Preventing incisional ventral hernias: important for patients but ignored by surgical specialities? A critical review.

    Get PDF
Purpose Incisional ventral hernias (IHs) are a common complication across all surgical specialities requiring access to the abdomen, pelvis, and retroperitoneum. This public health issue continues to be widely ignored, resulting in appreciable morbidity and expenses. In this critical review, the issue is explored by an interdisciplinary group. Methods A group of European surgeons encompassing representatives from abdominal wall, vascular, urological, gynecological, colorectal and hepato-pancreatico-biliary surgery have reviewed the occurrence of His in these disciplines. Results Incisional hernias are a major public health issue with appreciable morbidity and cost implications. General surgeons are commonly called upon to repair IHs following an initial operation by others. Measures that may collectively reduce the frequency of IH across specialities include better planning and preparation (e.g. a fit patient, no time pressure, an experienced operator). A minimally invasive technique should be employed where appropriate. Our main recommendations in midline incisions include using the ‘small bites’ suture technique with a ≥ 4:1 suture-to-wound length, and adding prophylactic mesh augmentation in patients more likely to suffer herniation. For off-midline incisions, more research of this problem is essential. Conclusion Meticulous closure of the incision is significant for every patient. Raising awareness of the His is necessary in all surgical disciplines that work withing the abdomen or retroperitoneum. Across all specialties, surgeons should aim for a < 10% IH rate.pre-print146 K

Deep XMM-Newton observation of the Eta Chamaleontis cluster

The members of the Eta Chamaleontis cluster are in an evolutionary stage in which disks are rapidly evolving. It also presents some peculiarities, such as the large fraction of binaries and accretion disks, probably related with the cluster formation process. Its proximity makes this stellar group an ideal target for studying the relation between X-ray emission and those stellar parameters. The main objective of this work is to determine general X-ray properties of the cluster members in terms of coronal temperature, column density, emission measure, X-ray luminosity and variability. We also aim to establish the relation between the X-ray luminosity of these stars and other stellar parameters, such as binarity and presence of accretion disks. A study of flare energies for each flare event and their relation with some stellar parameters is also performed. We used proprietary data from a deep XMM-Newton observation pointed at the core of the Eta Chamaleontis cluster. Specific software for the reduction of XMM-Newton data was used for the analysis of our observation. For the detection of sources, we used the wavelet-based code PWDetect. General coronal properties were derived from plasma model fitting. We also determined variability of the Eta Chamaleontis members in the EPIC field-of-view. A total of six flare-like events were clearly detected in five different stars. For them, we derived coronal properties during the flare events and pseudo-quiescent state separately. In our observations, stars that underwent a flare event have higher X-ray luminosities in the pseudo-quiescent state than cluster members with similar spectral type with no indications of flaring, independently whether they have an accretion disk or not. Observed flare energies are typical of both pre-main and main-sequence M stars. We detected no difference between flare energies of stars with and without an accretion disk.Comment: Accepted for publication in Astronomy and Astrophysics. 12 pages, 30 individual figure

Beach Leveling Using a Remotely Piloted Aircraft System (RPAS): Problems and Solutions

    Get PDF
The size and great dynamism of coastal systems require faster and more automated mapping methods like the use of a remotely piloted aircraft system (RPAS) or unmanned aerial vehicle (UAV). This method allows for shorter intervals between surveys. The main problem for surveying using low-altitude digital photogrammetry in beach areas is their visual homogeneity. Obviously, the fewer the homologous points defined by the program, the lower the accuracy. Moreover, some factors influence the error performed in photogrammetric techniques, such as flight height, flight time, percentage of frame overlap (side and forward), and the number of ground control points (GCPs). A total of 72 different cases were conducted varying these factors, and the results were analyzed. Among the conclusions, it should be highlighted that the error for noon flights is almost double that for the early morning flights. Secondly, there is no appreciable difference regarding the side overlap. But, on the other side, RMSE increased to three times (from 0.05 to 0.15 m) when forward overlap decreased from 85% to 70%. Moreover, relative accuracy is 0.05% of the flying height which means a significant increase in error (66%) between flights performed at 60 and 100 m height). Furthermore, the median of the error for noon flights (0.12 m) is almost double that for the early morning flights (0.07 m) because of the higher percentage of grids with data for early flights. Therefore, beach levelings must never be performed at noon when carried out by RPAS. Eventually, a new parameter has been considered: the relationship between the number of GCPs and the surface to be monitored. A minimum value of 7 GCP/Ha should be taken into account when designing a beach leveling campaign using RPAS.Fundacion Campus Tecnologico de Algeciras; Coastal Engineering Research group (University of Cadiz

Skin Cancer Prevalence in Outdoor Workers of Ski Resorts

    Get PDF
Background. Snow reflectivity and altitude increase the exposure of ski resort workers to solar ultraviolet radiation. The aim was to assess the presence of skin cancer in ski resorts workers and compare it with other groups of outdoor workers reviewing published studies. Methods. An observational cross-sectional prospective study was conducted in the three largest ski resorts in Spain: Baqueira Beret, Lleida; Formigal, Huesca and Sierra Nevada, Granada. All outdoor workers including ski instructors were invited to participate in the study. The participants completed a validated questionnaire about sun exposure and underwent a skin examination. Results. 219 workers were included in the study (80% male; mean age 43.8 (SD 11.31) years). Actinic keratosis (AK) but no other skin cancers were detected in 32 participants (14.62%). Those with AK worked in the Southernmost ski resort, were more likely to have light colour hair, and were older and with higher photoaging grade than those without them. Conclusion. Compared to other studies, outdoor workers on ski resorts show a higher prevalence of AK than general population but a lower prevalence than other groups of outdoor workers. © 2020 Yolanda Gilaberte et al

Polyamines containing naphthyl groups as pH-regulated molecular machines driven by light

    Get PDF
A series of compounds made up by linking methylnaphthalene fragments at both ends of different polyamine chains have shown to behave as pH-regulated molecular machines driven by light and fluorescence emission studies have proved the formation of an excimer between the two naphthalene units whose appearance, fluorescence intensity and decay times depend on the pH value of the media.Albelda Gimeno, Maria Teresa, [email protected] ; Garcia-España Monsonis, Enrique, [email protected] ; Soriano Soto, Concepción, [email protected]

Experimental transition probabilities in NII lines

    Get PDF
Producción CientíficaThis work reports an extensive collection of 108 transition probabilities of NII lines in the visible spectral region, all of them measured in an emission experiment. Relative intensity measurements have been made on a pulsed discharge lamp and the absolute Aki values have been obtained by using the literature available data. Electron density and temperature range in this experiment from 0.2 to and from 17000 to 29000 K respectively. The first one has been simultaneously determined from two-wavelength interferometry and from the Stark broadening of HeI 471.3 nm, the second from Boltzmann-plot of NII lines and from NII/NI intensities ratios. The results are compared with the recent available literature.

Controlling a Mouse Pointer with a Single-Channel EEG Sensor

    Get PDF
Goals: The purpose of this study was to analyze the feasibility of using the information obtained from a one-channel electro-encephalography (EEG) signal to control a mouse pointer. We used a low-cost headset, with one dry sensor placed at the FP1 position, to steer a mouse pointer and make selections through a combination of the user’s attention level with the detection of voluntary blinks. There are two types of cursor movements: spinning and linear displacement. A sequence of blinks allows for switching between these movement types, while the attention level modulates the cursor’s speed. The influence of the attention level on performance was studied. Additionally, Fitts’ model and the evolution of the emotional states of participants, among other trajectory indicators, were analyzed. (2) Methods: Twenty participants distributed into two groups (Attention and No-Attention) performed three runs, on different days, in which 40 targets had to be reached and selected. Target positions and distances from the cursor’s initial position were chosen, providing eight different indices of difficulty (IDs). A self-assessment manikin (SAM) test and a final survey provided information about the system’s usability and the emotions of participants during the experiment. (3) Results: The performance was similar to some brain–computer interface (BCI) solutions found in the literature, with an averaged information transfer rate (ITR) of 7 bits/min. Concerning the cursor navigation, some trajectory indicators showed our proposed approach to be as good as common pointing devices, such as joysticks, trackballs, and so on. Only one of the 20 participants reported difficulty in managing the cursor and, according to the tests, most of them assessed the experience positively. Movement times and hit rates were significantly better for participants belonging to the attention group. (4) Conclusions: The proposed approach is a feasible low-cost solution to manage a mouse pointe
