    The organizational implications of medical imaging in the context of Malaysian hospitals

    This research investigated the implementation and use of medical imaging in the context of Malaysian hospitals. In this report medical imaging refers to PACS, RIS/HIS and imaging modalities which are linked through a computer network. The study examined how the internal context of a hospital and its external context together influenced the implementation of medical imaging, and how this in turn shaped organizational roles and relationships within the hospital itself. It further investigated how the implementation of the technology in one hospital affected its implementation in another hospital. The research used systems theory as the theoretical framework for the study. Methodologically, the study used a case-based approach and multiple methods to obtain data. The case studies included two hospital-based radiology departments in Malaysia. The outcomes of the research suggest that the implementation of medical imaging in community hospitals is shaped by the external context particularly the role played by the Ministry of Health. Furthermore, influences from both the internal and external contexts have a substantial impact on the process of implementing medical imaging and the extent of the benefits that the organization can gain. In the context of roles and social relationships, the findings revealed that the routine use of medical imaging has substantially affected radiographers’ roles, and the social relationships between non clinical personnel and clinicians. This study found no change in the relationship between radiographers and radiologists. Finally, the approaches to implementation taken in the hospitals studied were found to influence those taken by other hospitals. Overall, this study makes three important contributions. Firstly, it extends Barley’s (1986, 1990) research by explicitly demonstrating that the organization’s internal and external contexts together shape the implementation and use of technology, that the processes of implementing and using technology impact upon roles, relationships and networks and that a role-based approach alone is inadequate to examine the outcomes of deploying an advanced technology. Secondly, this study contends that scalability of technology in the context of developing countries is not necessarily linear. Finally, this study offers practical contributions that can benefit healthcare organizations in Malaysia

    A business model perspective for ICTs in public engagement

Previous studies suggest that such initiatives seem to be impeded by socio-technical integration barriers such as low sustainability, poor citizen acceptance, coordination difficulties, lack of understanding and failure to assess their impact. Motivated by these shortcomings, the paper develops and applies a business model perspective as an interceding framework for analysis and evaluation. The underlying principle behind this approach is that it is not technology per se which determines success, but rather the way in which the businessmodel of the technological artifact is configured and employed to achieve the strategic goals. The business model perspective is empirically demonstrated with the case of an online petitioning system implemented by a UK local authority. The case illustrates the importance of considering ICTs in public engagement from a holistic view to make them more manageable and assessable

    Cohesion policy in the light of place-based innovation support: New approaches in multi-actors, decentralised regional settings with bottom-up strategies?

    According to the Barca report and the OECD, the place-based development approach is a new paradigm of regional policy. It underlines the necessity to distribute policy design and implementation among different policy levels in order to tailor policy measures to the specific local conditions. Place-based initiatives inherit a strong bottom-up element in public governance. Taking the innovation orientation in European cohesion policy as a starting point, it is the objective of this paper to analyse whether the recent implementation of structural and cohesion policy shows indications for place-based policy designs and governance. Germany is used as a case study, because it can be expected that in federal systems multi-level and bottom-up policy structures are already strongly evident.     Semantic Information Systems (IS) Standards play a critical role in the development of the networked economy. While their importance is undoubted by all stakeholders—such as businesses, policy makers, researchers, developers—the current state of research leaves a number of questions unaddressed. Terminological confusion exists around the notions of “business semantics”, “business-to-business interoperability”, and “interoperability standards” amongst others. And, moreover, a comprehensive understanding about the characteristics of Semantic IS Standards is missing. The paper addresses this gap in literature by developing a characteristics framework for Semantic IS Standards. Two case studies are used to check the applicability of the framework in a “real-life” context. The framework lays the foundation for future research in an important field of the IS discipline and supports practitioners in their efforts to analyze, compare, and evaluate Semantic IS Standard

    Indicators: tools for informing, monitoring or controlling?

    Today, indicators are produced and used worldwide; across all levels and sectors of society; by public, private and civil society actors; for a variety of purposes, ranging from knowledge-provision to administrative control. While the use of quantitative data as policy support, including policy formulation, has a long history, recent decades have seen the rise of what some have called an ‘indicator industry’ (for example, Hezri and Hasan 2004), focused especially on the production of environmental and sustainability indicators, within a framework variously called ‘governance by numbers' (Miller 2001; Lascoumes and Le Galès 2005; Jackson 2011), ‘management by numbers’ in public service (for example, Hood 2007) or ‘numbers discourse’ (Jackson 2011, p. 23). Indicators are generally expected to enhance the rationality of policymaking and public debate by providing a supposedly more objective, robust, and reliable information base. Indicators can operate as ‘boundary objects’ (for example, Turnhout 2009; Star 2010), catering to both technocratic and deliberative ideals, by combining ‘hard facts’ and modelling with collective reasoning and ‘speculation’. Research and development work in the area has hitherto overwhelmingly concentrated on improving the technical quality of indicators, while the fate of indicators in policymaking and the associated sociopolitical aspects have attracted little attention. This chapter focuses on this neglected area of indicator research, by providing an overview of the multiple types of existing indicators, as well as their use and influence in various venues of policymaking.
