    Investigation of trigger factors and clinical characteristics in children with asthma

    AMAÇ: Astımlı çocuklarda tetikleyici faktörler ve klinik özellikleri incelemek amacıyla yapıldı. GEREÇ ve YÖNTEM: Tanımlayıcı nitelikteki çalıþmanın örneklemini ıstanbul'da bir Eğitim Araþtırma Hastanesinin, Çocuk Alerji ve ımmünoloji Polikliniğinde astım hastalığı nedeniyle takip edilen 106 astımlı çocuk oluþturdu. BULGULAR: Çocuklarda astımı tetikleyen etkenlerin sıklıkla ev tozu akarları, polenler ve evde sigara içilmesi olduğu görüldü. Çocuklarda solunum güçlüğü belirtilerinin sıklıkla koþtuğunda, üst solunum yolu enfeksiyonları geçirdiğinde ve gece görüldüğü saptandı. Çocukların %56,7'sinde son bir aylık dönemde gündüz haftada 1'den az astım belirtileri olduğu, %63,2'sinde belirtilerin gece ayda 2'den az olduğu ve %52,8'inin son 1 ayda astım belirtileri ile uyandığı görüldü. Astımda sık görülen klinik belirtilerin, gündüz geceden daha çok olduğu belirlendi. SONUÇ: Klinik belirtilerin bilinenin aksine gündüz daha çok bildirilmesi ve düzenli ilaç kullanan çocuk sayısının düþük olması, ailelerin astım kliniğini iyi bilmediğini veya yeterince izlemediğini düþündürdü.OBJECTIVE: This study aimed investigating of trigger factors and the clinical characteristics of children with asthma. MATERIALS and METHODS: The sample of this descriptive study consisted of 106 children with asthma attending the Child Allergy and Immunology Clinic of a Training and Research Hospital in Istanbul. RESULTS: It has been showed that factors triggering asthma in children were mite, pollens and smoking at home. It has been determined that children experienced symptoms of difficulty in breathing at night when they ran or had upper respiratory tract infections. 56,7% of children had less than one asthma symptom during the day per week in the last month, 63,2% had less than two symptoms at night per month, and 52,8% had awoken because of asthma symptoms in the last month. Common clinical symptoms of asthma were more prevalent during the day compared to night. CONCLUSION: Because the clinical symptoms at day were reported more frequently, which is in contrast with common knowledge, and the low number of child who take their medication regularly, we concluded that families did not have adequate information on the clinical presentation of asthma or followed the symptoms accurately.     Bladder Wall Telangiectasia in a Patient with Ataxia-Telangiectasia and How to Manage?

    Ataxia-telangiectasia (A-T) is a rare neurodegenerative, inherited disease causing severe morbidity. Oculocutaneous telangiectasias are almost constant findings among the affected cases as telangiectasia is considered the main clinical finding for diagnosis. Vascular abnormalities in organs have been reported infrequently but bladder wall telangiectasias are extremely rare. We aimed to report recurrent hemorrhage from bladder wall telangiectasia in a 9-year-old boy with A-T who had received intravenous cyclophosphamide for non-Hodgkin’s lymphoma. Since A-T patients are known to be more susceptible to chemical agents, we suggested that possibly cyclophosphamide was the drug which induced bladder wall injury in this patient

    Evaluation of the health-related quality of life in Turkish cystic fibrosis patients

    Background Prolongation of life expectancy in cystic fibrosis (CF) to the fourth decade makes health-related quality of life (HRQOL) an important issue in this population. This study aims to evaluate the effects of clinical and socio-demographic characteristics of patients on different aspects of HRQOL in a pediatric CF population. We also intend to assess some measures of reliability and validity of the Turkish CF Questionnaire-Revised (CFQ-R) forms. Methods Fifty-four CF patients and 46 parents / caregivers filled the appropriate CFQ-R forms. Disease severity was evaluated via modified Shwachman-Kulczycki (SK) scores, and additional clinical information was obtained from patient records. Results Forty-three percent (n = 23) of patients were female, the mean age of patients being 13.3 +/- 3.96 years. Self- and proxy-reported points showed a strong correlation (Spearman's rho > 0.6) on more observable domains of HRQOL (e.g. physical functioning). No significant difference was found between sexes on any HRQOL domain. When compared between age groups, treatment burden appeared to worsen with age, while social functioning improved. All three forms of the CFQ-R Turkish version showed acceptable internal consistency reliability and convergence with disease severity measures. Conclusions The CFQ-R showed a fair correlation with the Shwachman-Kulczycki score and the FEV1% value, which makes its longitudinal use possible in clinical practice. It is important to use self-reported evaluations as well as parent / caregiver-reported HRQOL evaluations, especially in young children. More studies are needed to assess the psychometric properties of Turkish CFQ-R forms
