Aim:Hepatocellular carcinoma (HCC) is the second most common cause of cancer related deaths worldwide. Therefore, early diagnosis is too important for HCC. The aim of this study was to evaluate the predictive value of albumin-bilirubin index (ALBI) and platelet-albumin-bilirubin index (PALBI) on the development of HCC in patients with cirrhosis.Materials and Methods:331 patients with liver cirrhosis due to chronic hepatitis B and chronic hepatitis B + D who were admitted to Demiroğlu Bilim University between 2004-2019 were included in the study. The patients were divided into two main groups, with and without HCC. Demographic data, body mass index (BMI), Child and MELD scores and laboratory findings of the patients were recorded and analyzed separately for the groups. Mean ALBI and PALBI values were calculated for both groups. The relationships between ALBI, PALBI and HCC were evaluated. Results:The mean age of the patients was 51.3 ± 9.18 and, the mean BMI was 30.04 ± 11.65. The HCC group consisted of 115 patients and the non-HCC cirrhosis group consisted of 216 patients. Mean ALBI and PALBI score values were significantly lower in HCC group than in non-HCC cirrhosis group (p<0,001). Area under curve values (AUC) were calculated 0.775 for the ALBI and 0.733 for the PALBI.Conclusion:Although ALBI and PALBI have been developed to predict the survival and prognosis of HCC, we concluded that ALBI and PALBI may also be useful predictors of HCC development in cirrhosis

The Coast Guard Command, which has a wide range of duties as saving human lives, protecting natural resources, preventing marine pollution and battle against smuggling, uses diesel main engines in its ships, as in other military and commercial ships. It is critical that the main engines operate smoothly at all times so that they can respond quickly while performing their duties, thus enabling fast and early detection of faults and preventing failures that are costly or take longer to repair. The aim of this study is to create and to develop a model based on current data, to select machine learning algorithms and ensemble methods, to develop and explain the most appropriate model for fast and accurate detection of malfunctions that may occur in 4-stroke high-speed diesel engines. Thus, it is aimed to be an exemplary study for a data-based decision support mechanism

Aim:The presence of hepatosteatosis (HS) in the donor has negative effects on the results of liver transplantation (LT). Therefore, the detection of donor HS is vital during the pre-transplant period. The aim of this study was to compare the efficacy of liver biopsy and radiological methods in the detection of HS in live liver donor candidates.Materials and Methods:Two hundred twenty-six healthy individuals who were admitted to Demiroğlu Bilim University as donor candidates for LT were included in the study. Demographic, histopathological, laboratory and imaging findings of the donors were retrospectively reviewed. Computed tomography (CT) and magnetic resonance imaging (MRI) scans of the donors were retrospectively reevaluated and liver fat measurements were recorded.Results:39% (88) of the patients were female and 61% (138) were male. In the study population, the mean age was 34.3±8.7 years, the mean weight was 78.0±12.6 kg, the mean height was 169.1±9.6 cm, and the mean body mass index was 27.2±4.0. 42% of donors had 5% HS in liver biopsy. Both CT and MRI showed significant correlations with biopsy in HS detection (p<0.05).Conclusion:In our study, it was found that MRI correlated with biopsy as much as CT and could be used easily in the detection of HS. The use of MRI in liver donors may be more appropriate for donor health prior to transplantation

OBJECTIVE: We investigated the effects on skeletal muscle of gadolinium based linear and macrocyclic radiocontrast agents applied at experimental intervals using histopathological methods. METHODS: Thirty-two male Sprague–Dawley rats were included in the study for histopathological analysis. No procedure was performed on the healthy control group. The sham group received 0.1 ml/kg intravenous (IV) saline solution through the tail vein 4 times weekly for 5 weeks. The gadodiamide group received total 2 mmol/kg IV gadodiamide through the tail vein 4 times weekly for 5 weeks. The gadoteric-acid group received 2 mmol/kg IV gadoteric acid through the tail vein 4 times weekly for 5 weeks. RESULTS: We determined no marked apoptotic myofibrils associated withcaspase-3 expression in these two groups. Furthermore, no calcineurin expression was observed in myofibrils in the two groups. However, quantitative analyses revealed a decrease in muscle-fiber area in the gadodiamide and gadoteric-acid groups compared to the control group (Respectively; p=0.001 and p=0.017). CONCLUSION: In our experimental study, linear and macrocyclic GBCAs applied at repeated doses played no role in myofibril damage induced by caspase-3 and calcineurin – nuclear factor of activated T-cells in skeletal muscle tissue

Off-pump versus on-pump coronary artery bypass grafting in acute coronary syndrome: a clinical analysis

Abstract

Background

Although off-pump coronary artery bypass (OPCAB) surgery has many beneficial effects compared with on-pump surgery, switch to on-pump surgery has significantly higher risks of operative mortality. Benefits of OPCAB over on-pump surgery strategies concerning myocardial revascularization are still debatable. We have aimed to develop an "algorithm of off-pump surgical strategy" on preventing conversion to on-pump. This clinical study reports our clinical outcome of OPCAB in patients with acute coronary syndrome.

Methods

Between January 2006 and December 2008, 198 patients with acute coronary syndrome were enrolled in the study. Decision of OPCAB (142 patients) or on-pump surgery (56 patients) was made according to patients' present clinical status and our surgical background. Cardiac enzymes, duration of the surgery, graft numbers, stay in intensive care unit were recorded.

Results

OPCAP group has shorter operation time (82.78 min versus 164.22 min, p < 0.001), lesser necessity for intra-aortic balloon pumping (3.5% versus 12.5%, p = 0.053), shorter duration of intensive care unit stay (p < 0.05) and hospital stay (p < 0.001) compared to on-pump patients. EuroSCORE level was lower in OPCAP group (p < 0.001). None of the patients of OPCAB group required conversion to on-pump technique.

Conclusions

The patients who admitted to the hospital with acute coronary syndrome within "golden hours" (within 6 hours after onset) had a greater chance for OPCAB surgery. This study proves that EuroSCORE is likely to be an important factor in deciding which surgical technique to use, but a further investigation is needed to verify. According to our findings, a careful evaluation of coronary angiography, hemodynamic status, quality of target coronary vessel and timing of surgery are important for OPCAB surgery to avoid conversion to on-pump. By a careful systematic evaluation of the patients as explained with this article, it can be prevent or reduce conversion to on-pump surgery during OPCAB surgery.</p

Replicated evidence that endophenotypic expression of schizophrenia polygenic risk is greater in healthy siblings of patients compared to controls, suggesting gene-environment interaction. The EUGEI study

    Get PDF
Background First-degree relatives of patients with psychotic disorder have higher levels of polygenic risk (PRS) for schizophrenia and higher levels of intermediate phenotypes. Methods We conducted, using two different samples for discovery (n = 336 controls and 649 siblings of patients with psychotic disorder) and replication (n = 1208 controls and 1106 siblings), an analysis of association between PRS on the one hand and psychopathological and cognitive intermediate phenotypes of schizophrenia on the other in a sample at average genetic risk (healthy controls) and a sample at higher than average risk (healthy siblings of patients). Two subthreshold psychosis phenotypes, as well as a standardised measure of cognitive ability, based on a short version of the WAIS-III short form, were used. In addition, a measure of jumping to conclusion bias (replication sample only) was tested for association with PRS. Results In both discovery and replication sample, evidence for an association between PRS and subthreshold psychosis phenotypes was observed in the relatives of patients, whereas in the controls no association was observed. Jumping to conclusion bias was similarly only associated with PRS in the sibling group. Cognitive ability was weakly negatively and non-significantly associated with PRS in both the sibling and the control group. Conclusions The degree of endophenotypic expression of schizophrenia polygenic risk depends on having a sibling with psychotic disorder, suggestive of underlying gene–environment interaction. Cognitive biases may better index genetic risk of disorder than traditional measures of neurocognition, which instead may reflect the population distribution of cognitive ability impacting the prognosis of psychotic disorder
