    Augmented EHR: Enrichment of EHR with Contents from Semantic Web Sources

    This work presents methods to combine data from the Semantic Web into existing EHRs, leading to an augmented EHR. An existing EHR extract is augmented by combining it with additional information from external sources, typically linked data sources. The starting point is a standardized EHR extract described by an archetype. The method consists of combining specific data from the original EHR with contents from the external information source by building a semantic representation, which is used to query the external source. The results are converted into a standardized EHR extract according to an archetype. This work sets the foundations to transform Semantic Web contents into normalized EHR extracts. Finally, to exemplify the approach, the work includes a practical use case in which the summarized EHR is augmented with drug–drug interactions and disease-related treatment information

    Multitasking Compensatory Saccadic Training Program for Hemianopia Patients: A New Approach With 3-Dimensional Real-World Objects

    Producción CientíficaPurpose: To examinewhether a noncomputerized multitasking compensatory saccadic training program (MCSTP) for patients with hemianopia, based on a reading regimen and eight exercises that recreate everyday visuomotor activities using threedimensional (3D) real-world objects, improves the visual ability/function, quality of life (QL), and functional independence (FI). Methods: The 3D-MCSTP included four in-office visits and two customized homebased daily training sessions over 12weeks. A quasiexperimental, pretest/posttest study designwas carried out with an intervention group (IG) (n = 20) and a no-training group (NTG) (n = 20) matched for age, hemianopia type, and brain injury duration. Results: The groups were comparable for the main baseline variables and all participants (n = 40) completed the study. The IGmainly showed significant improvements in visual-processing speed (57.34% ± 19.28%; P < 0.0001) and visual attention/retention ability (26.67% ± 19.21%; P < 0.0001), which also were significantly greater (P < 0.05) than in the NTG. Moreover, the IG showed large effect sizes (Cohen’s d) in 75% of the totalQL and FI dimensions analyzed; in contrast to the NTGthat showed negligiblemean effect sizes in 96% of these dimensions. Conclusions: The customized 3D-MCSTP was associated with a satisfactory response in the IG for improving complex visual processing, QL, and FI. Translational Relevance: Neurovisual rehabilitation of patientswith hemianopia seems more efficient when programs combine in-office visits and customized home-based training sessions based on real objects and simulating real-life conditions, than no treatment or previously reported computer-screen approaches, probably because of better stimulation of patients´ motivation and visual-processing speed brain mechanisms

    Visual processing speed in hemianopia patients secondary to acquired brain injury: a new assessment methodology

    Producción CientíficaBackground: There is a clinical need to identify diagnostic parameters that objectively quantify and monitor the effective visual ability of patients with homonymous visual field defects (HVFDs). Visual processing speed (VPS) is an objective measure of visual ability. It is the reaction time (RT) needed to correctly search and/or reach for a visual stimulus. VPS depends on six main brain processing systems: auditory-cognitive, attentional, working memory, visuocognitive, visuomotor, and executive. We designed a new assessment methodology capable of activating these six systems and measuring RTs to determine the VPS of patients with HVFDs. Methods: New software was designed for assessing subject visual stimulus search and reach times (S-RT and R-RT respectively), measured in seconds. Thirty-two different everyday visual stimuli were divided in four complexity groups that were presented along 8 radial visual field positions at three different eccentricities (10o, 20o, and 30o). Thus, for each HVFD and control subject, 96 S- and R-RT measures related to VPS were registered. Three additional variables were measured to gather objective data on the validity of the test: eye-hand coordination mistakes (ehcM), eye-hand coordination accuracy (ehcA), and degrees of head movement (dHM, measured by a head-tracker system). HVFD patients and healthy controls (30 each) matched by age and gender were included. Each subject was assessed in a single visit. VPS measurements for HFVD patients and control subjects were compared for the complete test, for each stimulus complexity group, and for each eccentricity. Results: VPS was significantly slower (p < 0.0001) in the HVFD group for the complete test, each stimulus complexity group, and each eccentricity. For the complete test, the VPS of the HVFD patients was 73.0% slower than controls. They also had 335.6% more ehcMs, 41.3% worse ehcA, and 189.0% more dHMs than the controls. Conclusions: Measurement of VPS by this new assessment methodology could be an effective tool for objectively quantifying the visual ability of HVFD patients. Future research should evaluate the effectiveness of this novel method for measuring the impact that any specific neurovisual rehabilitation program has for these patients

    Juvenile neurogenesis makes essential contributions to adult brain structure and plays a sex-dependent role in fear memories

    Postnatal neurogenesis (PNN) contributes neurons to olfactory bulb (OB) and dentate gyrus (DG) throughout juvenile development, but the quantitative amount, temporal dynamics and functional roles of this contribution have not been defined. By using transgenic mouse models for cell lineage tracing and conditional cell ablation, we found that juvenile neurogenesis gradually increased the total number of granule neurons by approximately 40% in OB, and by 25% in DG, between 2 weeks and 2 months of age, and that total numbers remained stable thereafter. These findings indicate that the overwhelming majority of net postnatal neuronal addition in these regions occurs during the juvenile period and that adult neurogenesis contributes primarily to replacement of granule cells in both regions. Behavioral analysis in our conditional cell ablation mouse model showed that complete loss of PNN throughout both the juvenile and young adult period produced a specific set of sex-dependent cognitive changes. We observed normal hippocampus-independent delay fear conditioning, but excessive generalization of fear to a novel auditory stimulus, which is consistent with a role for PNN in psychopathology. Standard contextual fear conditioning was intact, however, pre-exposure dependent contextual fear was impaired suggesting a specific role for PNN in incidental contextual learning. Contextual discrimination between two highly similar contexts was enhanced; suggesting either enhanced contextual pattern separation or impaired temporal integration. We also observed a reduced reliance on olfactory cues, consistent with a role for OB PNN in the efficient processing of olfactory information. Thus, juvenile neurogenesis adds substantively to the total numbers of granule neurons in OB and DG during periods of critical juvenile behavioral development, including weaning, early social interactions and sexual maturation, and plays a sex-dependent role in fear memories

    Bilateral ischemic optic neuropathy after transurethral prostatic resection: a case report

    BACKGROUND: Nonarteritic ischemic optic neuropathy affects the anterior portion of the optic nerve and is characterized by sudden, painless visual loss. The affected eye has a relative afferent pupillary defect. The typical funduscopic appearance includes optic disc edema, with associated nerve fiber layer hemorrhage. Risk factors include advanced age, systemic hypertension, nocturnal hypotension, diabetes mellitus, and a small cup-to-disc ratio. Bilateral presentation is rare. Postoperative optic neuropathy has been associated with nonocular surgery; risk factors include a combination of prolonged surgical times, acute systemic hypotension, anemia due to blood loss, or prone positioning. We report for the first time a patient with bilateral, simultaneous anterior ischemic optic neuropathy after elective transurethral prostatic resection. CASE PRESENTATION: A 66-year old man underwent surgery for benign prostatic hyperplasia. The preoperative blood pressure was 140/85 mmHg, hemoglobin 15.9 g/dL, and hematocrit 48.6%. Two hours postoperatively, the blood pressure, hemoglobin, and hematocrit dropped dramatically. One day later, transient horizontal diplopia developed. Funduscopy showed a congenitally small cup-to-disc ratio without papillary edema. Other ocular findings were unremarkable. By 4 days postoperatively, sudden and painless amaurosis bilaterally developed when the patient awoke with nausea and vomiting. Visual acuity was no light perception bilaterally. The optic discs were swollen with small hemorrhages. Scans of the head and orbits and electrolyte levels were normal. There were no responses on visual evoked potentials bilaterally. The blood pressure was 90/50 mm Hg, the hemoglobin 7.0 g/dL, and the hematocrit 22.9%, necessitating infusion of three units of packed red blood cells. The blood pressure, hematocrit, and hemoglobin increased to normal levels. Three months later the visual acuity remained no light perception. The pupils were unreactive and there was marked optic disc atrophy bilaterally. CONCLUSION: Bilateral and simultaneous acute ischemic optic neuropathy may be a rare but devastating surgical complication. The combination of anemia and hypotension may increase the risk of anterior ischemic optic neuropathy postoperatively after transurethral prostatic resection

    Evaluación del trabajo colaborativo en Iniciación a la Investigación en Biología

    Tras el trabajo previo de diseño de la asignatura Iniciación a la Investigación en Biología, el equipo multidisciplinar de profesores y profesoras de la Red Docente INVES ha desarrollado una metodología propia de trabajo en equipo, no sólo entre el diferente profesorado que la compone, sino también con el profesorado de la asignatura Estadística, con la que se comparten objetivos de aprendizaje comunes. Se ha optimizado el sistema de evaluación del trabajo colaborativo del alumnado, mediante el uso de rubricas y auto-evaluación. Dicho trabajo consiste en el diseño y desarrollo de un proyecto de investigación bibliométrico de temática biológica realizado por los estudiantes, propiciando la adquisición de competencias transversales mediante una dinámica de trabajo en grupo que culmina en la edición de unas Jornadas Científicas. Por otra parte, se han consensuado criterios comunes de evaluación continua, mejorando en la eficiencia de la evaluación, y determinado un incremento de la capacidad de aprendizaje del alumnado a lo largo de los cursos 2010-11 al 2013-14. La oferta formativa se completa mediante la formación de un grupo de Alto Rendimiento Académico con docencia en lengua inglesa. Esto permite al alumnado implementar el objetivo general de compresión de lengua extranjera inglés en lo relativo al ámbito científico

    Un equipo multidisciplinar de profesores y profesoras que componen la Red Docente INVES e imparten docencia en la asignatura Iniciación a la Investigación en Biología, ha desarrollado una metodología propia de trabajo en equipo, en coordinación con el profesorado de la asignatura Estadística, con la que se comparten objetivos de aprendizaje comunes. El sistema de evaluación del trabajo colaborativo del alumnado se ha optimizado mediante el uso de rúbricas y auto-evaluación. Se ha propiciado la adquisición de competencias transversales mediante una dinámica de trabajo en grupo. El diseño y desarrollo de un proyecto de investigación bibliométrico, de temática biológica, es realizado por los y las estudiantes, y culmina con la edición de unas Jornadas Científicas. Con el fin de mejorar la eficiencia de la evaluación, se han consensuado criterios comunes de evaluación continua entre el profesorado. Ello ha determinado un incremento de la capacidad de aprendizaje del alumnado a lo largo de los cursos 2010-11 al 2013-14. La lectura y compresión de textos científicos en inglés junto a la formación de un grupo de Alto Rendimiento Académico con docencia en lengua inglesa completa la oferta formativa, permitiendo al alumnado implementar el objetivo general de compresión de lengua extranjera inglés en lo relativo al ámbito científico

    The novel object recognition (NOR) task is a behavioral test commonly used to evaluate episodic-like declarative memory and it relies on the innate tendency of rodents to explore novelty. Here we present a maze used to evaluate NOR memory in mice that reduces the time of the assay while improving reliability of the measurements by increasing the exploratory behavior. This memory test, being performed in a two-arms maze, is suitable for several strains of mice (including inbreed and outbreed) and does not require extended training sessions allowing an accurate temporal assessment of memory formation. This particular maze increases the mouse exploration time and reduces variability compared to other arenas used before to assess NOR. As both long- and short-term NOR memory can be easily and accurately quantified using this paradigm, this improved methodology can be easily applied to study pharmacological, genetic or age-related modulation of cognitive function.     Gender differences in attentional bias after owning a virtual avatar with increased weight

    Introduction: Eating Disorder (ED) patients selectively attend to appearance cues in preference to other information, in a phenomenon known as Attentional Bias (AB). The latest VR Head Mounted Displays (HMD) offer the chance to include Eye-Tracking (ET) devices, and thus provide more objective measures of body-related attention. This study aims to combine VR and ET technologies and use a VR-based embodiment technique while measuring real-time attention patterns. Specifically, we assess gender differences in eye-gaze behaviors towards specific weight-related or non-weight-related body parts when participants own a virtual avatar with different body sizes. Method: Thirty-five college students (25 women and 10 men) were exposed to an immersive virtual environment in which they were embodied in three avatars with different body sizes: first, one with the same body size as the participant; second, one larger than the participant; and finally, repetition of the avatar with the same body size as the participant. To analyze the gaze data Weight-related Areas of Interest (WAOIs) and Non-Weight-related Areas of Interest (NW-AOIs) were defined. Fixation points and complete fixation time on each AOI were recorded at the three different assessment times. Raw data from the Pupil Labs eye tracking add-on for the HTC-Vive headset were subsequently transformed into percentages for further analysis. Results: Mixed between (Gender)-within (Time) analyses of variance showednon-statistically significant interaction between gender and time (p>.05) and a non-statistically significant difference in fixation points and complete fixation times (p>.05), over the three assessment times. However, a statistically significant gender difference was found in fixation points (F (1.33) =10,030, p= .003, η2 = 0.233) and complete fixation time (F (1.33) =13,017, p= .001, η2 = 0.28. Overall, women reported significantly higher levels of fixation points and complete fixation times in W-AOIs than men. Women showed an increasing gaze pattern towards W-AOIs at the three different assessment times, while men showed an opposite gaze pattern towards NW-AOIs at the three different times. Interestingly, the greatest differences between men and women were found at the third assessment, when they once again owned an avatar with the same body size as themselves. Conclusion: This study provides useful information about gender differences in gaze pattern behaviors while participants owned a virtual avatar with different body sizes. To our knowledge, this is the first study to compare gaze pattern behaviors between women and men using VR-based embodiment techniques and ET attentional bias assessment. The use of these two technologies opens a promising new area in the assessment or treatment of Eating Disorders and body image disturbances
