    Engaging low skilled employees in workplace learning

    This report sets out the findings of a study into the motivators and barriers to participation in workplace learning by low skilled employees. Low skilled in the UK is usually considered to be below NVQ level 2 qualifications. The study was carried out by the Employment Research Institute at Edinburgh Napier University on behalf of the UK Commission for Employment and Skills. The report presents the results of a survey of both employee and employer views on participation in workplace learning in the care sector in north east England and the hotel sector in Yorkshire and the Humber region. The hotels and catering sector has the highest relative and absolute skill gaps in low skilled occupations, with some 63 per cent of staff suffering skills gaps. In contrast, the care sector has an apparent strong training culture, with care staff expected to progress to NVQ level 3 and only five per cent of low skilled staff suffered a skills gap

    Probabilistic forecasting of heterogeneous consumer transaction-sales time series

    We present new Bayesian methodology for consumer sales forecasting. With a focus on multi-step ahead forecasting of daily sales of many supermarket items, we adapt dynamic count mixture models to forecast individual customer transactions, and introduce novel dynamic binary cascade models for predicting counts of items per transaction. These transactions-sales models can incorporate time-varying trend, seasonal, price, promotion, random effects and other outlet-specific predictors for individual items. Sequential Bayesian analysis involves fast, parallel filtering on sets of decoupled items and is adaptable across items that may exhibit widely varying characteristics. A multi-scale approach enables information sharing across items with related patterns over time to improve prediction while maintaining scalability to many items. A motivating case study in many-item, multi-period, multi-step ahead supermarket sales forecasting provides examples that demonstrate improved forecast accuracy in multiple metrics, and illustrates the benefits of full probabilistic models for forecast accuracy evaluation and comparison.     Adapting a Remote Laboratory Architecture to Support Collaboration and Supervision

    Interest in, and use of, remote laboratories has been rapidly growing. These laboratories provide remote access, via the internet, to real laboratory equipment. Under appropriate circumstances they can support or even replace traditional (proximal) laboratories, provide improved access at reduced cost, and encourage inter-institutional sharing of expensive resources. Most attention to date has been on the development of the core infrastructure that manages access and interaction, and to a lesser extent consideration of pedagogic issues such as which learning outcomes are best suited to this modality. There has however been a recent recognition of the importance of also considering how collaboration and supervision can also be supported. In this paper we discuss a novel approach to the integration of support for multi-user distributed access to a single laboratory instance. The approach retains the benefits of the lightweight client inherent in the underlying architecture

    Global analysis of night marine air temperature and its uncertainty since 1880: the HadNMAT2 Dataset

    An updated version of the Met Office Hadley Centre’s monthly night marine air temperature dataset is presented. It is available on a 5? latitude-longitude grid from 1880 as anomalies relative to 1961-1990 calendar-monthly climatological average night marine air temperature (NMAT). Adjustments are made for changes in observation height; these depend on estimates of the stability of the near surface atmospheric boundary layer. In previous versions of the dataset, ad hoc adjustments were also made for three periods and regions where poor observational practice was prevalent. These adjustments are re-examined. Estimates of uncertainty are calculated for every grid box and result from: measurement errors; uncertainty in adjustments applied to the observations; uncertainty in the measurement height and under-sampling. The new dataset is a clear improvement over previous versions in terms of coverage because of the recent digitization of historical observations from ships' logbooks. However, the periods prior to about 1890 and around World War 2 remain particularly uncertain and sampling is still sparse in some regions in other periods. A further improvement is the availability of uncertainty estimates for every grid box and every month. Previous versions required adjustments that were dependent on contemporary measurements of sea surface temperature (SST); to avoid these, the new dataset starts in 1880 rather than 1856. Overall agreement with variations of SST is better for the updated dataset than for previous versions, maintaining existing estimates of global warming and increasing confidence in the global record of temperature variability and change

    Temporal and visual source memory deficits among ecstasy/polydrug users

    Objectives: The present paper seeks to investigate whether source memory judgements are adversely affected by recreational illicit drug use. Method: Sixty-two ecstasy/polydrug users and 75 non ecstasy users completed a source memory task, in which they tried to determine whether or not a word had been previously presented and if so, attempted to recall the format, location and temporal position in which the word had occurred. Results: While not differing in terms of the number of hits and false positive responses, ecstasy/polydrug users adopted a more liberal decision criterion when judging if a word had been presented previously. With regard to source memory, users were less able to determine the format in which words had been presented (upper versus lower case). Female users did worse than female nonusers in determining which list (first or second) a word was from. Unexpectedly, the current frequency of cocaine use was negative associated with list and case source memory performance. Conclusions: Given the role that source memory plays in everyday cognition, those who use cocaine more frequently might have more difficulty in everyday tasks such as recalling the sources of crucial information or making use of contextual information as an aid to learning

    Microvascular resistance predicts myocardial salvage and infarct characteristics in ST-elevation myocardial infarction

    <b>Background:</b> The pathophysiology of myocardial injury and repair in patients with ST‐elevation myocardial infarction is incompletely understood. We investigated the relationships among culprit artery microvascular resistance, myocardial salvage, and ventricular function.<p></p> <b>Methods and Results:</b> The index of microvascular resistance (IMR) was measured by means of a pressure‐ and temperature‐sensitive coronary guidewire in 108 patients with ST‐elevation myocardial infarction (83% male) at the end of primary percutaneous coronary intervention. Paired cardiac MRI (cardiac magnetic resonance) scans were performed early (2 days; n=108) and late (3 months; n=96) after myocardial infarction. T2‐weighted‐ and late gadolinium–enhanced cardiac magnetic resonance delineated the ischemic area at risk and infarct size, respectively. Myocardial salvage was calculated by subtracting infarct size from area at risk. Univariable and multivariable models were constructed to determine the impact of IMR on cardiac magnetic resonance–derived surrogate outcomes. The median (interquartile range) IMR was 28 (17–42) mm Hg/s. The median (interquartile range) area at risk was 32% (24%–41%) of left ventricular mass, and the myocardial salvage index was 21% (11%–43%). IMR was a significant multivariable predictor of early myocardial salvage, with a multiplicative effect of 0.87 (95% confidence interval 0.82 to 0.92) per 20% increase in IMR; P<0.001. In patients with anterior myocardial infarction, IMR was a multivariable predictor of early and late myocardial salvage, with multiplicative effects of 0.82 (95% confidence interval 0.75 to 0.90; P<0.001) and 0.92 (95% confidence interval 0.88 to 0.96; P<0.001), respectively. IMR also predicted the presence and extent of microvascular obstruction and myocardial hemorrhage.<p></p> <b>Conclusion:</b> Microvascular resistance measured during primary percutaneous coronary intervention significantly predicts myocardial salvage, infarct characteristics, and left ventricular ejection fraction in patients with ST‐elevation myocardial infarction.<p></p&gt

    Pathological Response in Resectable Non-small Cell Lung Cancer: A Systematic Literature Review and Meta-Analysis

    BACKGROUND: Surrogate endpoints for overall survival in patients with resectable non-small cell lung cancer receiving neoadjuvant therapy are needed to provide earlier treatment outcome indicators and accelerate drug approval. This study\u27s main objectives were to investigate the association among pathological complete response, major pathological response, event-free survival and overall survival and to determine whether treatment effects on pathological complete response and event-free survival correlate with treatment effects on overall survival. METHODS: A comprehensive systematic literature review was conducted to identify neoadjuvant studies in resectable non-small cell lung cancer. Analysis at the patient level using frequentist and Bayesian random effects (hazard ratio [HR] for overall survival or event-free survival by pathological complete response or major pathological response status, yes vs no) and at the trial level using weighted least squares regressions (hazard ratio for overall survival or event-free survival vs pathological complete response, by treatment arm) were performed. RESULTS: In both meta-analyses, pathological complete response yielded favorable overall survival compared with no pathological complete response (frequentist, 20 studies and 6530 patients: HR = 0.49, 95% confidence interval = 0.42 to 0.57; Bayesian, 19 studies and 5988 patients: HR = 0.48, 95% probability interval = 0.43 to 0.55) and similarly for major pathological response (frequentist, 12 studies and 1193 patients: HR = 0.36, 95% confidence interval = 0.29 to 0.44; Bayesian, 11 studies and 1018 patients: HR = 0.33, 95% probability interval = 0.26 to 0.42). Across subgroups, estimates consistently showed better overall survival or event-free survival in pathological complete response or major pathological response compared with no pathological complete response or no major pathological response. Trial-level analyses showed a moderate to strong correlation between event-free survival and overall survival hazard ratios (R2 = 0.7159) but did not show a correlation between treatment effects on pathological complete response and overall survival or event-free survival. CONCLUSION: There was a strong and consistent association between pathological response and survival and a moderate to strong correlation between event-free survival and overall survival following neoadjuvant therapy for patients with resectable non-small cell lung cancer

    Safety of guidewire-based measurement of fractional flow reserve and the index of microvascular resistance using intravenous adenosine in patients with acute or recent myocardial infarction

    Aims: Coronary guidewire-based diagnostic assessments with hyperemia may cause iatrogenic complications. We assessed the safety of guidewire-based measurement of coronary physiology, using intravenous adenosine, in patients with an acute coronary syndrome. Methods: We prospectively enrolled invasively managed STEMI and NSTEMI patients in two simultaneously conducted studies in 6 centers (NCT01764334; NCT02072850). All of the participants underwent a diagnostic coronary guidewire study using intravenous adenosine (140 μg/kg/min) infusion for 1–2 min. The patients were prospectively assessed for the occurrence of serious adverse events (SAEs) and symptoms and invasively measured hemodynamics were also recorded. Results: 648 patients (n = 298 STEMI patients in 1 hospital; mean time to reperfusion 253 min; n = 350 NSTEMI in 6 hospitals; median time to angiography from index chest pain episode 3 (2, 5) days) were included between March 2011 and May 2013. Two NSTEMI patients (0.03% overall) experienced a coronary dissection related to the guidewire. No guidewire dissections occurred in the STEMI patients. Chest symptoms were reported in the majority (86%) of patient's symptoms during the adenosine infusion. No serious adverse events occurred during infusion of adenosine and all of the symptoms resolved after the infusion ceased. Conclusions: In this multicenter analysis, guidewire-based measurement of FFR and IMR using intravenous adenosine was safe in patients following STEMI or NSTEMI. Self-limiting symptoms were common but not associated with serious adverse events. Finally, coronary dissection in STEMI and NSTEMI patients was noted to be a rare phenomenon

    Nutrition and Social Eating Habits Among Seniors Living Independently

    Background: Older adults have unique nutritional needs due to physiologic changes that occur as part of the normal aging process. Maintaining adequate nutrition has the potential to reduce morbidity and mortality related to chronic disease, fall risk, dementia and Alzheimer’s disease. Aging also poses an increased risk of isolation and lack of social interaction, particularly noted at meal times. Unintentional weight loss is an independent risk factor for early mortality. Social eating is related to higher food intake, and meal programs can improve nutritional risk for vulnerable seniors. We partnered with the Cathedral Square Corporation (CSC) to assess nutrition and social eating in residents at Heineberg Senior Housing in Burlington, VT and conducted focus groups to determine general nutritional concerns and evaluate potential interventions.https://scholarworks.uvm.edu/comphp_gallery/1033/thumbnail.jp
