Postępowanie w stanach hiperglikemii u chorych z ostrymi zespołami wieńcowymi
W niniejszej pracy przedstawiono zagadnienie dotyczące częstości występowania hiperglikemii
oraz jej wpływ na rokowanie u pacjentów z ostrym zespołem wieńcowym (OZW), a także
zaproponowany przez Polskie Towarzystwo Diabetologiczne schemat leczenia hipoglikemizującego
tej grupy chorych. Hiperglikemię w OZW obserwuje się u około 70-80% pacjentów,
z czego 1/3 stanowią osoby chorujące wcześniej na cukrzycę, a ponad 2/3 osób ze świeżo
stwierdzanymi zaburzeniami gospodarki węglowodanowej. Zgodnie z analizą dotychczasowych
doniesień rokowanie zarówno krótko-, jak i długoterminowe w tej ostatniej grupie pacjentów
jest istotnie gorsze niż w grupie chorych na cukrzycę i rośnie już od wartości glikemii
uznanych za prawidłowe (tj. 110-130 mg/dl). Polskie Towarzystwo Diabetologiczne proponuje
stosowanie leczenia insuliną u każdego chorego na cukrzycę z wartością glikemii przy przyjęciu
powyżej 140 mg/dl i każdego pacjenta, który nie chorował dotychczas na cukrzycę, jeżeli
wartość glikemii przy przyjęciu jest wyższa od 180 mg/dl. W artykule opisano szczegółowo

Stress hyperglycaemia in patients with first myocardial infarction
Objective: To investigate the incidence of stress hyperglycaemia at first acute
myocardial infarction (MI) with ST-segment elevation, occurrence of stress hyperglycaemia
as a manifestation of previously undiagnosed abnormal glucose tolerance
(AGT), and its relation to stress hormone levels. Materials and
methods: The population of this prospective cohort study consisted of 243
patients. On admission glucose, adrenaline, noradrenaline and cortisol levels were
measured. Patients without previously diagnosed diabetes (n = 204) underwent an
oral glucose tolerance test on day 3 of hospitalisation and 3 months after discharge.
Results: Abnormal glucose tolerance at day 3 was observed in 92
(45.1%) patients without a previous diagnosis of diabetes mellitus and resolved
after 3 months in 46 (50.0%) patients (p < 0.0001). Stress hyperglycaemia,
defined as admission glycaemia ‡ 11.1 mmol ⁄ l, affected 34 (14.0%) study participants:
28 (54.9%) patients with diabetes vs. 3 (8.8%) subjects with newly
detected impaired glucose intolerance (p < 0.00001) and 1 (2.2%) person with
AGT at day 3 (p < 0.000001). Multivariable analysis identified elevated glycated
haemoglobin (HbA1c; p < 0.0000001), anterior MI (p < 0.05) and high admission
cortisol concentration (p < 0.001), but not catecholamines, as independent predictors
of stress hyperglycaemia. The receiver operating characteristic curve analysis
revealed the optimal cut-off values of 8.2% for HbA1c and 47.7 lg ⁄ dl for admission
cortisol with very good and sufficient diagnostic accuracies respectively. Conclusions:
Newly detected AGT in patients with a first MI is transient in 50% of
cases. Stress hyperglycaemia is a common finding in patients with a first MI with
ST-segment elevation and diabetes mellitus, but is rarely observed in individuals
with impaired glucose tolerance or transient AGT diagnosed during the acute
phase of MI. The risk factors of stress hyperglycaemia occurrence include elevated
HbA1c, anterior MI and high admission cortisol concentration
Value of oral glucose tolerance test in the acute phase of myocardial infarction
<p>Abstract</p> <p>Background</p> <p>Although European guidelines advise oral glucose tolerance test (OGTT) in patients with acute myocardial infarction (AMI) before or shortly after hospital discharge, data supporting this recommendation are inconclusive. We aimed to analyze whether disturbances in glucose metabolism diagnosed before hospital discharge in AMI patients represents a latent pre-existing condition or rather temporary finding. Additionally, we planned to investigate the value of pre-selected glycemic control parameters as predictors of long-term glucometabolic state.</p> <p>Methods</p> <p>We assessed admission glycemia, glycated hemoglobin, mean blood glucose concentration on days 1 and 2 in 200 patients with a first AMI but without overt disturbances of glucose metabolism. We also performed OGTT at discharge and 3 months after discharge.</p> <p>Results</p> <p>The prevalence of disturbances in glucose metabolism (as assessed by OGTT) at 3 months was significantly lower than at discharge (29% <it>vs</it>. 48%, p = 0.0001). Disturbances in glucose metabolism were not confirmed in 63% of patients with impaired glucose tolerance and in 36% of patients with diabetes mellitus diagnosed during the acute phase of AMI. Age >77 years, glucose ≥12.06 mmol/l at 120 minutes during OGTT before discharge and mean blood glucose level on day 2 >7.5 mmol/l were identified as independent predictors of disturbances in glucose metabolism at the 3-month follow-up.</p> <p>Conclusions</p> <p>Disturbances in glucose metabolism observed in patients with a first AMI are predominantly transient. Elderly age, high plasma glucose concentration at 120 minutes during OGTT at discharge and elevated mean blood glucose level on day 2 were associated with sustained disturbances in glucose metabolism.</p
Admission glycemia and inhospital or long term mortality in patients without acute life-threatening conditions
WSTĘP. Zaburzenia gospodarki węglowodanowej
są istotnym czynnikiem ryzyka rozwoju przyspieszonej
miażdżycy. Chorzy z hiperglikemią podczas
przyjęcia do szpitala, niezależnie od współistnienia
cukrzycy, cechują się wyższą śmiertelnością
w porównaniu z grupą osób z prawidłową glikemią.
Dotychczas opracowano niewiele doniesień dotyczących
wpływu glikemii podczas przyjęcia na ryzyko
zgonu u chorych hospitalizowanych z innych powodów
niż nagłe stany zagrożenia życia.
MATERIAŁ I METODY. Badaniem objęto 4944 pacjentów
hospitalizowanych w Klinice Endokrynologii i Diabetologii
CM UMK w Bydgoszczy. Badanych podzielono
na 3 grupy w zależności od wcześniejszego występowania
zaburzeń gospodarki węglowodanowej, pojawienia
się ich podczas hospitalizacji lub ich braku.
Oceniano wpływ glikemii podczas przyjęcia do szpitala (AG) na śmiertelność wewnątrzszpitalną i 2 lata po
wypisaniu chorych ze szpitala.
WYNIKI. W grupie osób z zaburzeniami gospodarki
węglowodanowej rozpoznanymi po raz pierwszy
w trakcie hospitalizacji (II) stwierdzono najwyższą
glikemię podczas przyjęcia - 15,2 mmol/l
(273,6 mg/dl). Najniższą glikemię wynoszącą 5,9 mmol/l
(105,7 mg/dl) zaobserwowano w grupie osób bez
zaburzeń węglowodanowych (III). W grupie chorych
na cukrzycę AG wynosiła 12,3 mmol/l (221,1
mg/dl) i różniła się istotnie w porównaniu z grupą II
(p < 0,0001) i III (p < 0,0001). Największy odsetek
zgonów wewnątrzszpitalnych (3,7%) odnotowano
w grupie III. Był on statystycznie wyższy od stwierdzanego
u chorych na cukrzycę (2,6%, p < 0,05).
Spośród 870 badanych zmarło 223 pacjentów: 94
(42%) z grupy I, 20 (9%) z grupy II i 109 (49%) z III.
Nie stwierdzono istotnych różnic w śmiertelności po
2 latach od wypisu w poszczególnych grupach.
WNIOSKI. Nie stwierdzono zależności między AG
a śmiertelnością wewnątrzszpitalną i 2-letnią wśród
chorych hospitalizowanych z przyczyn innych niż
nagłe stany zagrożenia życia.INTRODUCTION. Carbohydrate metabolism disorders
are important risk factors of atherosclerosis development.
Patients with admission hyperglycemia
(AG), irrespective of diabetes coexistence, have a higher
inhospital mortality rate in comparison to people
with normal glucose level. Nowadays there are
few papers refer to admission glucose effect on
mortality in patients hospitalized due to other reasons
than acute life-threatening conditions.
MATERIAL AND METHODS. The study group consisted
with 4944 patients admitted to the Endocrinology
and Diabetology Department CM UMK in Bydgoszcz.
Analyzed people were divided into 3 groups
according to presence or absence of the glucose
metabolism disorders. The effect of admission glycemia
on inhospital and 2 years mortality was evaluated.
RESULTS. In patients with newly diagnosed glucose
metabolism disorders (group II) the highest AG
- 15.2 mmol/l (273.6 mg/dl) was found. The lowest
value of AG - 5.9 mmol/l (105.7 mg/dl) was observed
in people without metabolic disorders (III). In
patients with diabetes AG was 12.3 mmol/l (221.1
mg/dl) and was significantly different in comparison
to groups II (p < 0.0001) and III (p < 0.0001). The
highest percent of inhospital death (3.7%) was in III
group. It was significantly higher than in patients
with diabetes (I) (2.6%, p < 0.05). Among 870 examined
patient 223 died: 94 (42%) in I group, 20 (9%)
in II group and 109 (49%) in III group. After 2 years
from the discharge there were no significantly differences
in mortality rate between groups.
CONCLUSION. There was no correlation between
admission glycemia and inhospital and long-term
mortality in patients without acute life-threatening
conditions
The assessment of the implementation of the Polish Diabetes Association recommendations in the scope of metabolic control in patients with diabetes treated in endocrinological-diabetological outpatient clinic
WSTĘP. Zalecenia Polskiego Towarzystwa Diabetologicznego (PTD) określają docelowe wartości w terapii
cukrzycy dotyczące ciśnienia tętniczego, gospodarki węglowodanowej i lipidowej. Celem pracy jest ocena realizacji tych zaleceń u chorych na cukrzycę leczonych w Przyklinicznej Poradni Endokrynologiczno-Diabetologicznej Szpitala Uniwersyteckiego nr 1 w Bydgoszczy.
MATERIAŁ I METODY. Analizie poddano dokumentację losowo wybranych 313 pacjentów (151 mężczyzn i 162 kobiet chorych na: cukrzycę typu 1 — 21,4%, cukrzycę typu 2 — 75,4%, inne typy cukrzycy — 3,2%) leczonych w 2007 roku w Poradni. Średni wiek pacjentów
wynosił 60,2 ± 15,3 roku, czas trwania cukrzycy — 11 ± 8 lat, BMI — 28,4 ± 7,9 kg/m2. Analizowano wartość skurczowego i rozkurczowego ciśnienia tętniczego, odsetek hemoglobiny glikowanej (HbA1c), stężenia cholesterolu całkowitego, cholesterolu
frakcji HDL, LDL i triglicerydów. Obliczano średnie wartości wymienionych parametrów oraz odsetek osób, które osiągnęły docelowe wartości według zaleceń PTD na 2007 i 2009 rok.
WYNIKI. Średnie wartości ocenianych parametrów oraz odsetek osób osiągających docelowe wartości, według zaleceń PTD na 2007 rok (w nawiasie), to: ciśnienie skurczowe 136 ± 19 mm Hg (35,3%), rozkurczowe 79 ± 10 mm Hg (44,5%), HbA1c 7,5 ± 1,6%
(32,2%), cholesterol całkowity 184,2 ± 38,6 mg/dl (43,5%), HDL 49,3 ± 15,2 mg/dl (58,5% mężczyzn i 67,3% kobiet), LDL 105,5 ± 29,3 mg/dl (39,8%),
triglicerydy 136,8 ± 75,3 mg/dl (69,8%). Docelową wartość HbA1c według zaleceń PTD na 2009 rok osiągnęło 17,4% chorych na cukrzycę typu 1, 58% chorych na cukrzycę typu 2 trwającą < 5 lat i 52,6%
chorych na cukrzycę typu 2 trwającą ≥ 5 lat. Złożony cel terapeutyczny według zaleceń PTD na 2007 i 2009 rok osiągnęło 4,9% pacjentów.
WNIOSKI. Uzyskane przez autorów niniejszej pracy wyniki wskazują na niedostateczną kontrolę metaboliczną u chorych na cukrzycę pozostających pod opieką specjalistyczną. Sugerują one potrzebę poprawy jakości opieki diabetologicznej w poradniach specjalistycznych. (Diabet. Prakt. 2010; 11, 5: 160–166)INTRODUCTION. In the Polish Diabetes Association
(PDA) recommendations treatment targets in the
scope of blood pressure, glycemic control and lipids
are defined. The aim of the study was to assess the
implementation of these recommendations in
patients with diabetes treated in endocrinologicaldiabetological
outpatient clinic.
MATERIAL AND METHODS. We analyzed randomly
chosen sample of 313 records of patients (151 men
and 162 women; 21.4% with type 1 diabetes, 75.4%
with type 2 diabetes, 3.2% with other types), treated
in Endocrinological-Diabetological Outpatient Clinic
in University Hospital in Bydgoszcz in 2007 year.
Mean age of patients was 60.2 ± 15.3 years, diabetes
duration 11 ± 8 years, BMI 28.4 ± 7.9 kg/m2. We
analyzed systolic and diastolic blood pressure, HbA1c
levels, triglicerydes, total cholesterol and HDL and
LDL cholesterol concentrations.
RESULTS. Mean values of examined parameters and
the percentage of subjects achieving therapeutic
goals according to PDA recommendations in 2007
(in brackets) are: systolic blood pressure 136
± 19 mm Hg (35.3%), diastolic blood pressure 79
± 10 mm Hg (44.5%), HbA1c 7.5 ± 1.6% (32.2%),
total cholesterol 184.2 ± 38.6 mg/dL (43.5%), HDL 49.3
± 15.2 mg/dL (58.5% men and 67.3% women), LDL
105.5 ± 29.3 mg/dL (39.8%), triglicerydes 136.8
± 75.3 mg/dL (69.8%). HbA1c target recommended by
PDA in 2009 year, was achieved by 17.4% of patients
with type 1 diabetes, 58% of subjects with type 2
and diabetes duration < 5 years and by 52.6% of
subjects with type 2 and diabetes duration ≥ 5 years.
CONCLUSIONS. Our results confirm insufficient
metabolic control in patients with diabetes treated
in specialist care. These results indicate the need of
improvement of quality of care for patients with

Occurrence and predictors of left ventricular systolic dysfunction at hospital discharge and in long−term follow−up after acute myocardial infarction treated with primary percutaneous coronary intervention
Background: Post-ST-segment elevation myocardial infarction (STEMI) left ventricular systolic dysfunction (LVSD) has been
identified as an important marker of poor prognosis.
Aim: To assess the prevalence and course of LVSD at hospital discharge and in long-term follow-up in STEMI patients treated
with primary percutaneous coronary intervention (pPCI).
Methods: We enrolled 205 patients (157 male, 48 female) with a first STEMI. Echocardiography was performed before
hospital discharge and 12 months after STEMI. Left ventricular systolic function (LVSF) parameters were assessed: left ventricular
ejection fraction (LVEF), wall motion score index (WMSI), and average peak systolic mitral annular velocity (S’) by tissue
Doppler echocardiography (TDE). B-type natriuretic peptide plasma concentration was measured at admission (BNPadmission)
and at discharge (BNPdischarge).
Results: We found moderate LVSD, both at hospital discharge and after 12 months. Significant global LVSD (LVEF £ 40%)
was observed in 34% of patients at discharge, and 21% after 12 months (p < 0.001). Significant regional LVSD (WMSI ≥ 1.7)
after 12 months was less frequent than at discharge (21% vs 33%; p < 0.001). More patients had significant longitudinal
LVSD (S’ £ 6.0 cm/s) after 12 months compared to discharge (28% vs 23%; p < 0.001). Severe global LVSD (LVEF £ 30%)
was rare. Univariate logistic regression analysis revealed the predictors of significant global LVSD at 12 months after STEMI to
be: anterior location of STEMI; pre-discharge echocardiographic parameters of LVSF and left ventricle size and mass; prepPCI
angiographic indices; ratio of the difference of BNPdischarge and BNPadmission to BNPadmission expressed as % (BNPdelta %); time
from onset of pain to balloon, and the use of abciximab. Multivariate logistic regression analysis found independent predictors
of significant global LVSD at 12 months to be: BNPdelta % and LVEF at discharge with optimal cut-off values of 728.2% for
BNPdelta % and 37% for LVEF.
Conclusions: Patients with a first STEMI treated with pPCI present moderate LVSD, both at hospital discharge and after
12 months. In long-term follow-up, we found an improvement in global LVSF, and, albeit a smaller, improvement in regional LVSF.
No improvement in longitudinal LVSF was observed. The increase of BNP during hospitalisation, and LVEF at discharge, are
independent predictors of significant global LVSD at 12 months after a first STEMI treated with pPCI. Pre-discharge peak systolic
mitral annular velocity obtained by TDE may be useful in predicting LVEF in long-term follow-up in this group of patients
Tamponada serca jako powikłanie implantacji elektrod o aktywnej fiksacji
W pracy zaprezentowano dwa przypadki wystąpienia wczesnej tamponady serca po udanej
implantacji stymulatora serca oraz kardiowertera-defibrylatora z użyciem elektrod o aktywnej
fiksacji. W obu przypadkach po zabiegu implantacji w badaniu echokardiograficznym ujawniono
istotną ilość płynu w worku osierdziowym. U jednej pacjentki nie wykazano perforacji
wolnej ściany mięśnia sercowego, wykonano perikardiotomię z następowym 24-godzinnym
drenażem worka osierdziowego bez repozycji elektrod. U drugiej chorej wykonano nakłucie
worka osierdziowego z drenażem osierdzia, a po potwierdzeniu perforacji wolnej ściany prawej
komory, elektrodę reponowano przeznaczyniowo z worka osierdziowego do przegrody międzykomorowej
w warunkach kardiochirurgicznej sali operacyjnej.
W niniejszej pracy przeanalizowano możliwe mechanizmy powstania i terapii tamponady
serca po implantacji elektrod o aktywnej fiksacji, uwzględniając dane z piśmiennictwa. (Folia

The incidence of risk factors of type 2 diabetes mellitus in relatives of the patients
WSTĘP. Celem niniejszej pracy była ocena występowania
zaburzeń gospodarki węglowodanowej u krewnych
I stopnia chorych na cukrzycę typu 2 zależnie od
obecności innych czynników ryzyka tej choroby.
MATERIAŁ I METODY. Ocenianą grupę tworzyło
42 krewnych I stopnia chorych leczonych w Przyklinicznej
Poradni Diabetologicznej Szpitala Uniwersyteckiego
w Bydgoszczy. Przeprowadzano badanie
podmiotowe, przedmiotowe oraz test doustnego
obciążenia 75 g glukozy.
WYNIKI. U 13 osób (31%) rozpoznano zaburzenia
gospodarki węglowodanowej (grupa ZGW), zaś u 29
ich nie stwierdzono (grupa BZGW). Badani z grupy
ZGW byli istotnie statystycznie starsi od osób z grupy
BZGW (59,8 ± 14,6 vs. 37,8 ± 15,5 roku; p < 0,001),
cechowały ich znamiennie większe rodzinne obciążenie
cukrzycą typu 2 (2,0 ± 1,1 vs. 1,1 ± 0,4; p < 0,001
- liczba krewnych w rodzinie) i istotnie wyższa liczba
czynników ryzyka choroby (2,9 ± 1,2 vs. 2,0 ± 1,2;
p < 0,01). Najczęstsze, istotne statystycznie okazało
się występowanie w wywiadzie nieprawidłowego
stężenia glukozy we krwi - 61,5% (BZGW - 6,9%). Obie grupy nie różniły się pod względem liczby chorych
rodziców, natomiast rodzeństwo chore na
cukrzycę miało 92,3% osób z grupy ZGW i tylko 13,8%
z grupy BZGW. Przynajmniej jedno dziecko chore na
cukrzycę miało 23% osób z grupy ZGW, natomiast
nikt z BZGW. Nie wykazano istotnych różnic między
grupami odnośnie wskaźnika masy ciała (28,3 ±
± 4,5 kg/m2 vs. 25,9 ± 4,9 kg/m2) oraz wskaźnika talia-
biodro (0,86 ± 0,08 vs. 0,83 ± 0,07, odpowiednio).
Cukrzyca ciążowa wystąpiła u 7,7% vs. 6,9% badanych,
urodzenie dziecka z masą ciała powyżej 4000 g
dotyczyło 7,7% versus 11,9%, a zespół policystycznych
jajników stwierdzono u 7,7% versus 3,4%.
WNIOSKI. U krewnych I stopnia chorych na cukrzycę
typu 2 wystąpienie zaburzeń gospodarki węglowodanowej
zależało od: wieku, sumy czynników ryzyka
cukrzycy, a zwłaszcza stwierdzenia nieprawidłowych
wartości glikemii w wywiadzie, liczby krewnych
chorych na cukrzycę typu 2, w szczególności chorego
rodzeństwa i dzieci.INTRODUCTION. The aim of the study was the evaluation
of glucose metabolism disturbances in I° relatives
of the patients with type 2 diabetes, depending
on the other present risk factors.
MATERIAL AND METHODS. The evaluated group
consisted of 42 I° relatives of the patients treated in Outpatient Diabetology Clinic of the University
Hospital in Bydgoszcz. A subjective, objective examination
and the oral glucose tolerance test were
carried out.
RESULTS. In 13 patients (31%) glucose metabolism
disturbances (ZGW group) were identified, in 29 they
were not stated (BZGW group). The examined ZGW
group patients were considerably statistically older
than the BZGW (59.8 ± 14.6 vs. 37.8 ± 15.5 years;
p < 0.001), had a remarkably greater inherited susceptibility
to type 2 diabetes (2.0 ± 1.1 vs. 1.1 ±
± 0.4; p < 0.001 of the number of relatives in the
family) and were characterized by a significantly higher
number of the diabetes risk factors (2.9 ± 1.2
vs. 2.0 ± 1.2; p < 0.01). The occurrence of the abnormal
glycaemia - 61.5% (BZGW - 6.9%) in the
anamnesis turned out to be the most common and
statistically important. Both groups did not differ in
the number of parents with diabetes, whereas 92.3%
patients of the ZGW and only 13.8% of the BZGW had
siblings with diabetes. 23% people with ZGW had at
least one child with diabetes while no-one thase with
BZGW. No significant differences between the groups,
regarding BMI were stated: 28.3 ± 4.5 kg/m2 versus
25.9 ± 4.9 kg/m2 and WHR: 0.86 ± 0.08 versus 0.83 ±
± 0.07 respectively. Gestational diabetes mellitus occurred
in 7.7% versus 6.9%, giving birth to a newborn
with birth weight above 4000 g: 7.7% versus 11.9%,
polycystic ovary syndrome: 7.7% versus 3.4%.
CONCLUSIONS. In the I° relatives of the patients with
type 2 diabetes the occurrence of glucose metabolism
disturbances depended on: age, the amount of
diabetes risk factors, especially stating abnormal glycaemia
values in the anamnesis, the number of relatives
with type 2 diabetes, siblings and children in
particular
Edukacja zdrowotna jako metoda poprawy realizacji programu terapeutycznego
Zrozumienie przez chorego sensu działań diagnostycznych i leczniczych sprzyja ich akceptacji
oraz zasadniczo poprawia efektywność leczenia. W edukacji zdrowotnej jest ważne, aby odbiorcom
przekazywać wiedzę na temat procesu podejmowania decyzji zdrowotnych wraz z ich
konsekwencjami, jednak nie należy im narzucać, jakie decyzje powinni podjąć. W ten sposób
odbiorca staje się partnerem w procesie nauczania. Spośród metod edukacji należy wymienić:
dokładne instruowanie pacjentów (instrukcje ustne, informacje pisemne, programy nauczania);
wzmożony kontakt z pacjentami i poradnictwo (automatyczny telefoniczny, wspierany
komputerowo monitoring i poradnictwo, kontrole telefoniczne, oddziaływanie na rodziny pacjentów);
różne sposoby poprawy wygody leczenia (opieka zdrowotna i zaopatrzenie w leki
w miejscu pracy, uproszczenie dawkowania leków); zwiększenie zaangażowania pacjentów
w samokontrolę; „przypominacze” (dostosowanie schematu przyjmowania leków do codziennych
nawyków pacjenta, specjalne pakowanie leków, przypominanie o wizytach kontrolnych i konieczności
uzupełnienia leków); inne działania poprawiające współpracę lub nagradzające
pacjenta za poprawę przestrzegania zaleceń i skuteczności leczenia (np. dostosowanie częstości
wizyt, częściowe pokrywanie kosztów aparatów do mierzenia ciśnienia tętniczego). Niezależnie

Albiglutide and cardiovascular outcomes in patients with type 2 diabetes and cardiovascular disease (Harmony Outcomes): a double-blind, randomised placebo-controlled trial
Background:
Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.
Methods:
We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. Findings:
Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p<0·0001 for non-inferiority; p=0·0006 for superiority). The incidence of acute pancreatitis (ten patients in the albiglutide group and seven patients in the placebo group), pancreatic cancer (six patients in the albiglutide group and five patients in the placebo group), medullary thyroid carcinoma (zero patients in both groups), and other serious adverse events did not differ between the two groups. There were three (<1%) deaths in the placebo group that were assessed by investigators, who were masked to study drug assignment, to be treatment-related and two (<1%) deaths in the albiglutide group.
Interpretation:
In patients with type 2 diabetes and cardiovascular disease, albiglutide was superior to placebo with respect to major adverse cardiovascular events. Evidence-based glucagon-like peptide 1 receptor agonists should therefore be considered as part of a comprehensive strategy to reduce the risk of cardiovascular events in patients with type 2 diabetes.
