    Biogeography and conservation of terrestrial afrotropical birds

    Includes bibliography.This study aimed to describe patterns of distribution in terrestrial Afrotropical birds, to investigate the causes of these patterns, and examine how aspects of distributional patterns may be used to prioritize local regions for conservation attention. Presence-only data were gathered and digitized at one-degree square scale for 1686 terrestrial bird species that breed on or regularly visit sub-Saharan Africa as non-breeding migrants. Biogeographical analysis of the 1437 species that are globally restricted to sub-Saharan Africa (Afrotropical endemics) revealed a suite of geographical areas that have a homogenous and characteristic avifaunal composition, termed avifaunal zones. The approach used in this study ensured representativeness in the resultant biogeographical classification scheme, which was not biased towards avifaunas that are species rich or that contain many narrow endemics, and further included avifaunas that consisted of few, but taxonomically and ecologically distinct species (e.g. the Namib Province). Analysis of zonal boundaries exhibiting high levels of turnover, defined specifically as species replacement, were distinguished from zonal boundaries that are characterised by species richness gradients. For instance, the northern forest-savanna boundary between the Guineo-Congolian and Northern Savanna Subregions was shown to consist of a sharp ecotone between forest and savanna, whereas the boundary between the Northern Savanna and Northern Arid Subregions was shown to be dominated by species drop-outs. This shows that whereas the Northern Savanna Subregion represents a unique avifauna that is distinct from that of the Guineo-Congolian Subregion, the Northern Arid Subregion is merely a depauparate subset of the Northern Savanna avifauna. Patterns of species richness and narrow endemism where shown to differ between species groups that exhibit different life history characteristics (e.g. residents vs. migrants) and distributional characteristics Atrotropical endemics vs. nonendemics).     Where are the major gaps in the reserve network for Africa's mammals?

    The original publication is available from http://www.cambridge.org/The establishment of protected areas for wildlife conservation in Africa was motivated by a number of different reasons (including hunting, recreation and wildlife conservation). The current reserve network provides good coverage of the distributions of the 194 species of larger mammals (> 3 kg) and 51 species of threatened larger mammals. However, it is less effective in covering the distribution of all 197 of Africa's threatened mammal species, which includes >140 smaller bodied species ( <3 kg) often restricted to habitat patches. A fully comprehensive network of areas for the conservation of African mammals, especially those facing extinction, is not yet in place, and further reserves may be needed in the Horn of Africa (Somalia in particular), the Cameroon Highlands, parts of the eastern African coastal forests and Eastern Arc Mountains, and parts of the Albertine Rift Mountains. More and larger reserve areas are also required to adequately cover all the species of South Africa. Parts of these gaps are already covered by government forest reserves, and the importance of this reserve category for the conservation of African mammals, especially threatened species, needs to be better recognized. As many of the gaps in reserve coverage are in areas of high human population and good agricultural potential, conservation goals may be difficult to achieve unless we supplement traditional reserves with novel approaches to maintain natural habitats and wildlife outside reserves.Postprint versio

    Cognitive Interviewing during Pretesting of the Prefinal Afrikaans for the Western Cape Disabilities of the Arm, Shoulder and Hand Questionnaire following Translation and Cross-Cultural Adaptation

    When patient-reported measures are translated and cross-culturally adapted into any language, the process should conclude with cognitive interviewing during pretesting. This article reports on translation and cross-cultural adaptation of the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire into Afrikaans (for the Western Cape). This qualitative component of a clinical measurement, longitudinal study was aimed at the pretesting and cognitive interviewing of the prefinal Afrikaans (for the Western Cape) DASH questionnaire highlighting the iterative nature thereof. Twenty-two females and eight males with upper limb conditions were recruited to participate at public health care facilities in the Western Cape of South Africa. Cognitive interviews were conducted as a reparative approach with an iterative process through retrospective verbal probing during a debriefing session with 30 participants once they answered all 30 items of the translated DASH questionnaire. The sample included Afrikaans-speaking persons from low socioeconomic backgrounds, with low levels of education and employment (24 of 30 were unemployed). Pragmatic factors and measurement issues were addressed during the interviews. This study provides confirmation that both pragmatic factors and measurement issues need consideration in an iterative process as part of a reparative methodology towards improving patient-reported measures and ensuring strong content validity

    An analysis of the recent fire regimes in the Angolan catchment of the Okavango Delta, Central Africa

    Background This paper presents an analysis of fire regimes in the poorly studied Angolan catchment of the Okavango Delta in Botswana. We used MODIS data to examine the frequency and seasonality of fires over 20 years (from 2000 to 2020) in three dominant vegetation types (miombo woodlands, open woodlands and grasslands, and short closed to open bushlands), and in areas where people were present, and where they were absent. Results The median fire return intervals for both open woodlands and grasslands and short bushlands were relatively short (1.9 and 2.2 years respectively). In miombo woodlands, fires were less frequent (median return periods of 4.5 years). Human population density had no discernible effect on the fire return intervals, but about 14% of the miombo woodlands experienced no fires over 20 years. Ongoing shifting cultivation within miombo woodlands has led to structural changes and the introduction of fire into this vegetation type where fires were rare or absent in the past. About 12% of the miombo did not burn during the period examined where people were present, whereas close to 20% of the sites remained unburnt where people were absent. This suggests that people did not change the fire return interval in any of the vegetation types studied, but that they altered the amount of the landscape that is flammable in miombo vegetation. Fires occurred between June and September, with a peak in the late dry season (August and September). Conclusions Historical research indicates that late dry-season fires are detrimental to miombo woodlands, and our analysis suggests that degradation in parts of the catchment has led to the introduction of fire to this previously fire-free and fire-sensitive vegetation type. Deforestation of miombo woodlands, and the consequent introduction of fire, is a cause for concern with respect to the ecological stability of the Okavango Delta. Managers should therefore aim to protect the remaining closed-canopy miombo stands from further clearing and to attempt to shift the timing of burns to the early dry season to reduce their intensity.Antecedentes Este trabajo presenta un análisis de los regímenes de fuegos en la pobremente estudiada cuenca del delta del Río Okavango en Bostwana. Usamos datos de MODIS para examinar la frecuencia y estacionalidad de los incendios durante 20 años (desde 2000 hasta 2020) en tres tipos vegetales dominantes (bosques de miombo, bosques abiertos de sabanas y pastizales, y arbustales bajos cerrados y abiertos), y en áreas donde había gente presente y donde ahora está ausente. Resultados La mediana del intervalo de retorno del fuego tanto para las sabanas, los pastizales y los arbustales bajos fue relativamente baja (1,9 y 2,2 años, respectivamente). En los bosques de miombo, los incendios fueron menos frecuentes (la mediana del intervalo de fuego fue de 4,5 años). La densidad de la población humana no tuvo un efecto discernible en los intervalos de retorno del fuego, aunque alrededor del 14% de los bosques de miombo no experimentaron incendios en los últimos 20 años. Las modificaciones en los cultivos dentro de los bosques de miombo llevó a cambios estructurales, y la introducción del fuego en este tipo de vegetación estuvo ausente o fue rara en el pasado. En las áreas donde la gente estuvo presente, alrededor del 12% del miombo no se quemó durante el período examinado, mientras que cerca del 20% de los sitios permanecieron sin quemarse en los lugares donde la gente estuvo ausente.     Occupational therapy hand assessment practices: Cause for concern?

    Introduction: Assessment is critical for measuring improvement, or lack thereof, and demonstrating the outcome of intervention. In response to the lack of research in this area, this study aimed to determine the assessment practices of occupational therapists working with clients with hand conditions. Methods: A quantitative cross sectional survey design was used. A convenience sample of occupational therapists was recruited from five provinces. Respondents completed a questionnaire developed for the study that comprised demographic information, assessments used, frequency of use and factors influencing assessment choice. Data were analysed with Statistica version 11. Results: Eighty-one respondents (n=114) completed questionnaires representing a 71% response rate. Goniometry (84.0%), manual muscle testing (76.5%) and testing for flexor tendon function (76.3%) were used most frequently. The most common reasons for not using assessments were lack of availability and unfamiliarity. Conclusion: It is of concern that the assessment practices of participants in this study focussed primarily on the assessment of body function and structure with few therapists using activity and participation measures. This could seriously limit the evidence needed to verify the outcomes achieved through occupational therapy intervention in the treatment of hand conditions

    Role of public and private funding in the rising caesarean section rate:a cohort study

    Objective: The caesarean section rates have been rising in the developed world for over two decades. This study assessed the involvement of the public and private health sectors in this increase. Design: Population-based, retrospective cohort study. Setting: Public and private hospitals in Western Australia. Participants: Included in this study were 155 646 births to nulliparous women during 1996-2008. Main outcome measures: Caesarean section rates were calculated separately for four patient type groups defined according to mothers' funding source at the time of birth (public/private) and type of delivery hospital ( public/private). The average annual per cent change (AAPC) for the caesarean section rates was calculated using joinpoint regression. Results: Overall, there were 45 903 caesarean sections performed (29%) during the study period, 24 803 in-labour and 21 100 prelabour. Until 2005, the rate of caesarean deliveries increased most rapidly on average annually for private patients delivering in private hospitals (AAPC=6.5%) compared with public patients in public hospitals (AAPC=4.3%, p&lt;0.0001). This increase could mostly be attributed to an increase in prelabour caesarean deliveries for this group of women and could not be explained by an increase in breech deliveries, placenta praevia or multiple pregnancies. Conclusions: Our results indicate that an increase in the prelabour caesarean delivery rate for private patients in private hospitals has been driving the increase in the caesarean section rate for nulliparous women since 1996. Future research with more detailed information on indication for the prelabour caesarean section is needed to understand the reasons for these findings

    Neonatal complications in public and private patients:a retrospective cohort study

    OBJECTIVE: To use propensity score methods to create similar groups of women delivering in public and private hospitals and determine any differences in mode of delivery and neonatal outcomes between the matched groups. DESIGN: Population-based, retrospective cohort study. SETTING: Public and private hospitals in Western Australia. PARTICIPANTS: Included were 93 802 public and 66 479 private singleton, term deliveries during 1998-2008, from which 32 757 public patients were matched with 32 757 private patients on the propensity score of maternal characteristics. MAIN OUTCOME MEASURES: Neonatal outcomes were compared in the propensity score-matched cohorts using conditional logistic regression, adjusted for antenatal risk factors and mode of delivery. Outcomes included Apgar score <7 at 5 min, neonatal resuscitation (endotracheal intubation or external cardiac massage) and admission to a neonatal special care unit. RESULTS: No significant differences in maternal characteristics were found between the propensity score-matched groups. Private patients were more likely than their matched public counterparts to undergo prelabour caesarean section (25.2% vs 18%, p<0.0001). Public patients had lower rates of neonatal unit admission (AOR 0.67, 95% CI 0.62 to 0.73) and neonatal resuscitation (AOR 0.73, 95% CI 0.56 to 0.95), but higher rates of low Apgar scores at 5 min (AOR 1.31, 95% CI 1.06 to 1.63) despite adjustment for antenatal factors. Additional adjustment for mode of delivery reduced the resuscitation risk (AOR 0.86, 95% CI  0.63 to 1.18) but did not significantly alter the other estimates. CONCLUSIONS: Propensity score methods can be used to generate comparable groups of public and private patients. Despite the rates of low Apgar scores being higher in public patients, the rates of special care admission were lower. Whether these findings stem from differences in paediatric services or clinical factors is yet to be determined
