Na oportunidade mantivemos um diálogo com Pedro Demo, o que significa conversar com uma fonte que tem trilhado caminhos próprios e que se revelam ricos para o entendimento da realidade brasileira. A sua atuação como sociólogo é abrangente, tendo sempre a preocupação de trazer à discussão temáticas importantes para diversas áreas do conhecimento. DA MUNICIPALIZAÇÃO INDUZIDA À ESTADUALIZAÇÃO DO ENSINO MÉDIO

In order to reach a quality school education, the Brazilian State chose a competitive cooperative federalism (CURY, 2010). Therefore, the State created, in the 90’s, the National Education System, to articulate the actions of each department. During this period, facing pressure of international organisms, national and local, under the legal permission, the following premise arouse: The best place for a child is at school. However, the Ceará State Government, within the period of 1995 and 2006, didn’t create more spaces to attend the demand for high school students, accomplishing only the replacement of the primary education service to the cities, without intern institutional planning. In this sense, this work aims to analyze the regime of cooperation among Federal Government-State-City, in promoting the constitutional right of basic education in Fortaleza City (1995 until 2006). Specifically, we intend to comprehend the concepts and models of State, federation and decentralization and to analyze the guidelines and dedication of policies on municipal basic education charge and State high school charge. We adopted, in this analysis, the exploratory research, quantitative and qualitative character, with a bibliographic, documental and empiric emphasis. The literary review discusses about concepts of decentralization, educational system and regime of cooperation (SAVIANI, 2009; CURY, 2010, ARRETCHE, 1999 e DUARTE, 2003). Concurring, we did documental analyses on plans, regulations, reports and government messages and eight interviews, half-structured, with managers of the system and the school. Understanding the Federative State as a system of independent states, but all united under a sovereign government, we pondered that the acceptance of the cooperation among its members mustn’t take away the coordinative function of the Federal Government on national educational policies. However, the managerial model, proposed by the State reformation, seeks for rationalization of expenses, with centralized measures, as the municipal charge, without considering the technical and financial capability, as well as the conditions of permanence and the quality of teaching offered by the cities. When comparing the progress of enrolment in basic and high schools, in Fortaleza City, within 1997 and 2005, we noticed a mere relocation of enrolments. We also noticed the lack of specific regulation of the National Educational System, at the same time that punctual rules, like the Law 12.452/95, which regulates  the municipal charge, and the accord firmed within the State and 124 townships, in 1997, to empower the regime of cooperation. This lack is justified by both misunderstanding about the wide concept of systems, within the complete concept of Federative State, and by the organizational tradition of the Portuguese State, which formation occurred based on the families and the people, mainly particularistic, communal, municipal, infused and convicted of the spirit of fraction (RAMOS, 2009), it means, a formation that is contrary to the idea of a national unit. From this point, the economic, cultural, technological and fiscal gaps among states and cities were accentuated and influenced negatively on the permanence in the quality of education in the city of Fortaleza.Para lograr una educación de calidad, el estado brasileño ha optado por un federalismo cooperativo concurrente (CURY, 2010). Así, se determina, en la década del 90, la creación del sistema nacional de educación, para articular las acciones de cada entidad. Durante este período, frente a la presión de los organismos internacionales, nacionales y locales, bajo el marco legal, la premisa de Coloque el niño está en la escuela. Sin embargo, el gobierno del estado de Ceará, en el período comprendido entre 1995-2006, no ha creado nuevos puestos de trabajo para satisfacer la demanda de la high School secundaria, realizar sólo la reasignación de asistencia a la escuela primaria a los municipios sin planeamiento interinstitucional. En este sentido, este trabajo pretende analizar el sistema de colaboración entre Unión-estado-provincia en promover el derecho constitucional a la educación básica en Fortaleza (1995-2006). Concretamente, se pretende entender los conceptos y modelos de estado, Federación y descentralización y analizar las pautas y municipalización de evolución política de la escuela primaria y media superior persuasión. Adoptó, en este análisis, investigación exploratoria, cuantitativa y cualitativa en la naturaleza, de tipo documental y literatura empírica. La revisión de la literatura discute los conceptos de descentralización, educación y acuerdos de colaboración (SAVIANI, 2009; CURY, 2010, ARRETCHE, 1999 y DUARTE, 2003). Análisis concomitante, documental de planes, normas, programas, informes y mensajes de gobierno y ocho entrevistas semi estructuradas, con los administradores del sistema escolar. Entendiendo el Estado Federal como Estados independientes, pero Unidos bajo un soberano del gobierno central, ha aparecido el reconocimiento que la colaboración entre sus seres queridos no debe quitar el papel del Coordinador sindical en la política educativa nacional. Sin embargo, el modelo propuesto por la reforma del estado administrador, busca la racionalización del gasto, con medidas descentralizadoras, tales como la descentralización, sin tener en cuenta la capacidad técnica y financiera, así como las condiciones de estancia y la calidad de la educación ofrecida por esta instancia. Al comparar la evolución de la matrícula en educación primaria y secundaria, en Fortaleza, entre 1997 y 2005, un mero cambio de las inscripciones. Tenga en cuenta la ausencia de reglamentos específicos del sistema nacional de educación, mientras que el registro de las regulaciones individuales, como la ley 95/normatizadora, 12.452 de descentralización y el acuerdo celebrado entre el estado y 124 municipios, en 1997, para fortalecer el régimen de colaboración. Ausencia que es justificado tanto por falta de comprensión del amplio concepto de sistemas, dentro del concepto completo del Estado Federal, como por la tradición, organización del estado portugués cuya formación se produjo desde las familias y el pueblo, eminentemente particularista, comunal, municipal, impregnados y convencido el espíritu de la fracción (RAMOS, 2009), es decir, una formación contra la idea de unidad nacional. Por lo tanto, las disparidades económicas, culturales, tecnológicas y fiscales mide entre acentos de Estados y municipios y de influencia negativamente la permanencia y la calidad de la educación en la ciudad de Fortaleza. Para alcançar uma educação escolar de qualidade, o Estado Brasileiro optou por um federalismo concorrente cooperativo (CURY, 2010). Assim, determina, na década de 90, a criação do Sistema Nacional de Educação, para articular as ações de cada ente. Nesse período, diante da pressão dos organismos internacionais, nacionais e locais, sob o marco legal, surge a premissa Lugar de criança é na escola. Entretanto, o Governo do Estado do Ceará, no período entre 1995-2006, não criou novas vagas para o atendimento da demanda do ensino médio, realizando apenas o remanejamento do atendimento do ensino fundamental para os municípios, sem planejamento interinstitucional. Nesse sentido, este trabalho objetiva analisar o regime de colaboração entre União-Estado-Município na promoção do direito constitucional à educação básica em Fortaleza (1995 a 2006). Especificamente, pretende-se compreender os conceitos e modelos de Estado, federação e descentralização e analisar as diretrizes e desdobramentos da política de municipalização do ensino fundamental e estadualização do médio. Adotou-se, nesta análise, a pesquisa exploratória, de natureza quanti-qualitativa, do tipo bibliográfica, documental e empírica. A revisão de literatura trata dos conceitos de descentralização, sistema de educação e regime de colaboração (SAVIANI, 2009; CURY, 2010, ARRETCHE, 1999 e DUARTE, 2003). Concomitante, realizou-se análise documental de planos, normas, programas, relatórios e mensagens governamentais e oito entrevistas, semiestruturadas, com gestores do sistema e da escola. Entendendo o Estado Federativo como sistema de estados independentes, mas unidos sob um governo central soberano, ponderou-se o reconhecimento de que a colaboração entre seus entes não deve retirar a função coordenadora da União na política educacional nacional. Todavia, o modelo gestor, proposto pela reforma estatal, busca a racionalização dos gastos, com medidas descentralizadoras, como a municipalização, sem considerar a capacidade técnica e financeira, bem como as condições de permanência e a qualidade do ensino oferecida por essa instância. Ao se comparar a evolução da matrícula no ensino fundamental e médio, em Fortaleza, entre 1997 e 2005, percebe-se um mero remanejamento de matrículas. Nota-se a ausência de regulamentação específica do Sistema Nacional de Educação, ao mesmo tempo em que se registram regulamentos pontuais, como a Lei 12.452/95, normatizadora da municipalização, e o Convênio celebrado entre o Estado e 124 municípios, em 1997, para fortalecer o regime de colaboração. Ausência que se justifica tanto pela incompreensão do conceito amplo de sistemas, dentro do conceito pleno de Estado Federativo, como pela tradição organizacional do Estado português, cuja formação ocorreu a partir das famílias e do povo, eminentemente particularista, comunal, municipalista, impregnado e convicto do espírito de fração (RAMOS, 2009), ou seja, uma formação contrária à ideia de unidade nacional. Daí, as disparidades econômicas, culturais, tecnológicas e fiscais entre os estados e municípios acentuarem-se e influenciarem negativamente a permanência e a qualidade do ensino no município de Fortaleza.&nbsp

Fraturas maxilofaciais associadas ao esporte: revisão integrativa

O trauma maxilofacial é uma condição desafiadora aos profissionais de saúde, que pode ser representado por injúrias e fraturas aos ossos da face, sendo considerado uma das agressões mais devastadoras dos centros de trauma devido as consequências emocionais e às possibilidades de deformidades dentofaciais aos pacientes. O objetivo deste estudo foi realizar uma revisão da literatura sobre traumas maxilofaciais associados ao esporte, bem como, destacar os sítios anatômicos mais acometidos e quais os esportes possuem maior risco aos traumas, destacando seus meios de prevenção e proteção. Para realizar o levantamento bibliográfico foi realizada a pesquisa na Biblioteca Virtual em Saúde, em que foram inclusos artigos em inglês, português e espanhol, publicados nos anos de 2016 a 2023, que abordaram a incidência de traumas maxilofaciais em atletas, bem como, as regiões anatômicas de maior envolvimento, o tipo de esporte e o uso ou não de protetores faciais e bucais. A partir destes dados, foram selecionados 25 artigos. Os resultados, de acordo com a revisão, apontaram o futebol como o principal esporte causador das fraturas faciais e o sítio anatômico mais acometido é o terço médio da face, além de haver grande predominância para o gênero masculino. Conclui-se que medidas são necessárias para atenuar a problemática, destacando a importância dos equipamentos de prevenção e proteção durante a prática esportiva

Is chlamydia screening and testing in Britain reaching young adults at risk of infection? Findings from the third National Survey of Sexual Attitudes and Lifestyles (Natsal-3).

BACKGROUND: In the context of widespread opportunistic chlamydia screening among young adults, we aimed to quantify chlamydia testing and diagnosis among 16-24 year olds in Britain in relation to risk factors for prevalent chlamydia infection. METHODS: Using data from sexually experienced (≥1 lifetime sexual partner) 16-year-old to 24-year-old participants in Britain's third National Survey of Sexual Attitudes and Lifestyles (conducted 2010-2012), we explored socio-demographic and behavioural factors associated with prevalent chlamydia infection (detected in urine; n=1832), self-reported testing and self-reported diagnosis in the last year (both n=3115). RESULTS: Chlamydia prevalence was 3.1% (95% CI 2.2% to 4.3%) in women and 2.3% (1.5% to 3.4%) in men. A total of 12.3% of women and 5.3% men had a previous chlamydia diagnosis. Factors associated with prevalent infection were also associated with testing and diagnosis (eg, increasing numbers of sexual partners), with some exceptions. For example, chlamydia prevalence was higher in women living in more deprived areas, whereas testing was not. In men, prevalence was higher in 20-24 than 16-19 year olds but testing was lower. Thirty per cent of women and 53.7% of men with ≥2 new sexual partners in the last year had not recently tested. CONCLUSIONS: In 2010-2012 in Britain, the proportion of young adults reporting chlamydia testing was generally higher in those reporting factors associated with chlamydia. However, many of those with risk factors had not been recently tested, leaving potential for undiagnosed infections. Greater screening and prevention efforts among individuals in deprived areas and those reporting risk factors for chlamydia may reduce undiagnosed prevalence and transmission

Comparación de los métodos de gota gruesa y tiras de diagnóstico rápido para el diagnóstico de la malaria en Luanda, Angola

Introducción: a pesar de que se ha logrado reducir la morbilidad y la mortalidad de la malaria en África, esta constituye aún un importante problema de salud en Angola, y su capital, Luanda. Objetivo: comparar la ejecución de la microscopía contra un método de tiras de diagnóstico rápido para el diagnóstico de la malaria. Material y Métodos: se llevó a cabo un estudio de corte transversal para evaluar la precisión de una prueba rápida para el diagnóstico del paludismo. Entre diciembre de 2013 y marzo  de 2014; se seleccionaron aleatoriamente 1 000 muestras de sangre de pacientes quienes acudieron a los laboratorios por diagnóstico sospechoso de malaria, las que fueron analizadas por gota gruesa, y una prueba de diagnóstico rápido (SD Bioline Malaria AntigenPf/Pv®). Se evaluó la concordancia, la validez y la seguridad de ambos métodos, y para ello las gotas gruesas, fueron consideradas como la "prueba de oro". Resultados: se obtuvo una mayor frecuencia de positividad con las TDR que con las gotas gruesas en dos de los centros de salud (p&lt;0,05).  Al comparar los valores de concordancia, validez y seguridad entre los métodos empleados para el diagnóstico de la malaria se obtuvieron valores de concordancia superiores con la gota gruesa que con las TDR. De la misma forma, la especificidad y los valores predictivos de los positivos también fueron superiores con las gotas gruesas (p&lt;0,01). Conclusiones: este estudio permite considerar esta técnica de diagnóstico rápido como un método adecuado y sensible para diagnosticar infecciones por P. falciparum en Luanda. Palabras clave: tiras de diagnóstico rápido, microscopía, gota gruesa, centros de salud de Angola.ABSTRACTIntroduction: despite of National programs for control have reduced the morbidity and mortality because of malaria in Africa, this parasitic disease is still an important public health problem in the continent, in the Republic of Angola, and its capital Luanda. Objective: to compare the skills of technicians on malaria microscopy and to compare the microscopy against a rapid diagnostic test (RTD) in health centers from Luanda. Material and Methods: among December 2013 y march 2014, one thousand samples were randomly selected from patients visiting laboratories because of suspected diagnosis of malaria. These samples were observed by 20 technicians in 10 health centers from Luanda. A blood sample was taken by every person in order to be examined by thick smear (TS) and a RTD (SD Bioline Malaria Antigen Pf/Pv®). The concordance, the validity, and safety of both methods were evaluated, and the gold standard was defined by thick smears observed by four malaria expert technicians. Results: the frequency of positivity obtained with RTD was higher than with TSin health centers from Kassequel, Samba, and Km 12 (p&lt;0.05). The comparison of concordance, validity, and safety between used methods for the diagnosis of malaria showed concordance values with TS higher than RTD. The specificity and predictive values for positives were upper with TS than RTD (p&lt;0,01). Conclusions: this study showed the RTDBioline Malaria Ag f/Pv®, is an adequate and sensible method for the diagnosis of P. falciparum infections in Luanda.Key words: rapid diagnostic tests, microscopy, Angola health centers, thick smear. Intervención Educativa para mejorar la calidad del Diagnóstico de la Malaria en la red de Laboratorios de Luanda, República de Angola

Introducción: La malaria continúa siendo uno de los más importantes problemas de salud  pública a nivel mundial que afecta con énfasis al continente africano y a Angola. Objetivos: Evaluar el nivel de conocimientos teóricos y prácticos en el diagnóstico de la malaria de un grupo de técnicos en Luanda antes y después de una intervención educativa. Material y Métodos: Entre abril y julio de 2013 se realizó un estudio experimental con grupo de control en 120 técnicos de la red de salud de Luanda; se hicieron dos evaluaciones teóricas y prácticas: una, al comienzo y otra, 3 meses después de una intervención educativa. Se incluyó un grupo adicional de control de 120 técnicos que fueron evaluados al mismo tiempo y en quienes no se hizo intervención. Los entrenamientos fueron de 10  días cumplimentando un programa de clases prácticas y teóricas. Resultados: Después de los entrenamientos en el grupo de Intervención, las notas ascendieron tras la segunda evaluación; mientras que en el grupo de control las calificaciones se mantuvieron sin cambios. Se observó que en el grupo de Intervención la probabilidad de estar desaprobado fue 129 veces mayor en la evaluación inicial para las pruebas teóricas y 108 veces mayor en los exámenes prácticos. Finalmente fueron identificados los principales aspectos en que fallaron los técnicos evaluados en los grupos de intervención y control. Conclusiones: Se demostró una mejoría en los conocimientos teóricos y prácticos de los técnicos de laboratorio dedicados al diagnóstico de la malaria después de la intervención educativa.Palabras clave: Malaria, conocimientos teóricos, evaluación práctica, diagnóstico microscópico.ABSTRACTIntroduction: The malaria still is one of the most important public health problems worldwide, specially the Africa as well as in Angola. Avaliação de revestimento contendo extrato de mostarda e da sanitização na conservação de batata-baroa

Batata-baroa (Arracacha xanthorriza Bancroft) é um produto de alta perecibilidade, devido à perda de massa fresca por transpiração e a deterioração causada por microrganismos. Este trabalho teve por objetivo avaliar o efeito de um revestimento com extrato de mostarda e da sanitização sobre o desenvolvimento de microrganismos em batata- baroa durante o armazenamento em temperatura ambiente. A bactéria fitopatogênica Erwinia carotovora, causadora da podridão-mole, foi inoculada sobre a superfície da batata-baroa, em alguns tratamentos, para acelerar a sua deterioração. Os tratamentos foram: sanitização; sanitização e inoculação; revestimento com extrato de mostarda; revestimento sem extrato de mostarda; revestimento, inoculação e com extrato de mostarda; revestimento, inoculação e sem extrato de mostarda; e sem tratamento. Foram avaliados o desenvolvimento de fungos filamentosos e leveduras e mesófilos aeróbicos; perda de peso; e número de lesões aos 0, 3 e 6 dias de armazenamento. As contagens de fungos filamentosos e leveduras e mesófilos aumentaram durante o armazenamento na ordem de 1,5 e 5,5 ciclos logarítmicos, respectivamente. A sanitização na concentração usada não foi suficiente para inibir o crescimento dos fungos filamentosos e leveduras. A baixa eficiência do extrato de mostarda pode estar relacionada com sua volatilidade. A perda de peso máxima para todos o tratamentos foi de 4,5%. No terceiro dia de armazenamento, o tratamento com sanitização e inoculação apresentou menor número de lesões, enquanto as batatas-baroas submetidas ao revestimento com extrato, inoculadas, e aquelas somente sanitizadas apresentaram elevado número de lesões em relação às demais. O revestimento proporcionou brilho e não interferiu na cor amarela do produto.Peruvian carrot (Arracacha xanthorriza Bancroft) is a highly perishable produce because of the fresh mass loss and deterioration by microorganisms. This work aimed to evaluate the effects of a coating containing mustard extract and the sanitation process on microorganism growth in Peruvian carrots during storage under ambient conditions ( 25 ± 2oC, 74 ± 2%UR). Erwinia carotovora was inoculated on Peruvian carrot surface, in some treatments, to accelerate the produce deterioration. Seven treatments were carried out: sanitation; sanitation and inoculation; coating with mustard extract; coating without mustard extract; inoculation and coating with mustard extract; inoculation and oating without mustard extract, and without treatment (control). Mold and yeast and aerobic mesophilic, mass loss and number of lesions at zero, three and six days of storage. Mold and yeast and mesophilic aerobic counting increased during storage period by 1.5 and 5.5 log-cycles, respectively. Sanitation with 200 ppm chlorine was not efficient to inhibit mold and yeast and mesophilic aerobic. The low efficiency of mustard extract may be related to its volatility. Maximum mass loss was 4.5% for all treatments. Smaller lesion number was found for the inoculated and sanitized treatment, while the Peruvian carrots inoculated and coated with mustard extract and those sanitized showed larger lesion number compared with the other treatments. The coating provided the produce with shining appearance and did not interfere with the yellow color

A Educação em Saúde na área da Parasitologia: a experiência dos projetos de extensão LiPar e Educac na “Semana de meninas e mulheres na Ciência” ocorrida na Universidade do Estado do Rio de Janeiro

As atividades de extensão universitária têm ganhado cada vez mais espaço, por mostrarem-se importantes instrumentos de construção do pensamento científico, colaborando para a incorporação de novos saberes e práticas para a sociedade e para os extensionistas. Os espaços não formais de ensino, por sua vez, revelam-se ótimos meios para tornar a construção desses saberes possível. Logo, através da associação das atividades de extensão em espaços não formais de ensino, é possível promover as temáticas de educação em saúde em parasitologia, buscando conscientizar e prevenir, de modo criativo e ativo, as parasitoses que ainda são um grande problema de saúde pública. Desta forma, esse trabalho se propôs relatar a experiência dos projetos de extensão da Universidade do Estado do Rio de Janeiro, LiPar (Liga de Parasitologia) e Educac, no evento “Semana de Meninas e Mulheres na Ciência”, organizado por mulheres de diferentes áreas das ciências e tecnologias. Assim, analisou-se de que forma o espaço não formal de ensino pode ajudar na prática educativa das atividades propostas pelas extensões e de que maneira esses projetos levam o conhecimento de educação em saúde em parasitologia nesses espaços. Além disso, por meio da técnica da Nuvem de Palavras, buscou-se identificar as principais dúvidas sobre os assuntos de parasitologia abordados e também as pré-concepções sobre o tema. Portanto, concluiu-seque o espaço não formal institucionalizado pode ser um grande aliado para o ensino de educação em saúde em parasitologia, pois permite utilizar diferentes metodologias que colaboram para a compreensão da teoria com a prática

O impacto da ultrassonografia no atendimento multidisciplinar na UTI

Introduction: The introduction of ultrasound in the Intensive Care Unit (ICU) represents a significant milestone, fostering substantial advancements in multidisciplinary practice and positively influencing the management of various critical conditions. The application of pulmonary ultrasound, in particular, emerges as a versatile and essential tool, directly impacting the quality of care provided. Methodology: A bibliographic search was conducted on the PUBMED platform, utilizing specific Mesh descriptors: Ultrasonography, Intensive Care Units, Patient Care Team. The search period spanned from 2013 to 2021, encompassing a total of 33 articles. Results: The application of ultrasound in the ICU has proven to be an extremely relevant tool for multidisciplinary care. Pulmonary ultrasound, when employed by intensivists, not only provides a precise assessment of the mentioned conditions but also plays a fundamental role in rapid decision-making and treatment optimization. Conclusion: Ultrasound emerges as a transformative tool in the ICU, shaping multidisciplinary practice and ushering in an era of more advanced and patient-centered intensive care. O período de busca abrangeu os anos de 2013 a 2021, totalizando 33 artigos. Resultado: Aplicação da ultrassonografia na UTI tem se mostrado uma ferramenta de extrema relevância para o atendimento multidisciplinar.
