    Menetrier's disease

A clinical case of a 57-year-old female patient with history of epilepsy diagnosed in childhood, currently being treated with magnesium valproate, she presented with oral intolerance, ascites and weight loss. The initial laboratory workup was significant for hypoalbuminemia (serum albumin 18 g/L), but her liver and renal function were normal and urinalysis was negative. Panendoscopy was performed with a microscopic report: histopathology reporting findings related to hypertrophic gastropathy, gastric foveolar hyperplasia, with chronic diffuse moderate edematous gastritis without data of acute activity, with Helicobacter pylori bacilli in small quantity (+), negative for metaplasia, dysplasia and/or malignancy compatible with Menetrier disease. The patient was evaluated by the general surgery service to perform a jejunostomy and/or total gastrectomy as an adjuvant treatment for the patient. The extent of the gastrectomy to be performed is the subject of debate. The patient refused surgical treatment, so she was voluntarily discharged to an outpatient clinic to begin monoclonal treatment. The patient died 3 weeks before starting monoclonal treatment. Therefore, untreated patients have a high mortality rate due to complications.

The purpose is to show the autotransplantation and surgical repositioning of a retained canine, and the apical filling of central and lateral resorbed incisors from a 12-year-old female patient, healthy and with clinical absence of left maxillary canine. Radiographically, the retained canine between the resorbed central and lateral incisors was observed. Root canal treatment of the canine was performed after 8 weeks; apical curettage and placement of bovine graft in inter-incisal zone was done after 4 months. During 6 months, orthodontic traction of the canine was carried out with no positive results, and 12 months after the autotransplantation, surgical repositioning was performed. Clinical-radiographic control at 30 days and 24 months showed absence of inflammation, restoration and integration of the tooth-supporting structures. Autotransplantation combined with surgical repositioning of the retained canine and the apical filling of two incisors achieved the harmonious, aesthetic, functional, dental and psychological preservation of the patient. Keywords: Autotransplantation; Endodontics; Endodontic Surgery; Root Resorption; Surgical Repositionin

Adverse effects of antipsychotics on sleep in patients with schizophrenia. Systematic review and meta-analysis

IntroductionOur objective was to conduct a systematic review and meta-analysis of adverse effects on sleep in patients with schizophrenia receiving antipsychotic treatment.MethodsA systematic search was performed in PubMed, Cochrane Central, Embase, Toxline, Ebsco, Virtual Health Library, Web of Science, SpringerLink, and in Database of abstracts of Reviews of Effects of Randomized Clinical Trials to identify eligible studies published from January 1990 to October 2021. The methodological quality of the studies was evaluated using the CONSORT list, and the Cochrane bias tool. Network meta-analysis was performed using the Bayesian random-effects model, with multivariate meta-regression to assess the association of interest.Results87 randomized clinical trials were identified that met the inclusion criteria, and 70 articles were included in the network meta-analysis. Regarding the methodological quality of the studies, 47 had a low or moderate bias risk. The most common adverse effects on sleep reported in the studies were insomnia, somnolence, and sedation. The results of the network meta-analysis showed that ziprasidone was associated with an increased risk of insomnia (OR, 1.56; 95% credible interval CrI, 1.18–2.06). Several of the included antipsychotics were associated with a significantly increased risk of somnolence; haloperidol (OR, 1.90; 95% CrI, 1.12–3.22), lurasidone (OR, 2.25; 95% CrI, 1.28–3.97) and ziprasidone (OR, 1.79; 95% CrI, 1.06–3.02) had the narrowest confidence intervals. In addition, perphenazine (OR, 5.33; 95% CrI, 1.92–14.83), haloperidol (OR, 2.61; 95% CrI, 1.14–5.99), and risperidone (OR, 2.41; 95% CrI, 1.21–4.80) were associated with an increased risk of sedation compared with placebo, and other antipsychotics did not differ. According to the SUCRAs for insomnia, chlorpromazine was ranked as the lowest risk of insomnia (57%), followed by clozapine (20%), while flupentixol (26 %) and perospirone (22.5%) were associated with a lower risk of somnolence. On the other hand, amisulpride (89.9%) was the safest option to reduce the risk of sedation.DiscussionInsomnia, sedation, and somnolence were the most frequent adverse effects on sleep among the different antipsychotics administered. The evidence shows that chlorpromazine, clozapine, flupentixol, perospirone, and amisulpride had favorable safety profiles. In contrast, ziprasidone, perphenazine, haloperidol, and risperidone were the least safe for sleep.Systematic review registrationhttps://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42017078052, identifier: PROSPERO 2017 CRD42017078052

Identifying Hyper-Heuristic Trends through a Text Mining Approach on the Current Literature

    No full text
Hyper-heuristics have arisen as methods that increase the generality of existing solvers. They have proven helpful for dealing with complex problems, particularly those related to combinatorial optimization. Their recent growth in popularity has increased the daily amount of text in the related literature. This information is primarily unstructured, mainly text that traditional computer data systems cannot process. Traditional systematic literature review studies exhibit multiple limitations, including high time consumption, lack of replicability, and subjectivity of the results. For this reason, text mining has become essential for researchers in recent years. Therefore, efficient text mining techniques are needed to extract meaningful information, patterns, and relationships. This study adopts a literature review of 963 journal and conference papers on hyper-heuristic-related works. We first describe the essential text mining techniques, including text preprocessing, word clouds, clustering, and frequent association rule learning in hyper-heuristic publications. With that information, we implement visualization tools to understand the most frequent relations and topics in the hyper-heuristic domain. The main findings highlight the most dominant topics in the literature. We use text mining analysis to find widespread manifestations, representing the significance of the different areas of hyper-heuristics. Furthermore, we apply clustering to provide seven categories showing the associations between the topics related to hyper-heuristic literature. The vast amount of data available that we find opens up a new opportunity for researchers to analyze the status of hyper-heuristics and help create strategic plans regarding the scope of hyper-heuristics. Lastly, we remark that future work will address the limitations of collecting information from multiple data sources and analyze book chapters related to hyper-heuristics

    Soliton Molecules in Self-Mode-Locked Ring-Cavity Er/Yb Double-Clad Fiber Laser

    No full text
In this paper, generation of soliton molecules by a self-mode-locked Er/Yb double clad fiber laser is experimentally demonstrated. The optical spectrum of the bound solitons exhibits eight well defined peaks at similar to 1563 nm with high modulation depth. The corresponding autocorrelation measurement shows a fundamental trace with several narrow peaks, suggesting the presence of bound solitons. The obtained results demonstrate the reliability of our simple laser configuration for the formation of more robust soliton molecules as the pump power is increased. The soliton molecules exhibit repetition rate of 19.54 MHz and average power of 1.2 W, achieved with a pump power of similar to 4.8 W
