    Constant angular velocity of the wrist during the lifting of a sphere.

    The primary objective of the experiments was to investigate the wrist motion of a person while they were carrying out a prehensile task from a clinical hand function test. A sixcamera movement system was used to observe the wrist motion of 10 participants. A very light sphere and a heavy sphere were used in the experiments to study any mass effects. While seated at a table, a participant moved a sphere over a small obstacle using their dominant hand. The participants were observed to move their wrist at a constant angular velocity. This phenomenon has not been reported previously. Theoretically, the muscles of the wrist provide an impulse of force at the start of the rotation while the forearm maintains a constant vertical force on a sphere. Light–heavy mean differences for the velocities, absolute velocities, angles and times taken showed no significant differences (pÂŒ0.05)

    The impact of patients' chronic disease on family quality of life : an experience from 26 specialties

    BACKGROUND: Previous studies have assessed family quality of life in individual disease areas and specialties. The aim of this study was to investigate the impact of disease on family members of patients over a wide range of specialties and identify key impact areas. This information is essential in order to reveal the extent of this impact and to allow strategies to be developed to support the family members of patients with chronic disease. METHODS: Semi-structured interviews were carried out with 133 family members of mostly chronically ill patients from 26 medical specialties. Family members were invited to discuss all areas of their lives that had been affected by having an unwell relative. Thematic analysis was carried out using NVivo9Âź software. RESULTS: Most family members were female (61%), the partner or spouse of the patient (56%), or the parent (22%). Their mean age was 56.1 years (range: 21-85 years) and the mean duration of the patient's disease was 8.9 years (range: 1 month to 60 years). Ten key themes of family quality of life were identified from interviews. The median number of themes reported by family members was six (range: 1-10). The key themes included: emotional impact (mentioned by 92% of subjects), daily activities (91%), family relationships (69%), sleep and health (67%), holidays (62%), involvement in medical care and support given to family members (61%), work and study (52%), financial impact (51%), social life (37%), and time planning (14%). Relationships between the themes were identified. CONCLUSION: This large scale multi-specialty study has demonstrated the significant, yet similar, impact that illness can have on the quality of life of patients' family members. Family quality of life is a previously neglected area of health care which needs to be addressed in order to provide appropriate support for the patient and the family unit.Peer reviewedFinal Published versio

    Redox regulation of type-I inositol trisphosphate receptors in intact mammalian cells.

    A sensitization of inositol 1,4,5-trisphosphate receptor (IP3R)-mediated Ca2+ release is associated with oxidative stress in multiple cell types. These effects are thought to be mediated by alterations in the redox state of critical thiols in the IP3R, but this has not been directly demonstrated in intact cells. Here, we utilized a combination of gel-shift assays with MPEG-maleimides and LC-MS/MS to monitor the redox state of recombinant IP3R1 expressed in HEK293 cells. We found that under basal conditions, ∌5 of the 60 cysteines are oxidized in IP3R1. Cell treatment with 50 ÎŒm thimerosal altered gel shifts, indicating oxidation of ∌20 cysteines. By contrast, the shifts induced by 0.5 mm H2O2 or other oxidants were much smaller. Monitoring of biotin-maleimide attachment to IP3R1 by LC-MS/MS with 71% coverage of the receptor sequence revealed modification of two cytosolic (Cys-292 and Cys-1415) and two intraluminal cysteines (Cys-2496 and Cys-2533) under basal conditions. The thimerosal treatment modified an additional eleven cysteines, but only three (Cys-206, Cys-767, and Cys-1459) were consistently oxidized in multiple experiments. H2O2 also oxidized Cys-206 and additionally oxidized two residues not modified by thimerosal (Cys-214 and Cys-1397). Potentiation of IP3R channel function by oxidants was measured with cysteine variants transfected into a HEK293 IP3R triple-knockout cell line, indicating that the functionally relevant redox-sensitive cysteines are predominantly clustered within the N-terminal suppressor domain of IP3R. To our knowledge, this study is the first that has used proteomic methods to assess the redox state of individual thiols in IP3R in intact cells. © 2018 Joseph et al

    The trauma memory quality questionnaire:Preliminary development and validation of a measure of trauma memory characteristics for children and adolescents

    It has been suggested that post-traumatic stress is related to the nature of an individual's trauma memories. While this hypothesis has received support in adults, few studies have examined this in children and adolescents. This article describes the development and validation of a measure of the nature of children's trauma memories, the Trauma Memory Quality Questionnaire (TMQQ), that might test this hypothesis and be of clinical use. The measure was standardised in two samples, a cross-sectional sample of non-clinic referred secondary school pupils (n=254), and a sample participating in a prospective study of children and adolescents who had attended a hospital Accident and Emergency department following an assault or a road traffic accident (n=106). The TMQQ was found to possess good internal consistency, criterion validity, and construct validity, but test-retest reliability has yet to be established

    Power-law distributions from additive preferential redistributions

    We introduce a non-growth model that generates the power-law distribution with the Zipf exponent. There are N elements, each of which is characterized by a quantity, and at each time step these quantities are redistributed through binary random interactions with a simple additive preferential rule, while the sum of quantities is conserved. The situation described by this model is similar to those of closed NN-particle systems when conservative two-body collisions are only allowed. We obtain stationary distributions of these quantities both analytically and numerically while varying parameters of the model, and find that the model exhibits the scaling behavior for some parameter ranges. Unlike well-known growth models, this alternative mechanism generates the power-law distribution when the growth is not expected and the dynamics of the system is based on interactions between elements.     The development and clinical evaluation of a 'traffic-light' design dermatology outpatient discharge information checklist

    Background Although multiple factors influence discharge decisions, there is no structured guidance to assist clinicians in making informed decisions. A discharge information checklist might improve the appropriateness of dermatology clinicians’ discharge decisions. Objectives To generate consensus among dermatologists on the content of an outpatient discharge checklist, to create one and to seek clinicians’ opinions on its usefulness. Methods Seventeen consultant dermatologists from five National Health Service trusts completed a 72‐item Delphi questionnaire. A five‐point Likert scale was used to rate each item for importance in contributing to a high‐quality discharge decision. Eighteen clinicians completed a questionnaire evaluating checklist use. Results Consensus was determined when ≄ 75% of consultants rated an item ‘very important’ or ‘important’. There was strong inter‐rater reliability (intraclass correlation coefficient = 0·958) and fair inter‐rater agreement (Fleiss kappa = 0·269). There were 26 consensus‐agreed items, condensed to 13 that formed the ‘traffic‐light’ checklist. These are disease‐related issues (diagnostic certainty, disease severity, treatment appropriateness, patient manageable in primary care, patient's benefit from follow‐up), patient empowerment issues (understanding diagnosis and treatment outcome, having a clear plan, treatment side‐effects, ability to self‐manage) and addressing concerns (patient concerns, easy reaccess to secondary care, whether patient and clinician are happy with the decision). Twelve clinicians (67%) found the checklist useful, 11 (61%) wanted to use it in future, 10 (56%) thought it was useful for training and three (17%) said it helped their thinking. Clinicians suggested its use for auditing and for training clinicians and administrators. Conclusions Items were identified to create an outpatient discharge information checklist, which demonstrated high acceptability

    On modeling player fitness in training for team sports with application to professional rugby

    It is increasingly important for professional sports teams to monitor player fitness in order to optimize performance. Models have been put forward linking fitness in training to performance in competition but rely on regular measurements of player fitness. As formal tests for measuring player fitness are typically time-consuming and inconvenient, measurements are taken infrequently. As such, it may be challenging to accurately predict performance in competition as player fitness is unknown.Alternatively, other data, such as how the players are feeling, may be measured more regularly. This data, however, may be biased as players may answer the questions differently and these differences may dominate the data. Linear Mixed Methods and Support Vector Machines were used to estimate player fitness from available covariates at times when explicit measures of fitness are unavailable. Using data provided by Glasgow Warriors Rugby Club, a case study was used to illustrate the application and value of these models. Both models performed well with R2 values ranging from 60% to 85%, demonstrating that the models largely captured the biases introduced by individual players

    Integrating soil moisture measurements into pasture growth forecasting in New Zealand's hill country

    Forecasting pasture growth in hill country landscapes requires information about soil water retention characteristics, which will help to quantify both water uptake, and its percolation below the root zone. Despite the importance of soil moisture data in pasture productivity predictions, current models use low-resolution estimates of water input into their soil water balance equations and plant growth simulations. As a result, they frequently fail to capture the spatial and temporal variability of soil moisture in hill country soils. Wireless Sensor Networks (WSN) are promising in-situ measurement systems for monitoring soil moisture dynamics with high temporal resolution in agricultural soils. This paper presents the deployment of a soil moisture sensing network, utilising WSN technology and multi-sensor probes, to monitor soil water changes over a hill country farm in the northern Wairarapa region of the North Island. Processed capacitance-based raw data was converted to volumetric water content by means of a factory calibration function to assess sensor accuracy and to calculate soil water storage within the pasture root zone. The derived volumetric soil moisture data was examined in terms of its dependence on the variability and influences of hill country landscape characteristics such as aspect. The integration of spatially distributed sensors and multi-depth soil moisture measurements from various hillslope positions showed that slope and aspect exerted a significant impact on soil moisture values. Furthermore, considerable differences were identified in soil water profile responses to significant rainfall events and subsequent soil water redistribution. Initial indications are that high-resolution time series of accurate multi-depth soil moisture measurements collected by a WSN are valuable for investigating root zone water movement. Sensor evaluation and data analysis suggest that these devices and their associated datasets are able to contribute to an improved understanding of drying and wetting cycles and soil moisture variability. Potentially, this will create an opportunity to generate improved pasture growth predictions in pastoral hill country environments
