    Preparing Students in Professional Programs for Rural Practice: A Case Study

    Understanding the supports and constraints available in rural communities is integral to the education of professionals who choose to practise in rural settings. Previous research has indicated that many professionals do not have an accurate understanding of rural contexts and how rural settings impact personal life and professional practice. To address this gap, an interprofessional course centring on professional practice in rural and remote communities was developed. This course was designed to be reflective of rural practice, not only in content but also in the way the course was delivered. Findings from this case study indicated that students’ understanding of the complexity of rural settings was enhanced on multiple levels. The interactive and experiential nature of the course allowed students to develop working relationships that increased both their understanding of the value of interprofessional collaboration as well as the professional opportunities that are available in rural areas.  Comprendre les soutiens accessibles dans les communautĂ©s rurales et leurs contraintes fait partie intĂ©grante de la formation des professionnels qui choisissent de pratiquer en milieu rural. Des recherches antĂ©rieures ont indiquĂ© que de nombreux professionnels n’ont pas une comprĂ©hension adĂ©quate des contextes ruraux ni des rĂ©percussions de ces paramĂštres sur la vie personnelle et la pratique professionnelle. Pour combler cette lacune, un cours interprofessionnel centrĂ© sur la pratique professionnelle dans les collectivitĂ©s rurales et Ă©loignĂ©es a Ă©tĂ© crĂ©Ă©. Ce cours se veut le reflet de la pratique rurale, non seulement dans son contenu mais aussi dans sa façon d’ĂȘtre enseignĂ©. Les rĂ©sultats de cette Ă©tude de cas ont indiquĂ© que la comprĂ©hension qu’ont les Ă©lĂšves de la complexitĂ© du milieu rural a Ă©tĂ© renforcĂ©e Ă  plusieurs niveaux.     Adipose-Derived Autotaxin Regulates Inflammation and Steatosis Associated with Diet-Induced Obesity

    Autotaxin (ATX) is a secreted enzyme that generates the bioactive lipid lysophosphatidic acid (LPA). We generated mice with global inducible post-natal inactivation or adipose-specific loss of the Enpp2 gene encoding ATX. The animals are phenotypically unremarkable and exhibit differences in adipocyte size and adipose tissue expression of inflammatory genes after high fat feeding without gross differences in fat distribution or body mass. Surprisingly, both models of Enpp2- deficiency exhibited marked protection from high fat diet-induced hepatic steatosis. This phenotype was not associated with differences in dietary fat absorption but may be accounted for by differences in hepatic expression of genes involved in de novo synthesis of triglycerides. These findings suggest that pharmacological inhibition of ATX might be protective against hepatic steatosis

    Association of Accelerometry-Measured Physical Activity and Cardiovascular Events in Mobility-Limited Older Adults: The LIFE (Lifestyle Interventions and Independence for Elders) Study.

    BACKGROUND:Data are sparse regarding the value of physical activity (PA) surveillance among older adults-particularly among those with mobility limitations. The objective of this study was to examine longitudinal associations between objectively measured daily PA and the incidence of cardiovascular events among older adults in the LIFE (Lifestyle Interventions and Independence for Elders) study. METHODS AND RESULTS:Cardiovascular events were adjudicated based on medical records review, and cardiovascular risk factors were controlled for in the analysis. Home-based activity data were collected by hip-worn accelerometers at baseline and at 6, 12, and 24 months postrandomization to either a physical activity or health education intervention. LIFE study participants (n=1590; age 78.9±5.2 [SD] years; 67.2% women) at baseline had an 11% lower incidence of experiencing a subsequent cardiovascular event per 500 steps taken per day based on activity data (hazard ratio, 0.89; 95% confidence interval, 0.84-0.96; P=0.001). At baseline, every 30 minutes spent performing activities ≄500 counts per minute (hazard ratio, 0.75; confidence interval, 0.65-0.89 [P=0.001]) were also associated with a lower incidence of cardiovascular events. Throughout follow-up (6, 12, and 24 months), both the number of steps per day (per 500 steps; hazard ratio, 0.90, confidence interval, 0.85-0.96 [P=0.001]) and duration of activity ≄500 counts per minute (per 30 minutes; hazard ratio, 0.76; confidence interval, 0.63-0.90 [P=0.002]) were significantly associated with lower cardiovascular event rates. CONCLUSIONS:Objective measurements of physical activity via accelerometry were associated with cardiovascular events among older adults with limited mobility (summary score >10 on the Short Physical Performance Battery) both using baseline and longitudinal data.     Data provenance to audit compliance with privacy policy in the Internet of Things

    Managing privacy in the IoT presents a significant challenge. We make the case that information obtained by auditing the flows of data can assist in demonstrating that the systems handling personal data satisfy regulatory and user requirements. Thus, components handling personal data should be audited to demonstrate that their actions comply with all such policies and requirements. A valuable side-effect of this approach is that such an auditing process will highlight areas where technical enforcement has been incompletely or incorrectly specified. There is a clear role for technical assistance in aligning privacy policy enforcement mechanisms with data protection regulations. The first step necessary in producing technology to accomplish this alignment is to gather evidence of data flows. We describe our work producing, representing and querying audit data and discuss outstanding challenges.Engineering and Applied Science

    Deciphering osteoarthritis genetics across 826,690 individuals from 9 populations

    Osteoarthritis affects over 300 million people worldwide. Here, we conduct a genome-wide association study meta-analysis across 826,690 individuals (177,517 with osteoarthritis) and identify 100 independently associated risk variants across 11 osteoarthritis phenotypes, 52 of which have not been associated with the disease before. We report thumb and spine osteoarthritis risk variants and identify differences in genetic effects between weight-bearing and non-weight-bearing joints. We identify sex-specific and early age-at-onset osteoarthritis risk loci. We integrate functional genomics data from primary patient tissues (including articular cartilage, subchondral bone, and osteophytic cartilage) and identify high-confidence effector genes. We provide evidence for genetic correlation with phenotypes related to pain, the main disease symptom, and identify likely causal genes linked to neuronal processes. Our results provide insights into key molecular players in disease processes and highlight attractive drug targets to accelerate translation
