Characterization of wear debris released from alumina-on-alumina hip prostheses: Analysis of retrieved femoral heads and peri-prosthetic tissues
We analyzed by SEM three alumina-on-alumina femoral heads obtained from three patients who underwent revision for an aseptic loosening of the acetabular component. In parallel, the peri-prosthetic tissues were analyzed histologically in search of wear debris coming from the ceramic. Stripe wears, abrasive streaks, craters, and areas with extensive biomaterial removal were evidenced on the three femoral heads by SEM. In the altered area, the structure of the ceramic composed of minute polyhedric grains packed together was evidenced. In the peri-prosthetic tissues, the alumina particles were present in different forms: larges particles appeared transparent and birefringent, small particles engulfed by the macrophages had a light brown tint and were not birefringent. Large particles corresponded to the grains observed by SEM. EDS microanalysis confirmed the presence of aluminum oxide in these particles. Predicting fracture outcomes from clinical registry data using artificial intelligence supplemented models for evidence-informed treatment (PRAISE) study protocol
BackgroundDistal radius (wrist) fractures are the second most common fracture admitted to hospital. The anatomical pattern of these types of injuries is diverse, with variation in clinical management, guidelines for management remain inconclusive, and the uptake of findings from clinical trials into routine practice limited. Robust predictive modelling, which considers both the characteristics of the fracture and patient, provides the best opportunity to reduce variation in care and improve patient outcomes. This type of data is housed in unstructured data sources with no particular format or schema. The âPredicting fracture outcomes from clinical Registry data using Artificial Intelligence (AI) Supplemented models for Evidence-informed treatment (PRAISE)â study aims to use AI methods on unstructured data to describe the fracture characteristics and test if using this information improves identification of key fracture characteristics and prediction of patient-reported outcome measures and clinical outcomes following wrist fractures compared to prediction models based on standard registry data.Methods and designAdult (16+ years) patients presenting to the emergency department, treated in a short stay unit, or admitted to hospital for >24h for management of a wrist fracture in four Victorian hospitals will be included in this study. The study will use routine registry data from the Victorian Orthopaedic Trauma Outcomes Registry (VOTOR), and electronic medical record (EMR) information (e.g. X-rays, surgical reports, radiology reports, images). A multimodal deep learning fracture reasoning system (DLFRS) will be developed that reasons on EMR information. Machine learning prediction models will test the performance with/without output from the DLFRS.DiscussionThe PRAISE study will establish the use of AI techniques to provide enhanced information about fracture characteristics in people with wrist fractures. Prediction models using AI derived characteristics are expected to provide better prediction of clinical and patient-reported outcomes following distal radius fracture
Isotopic composition (238U/235U) of some commonly used uranium reference materials
We have determined 238U/235U ratios for a suite of commonly used natural (CRM 112a, SRM 950a, and HU-1) and synthetic (IRMM 184 and CRM U500) uranium reference materials by thermal ionisation mass-spectrometry (TIMS) using the IRMM 3636 233U-236U double spike to accurately correct for mass fractionation. Total uncertainty on the 238U/235U determinations is estimated to be < 0.02% (2Ï). These natural 238U/235U values are different from the widely used âconsensusâ value (137.88), with each standard having lower 238U/235U values by up to 0.08%. The 238U/235U ratio determined for CRM U500 and IRMM 184 are within error of their certified values; however, the total uncertainty for CRM U500 is substantially reduced (from 0.1% to 0.02%). These reference materials are commonly used to assess mass spectrometer performance and accuracy, calibrate isotope tracers employed in U, U-Th and U-Pb isotopic studies, and as a reference for terrestrial and meteoritic 238U/235U variations. These new 238U/235U values will thus provide greater accuracy and reduced uncertainty for a wide variety of isotopic determinations
Equivalence of Narcissistic Personality Inventory constructs and correlates across scoring approaches and response formats
The prevalent scoring practice for the Narcissistic Personality Inventory (NPI) ignores the forced-choice nature of the items. The aim of this study was to investigate whether findings based on NPI scores reported in previous research can be confirmed when the forced-choice nature of the NPIâs original response format is appropriately modeled, and when NPI items are presented in different response formats (true/false or rating scale). The relationships between NPI facets and various criteria were robust across scoring approaches (mean score vs. model-based), but were only partly robust across response formats. In addition, the scoring approaches and response formats achieved equivalent measurements of the vanity facet and in part of the leadership facet, but differed with respect to the entitlement facet
First Light Measurements with the XMM-Newton Reflection Grating Spectrometers: Evidence for an Inverse First Ionisation Potential Effect and Anomalous Ne A bundance in the Coronae of HR 1099
The RS CVn binary system HR 1099 was extensively observed by the XMM-Newton
observatory in February 2000 as its first-light target. A total of 570 ks of
exposure time was accumulated with the Reflection Grating Spectrometers (RGS).
The integrated X-ray spectrum between 5-35 Angstrom is of unprecedented quality
and shows numerous features attributed to transitions of the elements C, N, O,
Ne, Mg, Si, S, Fe, and Ni. We perform an in-depth study of the elemental
composition of the average corona of this system, and find that the elemental
abundances strongly depend on the first ionisation potential (FIP) of the
elements. But different from the solar coronal case, we find an inverse FIP
effect, i.e., the abundances (relative to oxygen) increase with increasing FIP.
Possible scenarios, e.g., selective enrichment due to Ne-rich flare-like

IL1B and DEFB1 Polymorphisms Increase Susceptibility to Invasive Mold Infection After Solid-Organ Transplantation
Background.âSingle-nucleotide polymorphisms (SNPs) in immune genes have been associated with susceptibility to invasive mold infection (IMI) among hematopoietic stem cell but not solid-organ transplant (SOT) recipients. Methods.âTwenty-four SNPs from systematically selected genes were genotyped among 1101 SOT recipients (715 kidney transplant recipients, 190 liver transplant recipients, 102 lung transplant recipients, 79 heart transplant recipients, and 15 recipients of other transplants) from the Swiss Transplant Cohort Study. Association between SNPs and the end point were assessed by log-rank test and Cox regression models. Cytokine production upon Aspergillus stimulation was measured by enzyme-linked immunosorbent assay in peripheral blood mononuclear cells (PBMCs) from healthy volunteers and correlated with relevant genotypes. Results.âMold colonization (n = 45) and proven/probable IMI (n = 26) were associated with polymorphisms in the genes encoding interleukin 1ÎČ (IL1B; rs16944; recessive mode, P = .001 for colonization and P = .00005 for IMI, by the log-rank test), interleukin 1 receptor antagonist (IL1RN; rs419598; P = .01 and P = .02, respectively), and ÎČ-defensin 1 (DEFB1; rs1800972; P = .001 and P = .0002, respectively). The associations with IL1B and DEFB1 remained significant in a multivariate regression model (P = .002 for IL1B rs16944; P = .01 for DEFB1 rs1800972). The presence of 2 copies of the rare allele of rs16944 or rs419598 was associated with reduced Aspergillus-induced interleukin 1ÎČ and tumor necrosis factor α secretion by PBMCs. Divergent Associations of SlowâWave Sleep versus Rapid Eye Movement Sleep with Plasma AmyloidâBeta
Objective: Recent evidence shows that during slow-wave sleep (SWS), the brain is cleared from potentially toxic metabolites, such as the amyloid-beta protein. Poor sleep or elevated cortisol levels can worsen amyloid-beta clearance, potentially leading to the formation of amyloid plaques, a neuropathological hallmark of Alzheimer disease. Here, we explored how nocturnal neural and endocrine activity affects amyloid-beta fluctuations in the peripheral blood. Methods: We acquired simultaneous polysomnography and all-night blood sampling in 60 healthy volunteers aged 20â68 years. Nocturnal plasma concentrations of amyloid-beta-40, amyloid-beta-42, cortisol, and growth hormone were assessed every 20 minutes. Amyloid-beta fluctuations were modeled with sleep stages, (non)oscillatory power, and hormones as predictors while controlling for age and participant-specific random effects. Results: Amyloid-beta-40 and amyloid-beta-42 levels correlated positively with growth hormone concentrations, SWS proportion, and slow-wave (0.3â4Hz) oscillatory and high-band (30â48Hz) nonoscillatory power, but negatively with cortisol concentrations and rapid eye movement sleep (REM) proportion measured 40â100 minutes previously (all t values > j3j, p values < 0.003). Older participants showed higher amyloid-beta-40 levels. Interpretation: Slow-wave oscillations are associated with higher plasma amyloid-beta levels, whereas REM sleep is related to decreased amyloid-beta plasma levels, possibly representing changes in central amyloid-beta production or clearance. Strong associations between cortisol, growth hormone, and amyloid-beta presumably reflect the sleepregulating role of the corresponding releasing hormones. A positive association between age and amyloid-beta-40
may indicate that peripheral clearance becomes less efficient with age
