Laparoscopic classification of the impalpable testis: an update

    Get PDF
Purpose We present a classification for the nonpalpable testis (NPT) based on laparoscopic findings and suggest guidelines for the interpretation of these findings.Patients and methods From October 2002 to December 2010, 121 patients with NPT underwent laparoscopy at two tertiary centers of Pediatric surgery in Egypt. The lower abdomen and pelvis were inspected to identify the following structures and their inter-relationships: the internal inguinal ring (and its patency), spermatic vessels, vas deferens, and testis. The laparoscopic findings were documented by one of the authors who attended all procedures, and video recordings were available in some cases. Further management was dependent on laparoscopic findings, classification, and plan of treatment. Results The study included 117 patients with 142 nonpalpable testes. Their mean age was 4.9 years. Among patients with unilateral NPT, a contralateral palpable undescended testicle was always associated with a viable NPT (100%), whereas a contralateral scrotal testis had an equal chance (50%) of finding a viable NPT, without a significant difference whether it was right or left sided. Conclusion Failure of normal testicular descent leads to a spectrum of anatomical variations that can be precisely and safely defined by laparoscopy in about two-thirds of patients with nonpalpable testes. In the remaining one-third of patients, including inguinal exploration after laparoscopy can help us to exclude a missed viable inguinal testis. Keywords: cryptorchidism, laparoscopy, testi

Mitomycin C application in resistant caustic esophageal stricture

    Get PDF
Background/purpose Caustic esophageal strictures still represent a catastrophic problem in children of our country. Management protocol is usually started by regular esophageal dilatation in which multiple sessions may be needed until the resolution of dysphagia; however, in many cases endoscopic dilatation fails and therefore esophageal replacement is eventually required. Our aim is to assess the role of mitomycin C application in the management of caustic esophageal stricture refractory to regular endoscopic dilatation.Patients and methods Patients with resistant caustic esophageal stricture were subjected to topical mitomycin C application on stricture site after endoscopic dilatation. Patients were followed up clinically using validated dysphagia score to assess the improvement of dysphagia, radiologically by contrast esophagogram and endoscopically to confirm resolution of the stricture. Results were analyzed and compared with results of a control group managed at the institution by a regular dilatation protocol. Results During the period January 2008 to June 2010, 12 patients with resistant caustic esophageal stricture were followed at our unit of whom six had a short esophageal stricture (< 3 cm) and six had a long stricture ( >3 cm). Topical mitomycin C application resulted in clinical and radiological resolution of dysphagia and strictures in 83% and 66% respectively, compared with 44 (eight out of 18 patients with short stricture) and 25% (two out of eight patients with long stricture) in the control group. Conclusion Mitomycin C application can markedly improve the outcome of patients with caustic esophageal stricture, reducing the number of required sessions for endoscopic dilatation. However, double-blinded randomized controlled trial is needed to prove its effectiveness. Keywords: caustic esophageal stricture, esophageal dilatation, mitomycin

Combating the High Prevalence of Obesity among Egyptian Households: A Pilot Study: Port-Said Households/ مكافحة السمنة في المجتمع المصري: دراسة تجريبية في بورسعيد

Obesity is a major public health issue in Egypt and its repercussions are not only limited to the health dimension but also extend to affecting the productive capacity of the citizens. This adversely affects the overall fundamentals of the Egyptian economy. Moreover, there is a current political interest in promoting the health and well-being of the citizens which is reflected in the recent 100 Million Seha massive campaign that measured the weight and height of millions of Egyptians.By applying the decision-making rule which has to do adhere to the equity, administrative, and political criteria, we recommend adjusting the Egyptian subsidy system and making use of the medical centers. Solving tackling obesity requires intensive efforts at the level of the state, the regional community, and even the international level. Utilizing the current resources of the Ministry of Supply and Internal Trade is considerably convenient for controlling and preventing obesity in Egypt. The proposed recommendation, which is adjusting the Egyptian subsidy system, primarily depends on the role of the Ministry of Supply and Internal Trade in securing healthy food items in the ministries’ different distribution channels, along with providing healthier variants in the list of subsidized goods
