Sokołowicz.Guests and travel shoppers constantly search and book hotel rooms all over the world on various devices, shifting from one screen to another. Thanks to internet, “anytime, anywhere” access to information quite far outweighs its downsides. In the paper, the author focuses on the efficient use of the Internet as a tool which helps to communicate with clients. The reaction to inquiries sent by email influences both booking behavior and guest satisfaction as well. The objective of the survey presented in this paper is to test empirically response behavior to Internet enquiries. In particular, response time and information quality of received answers (information depth) were also analyzed. Sokołowicz.The paper presents the mapping of potential next industrial revolution clusters in Poland and Siberia. Deindustrialization of the cities and struggles with its consequences are one of the fundamental economic problems in current global economy. Some hope to find an answer to that problem is associated with the idea of next industrial revolution and reindustrialization initiatives. In the paper, projects aimed at developing next industrial revolution clusters are analyzed.     Наведено результати зміни пружних властивостей гірських порід внаслідок впливу ибухових навантажень. Оцінка пружних властивостей проведена за швидкістю розповсюдження ультразвукових хвиль в зразках гірських порід. Визначено, що коефіцієнт зниження амплітуди ультразвукового сигналу 0,6…1,0 свідчить про вертикальну орієнтацію технологічної тріщинуватості. Отримано необхідну кількість вибухових навантажень (4…6) для зниження енергоємності механічного руйнування вибою.Rocks’ elastic properties change under the influence of explosive stressing was showed. Evaluation of elastic properties was carried by ultrasonic waves distribution speed in rock samples. Was established that ultrasonic signal amplitude decrease coefficient 0,6…1,0 demonstrates vertical orientation of technological fracturing. The necessary quantity of explosion stressing (4…6) was determined for specific energy decreasing to drill а well.Приведены результаты изменения упругих свойств горных пород под воздействием взрывных нагружений. Оценка упругих свойств горных пород проведена посредством скорости распространения продольных ультразвуковых волн. Определено, что коэффициент снижения амплитуды ультразвукового сигнала 0,6…1,0 свидетельствует о вертикальной ориентации технологических трещин. Получено необходимое количество взрывных нагружений (4…6) для снижения энергоемкости разрушения забоя

    Early Gastric Cancer

    Early Gastric Cancer (EGC) is a carcinoma limited to the gastric mucosa or submucosa without the involvement of any lymph node. In Indonesia, the prevalence of EGC in 1980 was 2.2% and 1.7% for Jakarta and Medan, respectively. From 1980-1987 in Surabaya, the prevalence was 9.1% from all gastric cancers. Gastric mucosal abnormalities include atrophic gastritis, which is frequently accompanied by achlorhidria or hipochlorhidria and pernicious anemia, and the presence of an ulcer or polyp were believed to be precarcinogenic factors. Environmental factors, life style, age, sex, genetic factors, race, as well as dietary factors, especially intake of foods containing N-nitrosa (N-nitrosa compound) might play a role as risk factors for EGC. H.pylori infection also causes an increased risk for EGC. The diagnosis of EGC is based on physical examination, occult blood in stool sample, cytology, double contrast roentgenologic examination, gastroscopy, gastrobiopsy, and radioactive phosphor. There are no tumor markers specific for EGC. Histologically, EGC is classified into intestinal and diffuse infiltrative EGC. In 1962, the Japanese Research Society for Gastric Cancer made a classification for EGC based on gastroscopy, fluoroscopy, histopathology and microscopic examinations. In Japan, detection for EGC was performed by spraying the gastric mucosa with methylen blue during endoscopy, which will stain intestinal mucosa and spare normal mucosa. Early detection of EGC in Japan was performed through mass screening of people ages 40-50 years with recent dyspepsia, by means of endoscopy, biopsy, and upper GI tract radiological examinations. Endoscopic Ultrasonography (EUS) is the most accurate tool to determine EGC staging, particularly those with non-ulcerative lessions. The choices of treatment for EGC are surgical therapy or Endoscopic Mucosal Resection (EMR). EMR is a localized therapy, and it is indicated for EGC without metastases, for patients unwilling to undergo operation, or for those who are bad candidates for operation. The prognosis for EGC does not depend upon microscopic classification, but mostly on the depth of gastric mucosal invasion, spread to regional lymph nodes, and the presence of distant metastases. By establishing the diagnosis of EGC, the prognosis is usually better, for the treatment can be given at an earlier stage

    Early Acute Liver Failure in Severe Acute Hepatitis B

    Early acute liver failure is defined by the presence of a drop below 50% of the normal prothrombine ratio, jaundice, and clotting factors of less than 50% in any patient with acute liver disease. Clinical features and laboratory manifestations of viral hepatitis range from unapparent disease, asymptomatic infection, to culminant disease, which has the highest mortality rate of up to over 80 %. We report a case of a 22-year old man who was treated in a private hospital with early acute liver failure caused by hepatitis B infection. The diagnosis was based on clinical symptoms and laboratory test Results such as jaundice, hepatomegaly, ascites, 34% prothrombine ratio, elongated prothrombine time (37,1 seconds), hypoalbuminemia (1,9 g/dL), hyperbillirubinemia (total billirubin 26,35 mg/dL, direct billirubin 16,66 mg/dL, HbsAg (+), IgM anti- HBc (+), IgM anti HAV (-), and anti HCV (-). The patient suffered from jaundice for 6 weeks and on the third week, he suffered from ascites. He had improved clinical condition and laboratory test results with conservative therapy after the seventh week. At the end of the tenth week, the patient's clinical condition and laboratory test results had reached normal, with HBsAg (-) and HBsAb (+). Whether or not interferon should have been given to this patient is still arguable

    Comparison of Platelet to Lymphocyte Ratio between Degrees of the Barcelona Clinic Liver Cancer on Hepatocellular Carcinoma Patients at Haji Adam Malik General Hospital

    BACKGROUND: Hepatocellular carcinoma (HCC) is the fifth most common malignancy. The Barcelona Clinical Liver Cancer System (BCLC), guides the treatment of patients with HCC. Platelet to lymphocyte ratio (PLR) is an inflammatory marker used as a prognostic factor disease of HCC. An increase in PLR indicates higher host's inflammatory response and is associated with aggressive HCC behaviour, according to BCLC. AIM: This study aims to determine the PLRs between among the degrees of BCLC (The Barcelona Clinic Liver Cancer) in HCC patients at Haji Adam Malik General Hospital in Medan during 2015-2016. METHODS: This retrospective study involved 166 patients with HCC who were then classified by the BCLC guidelines. PLRs among the patient's degrees of BCLC were compared using Kruskal Wallis test. RESULTS: A total of 166 HCC patients, 129 (77.7%) were men and 37 (22.3%) were women. The PLR value ​​has a median value of 17841with the lowest value of 1776 and the highest value of 223684. There were differences in PLR levels with various BCLC stages in patients with HCC at Haji Adam Malik Hospital during 2015-2016 (p = 0.026). CONCLUSION: There were differences in PLR levels with various BCLC stages in patients with HCC at Haji Adam Malik Hospital during 2015-2016
