Failure to Predict High-risk Kawasaki Disease Patients in a Population-based Study Cohort in Germany
Background: Diverse scores on high-risk Kawasaki disease (KD) patients have proven a good prognostic validity in the Japanese population. However, data on non-Japanese have been inconclusive. Do the Kobayashi, Egami and Sano scores or application of up-to-date statistical methods (Random Forest) predict response to standard intravenous immunoglobulin (IVIG) therapy and the risk of persistent coronary artery aneurysm (CAA) in patients with KD in a mainly Caucasian population in Germany? Methods: Data on 442 children (German population-based survey, 2013 and 2014) were used to assess the prognostic validity of the Kobayashi, Egami and Sano scores for being refractory to IVIG treatment and for predicting the risk of persistent CAA. Additionally, an up-to-date statistical approach (Random Forest) was applied to identify a potentially more valid score. Results: A total of 301 children were eligible for assessment of their response to IVIG treatment. Among those, 177 children were followed-up for 1 year to identify persistent CAA. Although all scores were significantly associated with being refractory to IVIG (relative risk range between 2.32 and 3.73), the prognostic properties were low (likelihood ratio positive: 1.83-4.57;sensitivity in the range of 0.28-0.53). None of the scores was a significant predictor of CAA 1 year after acute illness. Application of statistical analysis such as Random Forest did not yield a more valid score. Conclusions: None of the available scores appears to be appropriate for identifying high-risk Caucasian children with KD who might need intensified therapy
Relation between syphilis cases and sex education of women in a family health unit in the city of Lauro de Freitas
OBJECTIVE: To verify the relation between syphilis cases and sex education in a Family Health Unit in the city of Lauro de Freitas. METHOD: This was a cross-sectional study, approved by the Research Ethics Committee of the Metropolitan Union for the Development of Education and Culture (UNIME). The target audience was women aged 18 to 60 years from a specific covered area of a Family Health Unit in Lauro de Freitas-BA. A questionnaire prepared by the researchers was applied to 114 participants. RESULTS: 55 (48.2%) of the women studied considered themselves black and 31 (27.2%) had more than 11 years of study (high school complete). 62 (54.4%) were legally single, 84 (74.3%) had an active sex life, 87 (77.0%) had a steady partner. 113 (99.1%) women believe it is women's right to demand condom use. Only 7 (8.8%) of those surveyed reported having syphilis. 79 (69.3%) considers that they have good knowledge regarding sex education. 104 (91.2%) answered that the contagion is through sexual intercourse without a condom, but 60 (52.6%) of them have little information about the disease in question. CONCLUSION: The present study shows that 52.6% of women know little or nothing about syphilis. In addition, it was noted that when asked about the ways of syphilis contamination, none of the 114 women interviewed were able to answer all the correct alternatives. So, it is possible to conclude that it is necessary to implement new interaction public policies between the family health unit and the community
A prĂĄtica educativa docente sobre o Projeto PolĂtico PedagĂłgico: um relato de experiĂȘncia junto Ă pĂłs-graduação em enfermagem
O objetivo deste estudo foi relatar a experiĂȘncia de pĂłs-graduandas na elaboração e aplicação de uma prĂĄtica educativa sobre o Projeto PolĂtico PedagĂłgico (PPP), adequado ao ensino de graduação, utilizando metodologias ativas de ensino-aprendizagem. A atividade foi apresentada para pĂłs-graduandos no contexto de uma disciplina de pĂłs-graduação, sob orientação das docentes responsĂĄveis. A aula ocorreu em 2015, teve a duração de trĂȘs horas, contou com a participação de 35 pĂłs-graduandos e teve como proposta desenvolver nos alunos condiçÔes de conceituar o PPP; entender sua importĂąncia, seus princĂpios e estrutura; compreender os processos que envolvem a sua elaboração e desenvolver pensamentos crĂticos e reflexivos sobre os desafios de sua implementação nas instituiçÔes de ensino. A aula foi desenvolvida em trĂȘs etapas. Na primeira, disponibilizou-se aos alunos um texto com uma situação-problema fictĂcia que tratava da elaboração de um PPP para resolver dificuldades de uma instituição de ensino. Na segunda, adaptou-se a metodologia da Aprendizagem Baseada em Problemas (ABP) ao objetivo proposto, buscando os conhecimentos prĂ©vios dos alunos acerca do PPP. E na terceira, utilizaramse tarjas de cartolina que continham as etapas de construção de um PPP, para que os alunos as discutissem e as dispusessem em sequĂȘncia correta. A atividade foi avaliada como satisfatĂłria. Para as pĂłs-graduandas, a experiĂȘncia de planejar e aplicar metodologias ativas no contexto de ensino-aprendizagem possibilitou: compreender a relevĂąncia de um PPP, construir coletivamente sua concepção e desenvolver a formação docente.The objective of this study was to report the experience of postgraduates in the elaboration and application of an educational practice on the Pedagogical Political Project (PPP), suitable for undergraduate education, using active teaching-learning methodologies. The activity was presented to postgraduate students in the context of a postgraduate course, under the guidance of the responsible professors. The class took place in 2015, lasted three hours, with the participation of 35 postgraduate students and had as a proposal to develop in the students conditions to conceptualize the PPP; understand its importance, its principles and structure; understand the processes involved in its elaboration and develop critical and reflective thoughts about the challenges of its implementation in educational institutions. The class was developed in three stages. In the first, the students were given a text with a fictitious problem situation that dealt with the elaboration of a PPP to solve difficulties of an educational institution. In the second, the methodology of Problem-Based Learning (PBL) was adapted to the proposed objective, seeking the previous knowledge of the students about the PPP. And in the third one, strips of card that contained the steps of construction of a PPP were used, so that the students discussed them and arranged them in correct sequence. The activity was evaluated as satisfactory. For postgraduate students, the experience of planning and applying active methodologies in the teaching-learning context made it possible to: understand the relevance of a PPP, to constructively construct its conception and to develop teacher training
Systems-pharmacology dissection of a drug synergy in imatinib-resistant CML
Occurrence of the BCR-ABL[superscript T315I] gatekeeper mutation is among the most pressing challenges in the therapy of chronic myeloid leukemia (CML). Several BCR-ABL inhibitors have multiple targets and pleiotropic effects that could be exploited for their synergistic potential. Testing combinations of such kinase inhibitors identified a strong synergy between danusertib and bosutinib that exclusively affected CML cells harboring BCR-ABL[superscript T315I]. To elucidate the underlying mechanisms, we applied a systems-level approach comprising phosphoproteomics, transcriptomics and chemical proteomics. Data integration revealed that both compounds targeted Mapk pathways downstream of BCR-ABL, resulting in impaired activity of c-Myc. Using pharmacological validation, we assessed that the relative contributions of danusertib and bosutinib could be mimicked individually by Mapk inhibitors and collectively by downregulation of c-Myc through Brd4 inhibition. Thus, integration of genome- and proteome-wide technologies enabled the elucidation of the mechanism by which a new drug synergy targets the dependency of BCR-ABL[superscript T315I] CML cells on c-Myc through nonobvious off targets

Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial
Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials.
Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure.
Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.

Primary Student Teachers' Diagnosed Mathematical Competence in Semester One of their Studies
Data collected from a diagnostics mathematics test taken by some primary student teachers are reported. Student responses were analysed using the Dichotomous Rasch Measurement Model. Error analyses enabled the identification of main misconceptions. Findings showed students performed relatively well with basic computations and visually presented data but struggled with word problems. The more complex and abstract the language used, the more difficult it became, implying that the critical skills of interpreting mathematical concepts, representations, and language and problem solving require explicit remediation. Implications for primary teacher education are provided
