Toxizitäten und Therapiemanagement der Erstlinien-Targeted-Therapie beim metastasierten Nierenzellkarzinom als Einflussfaktoren auf das progressionsfreie Überleben und Gesamtüberleben
Einführung: Die Behandlung des metastasierten Nierenzellkarzinoms ist mit den verschiedenen Medikamenten aus der Gruppe der VEGF-Inhibitoren sowie mTOR-Inhibitoren als lang etablierte Substanzen in der Erstlinientherapie sehr komplex. Besondere Bedeutung haben Prognose- und Prädiktionsfaktoren, die für ein Therapieansprechen und verlängertes Gesamtüberleben (OS) stehen können. Ziel dieser Arbeit war es, bereits etablierte Faktoren zu überprüfen und neue Faktoren besonders aus dem Bereich der Nebenwirkung unter der Erstlinientherapie zu benennen.
Methoden: Es wurde eine retrospektive Studie an 151 Patienten im Zeitraum von 2006 bis 2013 durchgeführt. Es erfolgte eine Unterteilung der Patienten in eine Gruppe mit einem progressionsfreien Überleben (PFS) von mindestens 6 Monaten und eine Gruppe mit weniger als 6 Monaten, um hieran Prognose-und Prädiktionsfaktoren zu ermitteln.
Die Länge des PFS und OS wurden bezüglich der verschiedenen Einflussfaktoren in der Kaplan-Meier Überlebensanalyse mittels Log-Rank-Test verglichen. Zur Identifizierung unabhängiger Einflussfaktoren wurde eine Cox-Regressionsanalyse durchgeführt.
Ergebnisse: Neben bekannten Einflussfaktoren wie dem Hand-Fuß-Syndrom (HFS) (p-Wert 0,0003), der Hypertonie (p-Wert 0,046), der Leukopenie (p-Wert 0,013) und der Hypothyreose (p-Wert 0,022) konnten auch neu die Diarrhoe (p-Wert 6 Monate auf.
An unabhängigen Prognosefaktoren für ein verlängertes PFS konnte die „Disease control“ Rate definiert als stabile Erkrankung (SD), partielle Remission (PR) oder komplette Remission (CR) mit einer Hazard Ratio (HR) von 0,050 (95% KI 0,023-0,108, p-Wert <0,0001) ermittelt werden (PFS im Median bei 13,27 Monaten versus 2,90 Monaten). Es konnte erstmalig eine Mukositis als prädiktiver Faktor für ein verlängertes PFS mit 12,87 Monaten im Vergleich zu 7,43 Monaten mit einer HR von 0,480 (95% KI 0,294-0,785; p-Wert 0,003) gesehen werden. Der letzte unabhängige, bisher nicht bekannte Einflussfaktor besteht in der Verabreichung einer supportiven medikamentösen Therapie mit einer Verlängerung des PFS von im Median 12,90 Monaten vs. 7,43 Monaten (HR 0,592 (95% KI 0,379-0,924, p-Wert 0,0021)).
Als unabhängiger Prognosefaktor für ein verlängertes OS von 38,97 Monaten vs. 15,87 Monaten wurde ebenfalls die „Disease control“ Rate (SD, PR und CR) der Erstlinientherapie mit einer HR von 0,227 (95% KI 0,131-0,393, p-Wert <0,0001) identifiziert. An unabhängigen Prognosefaktoren für ein verkürztes OS konnten eine primäre Metastasierung sowie erstmalig auch das Ausbilden einer kardiovaskulären Toxizität der Erstlinientherapie ermittelt werden.
Schlussfolgerung: Es konnten neue Prognose- und prädiktive Faktoren für das PFS und OS unter Targeted-Therapie (TT) der Erstlinie ermittelt werden. Diese gilt es in prospektiven Studien zu bestätigen.Introduction: Treatment of the metastatic renal cell carcinoma is, due to its different treatment options for the first line therapy, very complex. Therefore, it is crucial to identify possible predictive and prognostic factors, which can show longer progression free survival (PFS) and overall survival (OS). It is thus the objective of this study to verify established factors and identify new factors from the targeted therapy era, especially side effects of first line therapy, that could become predictive and/or prognostic for longer or shorter PFS and OS.
Methods: This retrospective study compared 151 patients during a period of seven years, who had received a first line therapy. To compare the median length of the PFS and OS the Kaplan-Meier survival analysis and the log-rank test were used. Independent prognostic factors were identified by multivariated cox-regression model. To furthermore identify influencing factors, patients were additionally divided into two subgroups, one group with a longer PFS (>= 6 months) and one group with a shorter PFS (< 6 months).
Results: In the two groups investigated, significant differences for the following side effects. hand-foot-syndrome (p-value 0,0003), reflux disease (p-value 0,021), hypertension (p-value 0,046), leucopenia (p-value 0,013), hypothyreosis (p-value 0,022) and diarrhea (p-value < 0,0001) could be identified. All these toxicities were found in more patients in the group with the longer PFS.
An independent prognostic factor for longer PFS was the „disease control“ rate (stable disease (SD), partial remission (PR) and complete remission (CR)). Here the hazard ratio (HR) was 0,050 (95% CI 0,023-0,108, p-value <0,0001) and the patients had a median PFS of 13,27 months vs. 2,90 months. Moreover a mucositis was found to be an independent factor for a longer PFS (12,87 months vs. 7,43 months). The HR was 0,480 (95% CI 0,294-0,785; p-value 0,003). An additional prognostic factor was the supportive medical treatment with a longer median PFS of 12,90 months vs. 7,43 months (HR 0,592 (95% CI 0,379-0,924, p-value 0,0021). “Disease control” rate could also be found as one independent prognostic factor for longer OS with 38,97 months vs. 15,87 months (HR 0,227 (95% CI 0,131-0,393, p-value <0,0001). Synchronous metastatic disease and for the first time cardiovascular toxicity were shown to be independent factors for shorter median OS.
Conclusion: New prognostic and predictive factors for a longer PFS and OS and furthermore for a shorter OS were identified in this study. These factors have to be confirmed in prospective studies

Das Ovarialkarzinom hat durch die häufige Entwicklung von Zytostatikaresistenzen eine
besonders schlechte Prognose, so dass der Überwindung dieser Resistenzen eine besonders
große Bedeutung zukommt. Die vorliegenden Untersuchungen dienten zur Analyse der
Proliferationshemmung und Apoptoseinduktion durch den Tumornekrosefaktor-Rezeptorassoziiertem
Apoptose-induzierenden Liganden (TRAIL) an HEY-Zelllinien mit selektiver
Zytostatikaresistenz gegen die beim Ovarialkarzinom eingesetzten Chemotherapeutika.
TRAIL stellt einen der wenigen Stoffe dar, mit denen die gezielte Therapie einer malignen
Erkrankung möglich ist, ohne physiologische Zellverbände zu beeinflussen.
Mit Hilfe verschiedener Proliferationsassays konnte im ersten Teil der Arbeit eine Dosisabhängige
Proliferationshemmung in allen Zelllinien nachgewiesen werden, wobei die Taxanresistenten
Varianten eine partielle TRAIL-Resistenz aufwiesen und die TRAIL-resistente
Kontroll-Zelllinie nicht beeinflusst wurde. Darüber hinaus konnte mittels Färbung der Zellen
mit dem DNA-Farbstoff DAPI bewiesen werden, dass TRAIL schon in geringen
Konzentrationen Apoptose in den Ovarialkarzinomzelllinien auslöst. Die Zellen zeigten hier
ein ähnliches Ansprechen auf TRAIL wie zuvor im Proliferationsassay.
Die Untersuchung der Proteinexpression der Schlüsselfaktoren des TRAIL-Signalweges
zeigte, dass alle vier Todesrezeptoren sowie pro- als auch antiapoptotische Proteine in allen
Zelllinien nachweisbar waren. Eine Ausnahme stellte die Caspase 8 dar, die in der TRAILresistenten
Linie nicht exprimiert wurde.
Ein Zusammenhang zwischen der Expression und dem Ansprechen der Zellen auf eine
TRAIL-Behandlung lies sich für das antiapoptotische Protein XIAP nachweisen. TRAIL
führte in den sensitiven Zelllinien zu einer stark verminderten Expression dieses Proteins,
wohingegen die partiell TRAIL-resistenten Taxan-resistenten und die TRAIL-resistente
Kontrollvariante keine bzw. eine geringe Herabreguation aufwiesen.
Die Behandlung mit 50 ng/ml TRAIL führte in den sensitive Zelllinien zu einer Spaltung der
inaktiven Procaspase 8 in ihre aktive, proapoptotische Form, welche in den Taxan-resistenten
Zelllinien nur unvollständig stattfand.
Im zweiten Teil der Arbeit konnte gezeigt werden, dass es durch die Kombination von TRAIL
mit den bei der Diabetestherapie eingesetzten PPARg-Liganden zu einem verstärkten
apoptotischen Effekt kam. Am ausgeprägtesten war dieser Synergismus durch die
Kombination mit 15d-PGJ2. Endlich ließ sich durch diese Kombinationsbehandlung auch inden partiell TRAIL-resistenten Taxan-resistenten Varianten eine Proliferationshemmung und
Apoptoseinduktion erzielen. Die Proteinexpressionsanalysen von DR5, Caspase 8, cFlip und
XIAP unterstreichen die Ergebnisse der Proliferationsassys.
Untersuchungen mit dem PPAR-Liganden-Inhibitors GW9662 wiesen darauf hin, dass die
Beeinflussung des Zellwachstums durch die PPAR-Liganden nicht über deren
herkömmlichen Signalweg erfolgte.
TRAIL zeigte in den durchgeführten Untersuchungen eine Hemmung der Proliferation und
Induktion der Apoptose in chemoresistenten Ovarialkarzinom-Zelllinien, welche sich durch
die Kombination mit den aus der Diabetestherapie bekannten PPARg-Liganden noch
verstärken ließ und einen positiv synergistischen Effekt auf die partiell TRAIL-resistenten
Taxan-resistenten Zelllinien ausübte.
Die Kombination aus TRAIL und PPAR-Liganden könnte somit eine vielversprechende
Möglichkeit zur Therapie des Ovarialkarzinoms sein
Analysis of the Effects of Day-Time vs. Night-Time Surgery on Renal Transplant Patient Outcomes
Sleep deprivation and disruption of the circadian rhythms could impair individual surgical performance and decision making. For this purpose, this study identified potential confounding factors on surgical renal transplant patient outcomes during day and night. Our retrospective cohort study of 215 adult renal cadaver transplant recipients, of which 132 recipients were allocated in the "day-time" group and 83 recipients in the "night-time" group, primarily stratified the patients into two cohorts, depending on the start time. Within a 24 h operational system, "day-time" was considered as being from 8 a.m. to 8 p.m. and "night-time" from 8 p.m. to 8 a.m.. Primary outcomes examined patient and graft survival after three months and one year. Secondary outcomes included the presence of acute rejection (AR) and delayed graft function (DGF), as well as the rate of postoperative complications. In log-rank testing, "day-time" surgery was associated with a significantly higher risk of patient death (p = 0.003), whereas long-term graft survival was unaffected by the operative time of day. The mean cold ischemia time (CIT), which was 12.4 ± 5.3 h in the "night-time" group, was significantly longer compared to 10.7 ± 3.6 for those during the day (p = 0.01). We observed that "night-time" kidney recipients experienced more wound complications. From our single-centre data, we conclude that night-time kidney transplantation does not increase the risk of adverse events or predispose the patient to a worse outcome. Nevertheless, further research is required to explore the effect of fatigue on nocturnal surgical performance
Human islet microtissues as an in vitro and an in vivo model system for diabetes
Loss of pancreatic β-cell function is a critical event in the pathophysiology of type 2 diabetes. However, studies of its underlying mechanisms as well as the discovery of novel targets and therapies have been hindered due to limitations in available experimental models. In this study we exploited the stable viability and function of standardized human islet microtissues to develop a disease-relevant, scalable, and reproducible model of β-cell dysfunction by exposing them to long-term glucotoxicity and glucolipotoxicity. Moreover, by establishing a method for highly-efficient and homogeneous viral transduction, we were able to monitor the loss of functional β-cell mass in vivo by transplanting reporter human islet microtissues into the anterior chamber of the eye of immune-deficient mice exposed to a diabetogenic diet for 12 weeks. This newly developed in vitro model as well as the described in vivo methodology represent a new set of tools that will facilitate the study of β-cell failure in type 2 diabetes and would accelerate the discovery of novel therapeutic agents

Uncertainty in Simulating Wheat Yields Under Climate Change
Projections of climate change impacts on crop yields are inherently uncertain1. Uncertainty is often quantified when projecting future greenhouse gas emissions and their influence on climate2. However, multi-model uncertainty analysis of crop responses to climate change is rare because systematic and objective comparisons among process-based crop simulation models1,3 are difficult4. Here we present the largest standardized model intercomparison for climate change impacts so far. We found that individual crop models are able to simulate measured wheat grain yields accurately under a range of environments, particularly if the input information is sufficient. However, simulated climate change impacts vary across models owing to differences in model structures and parameter values. A greater proportion of the uncertainty in climate change impact projections was due to variations among crop models than to variations among downscaled general circulation models. Uncertainties in simulated impacts increased with CO2 concentrations and associated warming. These impact uncertainties can be reduced by improving temperature and CO2 relationships in models and better quantified through use of multi-model ensembles. Less uncertainty in describing how climate change may affect agricultural productivity will aid adaptation strategy development and policymaking
