Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Abstract
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and lowâmiddle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of âsingle-useâ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for lowâmiddle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both highâ and lowâmiddleâincome countries
Characterization of pediatric patients with iron-deficiency anemia
Background: iron-deficiency anemia is the most widespread nutritional deficiency in the world. Objective: to characterize pediatric patients with a diagnosis of iron-deficiency anemia attended to at the Department of Hematology of the "MĂĄrtires de Las Tunas" Pediatric Hospital, from January 1 to December 31, 2016.Methods: a cross-sectional descriptive study was carried out with children diagnosed with iron-deficiency anemia attended to at the place and during the period herein mentioned. The sample consisted of 107 patients who presented microcytic anemia and serum iron values lower than 11 micromole/l; patients with thalassemia and anemia secondary to chronic processes were excluded. Data from the laboratory records were used and processed using descriptive statistics. Results: pre-school patients prevailed with 55 cases for 51,4 % followed by children younger than 1 year with 31 patients for 29 %. Concerning gender, there was a prevalence of the female sex in 56,1 %. Patients with serum iron between 7-8,9 micromole/ml prevailed in 57 % of the studied sample. Conclusions: the studied patients of this research were characterized.</p
Medico-legal characterization of victims of sexual crimes in minors
Background: sexual crimes in minors constitute a complex social phenomenon that is present in all the nations of the world. In Cuba there has been an increase in the record of incidents in this regard. Objective: to describe the epidemiological and medico-legal aspects of sexual crimes and the victimization of the victim in minors under 18 years of age, treated at the department of forensic medicine of the "Dr. Ernesto Guevara de la Serna" General Teaching Hospital, of the province of Las Tunas, from January 2014 to December 2015.Methods: a cross-sectional descriptive study was carried out. The universe was made up of minors who were victims of sexual crimes and went to the department and during the period of time declared in the objective. The sample consisted of 172 children whose parents or guardians agreed to participate in the study. The data were processed using descriptive statistics, by means of the percentage analysis. Results: it was found that sexual crimes affected mostly female minors (91,3 %), in ages between 12 and 18 years (52,3 %). The crimes that were more frequently categorized as crimes were lewd abuse and rape, committed by an acquaintance and occurring in the victim's home. 88,4 % of the minors were explored more than once before visiting the department of forensic medicine. Several alterations were registered in the habitual behavior of the victims. Conclusions: it was possible to describe the medical-legal characteristics of the minors included in the study who were victims of alleged sex crimes.</p
Science, art and medicine in Cuba during the 17th and 18th centuries
Medicine is the science dedicated to the study of life, health, diseases and death of human beings, and involves exercising scientific-technical knowledge for the maintenance and recovery of health. It is an art where the knowledge of the doctor is used to obtain a practical result that is to prevent a human being from getting sick, and if he does, to achieve the restoration of his health. The art of being a doctor implies the science with which one has to proceed and through which one can find solutions to health problems through scientific methods. For its part, art, through its various manifestations such as painting, photography, sculpture, literature, among others, has facilitated medicine knowledge about facts, characters, actions or investigations carried out by illustrious personalities of medicine and science, since the emergence of medicine in Cuba during the colonial era. These precedents constitute works of invaluable historical and scientific value, which have contributed important antecedents in the execution of the current health programs
