    Cirugía mínimamente invasiva de estrabismo modificada

    Introducción: La cirugía mínimamente invasiva de estrabismo ha revolucionado el concepto de la cirugía tradicional de estrabismo. Este procedimiento se realiza con mínima disrupción anatómica entre el músculo y el tejido perimuscular. Objetivo: Describir una propuesta de modificación a la técnica de cirugía mínimamente invasiva de estrabismo. Métodos: Se revisó la literatura en español e inglés, sin restricciones de fecha, para investigar modificaciones reportadas a la técnica de cirugía mínimamente invasiva de estrabismo. En las modificaciones revisadas, coexiste la posibilidad de que ocurra la complicación de músculo deslizado o músculo perdido. Para realizar una técnica quirúrgica segura se identificaron los aspectos que pueden influir en esta, que son el agarre muscular y su fijación escleral, sin ampliar las microincisiones. Resultados: En la técnica modificada se emplearon pinzas curvas de Moody-Castroviejo, para ampliar el campo operatorio. Se utilizó una sola sutura no reabsorbible de ácido poliglicólico para el agarre muscular; se pasa primero por el espesor muscular y después por los extremos superior e inferior de la inserción muscular, lo cual ofrece seguridad al procedimiento. Se propone el uso del Dardenne, instrumento oftalmológico utilizado en la realización de pupiloplastias, para traer el extremo opuesto de la sutura y realizar un nudo de Siepser; al cortar la sutura, quedan los cabos por debajo del túnel conjuntival, sin ampliar las microincisiones. Conclusiones: Las modificaciones a la técnica de cirugía mínimamente invasiva de estrabismo, que propone este trabajo, permiten mejor agarre y fijación del músculo

    Analysis of the Use and Applicability of Different Variables for the Prescription of Relative Intensity in Bench Press Exercise

    The aim of the study was to analyze the use of variables such as % of one-repetition maximum (1RM) and number of maximal repetitions (xRM) with execution velocity to define and control the intensity of resistance training in bench press exercise. Hence, exercise professionals will achieve better control of training through a greater understanding of its variables. Methods: In this cross-sectional study, fifty male physical education students were divided into four groups according to their relative strength ratio (RSR) and performed a 1RM bench press test (T1). In the second test, participants performed repetitions to exhaustion (T2), using a relative load corresponding to 70% 1RM determined through the mean propulsive velocity (MPV) obtained from the individual load–velocity relationship. This same test was repeated a week later (T3). Tests were monitored according to the MPV of each repetition and blood lactate values (LACT). Results: Regarding MPV, the best (fastest) repetition of the set (MPVrep Best) values were similar between groups (0.62 m·s−1–0.64 m·s−1), with significant differences in relation to the high RSR group (p 0.05). Average LACT values (5.72 mmol·L−1) showed significant differences between the Background: Medium RSR and Very Low RSR groups. No significant differences were found (p > 0.05) between T2 and T3 with regards to MNR, MPVrep Best, or MPVrep Last, with little variability seen between participants. Conclusions: The use of variables such as the 1RM, estimated using an absolute load value, or an MNR do not allow an adequate degree of precision to prescribe and control the relative intensity of resistance training.     Ozonoterapia como tratamento adjuvante em úlcera bacteriana da córnea grave

    Introduction: spontaneous evolution or poorly treated cases of corneal ulcer carry the risk of extension of the infection, with severe visual impairment and damage to the structural integrity of the eye. Objective: to describe the clinical-epidemiological characteristics of patients with severe bacterial corneal ulcer treated with local ozone therapy as an adjunct to the standard protocol treatment. Method: an observational, descriptive and cross-sectional study was carried out on 48 patients admitted to the Ophthalmology service of the Hospital General Docente “Dr. Juan Bruno Zayas Alfonso”, from Santiago de Cuba, in the period from January 2017 to December 2019. The study variables were: age, gender, predisposing factors, microorganisms, signs of clinical improvement, treatment results and complications. Chi-square test was used for statistical validation. Results: the average age of the patients was 54.1 years; 62.5% of them were men. Ocular trauma (63.3%) predominated in males, and ocular diseases (44.4%) in females. The improvement of the signs was more common after 14 days. Satisfactory results were achieved in 100% of patients with gram-positive organisms. 10.4% presented corneal perforation. Conclusions: ozone therapy is valid as a treatment for severe corneal ulcer of bacterial etiology, and responds to the search for alternatives for patients with resistance to the antibacterial treatments that are offered in the basic health system.Introducción: la evolución espontánea o los casos mal tratados de la úlcera corneal conllevan el riesgo de extensión de la infección, con severa afectación visual e integridad estructural del ojo. Objetivo: describir las características clínico-epidemiológicas de pacientes con úlcera corneal grave bacteriana tratada con ozonoterapia local coadyuvante al tratamiento protocolizado. Método: se realizó un estudio observacional, descriptivo y transversal con 48 pacientes ingresados en el servicio de Oftalmología del Hospital General Docente “Dr. Juan Bruno Zayas Alfonso”, de Santiago de Cuba, en el periodo de enero de 2017 a diciembre de 2019. Las variables del estudio fueron: edad, sexo, factores predisponentes, microorganismos, signos de mejoría clínica, resultados al tratamiento y complicaciones. Para la validación estadística se utilizó prueba Chi-cuadrado. Resultados: la edad promedio de los pacientes fue de 54,1 años, el 62,5 % eran hombres. Predominó el trauma ocular (63,3 %) en el sexo masculino y enfermedades oculares (44,4 %) en el femenino. La mejoría de los signos fue más representativa a los 14 días. Se logró resultados satisfactorios en el 100 % de los pacientes con microorganismos grampositivos. El 10,4 % presentó perforación corneal. Conclusiones: la ozonoterapia es una terapia válida como tratamiento en la úlcera corneal grave de etiología bacteriana y responde a la búsqueda de alternativas para pacientes con resistencia a los tratamientos antibacterianos que se ofertan en el cuadro básico de salud.Introdução: a evolução espontânea ou casos mal tratados de úlcera de córnea trazem o risco de extensão da infecção, com comprometimento visual grave e integridade estrutural do olho.     Primer consenso en leucemia linfocítica crónica de la agrupación mexicana para el estudio de la hematología: epidemiología, diagnóstico y tratamiento

    La leucemia linfocítica crónica (LLC) es la leucemia crónica menos frecuente en México. En consideración a los avances recientes, a una mejor clasificación pronóstica y a la introducción de nuevas modalidades terapéuticas, la Agrupación Mexicana para el Estudio de la Hematología organizó el primer consenso mexicano en leucemia linfocítica crónica. Este consenso se llevó a cabo en Cancún, Quintana Roo, México, en Septiembre del 2007. Los objetivos de esta reunión fueron actualizar y compartir los conocimientos de la enfermedad entre los especialistas mexicanos, con el fin de mejorar el diagnóstico y el pronóstico de la LLC en México.     J-PLUS : a catalogue of globular cluster candidates around the M 81/M 82/NGC 3077 triplet of galaxies

    Globular clusters (GCs) are proxies of the formation assemblies of their host galaxies. However, few studies exist targeting GC systems of spiral galaxies up to several effective radii. Through 12-band Javalambre Photometric Local Universe Survey (J-PLUS) imaging, we study the point sources around the M 81/M 82/NGC 3077 triplet in search of new GC candidates. We develop a tailored classification scheme to search for GC candidates based on their similarity to known GCs via a principal component analysis projection. Our method accounts for missing data and photometric errors. We report 642 new GC candidates in a region of 3.5 deg2 around the triplet, ranked according to their Gaia astrometric proper motions when available. We find tantalizing evidence for an overdensity of GC candidate sources forming a bridge connecting M 81 and M 82. Finally, the spatial distribution of the GC candidates (g − i) colours is consistent with halo/intra-cluster GCs, i.e. it gets bluer as they get further from the closest galaxy in the field. We further employ a regression-tree-based model to estimate the metallicity distribution of the GC candidates based on their J-PLUS bands. The metallicity distribution of the sample candidates is broad and displays a bump towards the metal-rich end. Our list increases the population of GC candidates around the triplet by threefold, stresses the usefulness of multiband surveys in finding these objects, and provides a testbed for further studies analysing their spatial distribution around nearby (spirals) galaxies

    Towards precision medicine: defining and characterizing adipose tissue dysfunction to identify early immunometabolic risk in symptom-free adults from the GEMM family study

    Interactions between macrophages and adipocytes are early molecular factors influencing adipose tissue (AT) dysfunction, resulting in high leptin, low adiponectin circulating levels and low-grade metaflammation, leading to insulin resistance (IR) with increased cardiovascular risk. We report the characterization of AT dysfunction through measurements of the adiponectin/leptin ratio (ALR), the adipo-insulin resistance index (Adipo-IRi), fasting/postprandial (F/P) immunometabolic phenotyping and direct F/P differential gene expression in AT biopsies obtained from symptom-free adults from the GEMM family study. AT dysfunction was evaluated through associations of the ALR with F/P insulin-glucose axis, lipid-lipoprotein metabolism, and inflammatory markers. A relevant pattern of negative associations between decreased ALR and markers of systemic low-grade metaflammation, HOMA, and postprandial cardiovascular risk hyperinsulinemic, triglyceride and GLP-1 curves was found. We also analysed their plasma non-coding microRNAs and shotgun lipidomics profiles finding trends that may reflect a pattern of adipose tissue dysfunction in the fed and fasted state. Direct gene differential expression data showed initial patterns of AT molecular signatures of key immunometabolic genes involved in AT expansion, angiogenic remodelling and immune cell migration. These data reinforce the central, early role of AT dysfunction at the molecular and systemic level in the pathogenesis of IR and immunometabolic disorders

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Con responsabilidad, se organizó un programa cuya finalidad fuera publicitar con transparencia dichos avances, a través de un esfuerzo de rendición de cuentas a la comunidad inmediata, la universitaria, y a la comunidad abierta, la sociedad que la principal referencia para tal efecto. El programa se concretiza a través del presente libro, conformado con una inspiración de investigación multidisciplinaria; sin embargo, para llegar a tal fin, el reto es realizar el proceso de búsqueda y generación de conocimiento transitando hacia la colaboración de los cuerpos académicos, que puedan construir nuevos conocimientos fortalecidos por la convergencia de diferentes campos del saber. En consecuencia, la primera etapa de esta estrategia es la publicidad de los trabajos investigativos ejercidos, para hacer un balance al día, pero también proyectar el futuro de cada campo y área del conocimiento. La organización explicativa está organizada por tres bloques representativos del quehacer en la generación de conocimiento del Centro Universitario, un primer bloque centra el interés en las humanidades, educación y sustentabilidad; el segundo bloque lo integra la reflexión científica sobre la construcción democrática, derechos humanos y equidad de género; en el tercer segmento se destina a la seguridad alimentaria, salud pública y sistemas agropecuarios. La actualidad de la investigación eleva la producción lograda y lo que en el momento se encuentra en construcción y los alcances que produce para la docencia, la investigación misma, y para la sociedad en general. La prospectiva es un área que todos los capítulos desarrollan con el propósito de delinear los alcances innovadores por andar en teoría, metodología e incluso en los saberes mismo

    Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
