    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Etablierung einer Methodik zur Konditionierung von Blutgefäßen im Durchflussmodell

    Studien zur Konditionierung von Gefäßmaterial verschiedenster Art in vitro erfordern ein Kreislaufmodell, welches zum einen weitgehend physiologische Verhältnisse zur Verfügung stellt und zum anderen eine größtmögliche Flexibilität in der Beeinflussung der Konditionierungsparameter und in der Methode der Messwerterhebung bietet. Auf Basis der derzeitigen Literatur über Bioreaktoren und Erkenntnisse zur Blutgefäßkonditionierung entwickelten wir ein neuartiges Perfusions-/Superfusionssystem, welches den zu Beginn genannten Forderungen gerecht werden soll. Für einen routinemäßigen Betrieb und Erstellung physiologischer Milieus auch über längere Zeiträume wurde die Anlage weiter optimiert und verschiedene Methodiken erarbeitet. Der Bioreaktor ist in der Lage, physiologische Druckkurven, Pulsationen, Volumenflüsse und Gaspartialdrücke zu erstellen, eine Änderung dieser Parameter ist jederzeit möglich. Durch Superfusionsfluss und Rotationseinrichtung wird eine Blutgefäßzüchtung durch Rotationsbesiedelung mit extraluminaler Nährstoffversorgung durchführbar. Zur direkten Bestätigung einiger dieser Anlageneigenschaften wurden Rindervenen über vier und acht Tage sowie unter physiologischen Scherkräften (2,2 dyn/cm2 gegenüber 0,5 dyn/cm2) und unter zusätzlichem intraluminalen Druck (20 mmHg) über vier Tage inkubiert. Dabei wurden täglich Messungen von Elektrolyt- Glucose- und Laktatkonzentrationen sowie Gaspartialdrücken und pH-Wert vorgenommen, außerdem Gefäßfunktion und -morphologie vor und nach Inkubation bestimmt. Es zeigten sich konstante pCO2- und pH-Werte. Die Elektrolytkonzentrationen stiegen leicht an, am ehesten durch konstanten minimalen Wasserverlust (ca. 4,62 %/Tag) bedingt, welcher sich z.B. durch einen Perfusor leicht ausgleichen ließe. Entsprechend den Änderungen des Sauerstoffpartialdrucks über die Rindervene sowie der Laktat- und D-Glukosekonzentrationen über die Zeit präsentierten die Rindervenen neben einem aeroben Stoffwechsel einen anaeroben Stoffwechsel mit einem glykolytischen Metabolismus unter allen Inkubationsbedingungen außer unter erhöhten Scherkräften mit zusätzlichem intraluminalen Druck. Letztere bewirkten eine im Vergleich zu dem Verbrauch von D-Glukose vermehrte Laktatproduktion, was eine anaerobe Verstoffwechselung weiterer derzeit noch unklarer Substanzen vermuten lässt. Unser Perfusions-/Superfusionssystem ist zusammen mit der entwickelten Methodik in der Lage, Blutgefäßforschung und -konditionierung, insbesondere im Bereich der Blutgefäßzüchtung, in vitro zu betreiben und somit dazu beizutragen, Ansatzpunkte für das Verständnis und die Behandlung vaskulärer Krankheitsbilder zu liefern

    Metabolic Requirements of Blood Vessels in a Perfusion Bioreactor

    Small caliber vessel grafts are one of the major challenges of vascular tissue engineering. A variety of processes have been developed to create vascular grafts from scaffolds and donor cells in bioreactors. In order to optimize such processes, this study focused on monitoring vessel metabolism under conditions typically used in perfusion protocols. Bovine veins were perfused in a bioreactor for four days. Group 1 vessels served as controls and were perfused with standard medium. Medium of group 2 was adjusted to the viscosity of blood. Group 3 vessels were additionally challenged with elevated luminal pressure. Contractile function was assessed in an organ bath. Tissue viability was determined by tetrazolium dye reduction. Oxygen gradients, dextrose consumption, and lactate production were monitored using a blood gas analyzer. KCl induced contractions did not differ between days 0 and 4. Norepinephrine dose-response curves of group 3 vessels achieved a higher maximum contraction on day 4, with no changes of EC50. Tissue viability was not altered by any of the perfusion conditions. Oxygen gradients across the vessels did not change with time but were elevated in group 2, with no signs of oxygen depletion. Dextrose consumption and lactate formation of group 1 and 2 vessels appeared to be stoichiometric. In contrast, group 3 vessels produced more lactate than dextrose could supply. These results indicate that conventional oxygenation of culture media is sufficient to meet the oxygen consumption of a functional vessel. Elevated shear forces increased the oxygen demand without affecting dextrose consumption. Elevated shear forces and luminal pressure caused the utilization of alternative energy sources. Thus online monitoring of key metabolic parameters appears to be a desirable feature of perfusion bioreactors for vascular tissue engineering

    Improving the formability of linear flow split profiles by laser annealing

    The effect of laser annealing on the mechanical properties of severely deformed low alloy steel is discussed. Two different sets of annealing parameters were used to achieve improvements in formability. The resulting microstructures, tensile properties and hardness distributions are reviewed. Annealing at temperatures around 700 degrees C with very high heating and cooling rates leads to recrystallization at the surface and moderate grain growth in subsequent layers. The improvement in ductility is proven to be sufficient to prevent cracking during bending operations

    Epidemiologische Daten aus Sprech- und Singstimmprofilen von Grundschülern

    Hintergrund: Das Sprech- und Singstimmprofil ist auch bei Kindern essentieller Bestandteil einer multimodalen Stimmdiagnostik. In der Literatur finden sich nur spärliche Angaben zu alters- und geschlechtsspezifischen Normwertbereichen. Insbesondere besteht die Frage, ob sich unterschiedliche Stimmgewohnheiten bei Knaben und Mädchen im Stimmprofil widerspiegeln.Material und Methoden: An einer zufällig ausgewählten Stichprobe aus 161 Drittklässlern an 9 Leipziger Grundschulen (75 Knaben, 86 Mädchen, mittl. Alter 8,86a) wurden das Sprech- und Singstimmprofil (manueller Modus) unter Verwendung eines selbstkalibrierenden Mikrofons gemessen (DiVAS, Xion medical). Die Daten wurden von geschulten Medizinern und Musikpädagogen unter Verwendung einer SOP erhoben. Ausgewertet wurden die Frequenz und Intensität von 5 Messpunkten der Sprechstimme: (1) leisestes Sprechen, (2) Gesprächslautstärke, (3) Vortragslautstärke, (4) Rufstimme, (5) leisestes Sprechen (Rücknahmetest nach Seidner), aus dem Singstimmprofil maximaler und minimaler Schalldruckpegel (SPL max/min), höchste und niedrigste Grundfrequenz (F0max/F0min), maximale Phonationsdauer (MPT), Jitter und Dysphonia Severity Index (DSI).Ergebnisse: Es wurden die Daten von 161 Probanden vollständig ausgewertet. Sprechstimme: Für die Messpunkte ergaben sich folgende Mittelwerte der Steigerungsstufen (1) 214,16 Hz/49,16 dB(A), (2) 225,22 Hz/45,46 dB(A), (3) 251,76 Hz/62,14 dB(A), (4) 322,06 Hz/74,33 dB(A), (5) 227,37 Hz/52,36 dB(A). Im Vergleich zeigten Mädchen signifikant geringere Intensitäten in 2, 3 und 4 (p=0,020); bezüglich der Frequenz fanden sich keine Geschlechtsunterschiede. Im Vergleich waren Mädchen bei SPL min signifikant leiser (p<0,001), darüber hinaus ergaben sich keine Geschlechtsunterschiede.Fazit: Die Ergebnisse können erstmals für die Abschätzung von Normwertbereichen des Sprechstimmprofils im Grundschulalter dienen und bestätigten die klinischen und sozialen Erfahrungen, dass Jungen ihre Sprech- und Rufstimme häufig lauter einsetzen als Mädchen.     Effects of Shear Forces and Pressure on Blood Vessel Function and Metabolism in a Perfusion Bioreactor.

    Bovine saphenous veins (BSV) were incubated in a perfusion bioreactor to study vessel wall metabolism and wall structure under tissue engineering conditions. Group 1 vessels were perfused for 4 or 8 days. The viscosity of the medium was increased to that of blood in group 2. Group 3 vessels were additionally strained with luminal pressure. Groups 1-d through 3-d were similar except that BSV were endothelium-denuded before perfusion. Groups 1-a through 3-a used native vessels at elevated flow rates. Group 3 vessels responded significantly better to noradrenaline on day 4, whereas denuded vessels showed attenuated responses (p < 0.001). Tetrazolium dye reduction did not depend on perfusion conditions or time except for denuded vessels. pO(2) gradients across the vessels were independent of time and significantly higher in group 2 (p < 0.001). BSV converted glucose stoichiometrically to lactate except vessels of groups 3, 1-d, and 3-d which released more lactate than glucose could supply (p < 0.001). Group 1 vessels as well as all vessels perfused with elevated flow rates showed a loss of endothelial cells after 4 days, whereas group 2 and 3 vessels retained most of the endothelium. These data suggest that vessel metabolism was not limited by oxygen supply. Shear forces did not affect glucose metabolism but increased oxygen consumption and endothelial cell survival. Luminal pressure caused the utilization of energy sources other than glucose, as long as the endothelium was intact. Therefore, vessel metabolism needs to be monitored during tissue engineering procedures which challenge the constructs with mechanical stimuli
