    The role of midwives and nurse-midwives in promoting safe motherhood in Brazil

    O Brasil necessita melhorar a assistência à saúde materna e perinatal, haja vista os índices elevados de morbimortalidade e de cesáreas, o processo de medicalização do corpo feminino e a violação dos direitos sexuais e reprodutivos das mulheres durante o ciclo gravídico-puerperal. A partir da contribuição de pesquisadores e organismos internacionais dedicados ao estudo destas questões, este texto tem como finalidade oferecer subsídios teóricos que sustentem a proposta de que a promoção da maternidade segura requer a efetiva participação de obstetrizes e enfermeiras obstetras, profissionais que, a partir da perspectiva da promoção da saúde, podem colaborar na constituição de uma rede efetiva de cuidados, cuja premissa básica seja a atenção humanizada e baseada em evidências científicas a fim de melhorar a experiência da mulher e da família no processo de gestação, parto e pós-partoBrazil needs to improve maternal and perinatal health care, given the high rates of morbidity, mortality and cesarean sections, the medicalization process of the female body and the violation of the sexual and reproductive rights of women during pregnancy and childbirth.     This contribution, through the presentation of the results of the assessment process of the project “Non Da Soli-La Scuola Di Fronte All’emergenza: La Didattica A Distanza” promoted by Save the Children and supervised by the University of Milano-Bicocca, aims to outline the training process in order to define the educational role of the tutor, here defined as a figure who offers professional accompaniment to in-service teachers. This project was intended to provide through a tutoring program professional support to about 850 teachers from 70 schools distributed across 13 regions. Tutoring was provided remotely. The aim of the research is to describe the main features of the tutoring process taking into consideration the point of view of the participants -- teachers and tutors -- regarding the training methodologies adopted, educational functions enabled by tutors and the learning outcomes perceived by participants. Data was collected in July and December 2020 by way of questionnaires completed by the teachers and tutors. Descriptive uni-variate analysis and reflexive thematic analysis was applied to the data. Results indicate three main aspects of the tutoring process distributed across the following areas: a) the value of contextualizing and person-alizing the training program to meet the educational needs of the teacher; b) the value of collaboration between tutors and teachers and among teachers themselves; c) the challenge teachers faced in reconciling theory with practice. The discussion of the results draws attention to a need for continued exploration of tutoring as a training methodology for professional accompaniment of in-service teachers.Il contributo si propone di caratterizzare il tutoraggio come dispositivo formativo volto all’accompagna-mento professionale dei docenti in servizio mediante la presentazione dei risultati del processo di valuta-zione del progetto “Non Da Soli-La Scuola Di Fronte All’emergenza: La Didattica A Distanza”, realizzato da Save The Children con la supervisione scientifica dell’Università degli Studi Di Milano-Bicocca. Tale progetto ha inteso fornire un supporto professionale a distanza a circa 850 insegnanti di 70 scuole distribuite in 13 regioni attraverso percorsi di tutoraggio realizzati mediante piattaforme digitali di videoconferenza. Obiet-tivo della ricerca è delineare le caratteristiche principali del dispositivo formativo di tutoraggio attraverso la considerazione del punto di vista dei soggetti coinvolti – docenti e tutor – sulle metodologie formative adot-tate, sulle funzioni formative messe in atto e sugli apprendimenti percepiti dai docenti. I dati sono stati rac-colti a Luglio 2020 e Dicembre 2020 mediante la somministrazione di un questionario a insegnanti e tutor-formatori. Su tali dati è stata condotta un’analisi descrittiva di tipo univariato e un’analisi tematica ri-flessiva. I risultati mostrano che gli elementi maggiormente caratterizzanti il percorso di tutoraggio riguar-dano tre ambiti principali: a) l’utilità della contestualizzazione dell’intervento di accompagnamento e della sua individualizzazione rispetto ai bisogni formativi dei docenti; b) il valore della collaborazione tra tutor e insegnanti e tra insegnanti stessi; c) le difficoltà dei tutor nel facilitare processi di ricorsività tra teoria e pra-tica. La discussione dei risultati evidenzia la necessità di proseguire la linea di indagine sul tutoraggio come forma di accompagnamento professionale dei docenti in servizio

    Breast Cancer Preoperative Staging: Does Contrast-Enhanced Magnetic Resonance Mammography Modify Surgery?

    Women with newly diagnosed breast cancer may have lesions undetected by conventional imaging. Recently contrast-enhanced magnetic resonance mammography (CE-MRM) showed higher sensitivity in breast lesions detection. The present analysis was aimed at evaluating the benefit of preoperative CE-MRM in the surgical planning. From 2005 to 2009, 525 consecutive women (25–75 years) with breast cancer, newly diagnosed by mammography, ultrasound, and needle-biopsy, underwent CE-MRM. The median invasive tumour size was 19 mm. In 144 patients, CE-MRM identified additional lesions. After secondlook, 119 patients underwent additional biopsy. CE-MRM altered surgery in 118 patients: 57 received double lumpectomy or wider excision (41 beneficial), 41 required mastectomy (40 beneficial), and 20 underwent contra lateral surgery (18 beneficial). The overall false-positive rate was 27.1% (39/144). CE-MRM contributed significantly to the management of breast cancer, suggesting more extensive disease in 144/525 (27.4%) patients and changing the surgical plan in 118/525 (22.5%) patients (99/525, 18.8% beneficial)

    Desempenho funcional de crianças com mielomeningocele

    This study aimed at analysing the impact of level of lesion, locomotion, and neurological changes associated to meningomyelocele clinical framework concerning functional performance. For this prospective cross-sectional study 40 children with meningomyelocele were assessed as to level of lesion, ambulatory ability, Arnold-Chiari malformation, hydrocephalus, hydromyelia, tethered cord, and sphincter incontinence. Functional performance was evaluated by the Pediatric Evaluation of Disability Inventory (PEDI) which quantitatively assesses functional performance and independence in daily activities in the areas of self-care, mobility, and social functioning. Collected data were statistically analysed and the significance level set at pEste trabalho visou analisar o impacto dos níveis de lesão, deambulação e alterações neurológicas associadas ao quadro clínico da mielomeningocele sobre o desempenho funcional de seus portadores. Neste estudo transversal prospectivo foram examinadas 40 crianças com mielomeningocele, avaliando-se nível de lesão, deambulação, malformação de Arnold-Chiari, hidrocefalia, hidromielia, medula ancorada e incontinência esfincteriana.     Lack of cytotoxic and genotoxic effects of Minthostachys verticillata essential oil: Studies in vitro and in vivo

    Minthostachys verticillata (peperina) is an aromatic and medicinal plant with several uses and ethnobotanical properties. Numerous studies have demonstrated that its essential oil (Mv-EO) presents antimicrobial capacity and shows immunomodulating and anti-allergic properties in human cell lines. Thus, the goal of this study was to investigate the main chemical composition, analyzed by GC?FID, and the cyto-genotoxic effects of Mv-EO, using Vero cells, human PBMCs and mice bone marrow cells. The Mv-EO was rich in pulegone 60.5% and menthone 18.2%. Our results clearly show that Mv-EO is not cyto-genotoxic in vitro nor in vivo. It not induced cytotoxic effects, as indicated by trypan blue dye exclusion and NRU assays both in Vero cells and human PBMCs. In addition, Mv-EO (100?1000 μg/mL) not induced apoptotic effects on human PBMCs, as indicated by Hoechst staining and DNA fragmentation analysis by agarose gel electrophoresis. The in vivo assay showed that Mv-EO (25?500 mg/kg) not increased the frequency of micronucleus in bone marrow cells of mice. Further, the ratio of polychromatic/normochromatic erythrocytes was not modified. These findings suggest that Mv-EO appears to be safe as a therapeutic agent.Fil: Escobar, Franco Matias.     Minthostachys verticillata essentials oil and its major components: antiherpetic selective action in HEp-2 cells

    The medicinal herb Minthostachys verticillata was studied in various fields of natural medicine. Numerous studies were made to evaluate its antimicrobial properties. This work emphasizes in the analysis of the antiviral capacity of its Essential Oil (EO). The aim of this study was to determine the Therapeutic Index (TI) of the EO and its main chemical components. Suid herpesvirus type 1 or Pseudorabies virus (Prv) was proliferated in HEp-2 cells in presence of non-toxic concentrations of the oily samples. Antiviral action was quantified by reducing the number of plaques of lysis. Previously, the cytotoxicity of the samples was evaluated and 50% Cytotoxic Concentration values (CC50) were determined. The TI was calculated by relating the cytotoxic concentrations of each sample vs. those concentrations active against the virus. The results showed that the major components of the EO were pulegone, menthone and limonene, with CC50 of 352, 835 and 1155 μg/mL respectively vs. the CC50 of EO: 613 μg/mL. The antiviral action was exerted by pulegone and the EO at concentrations 20 to 30 times lowers than the toxic concentrations.
