    ESTIMATION OF H-p(3) AMONG STAFF MEMBERS IN TWO NUCLEAR MEDICINE UNITS IN FINLAND

    The eye lens exposure among 16 technicians in two nuclear medicine departments at university hospitals in Finland was investigated by measuring the operational quantity H-p(3) using EYE-D dosemeters. For all workers, the annual mean H-p(3) was estimated to be 1.1 mSv (max. 3.9 mSv). The relation between H-p(3) to routinely monitored personal dose equivalent H-p(10) was clearly correlated. Considering individual dose measurement periods (2-4 weeks), the H-p(3)/H-p(10) ratio was 0.7 (Pearson's coefficient r = 0.90, p 0.1 mSv vs. <0.1 mSv, respectively), i.e. higher Hp(10) predicts H-p(3) more reliably. Moreover, annual H-p(10) data from national dose register during 2009-2018 were used to derive the annual H-p(3) applying the H-p(3)/H-p(10) ratio. The data from Finnish nuclear medicine departments imply that routine measurements of H-p(3) among nuclear medicine technicians are not justified.Peer reviewe

    Ventilation during continuous compressions or at 30:2 compression-to-ventilation ratio results in similar arterial oxygen and carbon dioxide levels in an experimental model of prolonged cardiac arrest

    Background: In refractory out-of-hospital cardiac arrest, transportation to hospital with continuous chest compressions (CCC) from a chest compression device and ventilation with 100% oxygen through an advanced airway is common practice. Despite this, many patients are hypoxic and hypercapnic on arrival, possibly related to suboptimal ventilation due to the counterpressure caused by the CCC. We hypothesized that a compression/ventilation ratio of 30:2 would provide better ventilation and gas exchange compared to asynchronous CCC during prolonged experimental cardiopulmonary resuscitation (CPR).Methods: We randomized 30 anaesthetized domestic swine (weight approximately 50 kg) with electrically induced ventricular fibrillation to the CCC or 30:2 group and bag-valve ventilation with a fraction of inspired oxygen (FiO(2)) of 100%. We started CPR after a 5-min no-flow period and continued until 40 min from the induction of ventricular fibrillation. Chest compressions were performed with a Stryker Medical LUCAS (R) 2 mechanical chest compression device. We collected arterial blood gas samples every 5 min during the CPR, measured ventilation distribution during the CPR using electrical impedance tomography (EIT) and analysed post-mortem computed tomography (CT) scans for differences in lung aeration status.Results: The median (interquartile range [IQR]) partial pressure of oxygen (PaO2) at 30 min was 110 (52-117) mmHg for the 30:2 group and 70 (40-171) mmHg for the CCC group. The median (IQR) partial pressure of carbon dioxide (PaCO2) at 30 min was 70 (45-85) mmHg for the 30:2 group and 68 (42-84) mmHg for the CCC group. No statistically significant differences between the groups in PaO2 (p =0.40), PaCO2 (p = 0.79), lactate (p = 0.37), mean arterial pressure (MAP) (p = 0.47) or EtCO2 (p = 0.19) analysed with a linear mixed model were found. We found a deteriorating trend in PaO2, EtCO2 and MAP and rising PaCO2 and lactate levels through the intervention. There were no differences between the groups in the distribution of ventilation in the EIT data or the post-mortem CT findings.Conclusions: The 30:2 and CCC protocols resulted in similar gas exchange and lung pathology in an experimental prolonged mechanical CPR model.Peer reviewe

    What is known about the health and living conditions of the indigenous people of northern Scandinavia, the Sami?

    The Sami are the indigenous ethnic population of northern Scandinavia. Their health condition is poorly known, although the knowledge has improved over the last decade.The aim was to review the current information on mortality, diseases, and risk factor exposure in the Swedish Sami population.Health-related research on Sami cohorts published in scientific journals and anthologies was used to compare the health condition among the Sami and the majority non-Sami population. When relevant, data from the Sami populations in Swedish were compared with corresponding data from Norwegian and Finnish Sami populations.Life expectancy and mortality patterns of the Sami are similar to those of the majority population. Small differences in incidences of cancer and cardiovascular diseases have been reported. The traditional Sami lifestyle seems to contain elements that reduce the risk to develop cancer and cardiovascular diseases, e.g. physical activity, diet rich in antioxidants and unsaturated fatty acids, and a strong cultural identity. Reindeer herding is an important cultural activity among the Sami and is associated with high risks for accidents. Pain in the lower back, neck, shoulders, elbows, and hands are frequent among both men and women in reindeer-herding families. For men, these symptoms are related to high exposure to terrain vehicles, particularly snowmobile, whereas for women psychosocial risk factors seem to more important, e.g. poor social support, high effort, low reward, and high economical responsibilities.Although the health condition of the Sami population appears to be rather similar to that of the general Swedish population, a number of specific health problems have been identified, especially among the reindeer-herding Sami. Most of these problems have their origin in marginalization and poor knowledge of the reindeer husbandry and the Sami culture in the majority population. It is suggested that the most sustainable measure to improve the health among the reindeer-herding Sami would be to improve the conditions of the reindeer husbandry and the Sami culture

    Non-tuberculous Mycobacteria can Cause Disseminated Mycobacteriosis in Cats

    Mycobacteriosis caused by non-tuberculous mycobacteria (NTM) is a rising concern in human medicine both in immunocompromised and immunocompetent patients. In cats, mycobacteriosis caused by NTM is considered mostly to be a focal or dermal infection, with disseminated disease mostly caused by Mycobacterium avium. We describe three cases of disseminated mycobacteriosis in cats, caused by Mycobacterium malmoense, Mycobacterium branderi/shimoidei and M. avium, with no identified underlying immunosuppression. In all cases, extracellular mycobacteria were seen in the pulmonary epithelium, intestinal lumen and glomerular tufts, which could affect the shedding of the organism. The present study highlights the importance of mycobacteriosis as a differential even in immunocompetent animals. Considering the close relationship of owners and pets and the potential presence of free mycobacteria in secretions, cats should be considered as a possible environmental reservoir for mycobacteria.     An assessment of data sources, data quality and changes in national forest monitoring capacities in the Global Forest Resources Assessment 2005-2020

    Globally, countries report forest information to the Food and Agriculture Organization of the United Nations (FAO) Global Forest Resources Assessments (FRA) at regular intervals. While the status and trends of national forest monitoring capacities have been previously assessed for the tropics, this has not been systematically done worldwide. In this paper, we assess the use and quality of forest monitoring data sources for national reporting to the FRA in 236 countries and territories. More specifically, we (1) analyze the use of Remote Sensing (RS) for forest area monitoring and the use of National Forest Inventory (NFI) for monitoring forest area, growing stock, biomass, carbon stock, and other attributes in FRA 2005-2020, (2) assess data quality in FRA 2020 using FAO Tier-based indicators, and (3) zoom in to investigate changes in tropical forest monitoring capacities in FRA 2010 - 2020. Globally, the number of countries monitoring forest area using RS at good to very good capacities increased from 55 in FRA 2005 to 99 in FRA 2020. Likewise, the number of countries with good to very good NFI capacities increased from 48 in FRA 2005 to 102 in FRA 2020. This corresponds to ~85% of the global forest area monitored with one or more nationally-produced up-to-date RS products or NFI in FRA 2020. For large proportions of global forests, the highest quality data was used in FRA 2020 for reporting on forest area (93%), growing stock (85% ), biomass (76%), and carbon pools (61%). Overall, capacity improvements are more widespread in the tropics, which can be linked to continued international investments for forest monitoring especially in the context of Reducing Emissions from Deforestation and Forest Degradation in tropical countries (REDD+). More than 50% of the tropical countries with targeted international support improved both RS and NFI capacities in the period 2010-2020 on top of those that already had persistent good to very good capabilities. There is also a link between improvements in national capacities and improved governance measured against Worldwide Governance Indicators (WGI). Our findings – the first global study – suggest an ever-improving data basis for national reporting on forest resources in the context of climate and development commitments, e.g. the Paris Agreement and Sustainable Development Goals

    Characterization of low-density granulocytes in COVID-19

    Author summary The emergence of SARS-COV-2 and the ensuing COVID-19 disease has revealed an unprecedented need to understand the pathological mechanisms of acute respiratory infections in more detail. Granulocytes are highly abundant cells of the innate immunity, and thus first responders towards acute infections. However, their excessive activation can cause unwanted tissue damage and detrimental effects in humans. This study identifies a population of low-density granulocytes (LDGs) in COVID-19 patient samples, which has been poorly described in the context of acute infections so far. These cells were subclassified and found to be mainly of immature phenotypes. Further characterization revealed COVID-19 LDGs as a phenotypically diverse population with immunosuppressive characteristics, which seemed to be in line with an elevated recruitment and activation of granulocytes. Altogether, these findings suggest LDG may play a role in COVID-19 disease progression. Severe COVID-19 is characterized by extensive pulmonary complications, to which host immune responses are believed to play a role. As the major arm of innate immunity, neutrophils are one of the first cells recruited to the site of infection where their excessive activation can contribute to lung pathology. Low-density granulocytes (LDGs) are circulating neutrophils, whose numbers increase in some autoimmune diseases and cancer, but are poorly characterized in acute viral infections. Using flow cytometry, we detected a significant increase of LDGs in the blood of acute COVID-19 patients, compared to healthy controls. Based on their surface marker expression, COVID-19-related LDGs exhibit four different populations, which display distinctive stages of granulocytic development and most likely reflect emergency myelopoiesis. Moreover, COVID-19 LDGs show a link with an elevated recruitment and activation of neutrophils. Functional assays demonstrated the immunosuppressive capacities of these cells, which might contribute to impaired lymphocyte responses during acute disease. Taken together, our data confirms a significant granulocyte activation during COVID-19 and suggests that granulocytes of lower density play a role in disease progression.Peer reviewe

    Educational effects of early or later secondary school tracking in Germany

    This paper examines educational outcomes of pupils selected to secondary school types by different tracking regimes in a German state: Pupils are alternatively streamed after fourth grade or after sixth grade. Regression results indicate that, estimated on the mean, there are no negative effects of later tracking on educational outcomes in the middle of secondary school. Positive effects are observed for pupils with a less favorable family background. Quantile regressions reveal that the estimated effects of later tracking are positive for the lower quantiles but decrease monotonically over the conditional distribution of test scores
