    Feasibility of reducing frailty components in older adults with Alzheimer's dementia:a randomized controlled home-based exercise trial (AD-HOMEX)

    Objectives: There is a need for interventions to reduce frailty in older people with Alzheimer's dementia (AD). The purpose of this study was to investigate the effect of a home-based multimodal exercise program for older adults with AD (AD-HOMEX) on frailty. Design: A parallel single-blind randomized controlled trial comparing a home-based exercise program and usual care. Setting and participants: A home-based program in Brazil. Forty individuals aged 65 years or older with mild to moderate AD. Methods: The intervention group (IG) participated in a 16-week protocol involving three 60-minute sessions per week of progressive individualized physical exercises supervised by a physical therapist. The participants in the control group (CG) maintained their usual care. Frailty was assessed using the FRAIL questionnaire, the Edmonton Frail Scale (EFS) and a subjective assessment by the evaluator (SAE) at baseline and follow-up. Per-protocol analysis was performed. Results: Thirty-five participants completed the program (IG = 16; CG = 19). Frailty improved in the IG based on the EFS (P = .004) and FRAIL (P Conclusions and implications: AD-HOMEX seems to reduce frailty and improve frailty transition patterns.     Realidade virtual na função motora de membros inferiores pós-acidente vascular encefálico

    O Acidente Vascular Encefálico (AVE) pode gerar importantes alterações motoras, de tônus e sensitivas. A realidade virtual (RV), voltada para reabilitação, pode trazer benefícios em relação à aptidão física, atividades motoras e de equilíbrio. Objetivo: Investigar o efeito da terapia por RV no equilíbrio estático e dinâmico, descarga de peso no membro inferior afetado, tônus e recrutamento muscular, independência funcional e função sensório-motora de indivíduos pós-AVE. Métodos: Trata-se de estudo clínico, quasi-experimental e prospectivo em 6 indivíduos com diagnóstico de AVE. Antes e após intervenção com Nintendo Wii Fit Plus os indivíduos foram avaliados pela Escala de Equilíbrio de Berg, Timed “Up and Go”, Índice do Andar Dinâmico; Teste de Marcha (TM); Escala Modificada de Ashworth (EMA); Eletromiografia; Índice de Barthel e Escala Fugl Meyer (EFM). Para análise estatística utilizou-se o teste Kolmogorov-Smirnov, teste t e Wilcoxon. Foi adotado P 80%. Resultados: No TM inicial da 1ª e 15ª sessão (p = 0,03; d = 1,96; P = 96%), EMA dos músculos extensores de quadril (p = 0,04; d = 3,77; P 99%) e joelho (p = 0,04; d = 3,23; P = 99%) e flexores plantares (p = 0,01; d = 3,18; P = 99%) e EFM nas dimensões coordenação/velocidade (p = 0,02; d = 6,74; P = 100%) e sensibilidade (p = 0,01), foram observados resultados significativos com grande efeito e Power > 90%. Nos demais instrumentos não foram encontrados valores significativos. Conclusão: O programa de reabilitação, por RV, foi eficaz na melhora da descarga de peso no membro inferior afetado, tônus muscular e função sensório-motora nos indivíduos do estudoStrokes can generate significant motor, tonic and sensitive changes. Virtual reality (RV), focused on rehabilitation, can bring several benefits such as improvements on physical fitness, motor skills and balance. Objective: To assess the effects of the RV intervention to the static and dynamic balance, weight load on the affected limb, tone and muscle recruitment, functional independence and sensorimotor function in post-stroke individuals. Methods: Quasi-experimental and prospective clinical study with 6 subjects with diagnosis of stroke. Before and after the intervention with Nintendo Wii Fit Plus, the subjects were assessed by the Berg Balance Scale, Timed “Up and Go” and Dynamic Floor Index; Gait Test (GT); Modified Ashworth Scale (MAS); Electromyography; Barthel Index and Fugl-Meyer Scale (FMS). The Statistical analysis used Kolmogorov-Smirnov test, t test and Wilcoxon. Results: The 1st and 15th session evaluations have shown that GT (p = 0.03, d = 1.96, P = 96%), MAS for hip extensor muscles (p = 0.04; d = 3.77; P = 99%), knee extensors (p = 0.04; d = 3.23; P = 99%), plantar flexors (p = 0.01; d = 3.18; P = 99%), FMS in coordination/velocity dimensions (p = 0.02; d = 6.74; P = 100%) and sensitivity (p = 0.01) presented significant results, large effect size and power above 90%. Significant values were not found to the other evaluations. Conclusion: The RV rehabilitation program was effective and improved the weight-load in affected limb, muscle tone and sensorimotor function of the subject

    A list of land plants of Parque Nacional do Caparaó, Brazil, highlights the presence of sampling gaps within this protected area

    Brazilian protected areas are essential for plant conservation in the Atlantic Forest domain, one of the 36 global biodiversity hotspots. A major challenge for improving conservation actions is to know the plant richness, protected by these areas. Online databases offer an accessible way to build plant species lists and to provide relevant information about biodiversity. A list of land plants of “Parque Nacional do Caparaó” (PNC) was previously built using online databases and published on the website "Catálogo de Plantas das Unidades de Conservação do Brasil." Here, we provide and discuss additional information about plant species richness, endemism and conservation in the PNC that could not be included in the List. We documented 1,791 species of land plants as occurring in PNC, of which 63 are cited as threatened (CR, EN or VU) by the Brazilian National Red List, seven as data deficient (DD) and five as priorities for conservation.     Repercussão da terapia de suplementação de proteína em detrimento das alterações na composição muscular de idosos: uma revisão: Repercussion of protein supplementation therapy to the detriment of changes in muscle composition in the elderly: a review

    INTRODUÇÃO: O ato de envelhecer traz consigo inúmeras mudanças fisiológicas, dentre elas, destaca-se a sarcopenia, que por vezes pode levar a perda da capacidade funcional, podendo prejudicar a mobilidade e por fim acarretar em acidentes graves ou mortes. A atual concentração diária recomendada de proteína por quilograma não foi projetada para uma população em fase de envelhecimento, o que pode levar a uma concentração de proteína insuficiente. A suplementação proteica surgiu como forma alternativa de preservar a manutenção muscular. OBJETIVO: Analisar os efeitos da suplementação proteica na manutenção da capacidade funcional muscular na população idosa. METODOLOGIA: Para tanto, foi realizada uma revisão integrativa da literatura de aspecto qualitativo, no qual, a partir de uma pesquisa em bases de dados selecionadas, baseou-se em estudos que apresentaram efeitos da suplementação proteica na manutenção da capacidade funcional muscular de idosos. Ao final foram selecionados seis estudos que contemplavam o tema em questão. RESULTADOS E DISCUSSÃO: A melhora do estado nutricional está relacionada ao desempenho muscular, com base nisso, foram analisados nos estudos os seguintes aspectos: ganho de massa muscular, exercício físico em jejum, membros inferiores, velocidade da marcha e outros parâmetros funcionais e 25-hidroxivitamina D, todos colocando-se em comparação com a suplementação proteica como forma intervencionista e de manutenção da capacidade funcional muscular. Ainda, foi realizada uma análise da suplementação dietética com aminoácidos de cadeia ramificada (BCAAs) em relação a desnutrição. CONCLUSÃO: A análise dos dados supracitados revelou a relevância da busca pela melhoria na qualidade de vida e bem-estar da população senil, de maneira que o consumo diário recomendado de proteína seja preconizado como principal medida para manutenção da massa muscular nesta parcela populacional. Deve-se estimular o consumo diário de alimentos ricos em proteínas, tais como carnes, ovos, leite e derivados e suplementos alimentares, quando sua prescrição se faz necessária. Diante da corroboração da sarcopenia no aumento da incidência de quedas em idosos, se faz necessário orientar e estimular a população senil para a prática regular de exercício físico resistido, além do acompanhamento de equipe multidisciplinar

    BACKGROUND Leprosy or hansen’s disease is a spectral disease whose clinical forms mostly depends on host’s immune and genetic factors. Different Toll-like receptors (TLR) variants have been described associated with leprosy, but with some lack of replication across different populations. OBJECTIVES To evaluate the role of polymorphisms in genes TLR1, TLR2 and TLR4 and susceptibility to leprosy in a genetic case control study; to verify the association between genotypes of these markers and the immunological profile in the serum of patients with leprosy. METHODS Pre-designed TaqMan® assays were used to genotype markers at TLR1 (rs4833095, rs5743551), TLR2 (rs7656411, rs3804099) and TLR4 (rs1927914, rs1927911). A panel of cytokines and chemokines was accessed by enzime-linked immunosorbent assay (ELISA) test in the serum of a subgroup of patients with and without leprosy reactions. FINDINGS Our results show an association between the T allele of rs3804099 at the TLR2 gene and increased risk for leprosy per se [Odds ratio (OR) = 1.296, p = 0,022]. In addition, evaluating the association between different genotypes of the TLR1, 2 and 4 markers and cytokine/chemokine serological levels, IL-17 appears as an immunological marker regulated by the polymorphism of the three TLR genes evaluated, whereas different TLR1 genotypes were associated with differential production of IL-12p40 and MCP-1(CCL2). Furthermore, other relevant serum markers such as CXCL-10 and IL-6 seemed to be regulated by TLR2 variants and IL-1β was related to TLR4 genotypes. MAIN CONCLUSIONS All together our data points that the tested TLR markers may have a regulatory role in the immunity against Mycobacterium leprae, by driving the host’s production of key cytokines and chemokines involved in the pathogenesis of this disease

    Using online databases to produce comprehensive accounts of the vascular plants from the Brazilian protected areas: The Parque Nacional do Itatiaia as a case study

    Brazil is one of the most biodiverse countries in the world, with about 37,000 species of land plants. Part of this biodiversity is within protected areas. The development of online databases in the last years greatly improved the available biodiversity data. However, the existing databases do not provide information about the protected areas in which individual plant species occur. The lack of such information is a crucial gap for conservation actions. This study aimed to show how the information captured from online databases, cleaned by a protocol and verified by taxonomists allowed us to obtain a comprehensive list of the vascular plant species from the "Parque Nacional do Itatiaia", the first national park founded in Brazil. All existing records in the online database JABOT (15,100 vouchers) were downloaded, resulting in 11,783 vouchers identified at the species level. Overall, we documented 2,316 species belonging to 176 families and 837 genera of vascular plants in the "Parque Nacional do Itatiaia". Considering the whole vascular flora, 2,238 species are native and 78 are non-native.The "Parque Nacional do Itatiaia" houses 13% of the angiosperm and 37% of the fern species known from the Brazilian Atlantic Forest. Amongst these species, 82 have been cited as threatened, following IUCN categories (CR, EN or VU), seven are data deficient (DD) and 15 have been classified as a conservation priority, because they are only known from a single specimen collected before 1969
