Vergleichende Untersuchungen von Delayed Release-Systemen in AbhÀngigkeit von Wirkstoffeigenschaften
Die vorliegende Arbeit befasste sich mit der Möglichkeit und den Strategien zur Herstellung
von Delayed Release-Systemen in AbhÀngigkeit von Wirkstoffeigenschaften.
Dabei diente das Prinzip eines durch Sprengmittelquellung verursachten Aufplatzens
des FilmĂŒberzuges als Basis fĂŒr eine zeitlich verzögerte Wirkstofffreisetzung von
Hydrocortison sowie von Aminophyllin.
Als Grundlage wurden Formulierungen fĂŒr Tabletten, die einerseits eine hohe StabilitĂ€t
fĂŒr den anschlieĂenden Wirbelschichtprozess und andererseits eine rasche Freisetzung
des Wirkstoffes sicherstellen, untersucht. AuĂerdem wurden Pellets mit vergleichbaren
Freisetzungen hergestellt.
Ein besonderes Interesse galt der vergleichenden Untersuchung der EinflĂŒsse verschiedener
Filmbildner auf die Verzögerung der Freisetzung von Hydrocortison als
hydrophobem Wirkstoff gegenĂŒber dem hydrophilen Arzneistoff Aminophyllin aus
Tabletten.
Die Tabletten und Pellets wurden mit magensaftresistentem Filmbildner ĂŒberzogen,
um die Freisetzung durch den pH-Wert zu steuern.
Ferner wurde der Einfluss von ViskositÀt und von lipophilen ZusÀtzen bei wÀssrig
aufgebrachten Hydrogelbildnern auf die Verzögerung der Freisetzung vergleichend
untersucht.
Weiterhin wurden vier wasserunlösliche Polymere untersucht, von denen zwei Cellulosederivate
darstellen und zwei aus der Gruppe der vollsynthetischen Polymethylmethacrylat-
Derivate stammen. Ferner wurden organische Filmlösungen und
wÀssrige Dispersionen, unterschiedliche Weichmacher und Porenbildner sowie Antiklebemittel
und der Einfluss thermischer Nachbehandlung miteinander verglichen.
AuĂerdem waren Freisetzungsuntersuchungen in unterschiedlichen Medien zum
Vergleich der unterschiedlichen Darreichungsformen in Hinblick auf eine abzuschÀtzende
in-vitro/in-vivo-Beziehung von Bedeutung. Wichtig war ferner die LagerstabilitÀt
der Freisetzungen und die chemische StabilitÀt der Wirkstoffe nach Lagerung bei
definierten Temperaturen ĂŒber bis zu 2 Jahren.
Untersuchungen zur chronopharmakologischen Arzneistoffapplikation von Hydrocortisonhemisuccinat
mittels elektronischer Infusionspumpen wurden als weitere Möglichkeit
fĂŒr eine verzögerte Freisetzung einbezogen.
Im wesentlichen ergaben sich folgende Befunde:
1. FĂŒr hohe Wirkstoffdosen mit schwer verpressbaren Substanzen stellt die
Formulierung einer Tablette, die eine groĂe HĂ€rte aufweist und dennoch eine rasche
Freisetzung des Wirkstoffes gewÀhrleistet, eine besondere Aufgabe dar.
Mit Mannit und ExplotabÂź als Sprengmittel konnten Tabletten mit Aminophyllin
und ebenso auch mit Hydrocortison in niedriger Dosierung hergestellt werden.
Ferner wurde ein Einfluss des Sprengmittelanteils auf die Verzögerung der Freisetzung
von ĂŒberzogenen Tabletten festgestellt.
2. Eudragit S100, ein Filmbildner, der sich bei einem Schwellen-pH-Wert von etwa
7,5 löst, ergab fĂŒr Hydrocortisontabletten ĂberzĂŒge, die nach Herstellung bei
sauren pH-Werten bis einschlieĂlich pH 7,0 stabil waren, jedoch nach einer Lagerung
von 12 Monaten ihre StabilitĂ€t verloren. Gleiches galt fĂŒr Hydrocortisonpellets,
die jedoch bereits nach kĂŒrzerer Lagerung instabil wurden. Als Ursache
konnte eine VerflĂŒchtigung des Weichmachers nachgewiesen werden.
Mit dem hydrophilen basischen Aminophyllin war es nicht möglich, sÀureresistente
Tabletten, die ĂŒber pH 6,0 stabil waren, herzustellen.
3. Bei der Herstellung von ĂberzĂŒgen spielt der Zeitaufwand eine entsprechende
Rolle. FĂŒr ĂberzĂŒge mit hochviskosen quellenden Substanzen wird eine lange
Phase der Quellung und Lösung der Filmbildner fĂŒr die Ăberzugszubereitung
sowie fĂŒr den Wirbelschichtprozess benötigt. Durch Zusatz von lipophilen Stoffen
konnte sowohl die Prozesszeit verringert als auch die Verzögerung der Freisetzung
erhöht werden. Besonders wirtschaftlich ist die Verwendung von mittelviskoser
MethocelÂź K4M und 40 % Cetylalkohol. Die ĂberzĂŒge sind fĂŒr Hydrocortison
gut, fĂŒr Aminophyllin wegen der Diffusion durch den Film ungeeignet. In verschiedenen
Freisetzungsmedien wurden stark unterschiedliche Verzögerungen
festgestellt. Eine sehr gute LagerstabilitÀt war gegeben.
4. Ethylcellulose ist sowohl als organische Lösung als auch als wÀssrige Dispersion
aufgetragen fĂŒr Hydrocortison- und Aminophyllintabletten gleichermaĂen geeignet.
Ein Porenbildneranteil von 10 % Hydroxypropylmethylcellulose K4M im
FilmĂŒberzug ist fĂŒr eine reproduzierbare Verzögerung von fĂŒnf Stunden ideal.
Polyethylenglykol als Porenbildner erwies sich als ungeeignet. Einsatz des
hydrophilen Weichmachers Triethylcitrat ergab nach Ermittlung einer geeigneten
Konzentration bei gleichen Schichtdicken lÀngere Verzögerungszeiten als lipophiles
Acetyltributylcitrat. Dagegen war bei Verwendung von lipophilem
Weichmacher die FreisetzungsstabilitĂ€t ĂŒber zwei Jahre besser. Durch die
Schichtdicke konnte die Verzögerungszeit eingestellt werden. Ferner ist die AbhÀngigkeit
der Freisetzung vom Freisetzungsmedium sehr gering.
5. Mittels Hochdruckhomogenisation konnten wÀssrige Dispersionen mit Celluloseacetat
hergestellt werden, deren TeilchengröĂe mit im Handel befindlichen Dispersionen
anderer Polymere vergleichbar waren. Mit Triethylcitrat als Weichmacher
und MethocelÂź als Porenbildner eigneten diese sich fĂŒr Hydrocortisontabletten
gut. Aminophyllin wurde hingegen durch Diffusion schon vor dem Aufplatzen
freigesetzt. Hohe Zeitunterschiede mit dem Faktor 3 wurden bei den Freisetzungsuntersuchungen
in verschiedenen Medien festgestellt. Bei Lagerung kam
es nach 12 Monaten zu einer VerlÀngerung der verzögerten Freisetzung.
6. Eudragit RS-Film ist mit 10 % Porenbildner MethocelÂź K4M fĂŒr Hydrocortisontabletten
sehr gut geeignet. Mit kurzen Prozesszeiten und geringen Schichtdicken
wurden lange Verzögerungen der Freisetzung erreicht. Die Freisetzungszeiten
t60% variierten allerdings in verschiedenen Medien um den Faktor 2, wobei
diese nach Lagerung ĂŒber 12 Monate stabil waren.
Aminophyllin dagegen interagiert als ionische Substanz mit dem ebenfalls ionischen
Filmbildner und wurde durch Diffusion durch die Filmmembran freigesetzt
und ist daher unter diesen Bedingungen fĂŒr solche ĂberzĂŒge ungeeignet.
7. Das Polymer Eudragit NE eignet sich in Kombination mit dem Porenbildner Methocel
Âź K4M gegenĂŒber dem Porenbildner Polyethylenglykol 20.000 sehr gut fĂŒr
die Befilmung von Hydrocortisontabletten, da bei kurzen Herstellungszeiten
lange Verzögerungszeiten eingestellt werden konnten. Jedoch verdoppelte sich die Verzögerungszeit in verschiedenen Freisetzungsmedien, wÀhrend die LagerstabilitÀt
sehr gut war.
FĂŒr Aminophyllin ist Eudragit NE ungeeignet. Die Tabletten blĂ€hten sich im
Freisetzungsmedium auf und setzten den Wirkstoff durch Diffusion frei.
8. Ein wichtiges Kriterium fĂŒr die Bewertung eines Filmbildners fĂŒr ein Delayed Release-
System stellt die Robustheit der Freisetzungseigenschaften in verschiedenen
Freisetzungsmedien dar. Je unabhÀngiger von unterschiedlichen IonenstÀrken
und pH-Werten die Freisetzung erfolgt, desto besser ist auch eine Ăbertragung
der in-vitro-Befunde auf in-vivo Bedingungen denkbar. Bei in-vivo Anwendung
mĂŒssen zu den in-vitro Bedingungen noch weitere Parameter berĂŒcksichtigt
werden wie die MotilitĂ€t des Magen-Darm-Traktes sowie die FlĂŒssigkeitsverhĂ€ltnisse,
ferner Enzyme, Gallensalze und Nahrungseinfluss.
9. Die thermische Nachbehandlung zur beschleunigten Nachverfilmung ist zwar fĂŒr
Aminophyllintabletten mit einer GelbfÀrbung der OberflÀche verbunden, trÀgt jedoch
bei allen Filmbildnern zu einer VerlÀngerung und Stabilisierung der Verzögerungszeit
bei.
10. Die Wirkstoffe waren in allen Zubereitungen nach Lagerung von bis zu 24 Monaten
bei 20 °C und 30 °C stabil.
11. Wie die vorliegenden Untersuchungen zeigen, sind Freisetzungseigenschaften
von Wirkstoffen fĂŒr Delayed Release-Systeme zur peroralen Anwendung in hohem
Masse insbesondere von den physikalischen Eigenschaften der Wirkstoffe
abhÀngig. Konstruktionen von Arzneiformen mit allgemeiner Anwendbarkeit sind
deshalb in der Regel Wunschvorstellungen.
12. Die chemische StabilitÀt von Hydrocortisonhemisuccinat erwies sich bei der Verwendung
in elektronischen Infusionspumpen als ausreichend. Auch die Dosierungsgenauigkeit
der untersuchten Pumpen wies nur geringe Standardabweichungen
auf. Eine Anwendung der Pumpen als Delayed Release-System fĂŒr die
zeitverzögerte parenterale Applikation anderer wasserlöslicher und ausreichend
stabiler Arzneistoffe erscheint als möglich
ZellulÀrer Lipidimport durch Klasse B Scavenger Rezeptoren und die Biogenese von MembrandomÀnen
Phospholipids form the matrix of plasmamebranes (PM) consisting of an asymmetric lipid bilayer. The high amounts of sphingolipids and cholesterol in the PM are furthermore the basis to form liquid-ordered domains / rafts by saturated long chain fatty acid interactions. Hence, the lipid trafficking pathways enabling the membrane biogenesis and lateral segregation into membrane compounds remain largely undefined. Here we could demonstrate that the scavenger receptor CD36 selectively mediates the lipid uptake of sphingomyelin (SM) and phosphatidylcholine (PC) into PM in an endocytosis independent way. In human monocytes we could further show that the selective phospholipid uptake for SM, PC and phosphatidylethanolamine was almost exclusively promoted by CD36 and SR-BI. Whereas CD36 was mainly localized in rafts, we found about 2/3rds of SR-BI located in non-rafts. Thereby, the main cell entrance for phospholipids and cholesterol was mediated by SR-BI into non-raft compartments. SR-BI was then internalized by clathrin-coated pits and promoted the formation of intracellular microdomains in early endosomes by recruitment of the newly acquired raft lipids. Thus, we propose a leading role for SR-BI in generating new rafts by the incorporation of the raft building lipids SM and cholesterol. These may provide a pool for the regeneration of raft domains within the PM
Vaccine associated benign headache and cutaneous hemorrhage after ChAdOx1 nCoV-19 vaccine: A cohort study
Objectives
Fatal complications have occurred after vaccination with ChAdOx1 nCoV-19, a vaccine against Covid-19. Vaccine-induced immune thrombotic thrombocytopenia (VITT) with severe outcome is characterized by venous thrombosis, predominantly in cerebral veins, thrombocytopenia and anti-PF4/polyanion antibodies. Prolonged headaches and cutaneous hemorrhages, frequently observed after the ChAdOx1 nCoV-19 vaccine, have therefore caused anxiety among vaccinees. We investigated whether these symptoms represent a mild form of VITT, with a potential for aggravation, e.g. in case of a second vaccination dose, or a different entity of vaccine complications
Materials and methods
We included previously healthy individuals who had a combination of headache and spontaneous severe cutaneous hemorrhages emerging after the 1st dose of the ChAdOx1 nCoV-19 vaccine. Twelve individuals were found to meet the inclusion criteria, and a phone interview, cerebral MRI, assessment of platelet counts, anti PF4/polyanion antibodies and other laboratory tests were performed.
Results
None of the symptomatic vaccinees had cerebral vein thrombosis, hemorrhage or other pathology on MRI. Platelet counts were within normal range and no anti-PF4/polyanion platelet activating antibodies were found. Moreover, vasculitis markers, platelet activation markers and thrombin generation were normal. Furthermore, almost all symptoms resolved, and none had recurrence of symptoms after further vaccination with mRNA vaccines against Covid-19.
Conclusions
The combination of headaches and subcutaneous hemorrhage did not represent VITT and no other specific coagulation disorder or intracranial pathology was found. However, symptoms initially mimicking VITT demand vigilance and low threshold for a clinical evaluation combined with platelet counts and Dâdimer
New Alzheimer Amyloid ÎČ Responsive Genes Identified in Human Neuroblastoma Cells by Hierarchical Clustering
Alzheimer's disease (AD) is characterized by neuronal degeneration and cell loss. AÎČ42, in contrast to AÎČ40, is thought to be the pathogenic form triggering the pathological cascade in AD. In order to unravel overall gene regulation we monitored the transcriptomic responses to increased or decreased AÎČ40 and AÎČ42 levels, generated and derived from its precursor C99 (C-terminal fragment of APP comprising 99 amino acids) in human neuroblastoma cells. We identified fourteen differentially expressed transcripts by hierarchical clustering and discussed their involvement in AD. These fourteen transcripts were grouped into two main clusters each showing distinct differential expression patterns depending on AÎČ40 and AÎČ42 levels. Among these transcripts we discovered an unexpected inverse and strong differential expression of neurogenin 2 (NEUROG2) and KIAA0125 in all examined cell clones. C99-overexpression had a similar effect on NEUROG2 and KIAA0125 expression as a decreased AÎČ42/AÎČ40 ratio. Importantly however, an increased AÎČ42/AÎČ40 ratio, which is typical of AD, had an inverse expression pattern of NEUROG2 and KIAA0125: An increased AÎČ42/AÎČ40 ratio up-regulated NEUROG2, but down-regulated KIAA0125, whereas the opposite regulation pattern was observed for a decreased AÎČ42/AÎČ40 ratio. We discuss the possibilities that the so far uncharacterized KIAA0125 might be a counter player of NEUROG2 and that KIAA0125 could be involved in neurogenesis, due to the involvement of NEUROG2 in developmental neural processes
Decompressive surgery in cerebral venous sinus thrombosis due to vaccine-induced immune thrombotic thrombocytopenia
Background and purpose: Cerebral venous sinus thrombosis due to vaccine-induced im-mune thrombotic thrombocytopenia (CVST-VITT) is an adverse drug reaction occurring after severe acute respiratory syndrome coronavirus 2 (SARS-CoV- 2) vaccination. CVST-VITT patients often present with large intracerebral haemorrhages and a high proportion undergoes decompressive surgery. Clinical characteristics, therapeutic management and outcomes of CVST-VITT patients who underwent decompressive surgery are described and predictors of in- hospital mortality in these patients are explored.Methods: Data from an ongoing international registry of patients who developed CVST within 28 days of SARS-CoV- 2 vaccination, reported between 29 March 2021 and 10 May 2022, were used. Definite, probable and possible VITT cases, as defined by Pavord et al. (N Engl J Med 2021; 385: 1680â1689), were included. Results: Decompressive surgery was performed in 34/128 (27%) patients with CVST- VITT. In- hospital mortality was 22/34 (65%) in the surgical and 27/94 (29%) in the non- surgical group (p< 0.001). In all surgical cases, the cause of death was brain herniation. The highest mortality rates were found amongst patients with preoperative coma (17/18, 94% vs. 4/14, 29% in the non-comatose; p< 0.001) and bilaterally absent pupillary re-flexes (7/7, 100% vs. 6/9, 67% with unilaterally reactive pupil, and 4/11, 36% with bi-laterally reactive pupils; p= 0.023). Postoperative imaging revealed worsening of index haemorrhagic lesion in 19 (70%) patients and new haemorrhagic lesions in 16 (59%) pa-tients. At a median follow-up of 6 months, 8/10 of surgical CVST-VITT who survived ad-mission were functionally independent.Conclusions: Almost two-thirds of surgical CVST-VITT patients died during hospital ad-mission. Preoperative coma and bilateral absence of pupillary responses were associated with higher mortality rates. Survivors often achieved functional independence.Peer reviewe
Influence of obesity-related risk factors in the aetiology of glioma
BACKGROUND: Obesity and related factors have been implicated as possible aetiological factors for the development of glioma in epidemiological observation studies. We used genetic markers in a Mendelian randomisation framework to examine whether obesity-related traits influence glioma risk. This methodology reduces bias from confounding and is not affected by reverse causation. METHODS: Genetic instruments were identified for 10 key obesity-related risk factors, and their association with glioma risk was evaluated using data from a genome-wide association study of 12,488 glioma patients and 18,169 controls. The estimated odds ratio of glioma associated with each of the genetically defined obesity-related traits was used to infer evidence for a causal relationship. RESULTS: No convincing association with glioma risk was seen for genetic instruments for body mass index, waist-to-hip ratio, lipids, type-2 diabetes, hyperglycaemia or insulin resistance. Similarly, we found no evidence to support a relationship between obesity-related traits with subtypes of glioma-glioblastoma (GBM) or non-GBM tumours. CONCLUSIONS: This study provides no evidence to implicate obesity-related factors as causes of glioma
The German National Pandemic Cohort Network (NAPKON): rationale, study design and baseline characteristics

European Journal of Epidemiology . 2022.The German government initiated the Network University Medicine (NUM) in early 2020 to improve national research activities on the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) pandemic. To this end, 36 German Academic Medical Centers started to collaborate on 13 projects, with the largest being the National Pandemic Cohort Network (NAPKON). The NAPKON's goal is creating the most comprehensive Coronavirus Disease 2019 (COVID-19) cohort in Germany. Within NAPKON, adult and pediatric patients are observed in three complementary cohort platforms (Cross-Sectoral, High-Resolution and Population-Based) from the initial infection until up to three years of follow-up. Study procedures comprise comprehensive clinical and imaging diagnostics, quality-of-life assessment, patient-reported outcomes and biosampling. The three cohort platforms build on four infrastructure core units (Interaction, Biosampling, Epidemiology, and Integration) and collaborations with NUM projects. Key components of the data capture, regulatory, and data privacy are based on the German Centre for Cardiovascular Research. By April 01, 2022, 34 university and 40 non-university hospitals have enrolled 5298 patients with local data quality reviews performed on 4727 (89%). 47% were female, the median age was 52 (IQR 36-62-) and 50 pediatric cases were included. 44% of patients were hospitalized, 15% admitted to an intensive care unit, and 12% of patients deceased while enrolled. 8845 visits with biosampling in 4349 patients were conducted by April 03, 2022. Management of Cerebral Venous Thrombosis Due to Adenoviral COVID-19 Vaccination
Objective Cerebral venous thrombosis (CVT) caused by vaccine-induced immune thrombotic thrombocytopenia (VITT) is a rare adverse effect of adenovirus-based severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) vaccines. In March 2021, after autoimmune pathogenesis of VITT was discovered, treatment recommendations were developed. These comprised immunomodulation, non-heparin anticoagulants, and avoidance of platelet transfusion. The aim of this study was to evaluate adherence to these recommendations and its association with mortality. Methods We used data from an international prospective registry of patients with CVT after the adenovirus-based SARS-CoV-2 vaccination. We analyzed possible, probable, or definite VITT-CVT cases included until January 18, 2022. Immunomodulation entailed administration of intravenous immunoglobulins and/or plasmapheresis. Results Ninety-nine patients with VITT-CVT from 71 hospitals in 17 countries were analyzed. Five of 38 (13%), 11 of 24 (46%), and 28 of 37 (76%) of the patients diagnosed in March, April, and from May onward, respectively, were treated in-line with VITT recommendations (p < 0.001). Overall, treatment according to recommendations had no statistically significant influence on mortality (14/44 [32%] vs 29/55 [52%], adjusted odds ratio [OR] = 0.43, 95% confidence interval [CI] = 0.16-1.19). However, patients who received immunomodulation had lower mortality (19/65 [29%] vs 24/34 [70%], adjusted OR = 0.19, 95% CI = 0.06-0.58). Treatment with non-heparin anticoagulants instead of heparins was not associated with lower mortality (17/51 [33%] vs 13/35 [37%], adjusted OR = 0.70, 95% CI = 0.24-2.04). Mortality was also not significantly influenced by platelet transfusion (17/27 [63%] vs 26/72 [36%], adjusted OR = 2.19, 95% CI = 0.74-6.54). Conclusions In patients with VITT-CVT, adherence to VITT treatment recommendations improved over time. Immunomodulation seems crucial for reducing mortality of VITT-CVT. ANN NEUROL 2022Peer reviewe
The Core Value Compass: visually evaluating the goodness of brands that do good
yesBrands that do good for the society as well as for
themselves are motivated by the core values they espouse,
which necessitates a better understanding of what qualities a
true core value must possess. The inherent tension within
brands that do good, between commercial interests to
increase competitiveness, and societal interests that are
closely linked to the brandâs authenticity, has largely been
overlooked. Hence, we develop and demonstrate a relatively
easy-to-apply visual tool for evaluating core values based on
a set of âgoodnessâ criteria derived from extant theory. The
Core Value Compass adopts a paradox-based, evolutionary
perspective by incorporating the inherent tensions within
true core values, and classifying them according to their
temporal orientation. Thus, we contribute towards a better
understanding of underlying tensions of core values and
provide a practical tool that paves the way for improved, and
indeed ethical, corporate branding strategies. Furthermore,
we demonstrate the Compassâ application using the case of a
public sector brand, which is a quintessential brand that does
good. Therefore, we also contribute to the nascent theoretical
discourse on public sector branding. This paper therefore
adds to the notable attempts to bridge the gap between theory
and practice in core values-based corporate branding
Height and body mass index and risk for primary tumours of the central nervous system
Tumours of the central nervous system (CNS) are often a devastating disease in spite of best medical treatment by surgery, radiotherapy and chemotherapy. Body height and body fatness are known risk factors for several cancers, and the World Health Organization (WHO) has recently announced that prevention is the most cost-effective long-term strategy for cancer control. Knowledge about risk factors for CNS tumours is still scarce.
The main objective of this thesis was to study the association between body height and body fatness and risk for different CNS tumour subgroups. Therefore, two large prospective population-based Norwegian cohorts, the Nord-TrĂžndelag Health Study (HUNT) and the Statens SkjermbildeundersĂžkelse (SSU), were linked to the Cancer Registry of Norway. In both cohorts, body height and weight had been measured at study baseline and study participants were followed for up to four decades to identify new cases of CNS tumours.
Overall, this observational cohort study demonstrates differing associations of height and body fatness in relation to different CNS tumour subgroup
