    Study of sociodemographic factors and their relationship with the morbidity among adolescent girls of Pune slum area

    Background: Socio-demographic factors such as age, gender, socioeconomic status, and education can influence health and nutritional outcomes, especially in adolescent girls. Aims and objectives of research work was to study of sociodemographic factors and their relationship with the morbidity among adolescent girls residing in slum areas of Pune region.Methods: A cross-sectional study of the urban slum population from October 2011 to September 2012. Adolescent girls of the age group 10-19 years who gave consent and are living in a slum area for more than 6 months were enrolled. Information on health status including morbidity was collected by health examination.Results: Majority of the adolescent girls (54.02%) had to age between 14-16 years. Majority of the adolescent girls (96.4%) were studying and were underweight (41.07%; of which majority 59.78% were in the age group 14-16years). Out of the 41.07% underweight girls, the majority (84.78%) had anaemia (p<0.05). A total 26.34% study subjects were stunted and 12.95% were thin (for both, commonly reported in the age group of 14-16 years). Majority of the girls (48.2%) belonged to class III SES. In class III socioeconomic status group mild, moderate and severe anaemia was reported in 50.45%, 41.51%, and 36.36% cases, respectively.Conclusions: Among the studied sociodemographic factors lower socioeconomic status and overcrowding had an impact on the morbidities. However, we noted a significant impact of morbidity in underweight girls

    Mucinous tubular and spindle cell carcinoma of the kidney

    Mucinous tubular and spindle cell carcinoma (MTSCC) is a unique neoplasm attributing to less than 1% of all renal cell carcinoma (RCC). The median age is 6th decade with a significant female preponderance.1 These tumors classically consist of tightly packed, elongated, and anastomosing tubules which merge with bland spindle cells in a myxoid stroma in variable proportions. Tubules with tufting or small papillae and foci of foamy macrophages may occur. The tumor cells are low-grade and may show clear cytoplasm or oncocytic changes with rare mitosis. Immunohistochemically, PAX8, CK7, AMACR, and CD10 are positive in this tumor.1,2 Though it is a morphological diagnosis, it can be challenging to differentiate from a solid variant of papillary RCC, sarcomatoid RCC, or myoid-predominant angiomyolipoma. Immunohistochemistry may be of little help due to overlapping profiles.3 Copy number analyses can help establish a diagnosis in challenging cases or core biopsies since these are associated with multiple chromosomal losses involving chromosomes 1, 4, 6, 8, 9, 13, 14, 15, and 22. Novel biomarkers like VSTM2A overexpression are also emerging, which can be detected by RNA in-situ hybridization. Recurrent Hippo pathway aberrations have been defined as the molecular signature of MTSCC with increased nuclear YAP1 protein expression.4 Adverse features such as necrosis, solid growth, single file infiltration, sarcomatoid transformation, lymphovascular invasion, and increased mitoses are indicators of metastatic disease. Although it has indolent behavior, rare cases with classic morphology have been seen to develop metastases.5-6 We describe gross and histopathological findings of mucinous tubular and spindle cell carcinoma in a 65-year-old male patient. He presented with right abdominal pain for one month. The pain was dull and mild to moderate in intensity. On contrast-enhanced ultrasound, a well-defined, smoothly marginated heterogeneous hyperechoic lesion measuring 5.5x6cm was noted at the upper pole of the right kidney. The lesion was hyper enhancing compared to the rest of the renal parenchyma. Based on the radiological findings, possibilities of chromophobe RCC and oncocytoma were considered. The abdominal computed tomography (CT) revealed a hyperdense lesion in the upper pole of the right kidney with a relatively well-defined margin measuring 63x65x69mm with no evidence of significant post-contrast enhancement in the either arterial, venous or delayed phase. The patient underwent a right radical nephrectomy. On gross examination, a well-encapsulated mass was seen wholly occupying the upper pole of the kidney measuring 6.9x6.5x5.5cm. The cut surface was variegated with grey-white, firm areas admixed with mucinous and hemorrhagic foci (Figure 1A). No capsular breach or perinephric fat extension was noted. The renal pelvis, sinus, and renal vessels were free of tumor. It was composed of the dominant epithelial element against the background of extracellular mucinous stroma (Figure 1C). The epithelial component contained tightly packed, elongated, tufted, and anastomosing tubules lined by low-grade cuboidal cells and are seen merging with bland spindle cells (Figure 1D). The nuclei display fine vesicular chromatin, inconspicuous nucleoli, and moderate cytoplasm with vacuolations. Mitotic figures are infrequent (&lt;1/10HPF). No necrosis, lymphovascular invasion, or high-grade transformation was found. However, foamy macrophage collections and lymphoid aggregates are admixed. Cholesterol clefts with focal foreign body giant cell response indicated the long-standing nature of the tumor.     Phosphorus (P) is essential for all organisms. In agriculture, P, nitrogen (N) and potassium (K) are applied in large amounts as fertilizer. Monoammonium phosphate (MAP), a conventional fertilizer, is highly water soluble, causing high P and N concentrations in agricultural runoff, harming aquatic life. Struvite (NH4MgPO4 ·6H2O), a waste-water derived product with potential for use as a P fertilizer; has low water solubility, and phosphorus in struvite is not as accessible to plants as it is in MAP. Arbuscular mycorrhizal fungi (AMF) form mutualisms in which they receive carbon from plants in exchange for other nutrients, and assist in P uptake by more thorough soil exploration, a higher P affinity than that of plant roots, modification of the rhizosphere through exudates, and hyphal storage of absorbed P2.     Transcatheter closure of patent ductus arteriosus using duct occluder: an eleven-year experience from a single tertiary cardiac center

    Background: Patent Ductus Arteriosus (PDA) is a common congenital disorder. As an isolated lesion, PDA constitutes 6 to 11% of all congenital heart disease. PDA needs closure to eliminate pulmonary over circulation leading to volume overload of left ventricle, pulmonary vascular obstructed disease.Methods: This retrospective study was carried out in pediatric cardiology unit of Institute of Postgraduate Medical Education and Research, Kolkata from September 2005 to August 2016, which included 503 patients.Results: Device closure was attempted in 492 patient’s Procedural success was achieved in 85% cases on table, in who check aortogram revealed complete closure of PDA. In 15% cases, residual shunt was present. In 12% of cases, residual shunt disappeared during follow-up echocardiogram over 6-month follow-up. In 3% cases, small shunt remained at 6-month and 1-year follow up.Conclusion: Transcatheter closure of PDA by duct occluder is safe and effective with good mid-term outcome. The optimum assessment of ductul size and anatomy is crucial for optimum device size, which prevents residual shunt, device embolization and protrusion

    On-farm seed priming interventions in agronomic crops

    Priming techniques are gaining importance in agriculture with the increase in environmental stresses. Resource-poor farmers are in urgent need of such techniques as they are simple, economical, and value-added intervention associated with low-risk bearing factors. Seed enhancement methods are key to improve seed performance and achieve a good stand establishment. Worldwide beneficial effects of priming are recorded. But these technologies have still not reached most farmers. This review highlights the importance of on-farm priming strategies in modern crop production system to yield better productivity and obtain higher economic returns. Stimulation of the pre-germination metabolic changes by priming is necessary to overcome the environmental challenges that a plant can encounter. Thus, the study also focuses on mechanisms associated with priming-induced stress tolerance of crops. Various safe practical methods of seed priming can be easily adopted by the farming community to alleviate the levels of different stresses which can hamper productivity. Simultaneously they can produce good quality seeds and use them further for the next crop cycle cutting the costs of seed purchase.</p

    Distinct GDP/GTP bound states of the tandem G-domains of EngA regulate ribosome binding

    EngA, a unique GTPase containing a KH-domain preceded by two consecutive G-domains, displays distinct nucleotide binding and hydrolysis activities. So far, Escherichia coli EngA is reported to bind the 50S ribosomal subunit in the guanosine-5′-trihosphate (GTP) bound state. Here, for the first time, using mutations that allow isolating the activities of the two G-domains, GD1 and GD2, we show that apart from 50S, EngA also binds the 30S and 70S subunits. We identify that the key requirement for any EngA–ribosome association is GTP binding to GD2. In this state, EngA displays a weak 50S association, which is further stabilized when GD1 too binds GTP. Exchanging bound GTP with guanosine-5′-diphosphate (GDP), at GD1, results in interactions with 50S, 30S and 70S. Therefore, it appears that GD1 employs GTP hydrolysis as a means to regulate the differential specificity of EngA to either 50S alone or to 50S, 30S and 70S subunits. Furthermore, using constructs lacking either GD1 or both GD1 and GD2, we infer that GD1, when bound to GTP and GDP, adopts distinct conformations to mask or unmask the 30S binding site on EngA. Our results suggest a model where distinct nucleotide-bound states of the two G-domains regulate formation of specific EngA–ribosome complexes

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.
