    Identifying Factors to Increase the Adoption of Integrated Pest Management Practices: An Audience Segmentation Study

    Tailoring programs based on audience needs and priorities is a critical component of effective programs. Audience segmentation research provides an opportunity to identify subgroups based on characteristics that affect their willingness to adopt a certain behavior and allows extension programs to fulfill a variety of needs and target the most important groups within a population. Currently there is minimal research exploring the factors that influence the sustained adoption of environmentally responsible pest management practices. As a result, the purpose of this study was to explore potential extension audience segmentation strategies by evaluating factors related to differences in residents’ intentions to use integrated pest management (IPM) practices.We used online surveys to collect data from a sample of 3,588 residents. We found relatively high intentions to adopt IPM practices but there remains room to increase adoption levels. Responses of undecided ranged from 14.7% to 23.1% across all pest management practices, which demonstrates the potential for an audience to be influenced to adopt an IPM approach. We also found that demographics, complementary conservation behaviors and engagement with the natural environment characterize meaningful subgroups to guide extension program design. Interestingly, we found an interrelationship between the intention and actual adoption of other environmentally responsible practices and the intention to adopt IPM practices. Respondents with greater intent to engage in IPM were also more engaged in general conservation, water conservation and fertilizer best practices. This demonstrates that previous engagement with other environmentally responsible practices may influence the adoption of IPM. Respondents with greater intent to engage in IPM also had less negative experiences with water quality and availability, implying the possible role negative consequences can play as teaching tools in extension program

    The welfare and access to veterinary health services of mules working the mountain trails in the Gorkha region, Nepal

    Working equid populations are mainly present in low to middle-income countries, their work directly contributing to the lives of people reliant on their presence. Although assistance from working equids is important to support people and their communities in these regions, their welfare is often poor. This study aims to provide insight into the welfare status of mules distributing supplies in the Gorkha region of Nepal; a population of working equids which has been largely overlooked and under recorded. The welfare of mules was assessed via the Equid Assessment Research and Scoping (EARS) tool using a trained assessor; livelihood surveys gathered basic demographic and ownership information; and semi-structured interviews gained the perspectives of 26 key informants. Mule body condition was found to be ideal in many cases, but their management was in the majority of cases inappropriate; characterised by integumentary trauma from equipment use and inhumane handling, unsuitable dietary provision, and insufficient access to water. This difficult situation was compounded by inadequate access to suitably qualified, experienced veterinary professionals able to offer appropriate levels of support. Organisations aiming to improve welfare in these remote locations need a multifaceted approach where owners are facilitated and empowered to improve the welfare of their own equids; in addition, industry professionals are encouraged to improve training and provision within veterinary service

    Metacognitive Reflection and Insight Therapy: A Recovery-Oriented Treatment Approach for Psychosis

    Recent research has suggested that recovery from psychosis is a complex process that involves recapturing a coherent sense of self and personal agency. This poses important challenges to existing treatment models. While current evidence-based practices are designed to ameliorate symptoms and skill deficits, they are less able to address issues of subjectivity and self-experience. In this paper, we present Metacognitive Insight and Reflection Therapy (MERIT), a treatment approach that is explicitly concerned with self-experience in psychosis. This approach uses the term metacognition to describe those cognitive processes that underpin self-experience and posits that addressing metacognitive deficits will aid persons diagnosed with psychosis in making sense of the challenges they face and deciding how to effectively manage them. This review will first explore the conceptualization of psychosis as the interruption of a life and how persons experience themselves, and then discuss in more depth the construct of metacognition. We will next examine the background, practices and evidence supporting MERIT. This will be followed by a discussion of how MERIT overlaps with other emerging treatments as well as how it differs. MERIT’s capacity to engage patients who reject the idea that they have mental illness as well as cope with entrenched illness identities is highlighted. Finally, limitations and directions for future research are discussed

    Low burden strategies are needed to reduce smoking in rural healthcare settings: A lesson from cancer clinics

    Rural populations face significant smoking-related health disparities, such as a higher prevalence of lung cancer and cancer mortality, higher prevalence of smoking, and lower likelihood of receiving cessation treatment than urban counterparts. A significant proportion of health disparities in rural populations could be eliminated with low-barrier, easy-access treatment delivery methods for smoking cessation. In this study, we assessed treatment engagement among patients in rural and urban settings. Then, we examined the effect of an electronic health record-based smoking cessation module on patient receipt of evidence-based cessation care. As part of a quality improvement project, we retrospectively observed 479,798 unique patients accounting for 1,426,089 outpatient clinical encounters from June 2018-March 2019 across 766 clinics in the greater St. Louis, southern Illinois, and mid-Missouri regions. Smoking prevalence was higher in rural versus urban clinics (20.7% vs. 13.9%, 6.7% [6.3, 7.1], odds ratio = 1.6 [1.6, 1.6]

    Classifying depression symptom severity: Assessment of speech representations in personalized and generalized machine learning models

    There is an urgent need for new methods that improve the management and treatment of Major Depressive Disorder (MDD). Speech has long been regarded as a promising digital marker in this regard, with many works highlighting that speech changes associated with MDD can be captured through machine learning models. Typically, findings are based on cross-sectional data, with little work exploring the advantages of personalization in building more robust and reliable models. This work assesses the strengths of different combinations of speech representations and machine learning models, in personalized and generalized settings in a two-class depression severity classification paradigm. Key results on a longitudinal dataset highlight the benefits of personalization.     Contrasting metacognitive profiles and their association with negative symptoms in groups with schizophrenia, early psychosis and depression in a Russian sample

    Research has suggested that negative symptoms in psychotic disorders may be in part fueled by deficits in metacognition or the ability to form integrated ideas about oneself and others. One limitation of this work is that it has largely come from North America and Western Europe. To further the literature, we assessed symptoms using the Positive and Negative Syndrome Scale and Metacognition using the Metacognitive Assessment Scale - Abbreviated in a sample of outpatients with prolonged schizophrenia (n = 41), early episode psychosis (n = 37) and major depression (n = 30) gathered in Moscow, Russia. Verbal memory was assessed for use as a potential covariate. ANOVA revealed the two groups with psychosis had significantly poorer metacognitive function in terms of self-reflectivity and awareness of the other, than the group with depression. In both psychosis groups negative symptoms were more robustly related to metacognition than other forms of symptoms after controlling for neurocognition. Results support the possibility that metacognitive deficits are a psychological factor which cross culturally contributes to negative symptoms and point to metacognition as a potentially important target for intervention

    Age, Spatial, and Temporal Variations in Hospital Admissions with Malaria in Kilifi County, Kenya: A 25-Year Longitudinal Observational Study

    Background Encouraging progress has been seen with reductions in Plasmodium falciparum malaria transmission in some parts of Africa. Reduced transmission might lead to increasing susceptibility to malaria among older children, which has implications for ongoing control strategies. Methods and findings We conducted a longitudinal observational study of children admitted to Kilifi County Hospital in Kenya and linked to data on residence and Insecticide Treated Net (ITN) ownership. This included data from 69,104 children admitted to Kilifi County Hospital aged from 3 months to 13 years between 1st January 1990 and 31st December 2014. The variation in malaria slide positivity among admissions was examined in logistic regression models using the predictors; location of residence, calendar time, child’s age, ITN use and Enhanced Vegetation Index (a proxy for soil moisture). The proportion of malaria slide positive admissions declined from 0.56 with 95% confidence interval (95%CI) 0.54 to 0.58 in 1998 to 0.07 95%CI 0.06 to 0.08 in 2009, but then increased again through to 0.24 95%CI 0.22 to 0.25 in 2014. Older children accounted for most of the increase after 2009 (0.035 95%CI (0.030 to 0.040) among young children compared to 0.22 95%CI 0.21 to 0.23 in older children). There was a non-linear relationship between malaria risk and prevalence of ITN use within a 2km radius of an admitted child’s residence such that the predicted malaria positive fraction varied from ~0.4 to <0.1 as the prevalence of ITN use varied from 20% to 80%. In this observational analysis we were unable to determine the cause of the decline in malaria between 1998 and 2009, which pre-dated the dramatic scale-up in ITN distribution and use. Conclusion Following a period of reduced transmission a cohort of older children emerged who have increased susceptibility to malaria. Further reductions in malaria transmission are needed to mitigate against the increasing burden among older children and universal ITN coverage is a promising strategy to achieve this

    Late quaternary biotic homogenization of North American mammalian faunas

    Biotic homogenization-increasing similarity of species composition among ecological communities-has been linked to anthropogenic processes operating over the last century. Fossil evidence, however, suggests that humans have had impacts on ecosystems for millennia. We quantify biotic homogenization of North American mammalian assemblages during the late Pleistocene through Holocene (similar to 30,000 ybp to recent), a timespan encompassing increased evidence of humans on the landscape (similar to 20,000-14,000 ybp). From similar to 10,000 ybp to recent, assemblages became significantly more homogenous (>100% increase in Jaccard similarity), a pattern that cannot be explained by changes in fossil record sampling. Homogenization was most pronounced among mammals larger than 1 kg and occurred in two phases. The first followed the megafaunal extinction at similar to 10,000 ybp. The second, more rapid phase began during human population growth and early agricultural intensification (similar to 2,000-1,000 ybp). We show that North American ecosystems were homogenizing for millennia, extending human impacts back similar to 10,000 years.Peer reviewe
