    A randomised controlled trial testing the rebound-preventing benefit of four days of prednisolone during the induction of oclacitinib therapy in dogs with atopic dermatitis

    Background A rebound of pruritus occasionally occurs after oclacitinib dose reduction in dogs with atopic dermatitis (AD). Objectives To determine whether an initial 4-day course of prednisolone decreases the probability of a pruritus rebound after reducing the frequency of oclacitinib administration. Animals Forty dogs with mild-to-moderate AD lesions and moderate-to-severe pruritus. Materials and Methods Dogs were randomised to receive oclacitinib at 0.4-0.6 mg/kg twice daily for 14 days then once daily, alone or with prednisolone at 0.5 mg/kg, orally, twice daily for the first 4 days. Clinicians graded the Canine Atopic Dermatitis Extent and Severity Index (CADESI)4 and 2D-investigator global assessment (IGA) before and after 28 days; owners assessed the pruritis Visual Analog Scale (PVAS)10 and Owner Global Assessment of Treatment Efficacy (OGATE) on Day (D)0, D4, D14, D21 and D28. We considered a rebound any increase greater than one PVAS10 grade at D21 compared to D14. Results On D21, there were significantly fewer rebounds in the dogs receiving prednisolone (three of 20, 15%) compared to those given oclacitinib alone (nine of 20, 45%; Fisher's test, p = 0.041). Compared to oclacitinib monotherapy, the concurrent administration of prednisolone for the first 4 days led to significantly lower PVAS10 on D4 and D28, CADESI4 and 2D-IGA on D28, and OGATE on D21 and D28 (Wilcoxon-Mann-Whitney U-tests). Adverse effects of therapy were minor, intermittent and self-resolving. Conclusions and Clinical Relevance The initial addition of 4 days of prednisolone significantly decreased the probability of a rebound of pruritus 1 week after oclacitinib dose reduction. This short concomitant glucocorticoid administration led to a higher skin lesion improvement and improved perception of treatment efficacy with minimal adverse effects

    Gambling and Impulsivity Traits: A Recipe for Criminal Behavior?

    Gambling disorder (GD) is a psychiatric condition that was recently recategorized as a non-substance-related addiction in the Diagnostic and Statistical Manual of Mental Health Disorders. Criminal activity is commonly associated with gambling; however, few empirical studies to date have examined sociodemographic and psychological variables in this population. In this study, we explored criminal behavior history in a sample of consecutively recruited treatment-seeking gamblers (n = 382) and compared subjects with a history of illegal acts (n = 103, 26.9%) to those with no criminal record (n = 279, 73.1%). Impulsivity and personality traits were specifically explored, along with other gambling-related severity factors. We found that gamblers who engaged in illegal activity were more likely to endorse high levels of urgency (i.e., the tendency to act out when experiencing heightened emotional states) and increased lack of premeditation. Gamblers with a history of criminal behavior also had greater GD severity levels and gambling-related debts. Additionally, these gamblers reported lower levels of self-directedness, which is characterized by difficulty in establishing and redirecting behavior toward one's goals. Likewise, gamblers who had conducted criminal acts showed a tendency to engage in greater risk-taking behavior. These results shed new light on this understudied population and provide insights for developing targeted harm-prevention interventions and treatment protocols

    Association Between Preexisting Versus Newly Identified Atrial Fibrillation and Outcomes of Patients With Acute Pulmonary Embolism

    Background Atrial fibrillation (AF) may exist before or occur early in the course of pulmonary embolism (PE). We determined the PE outcomes based on the presence and timing of AF. Methods and Results Using the data from a multicenter PE registry, we identified 3 groups: (1) those with preexisting AF, (2) patients with new AF within 2 days from acute PE (incident AF), and (3) patients without AF. We assessed the 90-day and 1-year risk of mortality and stroke in patients with AF, compared with those without AF (reference group). Among 16 497 patients with PE, 792 had preexisting AF. These patients had increased odds of 90-day all-cause (odds ratio [OR], 2.81; 95% CI, 2.33-3.38) and PE-related mortality (OR, 2.38; 95% CI, 1.37-4.14) and increased 1-year hazard for ischemic stroke (hazard ratio, 5.48; 95% CI, 3.10-9.69) compared with those without AF. After multivariable adjustment, preexisting AF was associated with significantly increased odds of all-cause mortality (OR, 1.91; 95% CI, 1.57-2.32) but not PE-related mortality (OR, 1.50; 95% CI, 0.85-2.66). Among 16 497 patients with PE, 445 developed new incident AF within 2 days of acute PE. Incident AF was associated with increased odds of 90-day all-cause (OR, 2.28; 95% CI, 1.75-2.97) and PE-related (OR, 3.64; 95% CI, 2.01-6.59) mortality but not stroke. Findings were similar in multivariable analyses. Conclusions In patients with acute symptomatic PE, both preexisting AF and incident AF predict adverse clinical outcomes.     Ricardo García Cárcel (Requena, 1948) estudió Historia en Valencia bajo el magisterio de Joan Reglà, con quien formó parte del primer profesorado de historia moderna en la Universidad Autónoma de Barcelona. En esta universidad, desde hace prácticamente cincuenta años, ha desarrollado una extraordinaria labor docente y de investigación marcada por un sagaz instinto histórico, que le ha convertido en pionero de casi todo lo que ha estudiado: las Germanías, la historia de la Cataluña moderna, la Inquisición, las culturas del Siglo de Oro, la Leyenda Negra, Felipe II, Felipe V, Austrias y Borbones, la guerra de la Independencia, la historia cultural, los mitos de la historia de España... Muy pocos tienen su capacidad para reflexionar, ordenar, analizar, conceptualizar y proponer una visión amplia y llena de matices sobre el pasado y las interpretaciones historiográficas. A su laboriosidad inimitable se añade una dedicación sin límites en el asesoramiento de alumnos e investigadores e impulsando revistas, dosieres, seminarios o publicaciones colectivas.     A través de varios manuscritos conservados en la Biblioteca Capitular y Colombina de Sevilla y en la Biblioteca Nacional de España se traza un recorrido en el que se ahonda sobre la participación de diferentes actores en la producción libraria durante la Baja Edad Media. Siguiendo el modelo planteado en el siglo XIII por el franciscano Buenaventura de Bagnoregio, se estudian diferentes manuscritos atendiendo a la actividad de escritores, compiladores y autores, a los que se añaden los traductores, haciendo especial énfasis en el análisis de su escritura. Del mismo modo, se traza la evolución que experimentó el libro con la irrupción de la imprenta y los cambios que se fueron introduciendo a lo largo del siglo XVI.Through several manuscripts preserved in the Biblioteca Capitular y Colombina of Seville and in the National Library of Spain, a route is drawn in which it delves into the participation of different actors in the production of books during the Late Middle Ages. Following the model proposed in the 13th century by the Franciscan Buenaventura de Bagnoregio, different manuscripts are studied in response to the activity of writers, compilers and authors, to which translators are added, with special emphasis on the analysis of their writing. In the same way, the evolution that the book underwent with the irruption of the printing press and the changes that were introduced throughout the sixteenth century are traced

    Recientemente se ha promovido la idea de que nuestra concepción de inteligencia debería expandirse para incluir un nuevo concepto, la inteligencia emocional (IE). El desarrollo de modelosteóricos de IE ha ido paralelo al desarrollo de instrumentos de medidas para evaluar el concepto. En los últimos años se ha producido una proliferación de medidas de IE; sin embargo, muchas de ellas no son científicas ni miden el concepto con exactitud. En este artículo, realizamos una panorámica general de las medidas científicas de IE informando de las versiones validadas para población de lengua española. Igualmente, se revisa la fiabilidad, validez y métodos de evaluación de cada una de ellas. Se realiza una distinción entre medidas de auto-informe y medidas de habilidad de IE en función de cómo la IE es definida y el planteamiento de medición utilizado. En las medidas de auto-informe se pide a las personas que estimen su nivel de IE y, por tanto, proporciona una medida de IE percibida. En cambio, las medidas de habilidad evalúan los niveles de ejecución de las personas en tareas emocionales y proporcionan una medida de la IE actual. Finalmente, se subraya la necesidad de más investigación en el campo de la evaluación de la IE y se discuten algunas ventajas y desventajas sobre las medidas
