Understanding behaviour change in context: examining the role of midstream social marketing programmes
This research examines how midstream social marketing programmes that adopt a relational and community-based approach create opportunities for individuals to make incremental changes to health behaviour. Specifically, it applies Bourdieusian theory to explore how interactions between community healthcare workers (CHWs) and members of the public generate impetus for change and foster individual agency for improved health. Qualitative interviews were carried out with members of the public and CHWs engaged in a Smokefree homes and cars initiative. The findings suggest that although CHWs are challenged by resource constraints, their practices in working with individuals and families build trust and enable dialogue that bridges smoking-related health insight with home logics. These interactions can promote individual agency with a transformative effect through small changes to smoking-related dispositions, norms and practices. However, tensions with the habitus of other household members and other capital deficits can inhibit progress towards embedding new practices. The study concludes that interventions built upon community relationships show potential for addressing limitations of information-focused campaigns but there is a need to also respond to key social structures relating to the field of action for new health dispositions to become embedded in practice
First AMBER/VLTI observations of hot massive stars
AMBER is the first near infrared focal instrument of the VLTI. It combines
three telescopes and produces spectrally resolved interferometric measures.
This paper discusses some preliminary results of the first scientific
observations of AMBER with three Unit Telescopes at medium (1500) and high
(12000) spectral resolution. We derive a first set of constraints on the
structure of the circumstellar material around the Wolf Rayet Gamma2 Velorum
and the LBV Eta Carinae
Direct constraint on the distance of y2 Velorum from AMBER/VLTI observations
In this work, we present the first AMBER observations, of the Wolf-Rayet and
O (WR+O) star binary system y2 Velorum. The AMBER instrument was used with the
telescopes UT2, UT3, and UT4 on baselines ranging from 46m to 85m. It delivered
spectrally dispersed visibilities, as well as differential and closure phases,
with a resolution R = 1500 in the spectral band 1.95-2.17 micron. We interpret
these data in the context of a binary system with unresolved components,
neglecting in a first approximation the wind-wind collision zone flux
contribution. We show that the AMBER observables result primarily from the
contribution of the individual components of the WR+O binary system. We discuss
several interpretations of the residuals, and speculate on the detection of an
additional continuum component, originating from the free-free emission
associated with the wind-wind collision zone (WWCZ), and contributing at most
to the observed K-band flux at the 5% level. The expected absolute separation
and position angle at the time of observations were 5.1±0.9mas and
66±15° respectively. However, we infer a separation of
3.62+0.11-0.30 mas and a position angle of 73+9-11°. Our analysis thus
implies that the binary system lies at a distance of 368+38-13 pc, in agreement
with recent spectrophotometric estimates, but significantly larger than the
Hipparcos value of 258+41-31 pc
Near-Infrared interferometry of Eta Carinae with high spatial and spectral resolution using the VLTI and the AMBER instrument
We present the first NIR spectro-interferometry of the LBV Eta Carinae. The K
band observations were performed with the AMBER instrument of the ESO Very
Large Telescope Interferometer using three 8.2m Unit Telescopes with baselines
from 42 to 89m. The aim of this work is to study the wavelength dependence of
Eta Car's optically thick wind region with a high spatial resolution of 5 mas
(11 AU) and high spectral resolution. The medium spectral resolution
observations (R=1,500) were performed in the wavelength range around both the
HeI 2.059 micron and the Br gamma 2.166 micron emission lines, the high
spectral resolution observations (R=12,000) only in the Br gamma line region.
In the K-band continuum, a diameter of 4.0 +/-0.2 mas (Gaussian FWHM, fit range
28-89m) was measured for Eta Car's optically thick wind region. If we fit
Hillier et al. (2001) model visibilities to the observed AMBER visibilities, we
obtain 50 % encircled-energy diameters of 4.2, 6.5 and 9.6mas in the 2.17
micron continuum, the HeI, and the Br gamma emission lines, respectively. In
the continuum near the Br gamma line, an elongation along a position angle of
120+/-15 degrees was found, consistent with previous VLTI/VINCI measurements by
van Boekel et al. (2003). We compare the measured visibilities with predictions
of the radiative transfer model of Hillier et al. (2001), finding good
agreement. Furthermore, we discuss the detectability of the hypothetical hot
binary companion. For the interpretation of the non-zero differential and
closure phases measured within the Br gamma line, we present a simple geometric
model of an inclined, latitude-dependent wind zone. Our observations support
theoretical models of anisotropic winds from fast-rotating, luminous hot stars

Idiopathic toe-walking in children, adolescents and young adults: a matter of local or generalised stiffness?
<p>Abstract</p> <p>Background</p> <p>Idiopathic Toe Walking (ITW) is present in children older than 3 years of age still walking on their toes without signs of neurological, orthopaedic or psychiatric diseases. ITW has been estimated to occur in 7% to 24% of the childhood population. To study associations between Idiopathic Toe Walking (ITW) and decrease in range of joint motion of the ankle joint. To study associations between ITW (with stiff ankles) and stiffness in other joints, muscle strength and bone density.</p> <p>Methods</p> <p>In a cross-sectional study, 362 healthy children, adolescents and young adults (mean age (sd): 14.2 (3.9) years) participated. Range of joint motion (ROM), muscle strength, anthropometrics sport activities and bone density were measured.</p> <p>Results</p> <p>A prevalence of 12% of ITW was found. Nine percent had ITW and severely restricted ROM of the ankle joint. Children with ITW had three times higher chance of severe ROM restriction of the ankle joint. Participants with ITW and stiff ankle joints had a decreased ROM in other joints, whereas bone density and muscle strength were comparable.</p> <p>Conclusion</p> <p>ITW and a decrease in ankle joint ROM might be due to local stiffness. Differential etiological diagnosis should be considered.</p
Idiopathic toe walking and sensory processing dysfunction
<p>Abstract</p> <p>Background</p> <p>It is generally understood that toe walking involves the absence or limitation of heel strike in the contact phase of the gait cycle. Toe walking has been identified as a symptom of disease processes, trauma and/or neurogenic influences. When there is no obvious cause of the gait pattern, a diagnosis of idiopathic toe walking (ITW) is made. Although there has been limited research into the pathophysiology of ITW, there has been an increasing number of contemporary texts and practitioner debates proposing that this gait pattern is linked to a sensory processing dysfunction (SPD). The purpose of this paper is to examine the literature and provide a summary of what is known about the relationship between toe walking and SPD.</p> <p>Method</p> <p>Forty-nine articles were reviewed, predominantly sourced from peer reviewed journals. Five contemporary texts were also reviewed. The literature styles consisted of author opinion pieces, letters to the editor, clinical trials, case studies, classification studies, poster/conference abstracts and narrative literature reviews. Literature was assessed and graded according to level of evidence.</p> <p>Results</p> <p>Only one small prospective, descriptive study without control has been conducted in relation to idiopathic toe walking and sensory processing. A cross-sectional study into the prevalence of idiopathic toe walking proposed sensory processing as being a reason for the difference. A proposed link between ITW and sensory processing was found within four contemporary texts and one conference abstract.</p> <p>Conclusion</p> <p>Based on the limited conclusive evidence available, the relationship between ITW and sensory processing has not been confirmed. Given the limited number and types of studies together with the growing body of anecdotal evidence it is proposed that further investigation of this relationship would be advantageous.</p
Co-morbidity and patterns of care in stimulant-treated children with ADHD in the Netherlands
This study aimed at investigating the use of psychosocial interventions and psychotropic co-medication among stimulant-treated children with attention-deficit hyperactivity disorder (ADHD) in relation to the presence of psychiatric co-morbidity. Stimulant users younger than 16Ā years were identified in 115 pharmacies and a questionnaire was sent to their stimulant prescribing physician. Of 773 questionnaires sent out, 556 were returned and were suitable for analysis (72%). The results are based on 510 questionnaires concerning stimulant-treated children for whom a diagnosis of ADHD was reported. Of the 510 children diagnosed with ADHD, 31% had also received one or more other psychiatric diagnoses, mainly pervasive developmental disorder or oppositional defiant disorder/conduct disorder. We found an association between the presence of co-morbidity and the use of psychosocial interventions for the child (PĀ <Ā 0.001) and the parents (PĀ <Ā 0.001). In the ADHD-only group, 26% did not receive any form of additional interventions, while psychosocial interventions varied from 8 to 18% in children with ADHD and psychiatric co-morbidity. The presence of diagnostic co-morbidity was also associated with the use of psychotropic co-medication (overall, PĀ =Ā 0.012) and antipsychotics (PĀ <Ā 0.001). Stimulant-treated youths with ADHD and psychiatric co-morbidity received more psychosocial interventions and psychotropic co-medication than children with ADHD-only. The type of psychosocial interventions and psychotropic co-medication received by the children and their parents, depended on the specific co-morbid psychiatric disorder being present
Constraining the wind launching region in Herbig Ae stars: AMBER/VLTI spectroscopy of HD 104237
This is the author accepted manuscript. The final version is available from EDP Sciences via the DOI in this record.Aims. We investigate the origin of the BrĪ³ emission of the Herbig Ae star HD 104237 on Astronomical Unit (AU) scales.
Methods. Using AMBER/VLTI at a spectral resolution R = 1500 we spatially resolve the emission in both the BrĪ³ line and the adjacent continuum.
Results. The visibility does not vary between the continuum and the BrĪ³ line, even though the line is strongly detected in the spectrum, with a peak
intensity 35% above the continuum. This demonstrates that the line and continuum emission have similar size scales. We assume that the K-band
continuum excess originates in a āpuffed-upā inner rim of the circumstellar disk, and discuss the likely origin of BrĪ³.
Conclusions. We conclude that this emission most likely arises from a compact disk wind, launched from a region 0.2ā0.5 AU from the star, with
a spatial extent similar to that of the near infrared continuum emission region, i.e., very close to the inner rim location.This work has been partly supported by the
