    Desarrollo sostenible en el comercio según la perspectiva del consumidor: análisis de sus efectos en hipermercados

    La sostenibilidad es un tema de interés para la academia y para el sector empresarial por todas las preocupaciones que implica a nivel mundial. Debido al entorno en donde se desenvuelven las empresas del sector minorista, tienen mayores oportunidades de evidenciar las gestiones sostenibles, por ello en la literatura del marketing se ha estudiado ampliamente las acciones de sostenibilidad que realizan las empresas. Sin embargo, el análisis del compromiso del minorista con el desarrollo sostenible percibido por el consumidor, es un constructo que recientemente se está estudiando y su área de aplicación principalmente ha sido en el continente europeo, por lo que esta Tesis Doctoral tiene como objetivo principal: examinar el impacto del desarrollo sostenible percibido por el consumidor y el comportamiento del consumidor ético frente a los hipermercados; y, determinar cómo contribuyen, junto con otras variables clásicas del marketing, tales como el valor percibido, la calidad percibida y la imagen de la tienda, a generar capital de marca, lealtad y comunicación boca-oído en los clientes. Para alcanzar el objetivo se han llevado a cabo dos estudios en el contexto de una economía emergente como es Ecuador, en los años 2017 y 2019. Sus resultados se han publicado en tres artículos científicos que dan contenido a esta Tesis Doctoral por compendio, convirtiéndose en una oportunidad para reforzar el valor de las contribuciones de este estudio, dado que hasta el momento la mayoría de las investigaciones de referencia se habían basado en economías occidentales desarrolladas. Los resultados permiten concluir que el compromiso del minorista con el desarrollo sostenible percibido por el consumidor en Ecuador es un tema emergente, en donde los consumidores reconocen las acciones sostenibles de los minoristas y saben que tienen el poder de premiar a los minoristas al momento de decidir su compra. Además, el compromiso del minorista con el desarrollo sostenible percibido por el consumidor afecta de manera positiva al valor percibido, capital de marca, a la comunicación boca-oído y la lealtad. Esta investigación aporta con información relevante para reducir la brecha existente en la literatura asociada con el compromiso del minorista con el desarrollo sostenible percibido por el consumidor, así como el consumidor de mentalidad ética y la cadena de relaciones con las variables de marketing, basado en el cliente de hipermercados. Así, en la Tesis Doctoral se presenta una serie de conclusiones sustanciales, que aportan a la academia, se proponen sugerencias prácticas para orientar a los profesionales de la gran distribución minorista en el desarrollo de estrategias de marketing relacionadas con la sostenibilidad y el consumo ético, para incrementar las ventajas de diferenciación con la competencia. Finalmente, se declaran algunas limitaciones y se proponen nuevas oportunidades de investigación que podrían ser consideradas en contribuciones futuras.Sustainability is a topic of interest for academia and for the business sector due to all the concerns it implies worldwide. Due to the environment in which companies in the retail sector operate, they have greater opportunities to demonstrate sustainable management, which is why the marketing literature has widely studied the sustainability actions carried out by companies. However, the analysis of the retailer's commitment to sustainable development as perceived by the consumer is a construct that has recently been studied and its area of application has mainly been on the European continent, so this Doctoral Thesis has as its main objective: examine the impact of sustainable development perceived by the consumer and ethical consumer behavior towards hypermarkets; and determine how they contribute, together with other classic marketing variables, such as perceived value, perceived quality and store image, to generate brand equity, loyalty and word-of-mouth communication among customers. To achieve the objective, two studies have been carried out in the context of an emerging economy such as Ecuador, in the years 2017 and 2019. Their results have been published in three scientific articles that give content to this Doctoral Thesis by compendium, becoming an opportunity to reinforce the value of the contributions of this study, given that until now most of the reference research had been based on developed Western economies. The results allow us to conclude that the retailer's commitment to sustainable development perceived by the consumer in Ecuador is an emerging issue, where consumers recognize the sustainable actions of retailers and know that they have the power to reward retailers when making decisions. your purchase. Thus, in the Doctoral Thesis a series of substantial conclusions are presented, which contribute to the academy, practical suggestions are proposed to guide professionals in large retail distribution in the development of marketing strategies related to sustainability and ethical consumption, to increase the advantages of differentiation with the competition. Finally, some limitations are stated and new research opportunities that could be considered in future contributions are proposed

    Regional economic effects of COVID-19 on the self-employed: The case of Cantabria

    El objetivo de este trabajo es analizar y estimar los efectos que la crisis sanitaria desatada por la enfermedad COVID-19 tendrá sobre los trabajadores autónomos en la Comunidad Autónoma de Cantabria a través de la simulación de tres posibles escenarios de contingencia (base, favorable y adverso). Los resultados obtenidos muestran que, si los trabajadores autónomos afectados por las caídas de actividad económica se diesen de baja en la Seguridad Social, esto equivaldría a una caída anual de afiliación estimada en un 2,95% en el escenario base, un 0,43% en el favorable y un 4,48% en el caso más desfavorable.The economic crisis that, as a consequence of the health pandemic, has been unleashed, will affect to a greater extent the weakest groups of our labour market. In this regard, self-employed workers may face a particularly dramatic situation. Thus, due to the lock down measures established to mitigate the advance of the coronavirus, at this time of uncertainty, millions of workers in general, and self-employed in particular, have had to radically reduce their economic activity, paralyzing numerous projects and/or jobs. Consequently, their income has been suddenly and indefinitely reduced. Given these circumstances, at the present time, it is especially relevant to analyse the effects that COVID-19 is generating on employment and, more particularly, on the group of self-employed workers.     Tres constructos psicológicos como son autoconcepto físico, optimismo y motivación podrían estar más relacionados en el ámbito del deporte de lo que parece a priori. El objeto de este estudio es analizar cómo se relacionan estas variables. Se espera que se revelen algunas correlaciones positivas entre los distintos constructos y su relación con ambos géneros. Para ver la relación entre las tres variables se contó con una muestra de 113 participantes (51 mujeres y 62 hombres), de edades comprendidas entre 18 y 60 años. Los resultados obtenidos mostraron la relación entre autoconcepto físico y optimismo, y optimismo y motivación sin diferencias entre hombres y mujeres. Sin embargo también se ha encontrado una relación negativa entre autoconcepto y motivación, contraria a lo que cabría esperar. Tres constructos psicológicos como son autoconcepto físico, optimismo y motivación podrían estar más relacionados en el ámbito del deporte de lo que parece a priori.Three psychological constructs such as the physical self, optimism and motivation may be more related to the field of sport than it seems a priori. The purpose of this study is to analyze how these variables are related. It is expected that some positive correlations between constructs and their relationship with both genders are revealed. To see the relationship between the three variables had a sample of 113 participants (51 women and 62 men), aged between 18 and 60. The results showed the relationship between physical self and optimism, and optimism and motivation no difference between men and women.     Magnetic resonance imaging reference values for cardiac morphology, function and tissue composition in adolescents.

    BACKGROUND Cardiovascular magnetic resonance (CMR) is a precise tool for the assessment of cardiac anatomy, function, and tissue composition. However, studies providing CMR reference values in adolescence are scarce. We aim to provide sex-specific CMR reference values for biventricular and atrial dimensions and function and myocardial relaxation times in this population. METHODS Adolescents aged 15-18 years with no known cardiovascular disease underwent a non-contrast 3-T CMR scan between March 2021 and October 2021. The imaging protocol included a cine steady-state free-precession sequence for the analysis of chamber size and function, as well as T2-GraSE and native MOLLI T1-mapping for the characterization of myocardial tissue. FINDINGS CMR scans were performed in 123 adolescents (mean age 16 ± 0.5 years, 52% girls). Mean left and right ventricular end-diastolic indexed volumes were higher in boys than in girls (91.7 ± 11.6 vs 78.1 ± 8.3 ml/m2, p < 0.001; and 101.3 ± 14.1 vs 84.1 ± 10.5 ml/m2, p < 0.001), as was the indexed left ventricular mass (48.5 ± 9.6 vs 36.6 ± 6.0 g/m2, p < 0.001). Left ventricular ejection fraction showed no significant difference by sex (62.2 ± 4.1 vs 62.8 ± 4.2%, p = 0.412), whereas right ventricular ejection fraction trended slightly lower in boys (55.4 ± 4.7 vs. 56.8 ± 4.4%, p = 0.085). Indexed atrial size and function parameters did not differ significantly between sexes. Global myocardial native T1 relaxation time was lower in boys than in girls (1215 ± 23 vs 1252 ± 28 ms, p < 0.001), whereas global myocardial T2 relaxation time did not differ by sex (44.4 ± 2.0 vs 44.1 ± 2.4 ms, p = 0.384). Sex-stratified comprehensive percentile tables are provided for most relevant cardiac parameters. INTERPRETATION This cross-sectional study provides overall and sex-stratified CMR reference values for cardiac dimensions and function, and myocardial tissue properties, in adolescents. This information is useful for clinical practice and may help in the differential diagnosis of cardiac diseases, such as cardiomyopathies and myocarditis, in this population.     Multicenter evaluation of the Panbio™ COVID-19 rapid antigen-detection test for the diagnosis of SARS-CoV-2 infection

    Objetives: The standard RT-PCR assay for coronavirus disease 2019 (COVID-19) is laborious and time-consuming, limiting testing availability. Rapid antigen-detection tests are faster and less expensive; however, the reliability of these tests must be validated before they can be used widely. The objective of this study was to determine the performance of the Panbio™ COVID-19 Ag Rapid Test Device (PanbioRT) (Abbott) in detecting severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in nasopharyngeal swab specimens. Methods: This prospective multicentre study was carried out in ten Spanish university hospitals and included individuals with clinical symptoms or epidemiological criteria of COVID-19. Only individuals with ≤7 days from the onset of symptoms or from exposure to a confirmed case of COVID-19 were included. Two nasopharyngeal samples were taken to perform the PanbioRT as a point-of-care test and a diagnostic RT-PCR test. Results: Among the 958 patients studied, 325 (90.5%) had true-positive results. The overall sensitivity and specificity for the PanbioRT were 90.5% (95%CI 87.5-93.6) and 98.8% (95%CI 98-99.7), respectively. Sensitivity in participants who had a threshold cycle (CT) < 25 for the RT-PCR test was 99.5% (95%CI 98.4-100), and in participants with ≤5 days of the clinical course it was 91.8% (95%CI 88.8-94.8). Agreement between techniques was 95.7% (κ score 0.90; 95%CI 0.88-0.93). Conclusions: The PanbioRT performs well clinically, with even more reliable results for patients with a shorter clinical course of the disease or a higher viral load. The results must be interpreted based on the local epidemiological context.S

    A Functional Pipeline of Genome-Wide Association Data Leads to Midostaurin as a Repurposed Drug for Alzheimer’s Disease

    Genome-wide association studies (GWAS) constitute a powerful tool to identify the different biochemical pathways associated with disease. This knowledge can be used to prioritize drugs targeting these routes, paving the road to clinical application. Here, we describe DAGGER (Drug Repositioning by Analysis of GWAS and Gene Expression in R), a straightforward pipeline to find currently approved drugs with repurposing potential. As a proof of concept, we analyzed a meta-GWAS of 1.6 × 107 single-nucleotide polymorphisms performed on Alzheimer’s disease (AD). Our pipeline uses the Genotype-Tissue Expression (GTEx) and Drug Gene Interaction (DGI) databases for a rational prioritization of 22 druggable targets. Next, we performed a two-stage in vivo functional assay. We used a C. elegans humanized model over-expressing the Aβ1-42 peptide. We assayed the five top-scoring candidate drugs, finding midostaurin, a multitarget protein kinase inhibitor, to be a protective drug. Next, 3xTg AD transgenic mice were used for a final evaluation of midostaurin’s effect. Behavioral testing after three weeks of 20 mg/kg intraperitoneal treatment revealed a significant improvement in behavior, including locomotion, anxiety-like behavior, and new-place recognition.     Resistance to taxanes in triple negative breast cancer associates with the dynamics of a CD49f+ tumor initiating population

    Taxanes are a mainstay of treatment for breast cancer, but resistance often develops followed by metastatic disease and mortality. Aiming to reveal the mechanisms underlying taxane resistance, we used breast cancer patient-derived orthoxenografts (PDX). Mimicking clinical behavior, triple-negative breast tumors (TNBCs) from PDX models were more sensitive to docetaxel than luminal tumors, but they progressively acquired resistance upon continuous drug administration. Mechanistically, we found that a CD49f+ chemoresistant population with tumor-initiating ability is present in sensitive tumors and expands during the acquisition of drug resistance. In the absence of the drug, the resistant CD49f+ population shrinks and taxane sensitivity is restored. We describe a transcriptional signature of resistance, predictive of recurrent disease after chemotherapy in TNBC. Together, these findings identify a CD49f+ population enriched in tumor-initiating ability and chemoresistance properties and evidence a drug holiday effect on the acquired resistance to docetaxel in triple-negative breast cancer

    Multiple health behaviour change primary care intervention for smoking cessation, physical activity and healthy diet in adults 45 to 75 years old (EIRA study): a hybrid effectiveness-implementation cluster randomised trial

    Methods: A cluster randomised effectiveness-implementation hybrid trial-type 2 with two parallel groups was conducted in 25 Spanish Primary Health Care (PHC) centres (3062 participants): 12 centres (1481 participants) were randomised to the intervention and 13 (1581 participants) to the control group (usual care). The intervention was based on the Transtheoretical Model and focused on all target behaviours using individual, group and community approaches. PHC professionals made it during routine care. The implementation strategy was based on the Consolidated Framework for Implementation Research (CFIR). Data were analysed using generalised linear mixed models, accounting for clustering. A mixed-methods data analysis was used to evaluate implementation outcomes (adoption, acceptability, appropriateness, feasibility and fidelity) and determinants of implementation success. Results: 14.5% of participants in the intervention group and 8.9% in the usual care group showed a positive change in two or all the target behaviours. Intervention was more effective in promoting dietary behaviour change (31.9% vs 21.4%). The overall adoption rate by professionals was 48.7%. Early and final appropriateness were perceived by professionals as moderate. Early acceptability was high, whereas final acceptability was only moderate. Initial and final acceptability as perceived by the participants was high, and appropriateness moderate. Consent and recruitment rates were 82.0% and 65.5%, respectively, intervention uptake was 89.5% and completion rate 74.7%. The global value of the percentage of approaches with fidelity ≥50% was 16.7%. Eight CFIR constructs distinguished between high and low implementation, five corresponding to the Inner Setting domain. Conclusions: Compared to usual care, the EIRA intervention was more effective in promoting MHBC and dietary behaviour change. Implementation outcomes were satisfactory except for the fidelity to the planned intervention, which was low. The organisational and structural contexts of the centres proved to be significant determinants of implementation effectiveness

    Nutrición parenteral domiciliaria en España, 2019: informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA

    RESUMEN Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD
