    Computational neurorehabilitation: modeling plasticity and learning to predict recovery

    Despite progress in using computational approaches to inform medicine and neuroscience in the last 30 years, there have been few attempts to model the mechanisms underlying sensorimotor rehabilitation. We argue that a fundamental understanding of neurologic recovery, and as a result accurate predictions at the individual level, will be facilitated by developing computational models of the salient neural processes, including plasticity and learning systems of the brain, and integrating them into a context specific to rehabilitation. Here, we therefore discuss Computational Neurorehabilitation, a newly emerging field aimed at modeling plasticity and motor learning to understand and improve movement recovery of individuals with neurologic impairment. We first explain how the emergence of robotics and wearable sensors for rehabilitation is providing data that make development and testing of such models increasingly feasible. We then review key aspects of plasticity and motor learning that such models will incorporate. We proceed by discussing how computational neurorehabilitation models relate to the current benchmark in rehabilitation modeling – regression-based, prognostic modeling. We then critically discuss the first computational neurorehabilitation models, which have primarily focused on modeling rehabilitation of the upper extremity after stroke, and show how even simple models have produced novel ideas for future investigation. Finally, we conclude with key directions for future research, anticipating that soon we will see the emergence of mechanistic models of motor recovery that are informed by clinical imaging results and driven by the actual movement content of rehabilitation therapy as well as wearable sensor-based records of daily activity

    Combining brain-computer interfaces and assistive technologies: state-of-the-art and challenges

    In recent years, new research has brought the field of EEG-based Brain-Computer Interfacing (BCI) out of its infancy and into a phase of relative maturity through many demonstrated prototypes such as brain-controlled wheelchairs, keyboards, and computer games. With this proof-of-concept phase in the past, the time is now ripe to focus on the development of practical BCI technologies that can be brought out of the lab and into real-world applications. In particular, we focus on the prospect of improving the lives of countless disabled individuals through a combination of BCI technology with existing assistive technologies (AT). In pursuit of more practical BCIs for use outside of the lab, in this paper, we identify four application areas where disabled individuals could greatly benefit from advancements in BCI technology, namely,“Communication and Control”, “Motor Substitution”, “Entertainment”, and “Motor Recovery”. We review the current state of the art and possible future developments, while discussing the main research issues in these four areas. In particular, we expect the most progress in the development of technologies such as hybrid BCI architectures, user-machine adaptation algorithms, the exploitation of users’ mental states for BCI reliability and confidence measures, the incorporation of principles in human-computer interaction (HCI) to improve BCI usability, and the development of novel BCI technology including better EEG devices

    Classification of functional and non-functional arm use by inertial measurement units in individuals with upper limb impairment after stroke

    Background: Arm use metrics derived from wrist-mounted movement sensors are widely used to quantify the upper limb performance in real-life conditions of individuals with stroke throughout motor recovery. The calculation of real-world use metrics, such as arm use duration and laterality preferences, relies on accurately identifying functional movements. Hence, classifying upper limb activity into functional and non-functional classes is paramount. Acceleration thresholds are conventionally used to distinguish these classes. However, these methods are challenged by the high inter and intra-individual variability of movement patterns. In this study, we developed and validated a machine learning classifier for this task and compared it to methods using conventional and optimal thresholds. Methods: Individuals after stroke were video-recorded in their home environment performing semi-naturalistic daily tasks while wearing wrist-mounted inertial measurement units. Data were labeled frame-by-frame following the Taxonomy of Functional Upper Limb Motion definitions, excluding whole-body movements, and sequenced into 1-s epochs. Actigraph counts were computed, and an optimal threshold for functional movement was determined by receiver operating characteristic curve analyses on group and individual levels. A logistic regression classifier was trained on the same labels using time and frequency domain features. Performance measures were compared between all classification methods. Results: Video data (6.5 h) of 14 individuals with mild-to-severe upper limb impairment were labeled. Optimal activity count thresholds were ≥20.1 for the affected side and ≥38.6 for the unaffected side and showed high predictive power with an area under the curve (95% CI) of 0.88 (0.87,0.89) and 0.86 (0.85, 0.87), respectively. A classification accuracy of around 80% was equivalent to the optimal threshold and machine learning methods and outperformed the conventional threshold by ∼10%. Optimal thresholds and machine learning methods showed superior specificity (75-82%) to conventional thresholds (58-66%) across unilateral and bilateral activities. Conclusion: This work compares the validity of methods classifying stroke survivors' real-life arm activities measured by wrist-worn sensors excluding whole-body movements. The determined optimal thresholds and machine learning classifiers achieved an equivalent accuracy and higher specificity than conventional thresholds. Our open-sourced classifier or optimal thresholds should be used to specify the intensity and duration of arm use

    Computational Analysis of Upper Extremity Movements for People Post-Stroke

    Wearable sensors have been beneficial in assessing motor impairment after stroke. Individuals who have experienced stroke may benefit from the use of wearable sensors to quantify and assess quality of motions in unobserved environments. Seven individuals participated in a study wherein they performed various gestures from the Fugl-Meyer Assessment (FMA), a measure of post-stroke impairment. Participants performed these gestures while being monitored by wearable sensors placed on each wrist. A series of MATLAB functions were written to process recorded sensor data, extract meaningful features from the data, and prepare those features for further use with various machine learning techniques. A combination of linear and nonlinear regression was applied to frequency domain values from each gesture to determine which can more accurately predict the time spent performing the gesture, and the associated gesture FMA score. General performance suggests that linear regression techniques appear to better fit paretic gestures, while nonlinear regression techniques appear to better fit non-paretic gestures. A use of classifier techniques were used to determine if a classifier can distinguish between paretic and non-paretic gestures. The combinations include determining if a higher performance is obtained through the use of either accelerometer, rate gyroscope, or both modalities combined. Our findings indicate that, for upper-extremity motion, classifiers trained using a combination of accelerometer and rate gyroscope data performed the best (accuracy of 73.1%). Classifiers trained using accelerometer data alone and rate gyroscope data alone performed slightly worse than the combined data classifier (70.2% and 65.7%, respectively). These results suggest specific features and methods suitable for the quantification of impairment after stroke

    Development of a Unique Whole-Brain Model for Upper Extremity Neuroprosthetic Control

    Neuroprostheses are at the forefront of upper extremity function restoration. However, contemporary controllers of these neuroprostheses do not adequately address the natural brain strategies related to planning, execution and mediation of upper extremity movements. These lead to restrictions in providing complete and lasting restoration of function. This dissertation develops a novel whole-brain model of neuronal activation with the goal of providing a robust platform for an improved upper extremity neuroprosthetic controller. Experiments (N=36 total) used goal-oriented upper extremity movements with real-world objects in an MRI scanner while measuring brain activation during functional magnetic resonance imaging (fMRI). The resulting data was used to understand neuromotor strategies using brain anatomical and temporal activation patterns. The study\u27s fMRI paradigm is unique and the use of goal-oriented movements and real-world objects are crucial to providing accurate information about motor task strategy and cortical representation of reaching and grasping. Results are used to develop a novel whole-brain model using a machine learning algorithm. When tested on human subject data, it was determined that the model was able to accurately distinguish functional motor tasks with no prior knowledge. The proof of concept model created in this work should lead to improved prostheses for the treatment of chronic upper extremity physical dysfunction

    The ENIGMA Stroke Recovery Working Group: Big data neuroimaging to study brain–behavior relationships after stroke

    The goal of the Enhancing Neuroimaging Genetics through Meta‐Analysis (ENIGMA) Stroke Recovery working group is to understand brain and behavior relationships using well‐powered meta‐ and mega‐analytic approaches. ENIGMA Stroke Recovery has data from over 2,100 stroke patients collected across 39 research studies and 10 countries around the world, comprising the largest multisite retrospective stroke data collaboration to date. This article outlines the efforts taken by the ENIGMA Stroke Recovery working group to develop neuroinformatics protocols and methods to manage multisite stroke brain magnetic resonance imaging, behavioral and demographics data. Specifically, the processes for scalable data intake and preprocessing, multisite data harmonization, and large‐scale stroke lesion analysis are described, and challenges unique to this type of big data collaboration in stroke research are discussed. Finally, future directions and limitations, as well as recommendations for improved data harmonization through prospective data collection and data management, are provided

    Enhancing Nervous System Recovery through Neurobiologics, Neural Interface Training, and Neurorehabilitation.

    After an initial period of recovery, human neurological injury has long been thought to be static. In order to improve quality of life for those suffering from stroke, spinal cord injury, or traumatic brain injury, researchers have been working to restore the nervous system and reduce neurological deficits through a number of mechanisms. For example, neurobiologists have been identifying and manipulating components of the intra- and extracellular milieu to alter the regenerative potential of neurons, neuro-engineers have been producing brain-machine and neural interfaces that circumvent lesions to restore functionality, and neurorehabilitation experts have been developing new ways to revitalize the nervous system even in chronic disease. While each of these areas holds promise, their individual paths to clinical relevance remain difficult. Nonetheless, these methods are now able to synergistically enhance recovery of native motor function to levels which were previously believed to be impossible. Furthermore, such recovery can even persist after training, and for the first time there is evidence of functional axonal regrowth and rewiring in the central nervous system of animal models. To attain this type of regeneration, rehabilitation paradigms that pair cortically-based intent with activation of affected circuits and positive neurofeedback appear to be required-a phenomenon which raises new and far reaching questions about the underlying relationship between conscious action and neural repair. For this reason, we argue that multi-modal therapy will be necessary to facilitate a truly robust recovery, and that the success of investigational microscopic techniques may depend on their integration into macroscopic frameworks that include task-based neurorehabilitation. We further identify critical components of future neural repair strategies and explore the most updated knowledge, progress, and challenges in the fields of cellular neuronal repair, neural interfacing, and neurorehabilitation, all with the goal of better understanding neurological injury and how to improve recovery

    Developing Predictive Models for Upper Extremity Post–Stroke Motion Quality Estimation Using Decision Trees and Bagging Forest

    Stroke is one of the leading causes of long–term disability. Approximately twothirds of stroke survivors require long-term rehabilitation, which suggests the importance of understanding the post-stroke recovery process during his activities of daily living. This problem is formulated as quantifying and estimating the poststroke movement quality in real world settings. To address this need, we have developed an approach that quantifies physical activities and can evaluate the performance quality. Wearable accelerometer and gyroscope are used to measure the upper extremity motions and to develop a mathematical framework to objectively relates sensors’ data to clinical performance indices. In this article we employ two machine learning classification methods, Bootstrap Aggregating (Bagging) Forest and Decision Tree (DT), to relate the post-stroke kinematic data to quality of the corresponding motion. We then compare the accuracy of the resulted two prediction models using cross-validation approaches. Our findings indicate that Bagging forest approach is superior to the computationally simpler DTs for unstable data sets including those derived from stroke survivors in this project

    Restoring brain function after stroke - bridging the gap between animals and humans

    Stroke is the leading cause of complex adult disability in the world. Recovery from stroke is often incomplete, which leaves many people dependent on others for their care. The improvement of long-term outcomes should, therefore, be a clinical and research priority. As a result of advances in our understanding of the biological mechanisms involved in recovery and repair after stroke, therapeutic opportunities to promote recovery through manipulation of poststroke plasticity have never been greater. This work has almost exclusively been carried out in preclinical animal models of stroke with little translation into human studies. The challenge ahead is to develop a mechanistic understanding of recovery from stroke in humans. Advances in neuroimaging techniques now enable us to reconcile behavioural accounts of recovery with molecular and cellular changes. Consequently, clinical trials can be designed in a stratified manner that takes into account when an intervention should be delivered and who is most likely to benefit. This approach is expected to lead to a substantial change in how restorative therapeutic strategies are delivered in patients after stroke
