan varyans %37 çıkmıştır. Araştırmanın bulgularından yola çıkarak öğretmenlerin örgütsel bağlılığını artıran güçlendirici liderlik davranışlarının bu etkiyi öğretmenlerin destekleyici çalışma ortamına yönelik algıları üzerinden gerçekleştirdiği söylenebilir

    Handheld dermatoscopy as an easy-to-use capillaroscopic instrument in rheumatoid arthritis: A cross-sectional study

    Background/aim: Nailfold video capillaroscopy is considered as a reliable method for evaluating peripheral microangiopathy in rheumatologic diseases. In this study, we aimed to demonstrate the utility of handheld dermatoscopy as an easy-to-use nailfold capillaroscopic instrument in patients with rheumatoid arthritis. Materials and methods: This cross-sectional study included patients with rheumatoid arthritis and healthy subjects. A handheld dermatoscopic examination of proximal nail fold was performed in each subject. The possible correlation of capillaroscopic findings with disease activity was evaluated using the disease activity score 28 (DAS28). Results: A total of 59 patients with rheumatoid arthritis and 60 healthy subjects were enrolled in the study. The presence of capillary enlargement, avascular areas, capillary deformities, and capillary vascular anomalies in the group of patients showed a statistically significant difference when compared with the healthy subjects. No correlation was found between the nail fold capillaroscopic findings and DAS28 score. Conclusion: Hand-held dermatoscopy seems to be a useful technique in the evaluation of nail fold capillary changes. We suggest that in patients with rheumatoid arthritis, when capillaroscopic examination is needed, it can be evaluated using handheld dermatoscopy. Selected patients who showed findings using this method can be further examined with classical capillaroscopy to obtain more quantitative data. © TÜBİTAK

    Melanocytes as the source of the increased melanisation in pigmented epithelial tumours: a holistic approach

    Background/aim: We aimed to elucidate the causes of the increased melanisation in basal cell carcinoma (BCC) and seborrheic keratosis (SK), and the role of melanocytes in this process. Materials and methods: This study was a retrospective-cohort study conducted in the pathology department of a university hospital between January 2019 and October 2020. Forty-nine SK and 30 pigmented BCC were included in our study. SRY-box transcription factor 10 (SOX10), CD68, and Masson???Fontana staining was used for analysis in all samples. A representative section of each specimen was photographed under ??400 magnification to facilitate the assessments of the morphology of the melanocytes and their following morphometric parameters: density, nuclear diameter, and distribution. The density of pigmented keratinocytes in the lesional epidermis was scored. The nuclear diameters of melanocytes located in the nonlesional epidermis, the density of the melanophages, and the presence or absence of ulceration and solar elastosis were also recorded. Results: The morphometric findings confirmed a statistically significant increase in melanocyte density in the BCC group compared with that in the SK group (p < 0.001). Moreover, the nuclear minor diameters in the melanocytes of the BCC sections were significantly higher than those in the SK specimens (p < 0.001). The epidermal melanocytes were distributed diffusely in almost all BCC specimens (96.7%), whereas they were mainly limited to the basal layer in the majority of the SK sections (59.2%). The number of epidermal melanised keratinocytes with a score of 3 was significantly higher in the SK group (n = 31; 63.2%) than in the BCC group (n = 6; 20%) (p = 0.001), and they were the main cells representing the pigmented appearance of the tumours. No significant difference was found between both tumour groups in terms of their melanophage density scores (p = 0.206). Conclusion: This study is the first step towards an objective quantification of the melanocytes in pigmented epithelial tumours and may provide a morphological background for future studies on these skin lesions

    The prediction of decision self esteem and decision making styles by mindfulness

    Aim of this research is to investigate whether mindfulness predict decision self esteem and decision making styles. For this aim the sample consists of 597 university students (323 female and 274 male). Mindful Attention Awareness Scale (MAAS) (adapted into Turkish by Ozyesil, Arslan, Kesici and Deniz, 2011) and Melbourne Decision Making Questionnaire I-II (adapted into Turkish by Deniz, 2004) was used to collect data. The results show that there is a significant negative correlation between mindfulness and Buck passing (r=-.20, p<.001), procrastination (r=-.21, p<.001) and hyper vigilance (r=-.22, p<.001) –subscales of decision making styles-. Also it was found that mindfulness is a significant predictor of decision making styles. According to findings of study, 3.9% of variance in buck passing, 4.6% of variance in procrastination and 4.7% of variance in hyper vigilance –subscales of decision making styles- are explained by mindfulnes

    Objectives: This study aims to investigate the accuracy and effectiveness of blind and fluoroscopic-guided intra-articular shoulder injections in patients with shoulder pain.Patients and methods: The study included 17 patients (6 males, 11 females; mean age 52.6&plusmn;9.9 years; range 36 to 66 years) with shoulder pain more than three months. First intra-articular joint injections were performed with anterior approach blindly. Following the injection and after confirming that the needle tip was intra-articular with fluoroscopy and contrast distribution, the procedure was completed using 3 mL of local anesthetic (prilocaine and bupivacaine) and 1 mL of steroid (40 mg methylprednisolone). When the contrast distribution was observed to be extra-articular at the first administration, a second injection was continued under fluoroscopy guidance. All of the injections were intraarticular with the continuation of the procedure. Pain intensity was measured with visual analog scale (VAS).Results: According to the contrast distribution viewed with fluoroscopy, first blind injections were intra-articular in 11 of the 17 shoulders (64.7%). Mean of initial VAS score was 7.11. Improved pain was observed in the clinical follow-ups at the first hour (mean VAS: 2.35), third day (mean VAS: 2.64), and at the end of the first month (mean VAS: 2.23). The mean durations for blind and fluoroscopic-guided procedures excluding patients' preparation time were 0.8 minutes and 4.2 minutes, respectively.Conclusion: Although blind intra-articular shoulder injections are inexpensive and easily applicable, injections should be performed under fluoroscopy or another guide to ensure that the needle is intra-articular, not peri-articular.Amaç: Bu çalışmada omuz ağrısı olan hastalarda kör ve floroskopi rehberli eklem içi omuz enjeksiyonlarının doğruluğu ve etkinliği araştırıldı.Hastalar ve yöntemler: Çalışmaya üç aydan uzun süredir omuz ağrısı olan 17 hasta (6 erkek, 11 kadın; ort. yaş 52.6±9.9 yıl; dağılım 36-66 yıl) dahil edildi. İlk eklem içi enjeksiyonlar ön yaklaşım ile kör olarak uygulandı. Enjeksiyon sonrası iğne ucunun eklem içerisinde olduğu floroskopi ve kontrast dağılımı ile doğrulandıktan sonra işlem 3 mL lokal anestezik (prilokain ve bupivakain) ve 1 mL steroid (40 mg metilprednizolon) ile tamamlandı. İlk uygulamada kontrast dağılımının eklem dışı olduğu gözlendiğinde, ikinci enjeksiyona floroskopi eşliğinde devam edildi. İşlemin devam ettirilmesi ile tüm enjeksiyonlar eklem içi oldu. Ağrı yoğunluğu görsel analog ölçeği (GAÖ) ile ölçüldü.Bulgular: Floroskopi ile bakılan kontrast dağılımına göre, 17 omzun 11'inde (%64.7) birinci kör enjeksiyonlar eklem içi idi. Başlangıç GAÖ skoru ortalaması 7.11 idi. Klinik takiplerde birinci saatte (ortalama GAÖ: 2.35), üçüncü günde (ortalama GAÖ: 2.64) ve birinci ayın sonunda (ortalama GAÖ: 2.23) ağrıda iyileşme gözlendi. Kör ve floroskopi rehberli uygulama için hasta hazırlanma süresi dışındaki ortalama süre sırasıyla 0.8 dakika ve 4.2 dakika idi.Sonuç: Kör eklem içi omuz enjeksiyonları ucuz ve kolay uygulanabilir olsa da iğnenin eklem çevresinde değil eklem içinde olduğundan emin olmak için enjeksiyonlar floroskopi ya da başka bir rehber eşliğinde yapılmalıdır

In this work, we derive the analytical formulation of the channel model for diffusion dominated movement, considering ring-shaped (i.e., patch) observing receivers and Poiseuille flow with the aim of localization of the transmitter cell. Then, we derive formulations using this channel model for two different application scenarios. We assume that the emission start time is known in the first scenario, and unknown in the second one. We successfully localize the transmitter cell using a single receiver for the first scenario, whereas two receivers are used to localize the transmitter cell in the second scenario.     Protective effects of cordycepin on the histopathological changes and oxidative stress induced by hepatic ischemia/reperfusion in rats

    Aim: To investigate the effects of cordycepin on the histopathological changes and oxidative stress induced by hepatic ischemia/reperfusion (I/R) in rats. Method: Forty male Wistar rats were randomly divided into 4 groups as group I (sham, n=10), group II (control, n=10), group III (I/R-untreated, n=10) and group IV (I/R-cordycepin, n=10). Liver ischemia was induced for 30 min then reperfusion was allowed for 1 h. At the end of the experiment, liver specimens and blood samples were taken for histopathological and antioxidant evaluations, and biochemical analysis. Results: The levels of IL6, IL-1β, and TNFα in the serum and liver tissues were higher in the I/R-untreated group compared to the I/R-cordycepin treated group. In the I/R-cordycepin group, serum MDA levels were decreased compared with the I/R-untreated group. The I/R-cordycepin treated group showed an increase in TAS levels, and a decrease in TOS levels compared with I/R-untreated group. The histopathological injury score were significantly lower in the I/R-cordycepin treated group compared to the I/R-untreated group.&nbsp; In the I/R-untreated group, the integrity of the hepatocyte cell lines deteriorated. Mononuclear inflammatory cells infiltrated the parenchyma regions, the sinuses dilated and there was diffuse congestion Preoperative treatment with cordycepine reduced histopathological abnormalities. Conclusion: Cordycepin has demonstrated significant hepatoprotective effects against I/R injury induced in rats through TAS elevation and reduction of TOS, MDA and proinflammatory cytokines

    Syndrome of Inappropriate Secretion of Antidiuretic Hormone Cholestasis and Pericardial Effusion Due to Brucellosis Infection: A Case Report

    Syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is an extremely rare complication of infectious diseases. A rare case of brucellosis complicated by syndrome of inappropriate secretion of antidiuretic hormone (SIADH) cholestasis and pericardial involvement is reported. A 27-year-old woman was admitted for fever, abdominal pain, and scleral icterus. Her medical history revealed no recent use of diuretic agents. In addition to cholestasis and elevated liver enzymes, euvolemic hyponatremia, hypouricemia, low plasma osmolality, and high urinary osmolality were also detected. Surrenal and thyroid tests were also within normal range. Echocardiography revealed minimal pericardial effusion with normal cardiac functions. The final diagnosis was SIADH due to Brucellosis. Hyponatremia, cholestasis, and pericardial disease were resolved with effective antibrucellar treatment with streptomycine and doxycycline. After completing treatment of brucellosis, there was not any more evidence of cholestasis and pericardial fluid

    Performans Esaslı Bütçeleme Sistemi (PEB), modern bütçe sistemlerinin gelişim sürecinde ulaşılan en son tekniklerden biridir. Sistem özü itibariyle; kamu kurumlarının piyasa odaklı bir yönetim anlayışına uygun bütçeler hazırlayabilmeleri için öncelikle vizyon ve misyonlarını içeren bir stratejik plan hazırlamalarını ve uygulayacakları bütçelerde gerçekleştirecekleri faaliyetlere en uygun düşecek ödeneğin tahsisini gerekli kılmaktadır. Bu çalışmada ilk olarak; PEB'e yönelik bir bütçe sisteminde gerekli adımları ortaya koymak amacıyla, kritik kontrol noktaları tespit dilmiştir. Daha sonra Türkiye'deki çeşitli kamu kurumlarındaki PEB sistemine yönelik mevcut durum analizi sonucunda kurumların belirlenen kritik kontrol noktalarına göre yürüttükleri faaliyet düzeyleri elde edilmiştir. Kritik kontrol noktalarından ve Türkiye'deki kamu kurumlarının analizinden elde edilen sonuçlar çerçevesinde PEB'e yönelik tüm adımlar tamamlanmadan sistemin etkin şekilde uygulanamayacağı genel sonucuna ulaşılmıştır
