The counterfeiting of medicinal products is a growing concern for patients, the pharmaceutical industry and national policy-makers worldwide. EU internal market rules for medicinal products for human use coupled by wide-ranging International cooperation, and proposed European legislative reforms are hoped to have a positive impact against the problem of counterfeit medicines - one of the greatest current threats to public health and safety.peer-reviewe

Clinical practice: Drug desensitization in children

    Get PDF
Immediate type allergic reactions to medication are potentially life threatening and can hamper drug therapy of several medical conditions. Exact incidence and prevalence data for these reactions in children are lacking. If no alternative drug treatment is available, a desensitization procedure may secure the continuation of necessary therapy. Desensitization is only appropriate in case of a strong suspicion of an IgE-mediated allergic reaction. It should be performed by trained clinicians (allergy specialists) in a hospital setting where treatment of a potential anaphylactic reaction can be done without any delay. In this article, literature describing desensitization procedures for several antibiotics, antineoplastic agents, and vaccines in children is reviewed. In general, desensitization schemes for children differ only in final dose from schemes for adults. Contradictory data were found regarding the protective effects of premedication with antihistamines and glucocorticoids

Hiccup as an uncommon symptom of pneumonia Paulina Terlecka1, Anna Grzywa-Celińska1, Justyna Emeryk-Maksymiuk2, Katarzyna Szmygin-Milanowska1, Janusz Milanowski1 1 - Chair and Department of Pneumonology, Oncology and Allergology, Medical University of Lublin, Poland 2 - Chair of Internal Medicine and Department of Internal Medicine in Nursing, Medical University of Lublin, Poland Corresponding author: Paulina Terlecka, MD Chair and Department of Pneumonology, Oncology and Allergology Medical University of Lublin 20-954 Lublin, Poland 8 Jaczewskiego St. phone number: +48817244431 fax number: +48817244823 e-mail: [email protected] Abstract Hiccup is a symptom resulting with uncontrolled contractions of the diaphragm and external intercostals muscles with inspiration and with simultaneous sudden closure of the glottis, that produces specific “hic” sound. Depending on the time of duration, hiccup can be classified as acute (up to 48h), chronic (48h to 1 month) and persistent (lasting longer than one month). The exact pathophysiology of this symptom still remains unknown, though it is well known, that short-term episodes of hiccup are mainly caused by oesophagus and stomach diseases (gastric and duodenal ulcers, gastritis, gastro-oesophageal reflux) or alcohol consumption. Other reasons of this phenomenon include the diseases with the stimulation of the vagus nerve or conditions with diaphragm irritation. It can be also psychogenic or resulting from central nervous system diseases. Short term episodes of hiccup happen to every person and do not cause any concern. Chronic hiccups can lead to significant worsening of quality of life including fatigue, nutritional restrictions, dehydration, weight loss, insomnia, respiratory disorders, depression or even suicidal thoughts. We present an interesting case with diagnostic and therapeutic procedures in a 65-year-old smoker, who was hospitalized in our Department because of pneumonia, with persistent hiccup as an additional complaint considerably deteriorating his quality of life. Key words: hiccup, symptom, pneumonia

EARLY EDUCATION OF ACUTE KIDNEY INJURY AND PRE-RENAL PROBLEMS IN ANAPHYLACTIC SHOCK PATIENTS

Acute kidney injury (AKI) may manifest in patients experiencing anaphylactic shock. Arterial hypotension significantly contributes to the pre-renal etiology of AKI. The condition of anaphylactic shock is a life-threatening situation that requires immediate intervention. The decrease in perfusion to the renal organs in patients with acute anaphylactic shock can lead to the development of acute kidney injury (AKI). This is characterized by an increase in the Urine (U) and Creatinine (Cr) ratio (>20:1) and is reversible if promptly managed through fluid resuscitation, which results in a decrease in serum creatinine levels. Anaphylactic shock can lead to the occurrence of pre-renal acute kidney injury (AKI), which can be reversible if promptly addressed

When alarm bells ring: emergency tinnitus

    Get PDF
OBJECTIVE: The aim of this study is to develop a diagnostic-therapeutic algorithm for those suffering from tinnitus who seek emergency aid. MATERIALS AND METHODS: A literature review has been performed on articles from the last 30 years. RESULTS: It is important to activate medical or surgical diagnostic and therapeutic strategies, in order to safeguard and rehabilitate the various functions affected. Psychiatric comorbidity is the most frequent pathological condition of those with serious or catastrophic tinnitus. In these cases, mortality risk is linked to suicide, morbidity to tinnitus-correlated distress. CONCLUSIONS: Tinnitus, mainly linked to loss of hearing, is a frequent symptom among the population at large. About 7% of those affected by tinnitus turn to their doctor to solve their problem, while between 0.5 and 2% request urgent medical assistance. Their cry for help may be the result of an acute onset of tinnitus or the rapid impairment of an already chronic condition. Tinnitus is not considered an urgent ear, nose and throat (ENT) condition by the Associazione Otorinolaringologi Ospedalieri Italiani (AOOI) [Italian Association of Hospital ENT], even though there are many pathological conditions, sometimes serious, associated with tinnitus and emergency action is necessary to reduce the risk of morbidity and mortality

Assessment Of Pictogram Use In Pediatric Oral Liquid Medication Labeling At Sultanah Bahiyah Hospital, Alor Setar, Kedah, Malaysia

Liquids are commonly the preferred form of oral medication delivery for children. Generally, using pictograms to illustrate medication instructions is viewed as a feasible approach to improve communication, but limited information is available on its usefulness in pediatric medicine

A Rapid and Simple Procedure for the Establishment of Human Normal and Cancer Renal Primary Cell Cultures from Surgical Specimens

The kidney is a target organ for the toxicity of several xenobiotics and is also highly susceptible to the development of malignant tumors. In both cases, in vitro studies provide insight to cellular damage, and represent adequate models to study either the mechanisms underlying the toxic effects of several nephrotoxicants or therapeutic approaches in renal cancer. The development of efficient methods for the establishment of human normal and tumor renal cell models is hence crucial. In this study, a technically simple and rapid protocol for the isolation and culture of human proximal tubular epithelial cells and human renal tumor cells from surgical specimens is presented. Tumor and normal tissues were processed by using the same methodology, based on mechanical disaggregation of tissue followed by enzymatic digestion and cell purification by sequential sieving. The overall procedure takes roughly one hour. The resulting cell preparations have excellent viabilities and yield. Establishment of primary cultures from all specimens was achieved successfully. The origin of primary cultured cells was established through morphological evaluation. Normal cells purity was confirmed by immunofluorescent staining and reverse transcription-polymerase chain reaction analysis for expression of specific markers

    Tinnitus

Tinnitus is a prevalent experience and, for those who are troubled by it, it can be debilitating. Risk factors include hearing loss, ototoxic medication, head injury and depression, and at presentation the possibility of otologic disease and of anxiety/depression should be considered. Effective drug treatments have proven elusive, though this is a vibrant theme in tinnitus research. Surgical intervention for any otological pathology associated with tinnitus may be effective for that condition, but the tinnitus may persist. Presently available treatments include the provision of hearing aids when a hearing loss is identified (even when mild or unilateral), wide band sound therapy and counselling. In some patients, cognitive behavioural therapy (CBT) is indicated though availability of tinnitus specific CBT is limited in the UK. Of these treatments the evidence base is strongest for a combination of sound therapy and CBT based counselling, though clinical trials are constrained by the heterogeneity of the tinnitus patient population. Research into mechanisms of tinnitus and effective treatments now abounds, and progress is keenly anticipated
