    Building and testing a cognitive approach to the calculus using interactive computer graphics

    This thesis consists of a theoretical building of a cognitive approach to the calculus and an empirical testing of the theory in the classroom. A cognitive approach to the teaching of a knowledge domain is defined to be one that aims to make the material potentially meaningful at every stage (in the sense of Ausubel). As a resource in such an approach, the notion of a generic organiser is introduced (after Dienes), which is an environment enabling the learner to explore examples of mathematical processes and concepts, providing cognitive experience to assist in the abstraction of higher order concepts embodied by the organiser. This allows the learner to build and test concepts in a mode 1 environment (in the sense of Skemp) rather than the more abstract modes of thinking typical in higher mathematics. The major hypothesis of the thesis is that appropriately designed generic organisers, supported by an appropriate learning environment, are able to provide students with global gestalts for mathematical processes and concepts at an earlier stage than occurs with current teaching methods. The building of the theory involves an in-depth study of cognitive development, of the cultural growth and theoretical content of the mathematics, followed by the design and programming of appropriate organisers for the teaching of the calculus. Generic organisers were designed for differentiation (gradient of a graph), integration (area), and differential equations, to be coherent ends in themselves as well as laying foundations for the formal theories of both standard and non-standard analysis. The testing is concerned with the program GRADIENT, which is designed to give a global gestalt of the dynamic concept of the gradient of a graph. Three experimental classes (one taught by the researcher in conjunction with the regular class teacher) used the software as an adjunct to the normal study of the calculus and five other classes acted as controls. Matched pairs were selected on a pre-test for the purpose of statistical comparison of performance on the post-test. Data was also collected from a third school where the organisers functioned less well, and from university mathematics students who had not used a computer. The generic organiser GRADIENT, supported by appropriate teaching, enabled the experimental students to gain a global gestalt of the gradient concept. They were able to sketch derivatives. for given graphs significantly better than the controls on the post-test, at a level comparable with more able students reading mathematics at university. Their conceptualizations of gradient and tangent transferred to a new situation involving functions given by different formulae on either side of the point in question, performing significantly better than the control students and at least as well, or better, than those at university

    ResumenSe ha considerado que el estrés contribuye a la patogénesis y la progresión de las enfermedades cardiovasculares (ECV). Se ha demostrado que la reducción del estrés mediante la Meditación Trascendental [Transcendental Meditation (TM)®] ha bajado los niveles de presión arterial (PA) y reducido el riesgo de ECV en adultos y adolescentes. Este artículo repasa los resultados que sugieren el impacto beneficioso de la TM en reducir la PA en adultos hipertensos en reposo y en adolescentes pre-hipertensos en reposo, durante un estrés agudo creado en el laboratorio y durante la actividad diaria normal. Dichos resultados tienen implicaciones importantes para la inclusión de la TM en los esfuerzos que se realizan para prevenir y tratar las ECV y sus consecuencias clínicas.AbstractStress has been thought to contribute to the pathogenesis and progression of cardiovascular diseases (CVD). Stress reduction via Trasncendental Meditation (TM)® has been shown to lower blood pressure (BP) levels and reduce CVD risk in adults and adolescents. This article reviews findings suggesting a beneficial BP-lowering impact of TM in hypertensive adults at rest and in pre-hypertensive adolescents at rest, during acute laboratory stress and during normal daily activity. These findings have important implications for inclusion of TM efforts to prevent and treat cardiovascular diseases and their clinical consequences.</p

    Financial Challenges of Cancer for Adolescents and Young Adults and Their Parent Caregivers

    This study examined the financial impact of cancer and the use of income support in adolescents and young adults (AYAs) with cancer and their parent caregivers. As part of a national Australian study exploring the psychosocial impacts of cancer, 196 AYAs ages 15 to 25 years, six to 24 months from diagnosis, and 204 parent caregivers from 18 cancer sites were surveyed. Logistic regression and chi-square analyses were conducted to assess the influence of clinical and sociodemographic variables on financial status. Qualitative responses were coded, and key themes were identified using thematic analysis. The findings indicate that more than half of AYAs and parents reported financial issues as a consequence of AYA cancer. Financial issues resulted from direct medical costs, associated costs from treatment, and indirect costs from loss of income. AYAs and parents reported that it was important for them to receive income support, both during and after cancer treatment. However, large proportions of those who reported needing income support had difficulty accessing it. AYAs and their families are substantially financially disadvantaged by cancer, many for a prolonged time. Patient- and family-centered assessments and interventions are required to reduce the financial burden of AYA cancer.Peer reviewe

    Comparative efficacy and safety of bimekizumab in axial spondyloarthritis: a systematic literature review and network meta-analysis

    OBJECTIVES: To compare the efficacy and safety of bimekizumab 160 mg every 4 weeks, a selective inhibitor of interleukin‑17F and 17A, with biologic/targeted synthetic disease-modifying anti-rheumatic drugs (b/tsDMARDs) in non-radiographic axial spondyloarthritis (nr-axSpA) and ankylosing spondylitis (AS). METHODS: A systematic literature review identified randomised controlled trials until January 2023 for inclusion in Bayesian network meta-analyses (NMAs), including three b/tsDMARDs exposure networks: predominantly-naïve, naïve, and experienced. Outcomes were Assessment of SpondyloArthritis international Society (ASAS)20, ASAS40, and ASAS partial remission (PR) response rates at 12-16 weeks. A safety NMA investigated discontinuations due to any reason and serious adverse events at 12-16 weeks. RESULTS: The NMA included 36 trials. The predominantly-naïve network provided the most comprehensive results. In the predominantly-naïve nr-axSpA analysis, bimekizumab had significantly higher ASAS20 response rates vs secukinumab 150 mg (with loading dose [LD]/without LD), and comparable response rates vs other active comparators. In the predominantly-naïve AS analysis, bimekizumab had significantly higher ASAS40 response rates vs secukinumab 150 mg (without LD), significantly higher ASAS-PR response rates vs secukinumab 150 mg (with LD), and comparable response rates vs other active comparators. Bimekizumab demonstrated similar safety to other b/tsDMARDs. CONCLUSION: Across ASAS outcomes, bimekizumab was comparable to most b/tsDMARDs, including ixekizumab, TNF inhibitors and upadacitinib, and achieved higher response rates vs secukinumab for some ASAS outcomes in predominantly b/tsDMARD-naïve nr-axSpA and AS patients at 12-16 weeks. In a pooled axSpA network, bimekizumab demonstrated comparable safety vs other b/tsDMARDs

    The physical demands of match-play in academy and senior soccer players from the Scottish Premiership

    The present study aimed to assess the physical match performance among senior and youth soccer players from an elite Scottish Premiership club during the 2021/2022 season. Twenty-two first team (25.9 ± 4.5 years, 78.3 ± 8.2 kg, 1.82 ± 0.07 cm) and 16 youth players (16.8 ± 0.9 years, 70.1 ± 6.8 kg, 177 ± 5.8 cm) were examined. A selection of physical match performance variables were measured using a global positioning system. Linear mixed-effect regressions revealed for all examined variables no significant differences between first team and U-18 players and no significant differences between playing level by position interaction. Across both teams, Centre Backs compared to Wing Backs, showed a 295 m (p &lt; 0.01) lower high-intensity distance, and performed on average 36 fewer very-high intensity decelerations (p = 0.03). Comparing to Wide Midfielders, Centre Backs showed lower total (1297 m, p = 0.01), high-intensity (350 m, p = 0.01), and sprint (167 m, p &lt; 0.01) distances. Sprint distance was also lower in Centre Backs vs. Strikers (118 m, p = 0.03), and in Central Midfielders vs. both Strikers (104 m, p = 0.03) and Wide Midfielders (154 m, p = 0.01). The present findings highlight the physical match performance of elite Scottish players and provide useful information within the context of understanding how methods of physical development of youth soccer are implemented in different countries
