    Structured Analysis and Review of Filter-Based Control Strategies for Hybrid Energy Storage Systems

    Hybrid energy storage systems (HESS), i.e., the combination of two different energy storage technologies, are widely discussed as a promising solution for energy storage problems. A common control scheme to allocate the power between these storages and the subject of this study is filter-based control, where a filter splits the input signal into a low-frequency and high-frequency part. It provides robust results and easy implementation, although more advanced strategies may perform better. Many publications use this controller for specific problems, but a structured analysis of this controller type that quantifies the advantages and disadvantages, traits, and setbacks is missing. This work fills this gap and structures, summarizes, and provides mathematical background and guidelines on filter-based control of hybrid energy storage systems. Numerical simulations are performed to quantify the impact of design variables, parameters, or the input signal by using a linear storage model with efficiency and self-discharge rate and a low-pass filter controller with constant energy feedback as a representative subtype of this control scheme. The present work proves the high cycle-reduction capabilities of filter-controlled HESS at the cost of overdimensioning compared to more advanced control strategies. It demonstrates that using a high-efficiency, high-power storage with a low self-discharge rate and high-energy storage leads to smaller overall dimensioning and losses than a single storage system.     Pretreatment thrombocytosis: A prognostic marker for oral squamous cell carcinoma?

    Purpose: Thrombocytosis associated with poorer prognoses seems to be a frequent preoperative finding in different kind of cancers. The aim of the present study was to evaluate whether thrombocytosis can be used as a prognostic marker for oral squamous cell carcinoma (SCC). Methods: Altogether, 288 patients with oral SCC were considered, as well as all platelet counts between 1 and 5days prior to surgical treatment, recurrence rate, and lymph node metastasis. The minimum follow-up time was 12months. Results: The mean preoperative thrombocyte score of the patients who received surgery was 259.55 ± 83.8 Tsd/μl; 273 out of 288 patients were in the normal thrombocyte range, and 12 had a thrombocytosis. From 51 patients with recurrence, three were in the thrombocytosis group, and 45 patients with recurrence were in the normal thrombocyte range.     Poster Einleitung: OSCEs werden immer häufiger in der Ausbildung von Studierenden eingesetzt. Die Einführung eines OSCEs im fach Chirurgie ist in Planung. Durch die große Anzahl von Studierenden pro Semester oder Studienjahr (400 Studierende in Frankfurt) ist die Durchführung einer OSCE Prüfung mit großem personellen Aufwand verbunden. Vor allem während der Prüfung müssen eine Vielzahl von Chirurgen simultan zu Prüfungszwecken zur Verfügung stehen. Ziel der Studie war es, zu überprüfen, ob eine video-basierte Bewertung einer „Nahtstation“ zu einem späteren Zeitpunkt zu gleichen Ergebnissen in der Bewertung der Leistung der Studierenden führt. Methode: 33 Studierende führten unter standardisierten Bedingungen eine Hautnaht an einem Modell durchzuführen. Die Studierenden wurden während der Prozedur von zwei prüfenden Chirurgen und zwei Studierenden im PJ (praktischen Jahr) beobachtet und anhand einer objektiv strukturierten Checkliste bewertet (Prozessevaluation). Die Prozedur wurde gleichzeitig auf Video aufgezeichnet und zu einem späteren Zeitpunkt zwei weiteren Chirurgen und zwei weiteren Studierenden im PJ zur Bewertung gezeigt. Ergebnisse: Der Vergleich zwischen "live“-prüfenden und "video“-prüfenden Chirurgen zeigt eine signifikant hohe Korrelation (r=0,87; p<0,01) und eine hohe Übereinstimmung (88,2%) in der Bewertung. Ebenso zeigen die prüfenden PJler eine signifikant hohe Korrelation (r=0,84; p<0,01). Die Übereinstimmung ist bei den PJlern mit (82.4%) etwas niedriger als bei den beteiligten Chirurgen. Zusammenfassung: Mit dieser Studie konnte zeigt werden, daß es bei der Beurteilung der Performance von Studierenden bei einer Hautnaht am Modell unter Anwendung von objektiv strukturierten Checklisten möglich ist, eine direkte Beobachtung der Studierenden durch eine video-basierte Beobachtung zu ersetzen. Eine "Nahtstation“ in einem OSCE kann somit während der Prüfungszeit ohne Prüfer auskommen und im Anschluß bewertet werden

    Review of recurrently mutated genes in craniosynostosis supports expansion of diagnostic gene panels

    Craniosynostosis, the premature fusion of the cranial sutures, affects ~1 in 2000 children. Although many patients with a genetically determined cause harbor a variant in one of just seven genes or have a chromosomal abnormality, over 60 genes are known to be recurrently mutated, thus comprising a long tail of rarer diagnoses. Genome sequencing for the diagnosis of rare diseases is increasingly used in clinical settings, but analysis of the data is labor intensive and involves a trade-off between achieving high sensitivity or high precision. PanelApp, a crowd-sourced disease-focused set of gene panels, was designed to enable prioritization of variants in known disease genes for a given pathology, allowing enhanced identification of true-positives. For heterogeneous disorders like craniosynostosis, these panels must be regularly updated to ensure that diagnoses are not being missed. We provide a systematic review of genetic literature on craniosynostosis over the last 5 years, including additional results from resequencing a 42-gene panel in 617 affected individuals. We identify 16 genes (representing a 25% uplift) that should be added to the list of bona fide craniosynostosis disease genes and discuss the insights that these new genes provide into pathophysiological mechanisms of craniosynostosis

    Vitamin D intoxication caused by ingestion of commercial cat food in three kittens

    Two siblings, a 6-month-old sexually intact male weighing 2.5 kg (cat 1) and a sexually intact female (cat 2) British Shorthair cat weighing 2.3 kg, were examined because of a 3-week history of polyuria, lethargy and laboured breathing. One year previously, another sibling (cat 3) had been presented because of similar, yet more severe, clinical signs at the age of 5 months. Physical examination revealed lethargy, dehydration and polypnoea with slightly increased inspiratory effort. Diagnostic investigation revealed severe hypercalcaemia (cats 1-3), renal azotaemia (cats 1 and 3) and a radiologically generalised miliary interstitial pattern of the lungs (cats 1-3) attributable to hypervitaminosis D caused by ingestion of commercial cat food. Cat 3 was euthanased. Cats 1 and 2 were treated with isotonic saline solution (180 ml/kg IV daily), sucralfate (30 mg/kg PO q12h), terbutaline (only cat 1: 0.1 mg/kg SC q4h), furosemide (1.5 mg/kg IV q8h) and tapering doses of prednisolone. Cat 2 was normal on day 14. Cat 1 had stable renal disease and was followed up to day 672. The radiological generalised military interstitial pattern of the lungs had improved markedly. Excessive cholecalciferol-containing commercially available cat food poses a great hazard to cats. Supportive treatment may result in long-term survival and improvement of radiological pulmonary abnormalities

    Health professionals' experience of treatment of patients whose community treatment order was revoked under new capacity-based mental health legislation in Norway: qualitative study

    Background Norway introduced capacity-based legislation in mental healthcare on 1 September 2017 with the aim of increasing patient autonomy and legal protection and reducing the use of coercion. The new legislation was expected to be particularly important for patients under community treatment orders (CTOs). Aims To explore health professionals’ experiences of how capacity-based legislation affects healthcare services for patients whose compulsory treatment order was revoked as a result of being assessed as having capacity to consent. Method Nine health professionals responsible for treatment and care of patients whose CTO was revoked owing to the new legislation were interviewed in depth from September 2019 to March 2020. We used a hermeneutic approach to the interviews and analysis of the transcripts. Results The participants found that capacity-based legislation raised their awareness of their responsibility for patient autonomy and involvement in treatment and care. They also felt a need for more frequent assessments of patients’ condition and capacity to consent and more flexibility between levels of care. Conclusions The study shows that health professionals found that capacity-based legislation raised their awareness of their responsibility for patient autonomy and involvement in treatment and care. They sought closer dialogue with patients, providing information and advice, and more frequently assessing patients’ condition to adjust treatment and care to enable them to retain their capacity to consent. This could be challenging and required competence, continuity and close collaboration between personnel in different healthcare services at primary and specialist level
