    An in vitro evaluation of two dentine adhesive systems to seal the pulp chamber using a glucose penetration model

    Objectives: To evaluate the sealing capability of Cavit TM G with or without Clearfil TM S3 Bond and Prime & Bond NT placed in the pulp chamber. Study Design: Forty single rooted premolars, extracted for orthodontic and periodontal reasons, with intact coronal surface and mature apices, were standardized to a length of 15 mm. The teeth were instrumented, filled with a gutta-percha master cone and divided into three groups to obturate the pulp chambers: Cavit TM G; Clearfil TM S3 Bond plus Cavit TM G and Prime & Bond® NT plus Cavit TM G. A glucose leakage model was used for evaluating the coronal microleakage. The Mann-Whitney test was used to evaluate the differences in the means of the glucose leakage. Results: An increase in glucose penetration was observed during the first week in groups Cavit TM G and Cavit TM G+PBNT. The glucose penetration values of all groups were similar at 30 and 45 days, and there were no significant differences among them in both time periods (p=0.736 and p=0.581, respectively).     Intraorifice sealing ability of different materials in endodontically treated teeth

    Objectives: To evaluate Cavit? G, ProRoot? MTA and Tetric® EvoFlow as intraorifice barriers to prevent coronal microleakage in root canal treatment. Study Design: Forty-two human single rooted teeth were divided randomly in three experimental groups of 10 specimens each and two control groups. The experimental groups were prepared with hand instrumentation and cold lateral condensed technique of the gutta-percha. Four millimetres of coronal gutta-percha were removed and replaced by one of the following filling materials: Cavit? G, Tetric® EvoFlow or ProRoot? MTA. In the experimental groups, leakage was measured by the concentration of leaked glucose in the apical reservoir at 1, 7, 30, and 45 days, using the enzymatic glucose oxidase method. Data were analyzed by means of Mann-Whitney U and Kruskal-Wallis tests at ?=0.05. Results: The glucose penetration results of three experimental groups increased gradually over time. No significant differences were found among groups at 24 hours and 1 week. At thirty and forty-five days, Cavit? and Tetric® EvoFlow values were significantly different (p=0.007 and p=0.023, respectively). Conclusions: The sealing ability of the Cavit? G, ProRoot? MTA and Tetric® EvoFlow used as intraorifice materials tends to be similar over time

    Intervención farmacéutica en el seguimiento de la terapia antirretroviral

    The elevated cost of antiretroviral treatment demands close follow-up to achieve the effectiveness and efficacy of the treatments implemented, the role of pharmacists in achieving such goals is crucial in the sense that they are mainly involved in directing antiretroviral treatments aimed at generating maximum benefit with the available resources. The aim of the present study was to achieve an appropriate clinical control of HIV+ patients through the correct use of the antiretroviral drugs prescribed. The study was carried out for external HIV-patients with antiretroviral treatment in a university hospital. One and a half years after implementing a Pharmaceutical Care Program, a total of 853 interviews were obtained from 386 HIV+ patients (79.6% of the total), achieving a 9% increase in optimal adherence. In turn, we observed 1060 adverse drugs events and 513 drug related problems. In consequence, we performed 1401 interventions in 365 patients. The degree of patient acceptance of the program had a mean score of 3.7 (scale 0-4). The number of patients whose antiretroviral plasma concentrations were monitored was 381, with a total of 849 determinations. Only 39% of them were within the therapeutic range established for these drugs. Based on pharmacokinetic information, the clinicians performed a dosage adjustment in 16 patients. The experience gained since the implementation of the HIV+ patient follow-up program has proved to be useful for the optimisation of treatments using antiretroviral drugs.La eficacia y el alto coste de la terapia antirretroviral exigen un estrecho seguimiento para lograr su máxima efectividad y eficiencia, siendo fundamental el papel del farmacéutico para alcanzarla mediante la individualización de los tratamientos. El objetivo de este estudio ha sido conseguir un adecuado control clínico del paciente VIH+ a través del uso correcto de los medicamentos antirretrovirales prescritos. Para ello se ha puesto en marcha un Programa de Atención Farmacéutica para los pacientes externos VIH+ en tratamiento antirretroviral en un hospital universitario. Después de un año y medio de iniciarlo se han realizado un total de 853 entrevistas en 386 pacientes VIH+ (79,6% del total), consiguiéndose un aumento del 9% en la proporción de pacientes con adherencia óptima. A su vez, se han detectado 1.060 acontecimientos adversos por medicamentos y 513 problemas relacionados con la medicación. En función de ello, se han realizado 1.401 intervenciones en 365 pacientes. El grado de aceptación de los pacientes al servicio de atención farmacéutica ha alcanzado una puntuación media de 3,27 (escala 0-4). El número de pacientes cuyas concentraciones plasmáticas de fármacos antirretrovirales se han monitorizado es de 381, con un total de 849 determinaciones. Sólo el 39% de ellas se encontraba dentro de los márgenes terapéuticos aceptados para estos fármacos. Basándose en la información farmacocinética, los clínicos han realizado el ajuste posológico en 16 pacientes. La experiencia obtenida desde la implantación del programa de seguimiento ha puesto de manifiesto su utilidad para la optimización de los tratamientos con fármacos antirretrovirales

    Analizamos el efecto de la innovación sobre la productividad en la empresa manufacturera española. Así identificamos y justificamos los siguientes factores de innovación que determinan la productividad: actividades de investigación y desarrollo (I+D), gastos I+D, patentes, innovaciones de producto, innovaciones de proceso, organización interna de la innovación, externalización de las actividades de I+D, financiación pública de los gastos de I+D y colaboración exterior de la innovación. Las conclusiones de este trabajo basado en un análisis de regresión múltiple para el año 2010 a nivel de empresa, demuestran que las innovaciones de proceso y las actividades de I+D (especialmente en actividades internalizadas) están en mejores condiciones de favorecer la productividad de la empresa manufacturera española.Analizamos el efecto de la innovación sobre la productividad en la empresa manufacturera española.     DC-DC transformer multiphase converter with transformer coupling for two-stage architecture

    This paper presents an analysis of a very high efficiency topology based on a multiphase converter where the coupling among the phases is done by means of transformers. Since energy is not stored in the transformers, it is transferred directly from the input to the output. This topology along with its control strategy, were previously reported in state of the art. Some advantages of the proposed concept are that, thanks to the minimum storage of energy, the transfer rate of the energy is decoupled from switching frequency and can be chosen in order to increase efficiency. The regulation capability of this topology is limited and the topology acts as a DC-DC transformer. In this paper, the proposed topology is analyzed in detail and design guidelines are presented in order to optimize the design of the converter. These guidelines are applied to a specific design in order to build an optimized prototype

    Association between tumor size and immunohistochemical expression of Ki-67, p53 and BCL2 in a node-negative breast cancer population selected from a breast cancer screening program

    BACKGROUND/AIM: Breast cancer is the most common type of cancer among women. Breast infiltrating ductal carcinoma (IDC) is the most common type of breast cancer, approximately 80% of all breast carcinomas. The aim of this study was to analyze the association of tumor size, evaluated after histopathological analysis, with different clinical and biological parameters in IDC. MATERIALS AND METHODS: The study group included 251 women with IDC without axillary lymph node involvement, aged between 27 and 81 years. Analyzed parameters were: age, histological grade, menopausal status, menarche, pregnancy, abortion, breastfeeding, contraceptive use, hormone replacement therapy, estrogen receptor (ER), progesterone receptor (PR), androgen receptor (AR), Ki-67, p53 and BCL2. RESULTS: Pathological tumor size was between 0.2 and 5.1 cm (1.43±0.86 cm). Tumors in 45 cases exceeded 2 cm, in eight 3 cm and only in one 5 cm. Pathological size was significantly associated with age >70 vs. 70 years, histological grade 3 and immunohistochemically-augmented expression of Ki-67 and p53
