    Developing strategies to improve fidelity of delivery of, and engagement with, a complex intervention to improve independence in dementia: a mixed methods study

    Background: It is important to evaluate fidelity of delivery and engagement during feasibility trials. However, there is little guidance on how to systematically develop strategies to improve implementation if problems arise. We aimed to: 1) Assess fidelity of delivery and engagement, 2) Identify factors influencing fidelity of delivery and engagement, and 3) Develop strategies to improve fidelity of delivery of, and engagement with, a complex intervention to improve independence in dementia, within a feasibility trial. Methods: A mixed methods evaluation of an intervention that aimed to improve independence in dementia. To assess fidelity of delivery and engagement, observation and self-report methods were used: 60% of audio-recorded intervention sessions were transcribed and reliably rated for fidelity. Providers (n = 12) and people with dementia/supporters (n = 34) were asked to complete checklists after each session. Descriptive statistics were used to analyse the data. To identify factors influencing fidelity and engagement, one-to-one semi-structured interviews were conducted with providers (n = 8), people with dementia (n = 7) and supporters (n = 7). Thematic analysis and content analysis were used to analyse data. To develop strategies, we followed four steps proposed by the authors of the Behaviour Change Wheel (1. Understanding the behaviour, 2. Identifying intervention functions, 3. Specifying intervention content, 4. Identifying mode of delivery). Results: Researcher ratings indicated moderate fidelity and provider/participant ratings indicated high fidelity of delivery. Knowledge, providers’ attributes, ease of adaptation of the intervention in relation to participants’ needs and logistical considerations influenced fidelity. We developed four strategies to improve fidelity of delivery of PRIDE: 1) showing a video, 2) giving an instruction sheet, 3) giving time to practice and 4) providing continued support. Participants reported high levels of engagement. Participants’ attributes, capability and opportunity influenced engagement. We developed four strategies to improve engagement with PRIDE: 1) a session summary document, 2) clear instructions, 3) time to practice activity and 4) providing regular compulsory telephone support. Conclusion: Fidelity of delivery and engagement are complex behaviours. This manuscript provides an example of how the Behaviour Change Wheel can be used during a feasibility trial to systematically develop strategies to improve implementation of complex interventions

    Occupational (Im)mobility in the Global Care Economy: The Case of Foreign-Trained Nurses in the Canadian Context

    The twenty-first century has witnessed a number of significant demographic and political shifts that have resulted in a care crisis. Addressing the deficit of care provision has led many nations to actively recruit migrant care labour, often under temporary forms of migration. The emergence of this phenomenon has resulted in a rich field of analysis using the lens of care, including the idea of the Global Care Chain. Revisions to this conceptualization have pushed for its extension beyond domestic workers in the home to include skilled workers in other institutional settings, particularly nurses in hospitals and long-term care settings. Reviewing relevant literature on migrant nurses, this article explores the labour market experiences of internationally educated nurses in Canada. The article reviews research on the barriers facing migrant nurses as they transfer their credentials to the Canadian context. Analysis of this literature suggests that internationally trained nurses experience a form of occupational (im)mobility, paradoxical, ambiguous and contingent processes that exploit global mobility, and results in the stratified incorporation of skilled migrant women into healthcare workplaces

    Adoption and Abandonment of Precision Soil Sampling in Cotton Production

    Technology adoption in precision agriculture has received considerable attention, while abandonment has received little. Our objective was to identify factors motivating adoption and abandonment of precision soil sampling in cotton. Results indicate younger producers who farmed more cotton area, owned more of their cropland, planted more non-cotton area, used a computer, or used a Personal Digital Assistant (PDA) were more likely to adopt precision soil sampling. Those with more cotton area or who owned livestock were more likely to abandon, while those who used precision soil sampling longer, used a PDA, or used variable-rate fertilizer application were less likely to abandon.Crop Production/Industries,

    Searching for Massive Outflows in Holmberg IX X-1 and NGC 1313 X-1: The Iron K Band

    We have analysed all the good quality XMM-Newton data publicly available for the bright ULXs Holmberg IX X-1 and NGC 1313 X-1, with the aim of searching for discrete emission or absorption features in the Fe K band that could provide observational evidence for the massive outflows predicted if these sources are accreting at substantially super-Eddington rates. We do not find statistically compelling evidence for any atomic lines, and the limits that are obtained have interesting consequences. Any features in the immediate Fe K energy band (6-7 keV) must have equivalent widths weaker than ~30 eV for Holmberg IX X-1, and weaker than ~50 eV for NGC 1313 X-1 (at 99 per cent confidence). In comparison to the sub-Eddington outflows observed in GRS 1915+105, which imprint iron absorption features with equivalent widths of ~30 eV, the limits obtained here appear quite stringent, particularly when Holmberg IX X-1 and NGC 1313 X-1 must be expelling at least 5-10 times as much material if they host black holes of similar masses. The difficulty in reconciling these observational limits with the presence of strong line-of-sight outflows suggests that either these sources are not launching such outflows, or that they must be directed away from our viewing angle.Comment: 11 pages, 5 figures, accepted for publication in MNRA

    Examining the nature of the ultraluminous X-ray source Holmberg II X-1

This is an open access article distributed under the terms of the Creative Commons Attribution License (CC BY), https://creativecommons.org/licenses/by/4.0/We present a comprehensive spectral analysis of the ultraluminous X-ray source Holmberg II X-1 using broadband and high-resolution X-ray spectra taken with the XMM-Newton satellite over a period of 19 yr, taking advantage of data from a recent campaign. We tested several models for the broadband spectra, including a double thermal component provided a reasonable description for the continuum between 0.3 and 10 keV and enabled us to constrain the properties of the accretion disc. The luminosity–temperature trends of the inner and outer disc components broadly agree with the expectations for a thin disc, although the exact values of the slopes are slightly sensitive to the adopted model. However, all tested models show L − T trends that deviate from a power law above a bolometric luminosity of about 5 × 1039 erg s−1, particularly for the hot thermal component associated with the inner accretion flow. Assuming that such deviations are due to the accretion rate exceeding its Eddington limit or, most likely, the super-critical rate, a compact object with a mass of 16–36 M⊙ was inferred, specifically, a stellar-mass black hole. The time-averaged (2021) high-resolution spectra present narrow emission lines at 1 keV primarily from Ne IX-X and a very strong at 0.5 keV from N VII, which indicate Ne–N-rich gas with non-solar abundances. This favours a nitrogen-rich donor star, such as a blue or red supergiant, which has escaped from its native stellar cluster characterised by a low-metallicity environment.Peer reviewe

    Discovery of a red supergiant counterpart to RX J004722.4-252051, a ULX in NGC 253

    We present two epochs of near-infrared spectroscopy of the candidate red supergiant counterpart to RX J004722.4–252051, a ULX in NGC 253. We measure radial velocities of the object and its approximate spectral type by cross-correlating our spectra with those of known red supergiants. Our VLT/X-shooter spectrum is best matched by that of early M-type supergiants, confirming the red supergiant nature of the candidate counterpart. The radial velocity of the spectrum, taken on 2014 August 23, is 417 ± 4 km s−1. This is consistent with the radial velocity measured in our spectrum taken with Magellan/MMIRS on 2013 June 28, of 410 ± 70 km s−1, although the large error on the latter implies that a radial velocity shift expected for a black hole of tens of M⊙ can easily be hidden. Using nebular emission lines we find that the radial velocity due to the rotation of NGC 253 is 351 ± 4 km s−1 at the position of the ULX. Thus the radial velocity of the counterpart confirms that the source is located in NGC 253, but also shows an offset with respect to the local bulk motion of the galaxy of 66 ± 6 km s−1. We argue that the most likely origin for this displacement lies either in a SN kick, requiring a system containing a ≳ 50 M⊙ black hole, and/or in orbital radial velocity variations in the ULX binary system, requiring a ≳ 100 M⊙ black hole. We therefore conclude that RX J004722.4–252051 is a strong candidate for a ULX containing a massive stellar black hole
